Characterizing migraine burden and treatment response using nationwide smartphone app-based data in Korea.

BACKGROUND: Migraine is a major source of disability in Korea, yet many patients remain underdiagnosed or undertreated. Traditional epidemiological studies are limited by recall bias and incomplete patient-reported outcomes. App-based headache diaries offer real-time, repeated measures of symptoms and treatment, enabling large-scale assessment to better understand migraine burden and treatment response. This study aimed to evaluate the large-scale implementation of a nationally distributed smartphone "Headache Diary" mobile application and describe migraine burden, acute treatment response, an

Clinical Practice Patterns in the Physiotherapy Management of Tension-Type Headache Among Spanish Physiotherapists.

Background: Tension-type headache (TTH) is the most prevalent primary headache disorder worldwide and represents a major source of disability related to chronic pain. Despite its high prevalence, uncertainty remains regarding optimal conservative management strategies, and limited evidence is available on how physiotherapists apply existing recommendations in routine clinical practice. Objective: The objective was to explore physiotherapists' perceptions, clinical experiences, and treatment strategies in the management of tension-type headache, with particular emphasis on commonly used interve

Tension-Type Headache: Toward an Integrative Multidimensional Framework for Clinical Stratification and Personalized Management.

Tension-type headache (TTH) is the most prevalent primary headache disorder worldwide, contributing substantially to individual disability and global socioeconomic burden. Despite its high prevalence, TTH remains clinically heterogeneous, with episodic and chronic forms influenced by the dynamic interplay of peripheral, central, psychosocial, and lifestyle-related mechanisms. Peripheral musculoskeletal factors, including craniocervical muscle alterations and myofascial trigger points, interact with central sensitization processes, while psychosocial stressors, coping strategies, and lifestyle

Reduced thalamic glutathione in migraine: a MEGA-PRESS spectroscopy study.

BACKGROUND: Migraine and tension-type headache (TTH) are highly prevalent primary headache disorders lacking objective diagnostic biomarkers. Oxidative stress has been implicated in migraine pathophysiology, with glutathione (GSH) serving as the brain's principal antioxidant. This study investigated whether thalamic GSH levels, measured using optimized MEGA-PRESS spectroscopy, could serve as a candidate biomarker for differentiating migraine from TTH. METHODS: This cross-sectional study enrolled 76 participants: 20 healthy controls (HC), 19 TTH patients, and 37 migraine patients (27 without a

Patterns of headache in patients with antiphospholipid syndrome in relation to autoantibodies.

BACKGROUND: The interplay between Antiphospholipid syndrome (APS) and migraine is very controversial; migraine is believed to be the most common neurologic symptom in APS, and there are claims that migraineurs have abnormally high Antiphospholipid antibodies (aPL), yet the reports differ widely. Nevertheless, APS could lead to various other types of headaches; both primary and secondary, leaving patients susceptible to exacerbation of symptoms, overlapping of headaches, and atypical presentations. AIM OF THE WORK: To describe types and characteristics of headache in a cohort of patients with

Selective association and familial aggregation of migraine in epilepsy: evidence from a dual-control study.

BACKGROUND: Headache, particularly migraine, is frequently reported in people with epilepsy, but the specificity of this association and the contribution of familial susceptibility is not well defined. We evaluated headache prevalence in patients with epilepsy, their first-degree relatives, and unrelated controls. METHODS: In this prospective case-control study conducted at a tertiary epilepsy centre, we enrolled 120 consecutive patients with epilepsy, 120 first-degree relatives (one per patient), and 120 age- and sex-matched unrelated controls. Epilepsy was classified according to the Intern

Over 600 million people aged 18-65 will have headache tomorrow: global 1-day prevalence and recall bias from a meta-analysis of individual participant data (N = 38,512) from the general populations of 15 countries.

BACKGROUND: Our recent estimate of the global 1-year prevalence of headache among those aged 18-65 years was 65%: considerably higher than previous estimates, but based solely on high-quality epidemiological data derived from a large population-based sample. Here we present complementary estimates of 1-day prevalence. METHODS: We performed a meta-analysis of individual participant data from cross-sectional surveys among population-representative samples (age range 18-65 years) from 15 countries and all world regions. All used the Headache-Attributed Restriction, Disability, Social Handicap an

[Genetics of primary headache disorders in children and adolescents].

This review describes current knowledge on the genetics of primary headache disorders in children and adolescents. The heritability for recurrent headache in children and adolescents is estimated at 70%, significantly higher than the heritability for both migraine and tension-type headache in adults. Hemiplegic migraine is a rare monogenic migraine subtype with causal variants identified in CACNA1A, ATP1A2, SCN1A, and PRRT. There is very limited knowledge of genetic risk variants for children and adolescents with polygenic primary headache disorders.

Matrix and graphical representation of the primary headache syndromes in the International Classification of Headache Disorders (ICHD3): a basis for automated diagnosis and analysis of criteria.

OBJECTIVE: We represent primary headaches within the International Headache Classification (ICHD3) in matrix form and show that this representation allows for automated diagnosis as well as additional insights into headache classification. BACKGROUND: ICHD3 has been the gold standard for clinical trials and research in headache medicine. Given its criteria-based language, ICHD3 can be interpreted as a set of logical statements and as a result can be encoded as the biadjacency matrix of a mathematical graph. This paper implements this interpretation of the classification and explores the clini

Multidisciplinary management of headache in developmental age: effects on school performance, quality of life, and therapeutic perspectives.

Primary headaches represent a major public health burden in the pediatric population, with a prevalence exceeding 60%. They significantly impair quality of life, school attendance, and family dynamics, often manifesting as a complex biopsychosocial condition. This review provides a comprehensive update on the epidemiology, clinical classification, and comorbidities of pediatric primary headaches, offering a critical analysis of current diagnostic pathways and evolving therapeutic strategies. We examined recent literature, ICHD-3 classification criteria, and international consensus guidelines (

Symptoms in hypertension: Diagnostic challenges, clinical implications and management strategies for headache, dizziness and vertigo.

Headache, dizziness and vertigo are non-specific but frequently reported symptoms in patients with hypertension. When evaluating headache, clinicians must systematically exclude primary headache disorders - such as migraine, tension-type headache and trigeminal autonomic cephalalgias - before attributing symptoms to hypertension. If a primary headache is suspected, referral to a neurologist is essential for specialized management. Hypertension-related headache must fulfil specific diagnostic criteria, including a high blood pressure level (≥180/120mmHg), a demonstrated causal relationship thro

Clinical features of chronic tension-type headache combined with hyperacusis: a cross-sectional study.

BACKGROUND: Chronic tension-type headache (CTTH) is a prevalent primary headache disorder that significantly affects quality of life. Hyperacusis, an abnormal intolerance to everyday sounds, is frequently observed in patients with CTTH, suggesting potential shared neural mechanisms. However, the clinical characteristics of patients with CTTH and comorbid hyperacusis remain poorly characterized. OBJECTIVES: This study aimed to investigate the clinical characteristics of CTTH patients with comorbid hyperacusis, focusing on headache profiles, auditory sensitivity patterns, and psychological corr

Comparative Efficacy of Botulinum Toxin Type A Versus Antihomotoxic Therapy in the Treatment of Bruxism-Related Masticatory Muscle Hyperactivity: A Randomized Clinical Trial Using Electromyographic Measurements.

BACKGROUND: Bruxism is currently defined, according to the international consensus of Lobbezoo and colleagues, as a repetitive masticatory muscle activity characterised by clenching or grinding of the teeth and/or bracing or thrusting of the mandible. It is a motor behaviour-not a disorder in itself-that may act as a risk factor for orofacial pain, tooth wear, masticatory muscle hypertrophy and temporomandibular disorders in susceptible individuals. Pharmacological management is indicated only when such sequelae are present. Botulinum toxin type A (BTX-A) has emerged as a widely investigated o
