Gene expression profiles of carotid plaques differ between patients with amaurosis fugax and those with cerebral transient ischaemic attack or stroke.

AIMS: In patients with atherosclerotic internal carotid artery stenosis, the risk of subsequent ischaemic stroke is higher in those presenting with a cerebral transient ischaemic attack (TIA) than in those with amaurosis fugax (AFX; transient monocular blindness). While AFX is associated with more stable histological characteristics, the underlying molecular mechanisms remain unclear. We hypothesized that biological processes and gene expression profiles in carotid plaques of patients with AFX differ from those with cerebral TIA and stroke. METHODS AND RESULTS: We analysed carotid plaques fro

Determinants of prehospital and in-hospital delay in patients with symptomatic carotid stenosis and their influence on the outcome after elective carotid endarterectomy.

BACKGROUND: This study analyses the determinants of prehospital (index event to admission) and in-hospital delay (admission to carotid endarterectomy (CEA)). In addition, the analysis addresses the association between prehospital or in-hospital delay and outcomes after CEA for symptomatic patients in German hospitals. MATERIALS AND METHODS: This retrospective analysis is based on the nationwide German statutory quality assurance database. 55 437 patients were included in the analysis. Prehospital delay was grouped as follows: 180-15, 14-8, 7-3, 2-0 days or 'in-hospital index event'. In-hospit

The diagnosis and treatment of asymptomatic and symptomatic patients with carotid artery stenosis.

Carotid artery atherosclerotic stenosis is an important annual cause of stroke in the United States. Moreover, the incidence of carotid artery stenosis is significantly increasing due to the widespread popularity of high fat and high salt diets, sedentary lifestyles, and the increasing age of the population. Of major importance to cardiovascular specialists is the fact that individuals with atherosclerotic carotid artery stenosis can have a prevalence of atherosclerotic coronary artery disease as high as 50 to 75%. Vascular screening for carotid artery stenosis with Doppler ultrasound should b

Higher Rate of Reinterventions after Transfemoral Carotid Artery Stenting in Symptomatic Patients: A Retrospective Stroke Center's Cohort Study Between 2015-2024.

BACKGROUND: Carotid artery stenosis is a significant contributor to ischemic strokes, and its surgical management includes carotid artery endarterectomy (CEA), transfemoral carotid artery stenting (TF-CAS), and transcarotid artery revascularization (TCAR). CEA has traditionally been preferred, but TF-CAS and TCAR are also excellent alternative options if the anatomy of the vessels allows them. This study reports our short- and mid-term outcomes after carotid artery revascularization in symptomatic patients at a stroke center. METHODS: This single-institution retrospective cohort study was con

Neuro-Ophthalmic Presentations of Polycythemia.

BACKGROUND: Ocular manifestations of hyperviscosity, particularly associated with the hemodynamic changes in polycythemia, can lead to various ocular vascular events, ranging from transient monocular blindness to potentially irreversible vision loss if left untreated. METHODS: This is a multicenter, retrospective case series and review of literature involving 4 patients presenting with neuro-ophthalmic symptoms related to underlying polycythemia. A thorough search for prior reported cases with similar manifestations to ours was conducted through the PubMed database. RESULTS: Four patients, 3

Outcomes of carotid endarterectomy in symptomatic octogenarian patients.

INTRODUCTION: Carotid endarterectomy (CEA) is the standard treatment for symptomatic carotid artery stenosis (ipsilateral cerebral ischemic symptoms in the last 3months - SCS) in patients suitable for open surgery. Given the increasing age of the general population, this treatment is performed in octogenarian patients with increasing frequency. However, this population was not included in the historical randomized controlled trials and there is a lack of available data on perioperative complications, specifically the severity of possible postoperative stroke. The aim of this study is to report

Identification of risk factors for permanent visual loss in patients with giant cell arteritis.

OBJECTIVES: permanent visual loss (PVL) is the most frequent ischemic complication of GCA. We aimed to evaluate whether clinical signs, symptoms, and blood test abnormalities at GCA diagnosis can predict PVL. PATIENTS AND METHODS: retrospective, multicenter study of patients with biopsy-proven GCA. The whole cohort was randomly split into a derivation and a validation dataset. Multivariable logistic regression (MVLR) was used to develop a prediction model and a predictive score. The model's performance was determined through the area under the curve (AUC). RESULTS: 620 patients were included

Intraplaque Hemorrhage Volume and Ischemic Stroke Risk.

BACKGROUND AND PURPOSE: Intraplaque hemorrhage (IPH) in carotid atherosclerotic plaques is the best established biomarker of plaque vulnerability. However, the relationship between IPH volume and ischemic neurologic symptoms remains scarcely studied. This study explored the association between carotid IPH volume and ischemic event severity. MATERIALS AND METHODS: A retrospective analysis was conducted on consecutive patients with suspected carotid atherosclerosis evaluated from December 2015 to January 2021. Patients underwent carotid plaque MRI using T1-weighted imaging with fat suppression

Management and Treatment of Carotid Stenosis: Overview of Therapeutic Possibilities and Comparison Between Interventional Radiology, Surgery and Hybrid Procedure.

Carotid stenosis is a common pathology in clinical practice and unfortunately carries a high risk of serious cerebrovascular events. The early recognition of carotid plaque and, consequently, a careful analysis by means of multimodal imaging are the necessary steps to undertake a correct management pathway, aimed at preventing or, if not possible, reducing the risk of atherogenic phenomena responsible for cerebral infarction. In particular, the presence or absence of clinical symptoms, understood as the occurrence of events such as TIAs in the last 6 months, non-disabling strokes or repeated e

CASCOM Study: CAR Score Predicting Restroke in Symptomatic Carotid Stenosis With Only Intensive Medical Therapy.

BACKGROUND: Carotid endarterectomy for symptomatic carotid stenosis is based on decades-old trials, while modern medical therapy has substantially improved outcomes. The carotid artery risk (CAR) score predicts stroke risk using clinical and imaging factors. This study evaluates the 2-year risk of recurrent ipsilateral stroke in patients with recent transient ischemic attack, amaurosis fugax, or minor stroke and ipsilateral symptomatic carotid stenosis treated with intensive medical therapy. METHODS: In this prospective, single-arm observational study conducted in Denmark from October 2020 to

Amaurosis fugax progressing to retinal artery occlusion with anterior migration of a retrobulbar embolus on orbital color Doppler imaging.

PURPOSE: Orbital color Doppler imaging (CDI) is useful in the evaluation of sudden monocular vision loss, providing information on etiology which may guide management. We present two cases of amaurosis fugax progressing to retinal artery occlusion (RAO) associated with migration of a hyperechoic particle within the central retinal artery (CRA) and altered vascular dynamics found on CDI. OBSERVATIONS: Both patients presented with amaurosis fugax, and CDI revealed a hyperechoic particle 2.8 mm from the optic nerve head in both patients. Patient 1 was found to have severe aortic stenosis and a t

Cerebrovascular imaging of carotid embolization: Amaurosis fugax and transient ischemic attack in motion.

We describe a 65-year-old male ophthalmologist who experienced a brief episode of transient monocular vision loss in his left eye, characterized as a "gray-out" lasting a few seconds. Utilizing his medical expertise, the patient recorded the blood flow in his ophthalmic artery, capturing a rare real-time visualization of embolus migration from the central to peripheral regions. The embolism resolved spontaneously, restoring normal vision. Duplex ultrasound revealed high-grade stenosis in the proximal left internal carotid artery, with a peak systolic velocity of 421 cm/s and an end-diastolic v

Vitreoretinal lymphoma presenting as amaurosis fugax.

PURPOSE: To describe a case of vitreoretinal lymphoma with unilateral vascular involvement. OBSERVATIONS: A 80-year-old male with recurrent amaurosis fugax (AF) in the left eye was seen in the ophthalmological department. During a period of seven months the patient developed central vein occlusion (CRVO) and later the panuveitis suspected to be acute retinal necrosis (ARN) before being diagnosed with vitreoretinal lymphoma (VRL). The patient preferred enucleation as treatment which allowed thorough histological examination revealing Epstein Barr virus-associated vitreoretinal lymphoma. CONCL

Association between index symptom and timing on perioperative stroke rate in patients undergoing carotid endarterectomy.

BACKGROUND AND PURPOSE: Carotid endarterectomy (CEA) in symptomatic carotid stenosis (SCS) may have a higher risk of perioperative stroke due to disease severity or hemorrhagic conversion. This study aimed to evaluate CEA outcomes for SCS and examine causes of post-operative stroke based on intervention timing and preoperative symptoms. METHODS: All CEAs performed for SCS from 2012 to 2023 across two metropolitan hospitals were analyzed. CEAs were performed with general anesthesia, patching, and shunting. Post-operative (30-day) strokes were classified as technical, hemorrhagic, or embolic an

Case report and literature review of transthyretin amyloidosis with p.Ser43Asn mutation presenting in China.

BACKGROUND: Familial transthyretin amyloidosis (ATTR) is a rare hereditary disorder marked by abnormal protein fibril deposits that impair multiple organ functions. Although over 130 mutations have been identified, the p.Ser43Asn variant, which has been associated with amyloidosis involving thyroxine-binding globulin (TBG), is scarcely reported in the literature, especially within the Chinese population. CASE PRESENTATION: We report a case of a 50-year-old male presenting with recurrent episodes of amaurosis fugax. The patient had a family history of cardiac disease. Auxiliary examinations re
