Paroxysmal migraine associated with vancomycin administration: A case report, a review of pharmacovigilance databases for similar cases and possible mechanisms.

BACKGROUND: Vancomycin is a core drug for the treatment of methicillin-resistant Staphylococcus aureus, but its neurologic adverse effects are rarely reported. This case is the first to report a possible association between intravenous vancomycin and episodic migraine. Rapid identification of drug-induced migraine can avoid unnecessary invasive testing and medication delays and has direct clinical value. OBJECTIVE: To report and document a case of episodic migraine that was highly correlated with the timing of intravenous vancomycin administration, assessing causality and its general clinical

A Curious Case of Galactorrhea in the Context of Gastroenteritis: A Case Report.

Metoclopramide is a prokinetic and antiemetic agent widely used in primary care. Its mechanism of action explains both its therapeutic effects, as well as certain adverse reactions, including hyperprolactinemia and galactorrhea. Metoclopramide-induced galactorrhea may prompt concern and lead to unnecessary diagnostic testing. This case report is about a healthy 21-year-old woman taking a combined oral contraceptive who was prescribed metoclopramide for gastroenteritis with vomiting. Forty-eight hours after starting treatment, she developed spontaneous, bilateral, multiductal, painless galactor

Extrapyramidal symptoms and effectiveness of continuous vs bolus intravenous metoclopramide: A systematic review and meta-analysis.

OBJECTIVE: Metoclopramide is widely used to treat nausea, vomiting, and headache. However, it may cause extrapyramidal symptoms (EPS) such as akathisia. Continuous intravenous (IV) infusion has been proposed as a safer alternative to bolus injection. This study aimed to compare the risk of EPS and effectiveness between continuous and bolus IV metoclopramide administration. METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) comparing continuous vs. bolus IV administration of metoclopramide. Databases searched included CENTRAL, MEDLINE, Embase, CI

Dental drugs with proarrhythmic risk in patients with Brugada syndrome: precaution instructions for practices in the field of orofacial pain.

Orofacial pain, diagnosed and treated in a subfield of dentistry, highly relies on long-term psychotropic, neuromodulating, and analgesic regimens that have the potential to alter cardiac ion channels and autonomic tone. In patients with inherited arrhythmia syndromes such as Brugada syndrome (BrS), these drugs may unmask a type 1 Brugada electrocardiographic pattern or trigger malignant ventricular arrhythmias and sudden cardiac death, yet most dental guidance addresses only short-term use of local anesthetics. In this narrative review, we synthesize evidence on the arrhythmogenic potential o

Isotretinoin-induced Intracranial Hypertension Presenting as Unilateral Pulsatile Tinnitus.

BACKGROUND: Pulsatile tinnitus (PT), a challenging condition for patients and clinicians alike, is often linked to vascular, neoplastic, and systemic disorders. The most common cause is idiopathic intracranial hypertension (IIH). However, physicians must recognize that not all intracranial hypertension (IH) is idiopathic and can be due to secondary causes, including cerebral venous anomalies, medications, and systemic medical conditions. OBJECTIVE: To highlight the importance of this entity, we report a case of drug-induced IH (DIIH), caused by isotretinoin (ISO), that presented with persiste

Itraconazole-Induced Motor Sensory Polyneuropathy in a Young Indian Male: An Unusual Case.

INTRODUCTION: Itraconazole, a widely prescribed triazole antifungal agent, is generally well tolerated but can rarely cause neurotoxicity. Motor-sensory polyneuropathy secondary to itraconazole use is an exceptionally uncommon and underrecognized adverse effect. Although reversibility has been reported in several cases, delayed diagnosis or continued exposure may result in persistent neurological impairment. This case report aims to highlight a rare but clinically significant adverse effect-motor-sensory polyneuropathy-associated with prolonged itraconazole use in an otherwise healthy young ad

Amiodarone-induced aseptic meningitis.

Drug-induced aseptic meningitis (DIAM) is a rare but serious adverse drug reaction. A broad spectrum of medications is linked to DIAM, with NSAIDs, human IVIG, antibiotics (particularly sulphonamides and penicillins), vaccines and monoclonal antibodies being among the most frequently reported. Amiodarone is a frequently used anti-arrhythmic medication and neurotoxic effects including tremor and peripheral neuropathy are well known. We present a case of a male in his late 70s admitted to the emergency department with a 5-week history of headache, vomiting and weight loss. Cerebrospinal fluid an

Hyperacusis-inducing drug candidates.

Hyperacusis is a troubling loudness hypersensitivity disorder occasionally involving pain around ear and face. The neural origins of hyperacusis and conditions that evoke it are poorly understood. To gain insights into the neural mechanisms that might give rise to hyperacusis, a pharmacovigilance analysis was conducted on 3833 case reports in which hyperacusis was listed as a drug-induced adverse event in the United States Food and Drug Administration Adverse Event Reporting System (FAERS). The 10 drug classes ranked by the number of reported hyperacusis cases were selective (1) serotonin reup

Pharmacogenetic mechanism of cilostazol-induced headaches: Splicing-mediated loss of ABCC5 gene function.

BACKGROUND AND PURPOSE: Cilostazol, a phosphodiesterase 3 inhibitor, causes intolerable headaches in over one third of patients, frequently leading to treatment discontinuation. We investigated whether ABCC5 variant rs7636910 (NM_005688.4:c.1146A > G) protects against cilostazol-induced headaches through altered cyclic nucleotide signalling. EXPERIMENTAL APPROACH: Analysis included 101 healthy Korean volunteers from Phase 1 clinical trials, with independent replication in a multi-ethnic cohort (n = 369) from the All of Us Research Program. Functional consequences of rs7636910 were evaluated t

Metoclopramide-Induced Hyperprolactinemia Mimicking Prolactinoma in a Young Woman Receiving Fragmented Care: A Case Report.

Hyperprolactinemia accompanied by headache and visual symptoms commonly prompts pituitary imaging to rule out prolactinoma. Several medications can elevate serum prolactin into ranges that overlap with prolactin-secreting adenomas, and metoclopramide, a dopamine D2 receptor antagonist used for gastroparesis and nausea, is among the most frequent offenders. Headache and blurred vision are also listed as adverse effects, reproducing the symptomatic triad clinicians associate with a pituitary tumor. We describe a 22-year-old woman with polycystic ovary syndrome who presented with a prolactin leve

New-onset alopecia areata during calcitonin gene-related peptide inhibitor therapy: first reported case.

Dear Editor, Calcitonin gene-related peptide (CGRP) is a neuropeptide with potent vasodilatory effects and a key role in trigeminovascular signaling, underlying the pathogenesis of migraine. Consequently, the inhibition of CGRP activity has been demonstrated to reduce the incidence of acute migraine attacks. Based on this evidence, monoclonal antibodies against CGRP or its receptor have been approved by the European Medicines Agency for prevention of both episodic and chronic migraine. Among CGRP inhibitors, galcanezumab has also been authorized for the treatment of episodic cluster headache.

Suspected Lamotrigine-Associated Haematological Adverse Reaction in the Context of Polypharmacy, Alcohol Use, HIV, and Recent Medication Changes.

This is a case report of an adult male with well-controlled HIV infection who developed fever, malaise, myalgia and headache within 24-48 hours of commencing lamotrigine. Laboratory investigations demonstrated acute leukopenia, neutropenia and thrombocytopenia, accompanied by elevated inflammatory markers. Following withdrawal of lamotrigine, constitutional symptoms improved rapidly, while haematological and inflammatory markers demonstrated an overall trend towards recovery over subsequent days, although some laboratory abnormalities transiently persisted or worsened before normalising.
