Transient monocular obscuration--?amaurosis fugax: a case report.

A 73-year-old white man with pseudophakia experienced repeated bouts of transient visual loss associated with erythropsia and colour desaturation. A diagnosis of atheromatous carotid vascular disease was considered, prompting carotid angiography, during which time the patient experienced transient aphasia. Subsequent examination during an episode of visual loss showed that a spontaneous anterior chamber haemorrhage was the cause of the visual complaints.

Mortality and stroke after amaurosis fugax.

One hundred and ten patients who had presented with amaurosis fugax and were treated medically were studied retrospectively. Follow up from the time of presentation was six to nineteen years with a median of eight years. The mortality and frequency of strokes in this group was compared with populations matched for age and sex. Life expectancy in patients with amaurosis fugax was reduced. Ischaemic heart disease was the most frequent cause of death and occurred at a greater rate than in the general population (p less than 0.01). The indicence of stroke was higher than in the Oxfordshire Communi

Arteriographic comparison of amaurosis fugax and hemispheric transient ischaemic attacks.

The results of carotid angiography were compared in 53 patients with amaurosis fugax and 92 with hemispheric transient ischaemic attacks (TIAs). Evidence of extracranial disease was found in 71% of those with retinal ischaemia and these patients rarely had evidence of intracranial disease (11%) or normal angiograms (15%). By contrast patients with hemispheric symptoms less frequently showed disease of the cervical carotid artery (36%) and more often had intracranial disease (27%) or normal angiograms (35%). It is suggested that amaurosis fugax is usually due to emboli originating in the intern

Amaurosis fugax as the presenting manifestation of dural arteriovenous malformation.

A 46-year-old woman under investigation for three episodes of amaurosis fugax in the left eye proved to have a left anterior-middle fossa dural arteriovenous malformation with pial venous drainage. The malformation received its main supply from the left middle meningeal artery, but its anterior part was fed by the recurrent meningeal branch of the left ophthalmic artery. Transient episodic lowering of retinal arterial pressure due to shunting of blood from the ophthalmic artery to the malformation is the most likely explanation for the occurrence of amaurosis fugax, indicating that this sympto

The post endarterectomy carotid bruit. Evaluation by Duplex scan.

The presence of a bruit after carotid endarterectomy may indicate a persistent or recurrent lesion. The authors noninvasively evaluated, by Duplex scanning, 18 asymptomatic postoperative patients who underwent a total of 23 carotid endarterectomies and who developed a postoperative bruit to determine the significance of the bruit. Eleven men and seven women were studied from 6 weeks to 2 years postsurgery. Ages ranged from 49 to 75 years (63.6 +/- 8.0 years). Indications for endarterectomy were: transient ischemic attacks (including amaurosis fugax), 17 vessels; completed stroke with significa

Ocular manifestations of carotid artery atheroma.

Although carotid endarterectomy (CE) is recommended in appropriate patients with bifurcation atheroma for the prevention of stroke, these patients are also at risk for visual loss, either temporary or permanent. The presentations of patients undergoing 512 consecutive CEs were reviewed to determine the preoperative frequency and risks of ocular disturbances. Visual symptoms occurred in 131 patients (25%). Temporary, monocular blindness (amaurosis fugax, AF) was reported by 99 patients (19%). Concomitant neurologic symptoms were present in 33 (33%) of the patients with AF, and carotid artery st

Comparison of amaurosis fugax and transient cerebral ischemia: a prospective clinical and arteriographic study.

We compared the clinical associations, arteriographic findings, and long-term outcome of 93 patients with amaurosis fugax and 212 patients with focal cerebral ischemia (transient ischemic attacks [TIAs]). The group of patients with cerebral TIAs included a significantly larger proportion of blacks and had a higher prevalence of hypertension than the group with amaurosis. Operable atherosclerotic lesions of the carotid arteries were more often associated with amaurosis (66%) than with cerebral TIAs (51%). The seven-year cumulative rate of cerebral infarction, however, was less in patients with

Idiopathic ischemic syndromes of the retina and optic nerve and their carotid origin.

Most eye problems referred to vascular surgeons for workup of carotid disease are either transient (amaurosis fugax) or partial (retinal branch artery occlusion) visual loss. Patients with more severe total infarcts of the retina (central retinal artery occlusion) or of the head of the optic nerve ("ischemic optic neuropathy") are seldom suspected of carotid disease and, in the latter case, are generally labeled idiopathic. An explanation for the difference in attitude towards these conditions is suggested. A prescribed carotid etiology for these various ischemic syndromes of the eye and optic

[Arterial occlusive diseases of the eye. Diagnostic contribution of Doppler sonography].

In a retrospective study, patients with 125 ocular arterial occlusions (71 men, 54 women) observed between 1978 and 1981 were analyzed with regard to etiology, therapy, and course of disease. Particular attention was given to information obtained by means of Doppler sonography. Among our patients the most common condition was occlusion of either the central retinal artery or its branches (62 cases), as opposed to 30 patients with apoplexia papillae, 6 with chronic ocular ischemia (or ischemic ophthalmopathy), and 4 with less common types of occlusion. In the patient collective as a whole, the

Branch retinal artery occlusion in the Churg-Strauss syndrome.

Amaurosis fugax followed by retinal infarction occurred as an early manifestation of exacerbation in a 46-year-old woman with allergic angiitis and granulomatosis (Churg-Strauss syndrome). There was evidence of widespread intraluminal branch retinal artery occlusions without accompanying retinal vasculitis. Low molecular weight dextran and heparin, in addition to systemic steroid therapy, appeared to be beneficial. A review of the ophthalmic manifestations of the Churg-Strauss syndrome and of other hypereosinophilic conditions suggests that thromboembolism resulting from a hypercoagulable stat

Amaurosis fugax in a patient with a left ventricular endocardial pacemaker.

A transvenous left ventricular endocardial pacemaker catheter is a potential source of systemic arterial embolization. The case of a woman who presented with left-eye amaurosis fugax is reported. The patient had a history of contralateral carotid atherosclerosis; however, the digital subtraction angiography of the carotid arteries was not sufficiently abnormal to account for her present symptoms. The patient had a history of two myocardial infarctions and the tachycardia-bradycardia syndrome for which she was treated with a demand ventricular pacemaker. The chest x-ray and electrocardiogram su

[A case of amaurosis fugax with anomalous origin of the ophthalmic artery and atheromatous stenosis of the external carotid artery].

Amaurosis fugax associated with anomalous origin of the ophthalmic artery without occlusion of internal carotid artery is very rare. We are reporting such a case of amaurosis fugax with anomalous origin of the ophthalmic artery and atheromatous stenosis of the external carotid artery. A 49-year-old-man was referred to our neurosurgical clinic because of transient monocular blindness. The patient was diagnosed as carotid TIA and was treated within aspirin and dipyridamole. In spite of this treatment, he experienced further transient dimness of vision in the right eye, several times during a per

Peripheral arterial occlusion and amaurosis fugax as the first manifestation of polycythemia vera. A case report.

A patient with polycythemia vera, who presented digital arterial occlusion and episodic monocular blindness as the first clinical signs of the disease is described. After an early diagnosis, reduction of the cellular blood volume, and intraarterial perfusions with adenosine triphosphate (Laevadosin) followed by reserpine (Serpasil) led to adequate local tissue perfusion. No surgical intervention was required and visual disturbances disappeared. The patient has now been in a stable phase of the disease for 7 months without subsequent treatment.

Pulsed Doppler imaging and spectrum analysis for detection of carotid artery disease.

One hundred carotid arteries in 53 patients suffering from transient cerebral ischaemia or amaurosis fugax have been studied prospectively. Internal carotid arteries were assessed by a six-channel pulsed Doppler imaging system combined with Doppler spectrum analysis and the findings compared with those obtained by X-ray contrast arteriography. Stenosis (greater than 25% diameter reduction) was correctly diagnosed by ultrasonography with a sensitivity of 93% and a specificity of 96%. Occlusion was correctly diagnosed with a sensitivity of 92% and a specificity of 97%. The ability to recognize a

External carotid endarterectomy.

Forty-nine external carotid revascularization procedures were performed in 36 patients. Indications included transient ischemic attacks, amaurosis fugax, and the procedure as a prelude to extracranial-to-intracranial bypass. Twenty patients had a preoperative neurologic defect. Twelve patients were classified as having bilateral and 24 as having unilateral carotid disease. Unilateral external carotid endarterectomy was performed in 29 patients, with the remaining patients undergoing procedures for additional occlusive lesions. Five patients had operation-related strokes (one death). Three of e

Atherosclerotic extracranial carotid artery aneurysms.

Twenty-four atherosclerotic extracranial carotid artery aneurysms were encountered in 21 patients during a 25-year period. These represented 46% of all extracranial carotid artery aneurysms diagnosed at the University of Michigan during this period. Neurologic symptoms including amaurosis fugax, transient ischemic attacks, and stroke were present in 50% of the patients. An asymptomatic pulsatile neck mass occurred in 33%. Surgical therapy was undertaken for 18 aneurysms, and nonoperative treatment was pursued in the remaining six aneurysms. Operative therapy included 14 aneurysmectomies and fo

The correlation between carotid plaque ulceration and cerebral infarction seen on CT scan.

The purpose of the study was to determine the association between cerebral infarction seen on CT scan and macroscopic ulceration of atheromatous carotid plaques in patients undergoing carotid endarterectomy. Following carotid endarterectomy in 65 patients, specimens were examined for the presence of ulceration without knowing the result of the preoperative CT brain scan. The 65 patients thus investigated underwent 68 carotid endarterectomies: 36 for a history of transient ischemic attacks (TIAs), 13 for amaurosis fugax, and six for prior strokes; 13 asymptomatic patients had prophylactic carot
