Statistical correlations were sought between the clinical data, the neurological signs and the results of angiography in 224 patients who had had panangiography for cerebral ischaemia. With advancing age there is an increase in the incidence of stenosis, but not of occlusion in the cervical portion of the carotid artery. Patients who suffer from peripheral or coronary occlusive artery disease have a higher incidence of stenosis and occlusion of the cervical carotid artery, but most are clinically silent. A 55-year-old male received a transvenous ventricular pacemaker for sick sinus syndrome. After pacemaker insertion, he developed recurrent episodes of amaurosis fugax. The source of thromboemboli was the pacemaker electrode malpositioned in the left ventricle. Position of the electrode was diagnosed correctly by two-dimensional echocardiography. The malpositioned electrode was successfully removed with cessation of recurrent thromboembolic phenomenon.

This study demonstrates the ability of Doppler ultrasound using both a simple Doppler technique and Duplex scanning to identify lesions at the carotid bifurcation. The procedure is non-invasive and is of little discomfort to the patient. Doppler ultrasound provides a sound basis for clinical evaluation of vascular disease.

The angiographic criteria for the significance of a carotid lesion and the contribution of embolic versus haemodynamic factors in stroke pathogenesis are currently unresolved. Nineteen subjects with retinal strokes of different degrees of severity (amaurosis fugax, occlusion of individual retinal arteries or of the central retinal artery) were examined by continuous-wave Doppler sonography. Evaluation of periorbital Doppler flow and direct examination of the major neck vessels were performed. Twelve subjects presented with homolateral arteriosclerotic lesions at the carotid bifurcation. A 31-year-old Japanese male with unilateral bouts of transient visual loss lasting for variable periods of time has been observed. In spite of the prolonged duration of some of these attacks (the longest attack lasted 7 1/2 hours), the patient sustained a relatively small amount of functional damage to the retina. Although the exact mechanism underlying these bouts of transient visual loss remains unknown, a discussion of the possible causes of such episodes, as well as the reasons for the relative preservation of vision in spite of the length of these attacks, is included.

Forty-four unselected patients with amaurosis fugax (AF) have been followed for 4.6 years (mean 2.6). Thirty per cent had atheromatous lesions, 20% had miscellaneous diagnoses (temporal arteritis 5, transitory ocular hypertension 2, glaucomatous iritis 1, benign intracraniel hypertension 1), 50% consisted of young, mainly women, in whom no cause was found. Prior to AF 2 had hemiplegia, 4 TCI, one optic atrophy and suspicion in 3. None died, one developed hemiparesis and one macular degeneration. An individual conservative attitude to AF seems justified in this material.

Fifty-nine patients had arteriography because of episodes of amaurosis fugax. Only one third of them had atherosclerotic lesions potentially treatable by carotid endarterectomy. Another third had normal arteriograms, some of these had migraine, heart disease or platelet disorders presumed to be the cause of their symptoms. Patients with amaurosis fugax are a heterogeneous group, and their visual symptoms should not be considered a specific indicator of stenosis of the internal carotid artery.

A series of 101 patients with carotid fibromuscular disease have been treated and followed for at least one year. Transient ischaemic attacks and amaurosis fugax were the most common presentation, while 22% of patients had had a completed stroke prior to surgery. Graduated dilatation was the most commonly used surgical method. In 150 operations, there were no deaths and three operative strokes. The operative repair has proven to be very durable and associated with a low incidence of neurologic events.

Three patients with orbital lesions had substantial visual loss while maintaining their eye in an eccentric position of gaze (gaze-evoked blindness). The acuity rapidly returned to baseline levels on return to primary position. Two patients had optic nerve sheath meningiomas and one a displaced portion of orbital bone in contact with the globe. Disc abnormalities and/or choroidal folds were present in all three cases. Gaze-evoked blindness is characterized by the presence of an intraconal mass lesion, but proptosis may be subtle or not present at all. Gaze-evoked blindness should be considered

Fifty-one patients with amaurosis fugax were followed for a mean of 4.9 years from their first attack. Females predominated under the age of 50 years in contrast to men who presented in the older age group. Minor permanent visual sequelae occurred in only three patients, but cerebral vascular symptoms were present at some time in just under half the group. A range of abnormalities was identified on cerebral angiography and some of these correlated with certain clinical findings. Twenty patients were treated by surgery and permanent relief of significant improvement in symptoms occurred in 14.
