BACKGROUND: The causes of persistent headache following concussion are poorly understood. The objective of this study is to explore analgesic overuse as a potential cause of chronic post-traumatic headache among adolescents referred to a headache clinic following concussion.
METHODS: A retrospective chart review was conducted of all adolescent concussion patients referred to our pediatric headache clinic over the 16-month period between August 1, 2011, and November 30, 2012. Those patients with chronic post-traumatic headaches of 3-12 months' duration who also met International Headache Socie

Both primary headaches and minor head injuries are common in children. If headache presents for the first time or becomes exacerbated soon after head injury it is described as post-traumatic headache (PTH). Acute PTH resolves within 3 months from injury, but chronic PTH continues beyond 3 months. The pathogenesis of PTH is not well understood. Several mechanisms were proposed such as axonal injuries and disturbances of cerebral metabolic processes. The clinical features of PTH are those of primary headache disorders such as migraine and tension-type headache and usually follow a favorable prog

BACKGROUND: Chronic post-traumatic headache (CPTH) after mild head injury can be difficult to manage. Research is scarce and successful interventions are lacking. To evaluate the effect of a group-based Cognitive Behavioural Therapy (CBT) intervention in relation to headache, pain perception, psychological symptoms and quality of life in patients with CPTH.
METHODS: Ninety patients with CPTH according to ICHD-2 criteria were enrolled from the Danish Headache Center into a randomized, controlled trial. Patients were randomly assigned to either a waiting list group or to a nine-week CBT group in

The clinical effectiveness of primary and secondary headache treatment by transcranial direct current stimulation (tDCS) with various locations of stimulating electrodes on the scalp was analyzed retrospectively. The results of the treatment were analyzed in 90 patients aged from 19 to 54 years (48 patients had migraine without aura, 32 - frequent episodic tension-type HAs, 10 - chronic tension-type HAs) and in 44 adolescents aged 11-16 years with chronic post-traumatic HAs after a mild head injury. Clinical effectiveness of tDCS with 70-150 μA current for 30-45 min via 6.25 cm(2) stimulating

Mild traumatic brain injury (mTBI) is a common complication of minor head injury and a serious problem in the Iraq war returnees. Effective treatment is not yet available. We have treated 23 patients with chronic post-traumatic headache by epidural saline and oxygen injection (ESOI) with efficacy of 96 %. Among them, ten cases were cured. Two out of these cured cases fulfilled the criteria of mTBI and their improvement were objectively demonstrated by a TriIRIS C9000 (Hamamatsu Photonics K.K.) that can monitor the accommodation and convergence function simultaneously. We show the treatment pro

BACKGROUND: The aetiology behind chronic post-traumatic headache (CPTH) after mild head injury is unclear and management is complicated. In order to optimize treatment strategies we aimed to characterize a CPTH population.
METHODS: Ninety patients with CPTH and 45 patients with chronic primary headaches were enrolled from the Danish Headache Center. All patients were interviewed about demographic and headache data. They completed the Harvard Trauma Questionnaire (HTQ), Rivermead Post Concussion Symptoms Questionnaire, SF-36 and a headache diary.
RESULTS: The CPTH group experienced more cogni

UNLABELLED: Post-traumatic headache (PTH) is the most common symptom found in the post-traumatic syndrome, whose onset occurs within seven days of the trauma. The condition is characterized as acute when it persists for up to 3 months. PTH beyond this period is considered chronic.
OBJECTIVES: The objective of this study was to determine the clinical features of chronic post-traumatic headache (cPTH) and its association with depression, anxiety and quality of life.
METHODS: A total of 73 female subjects were evaluated. Patients were divided into three groups: (a) group without headache, CONTR

BACKGROUND: Chronic headache is one of the most prominent symptoms among subjects with traumatic head injury (THI). Despite the relatively high prevalence of chronic post-traumatic headache (CPTHA) and its enormous effect on the already poor quality of life of subjects with THI, its mechanisms has not been studied in depth.
OBJECTIVE: To conducted quantitative somatosensory testing in THI subjects with and without chronic post-traumatic headache (CPTHA) in order to shed light on the yet, unknown pathophysiology of CPTHA.
METHODS: THI subjects with and without CPTHA and healthy controls under

Mild traumatic brain injury is very common in Western societies, affecting approximately 1.8 million individuals in the USA. Even though between 30% and 90% of patients develop post-traumatic headache, post-traumatic headache remains a very controversial disorder. Particularly when it comes to chronic post-traumatic headache following mild closed head injury and headache attributed to whiplash injury. Some experts are disputing its existence as a genuine disorder. Indistinct disease classification, unresolved pathophysiological mechanism, and the role of accident-related legal issues further f

BACKGROUND AND PURPOSE: Chronic post-traumatic headache attributed to mild head injury is a somewhat disputed headache diagnosis. A main object of this study was to assess the validity of this diagnosis by studying the headache pattern of concussed patients that participated in one historic (n = 131) and one prospective cohort (n = 217) study.
METHODS: Head injury patients were recruited from two hospitals in Kaunas, Lithuania. Controls were recruited amongst patients with minor orthopaedic traumas not involving the head and neck.
RESULTS: When data from the two studies were pooled, no diffe

The history of post-traumatic headache begins in the middle of the 19th century, and its latest iteration has been defined in the International Headache Classification of 2004. Contrary to the latter, there are instances when mild head injury without symptoms or signs of concussion may evoke the pathophysiological changes of migraine. The mechanisms of chronic post-traumatic headache and the associated syndrome are complex and include pathophysiological, psychological, and socioeconomic factors. Treatment of these headaches is similar to that of the primary headaches with particular attention

Chronic post-traumatic headache (PTH) always arises from some sort of head injury such as mild traumatic brain injury or concussion. The terminology and definitions for these injuries remain controversial and continue to evolve. This article reviews and updates current terminology and definitions for these head injuries, along with current concepts of pathophysiology. The second portion of this article considers the persistence of the other symptoms associated with chronic PTH, which constitute the post-concussive disorder. PTH is the most common symptom after a head injury, but post-concussiv

This was a prospective, observational study of children aged 3 to 15 years admitted to hospital with head injury (HI). Demographic data and information on the nature of the HI, and history of premorbid headache were collected. A structured telephone questionnaire was used to interview parents and children 2 months after injury and at 4-monthly intervals for up to 3 years, if headache was reported. One hundred and ninety children were admitted with HI. Data were available on 117 children (81 males, 36 females; mean age 8y 5mo [SD 3y 1mo]). HI was minor in 93 patients and significant in the rest

BACKGROUND: Chronic post-traumatic headache (CPH) is a complex clinical entity. The clinical features of CPH are variable and may resemble those of primary headaches (migraine, tension-type headache or cluster headache), as well as of cervicogenic headache. Therefore, the treatment must be individualized according to the headache type. There were few studies addressing the effectiveness of pharmacological treatment in patients with CPH.
AIM: The aim of the present study was to analyze the efficiency of pharmacological treatment with analgesic drugs and concomitant psychotropic drugs in outpat

The syndrome known as chronic post-traumatic headache (CPTH) is controversial, particularly when it occurs following mild closed head injury or "whiplash". Problems with the diagnosis include issues related to classification, epidemiology, pathophysiology (psychogenic vs. organic) and treatment. In addition, there are cultural differences that may influence prevalence and diagnosis, as in some countries with little medico-legal recourse the disorder is virtually unknown. In this paper we review some of these controversies clinicians are left to formulate their own understanding based on availa

We report here studies on the levels of autoantibodies (aAb) to AMPA glutamate receptors (GluR1 subunit) and NMDA glutamate receptors (NR2A subunit) in serum from 60 children aged 7-16 years with chronic posttraumatic headache (CPTHA) following mild craniocerebral trauma (CCT). The first group consisted of 48 children who had sustained cerebral concussion (CC), of which 34 had single-episode CC (subgroup 1a) and 14 had repeated CC (subgroup 1). The second group included 12 children with mild cerebral contusions (MCC). Serum glutamate receptor aAb levels were measured six months and one year af

Over the years, there has been much confusion and conflict regarding when a post-traumatic headache (PTH) should be classified as chronic. Chronic pain usually is described as pain persisting for longer than 6 months. For many years, a chronic PTH was considered to be a headache lasting longer than 2 months. Some useful definitions and criteria were published in 2003 by the International Headache Society, in the Second Edition of the Classification and Diagnostic Criteria for Headache Disorders. These criteria are reviewed and discussion follows regarding the problem of when a chronic PTH shou
