ERG in amaurosis fugax, depending on the position of the body. A case presentation.

A woman, aged 57, reported a considerable loss of vision in her right eye within approx. 60s after lying down. She regained her vision in the upright position within approx. 90s. The Ganzfeld ERG, recorded in the upright position, showed a moderately reduced amplitude, but was entirely absent when recorded in the supine position, closely correlated to the subjective fading of the vision. Although all other clinical tests revealed no further hint to the cause of this remarkable amaurosis fugax, a temporary hypoxy of the retina due to circulatory deficiencies is suspected.

[Amaurosis fungax. Studies on hemodynamics and platelet function].

Thirty-eight patients with amaurosis fugax were examined ophthalmologically. In 12 of these cases carotid occlusion was diagnosed by ophthalmodynamography and Doppler ultrasonography of the orbital vessels. Angiographic substantiation was possible in nine of these patients. The pathognomonic importance of amaurosis fugax for occlusive carotid disease is clearly shown by these findings. The plasma concentration of platelet specific proteins was examined in 32 patients and found to be pathologically increased in 13 cases. Spontaneous aggregation of platelets and circulating platelet aggregates w

Retinal fluorescein angiographic evidence for atheromatous microembolism. Demonstration of ophthalmoscopically occult emboli and post-embolic endothelial damage after attacks of amaurosis fugax.

There is evidence that microemboli responsible for amaurosis fugax may be atheromatous but it can elude ophthalmoscopic confirmation, because such emboli quickly fragment and disappear from view in the retinal vessels. This report documents 2 patients in whom fluorescein angiography of the retina provided evidence of microembolization after an attack of amaurosis although the fundus appeared normal. In one patient the angiogram revealed intraluminal material; in the other it showed fluorescein leakage from patent arteriolar bifurcations.

Amaurosis fugax associated with ophthalmic artery stenosis: clinical simulation of carotid artery disease.

A 72 year old woman complained of transient loss of vision in the left eye. She had undergone a left carotid endarterectomy 10 years previously. Reduced ophthalmic artery pressure was found on noninvasive carotid artery testing and cerebral angiography was performed. No lesion was evident in the carotid artery, but significant ophthalmic artery stenosis was identified. We report this case as showing the occurrence of amaurosis fugax in association with ophthalmic artery stenosis.

Neurologic complications of angiography for cerebrovascular disease.

A recent study reported a 12.2% incidence of cerebral complications (5.2% permanent) after cerebral angiography in 147 patients. Because this was markedly different from our experience, we initiated a retrospective study of 301 patients who had undergone cerebral angiography for hemisphere transient ischemia or amaurosis fugax. There was only a 1.3% rate of transient complications (none permanent) in our patients. The benefits of cerebral angiography in patients with suspected cerebrovascular occlusive disease overwhelm the small potential risk of the procedure.

Demonstration of pathological circulating endothelial cells in patients with amaurosis fugax.

The present study was performed in nine patients with amaurosis fugax (AF). Eight of the patients had had at least one attack of AF in the preceding 3 months, and the ninth, a woman aged 35 years, had also had a transient cerebral ischaemic attack (TIA) 5 months previously, with residual hemiparesis. Only one patient, a man aged 51 years, had been given 'anti-platelet' therapy before beginning this study, having had a mycocardial infarct some years before. We suggest that AF is due to an embolism of platelet aggregates in the ophthalmic artery and that it is damage to the vascular endothelium

Retinal emboli in patients with mitral valve prolapse.

Small fibrin-platelet emboli caused by prolapsed mitral valves may cause retinal occlusive disease with a wide range of ophthalmic manifestations, including amaurosis fugax in young people, retinal or choroidal arteriolar occlusion, and retinal neovascularization (atypical Eales' disease). Six patients with retinal occusive disease underwent extensive noninvasive cardiac and systemic tests and were found to have prolapsed mitral valves. Patients with unexplained ocular emboli should be examined by a cardiologist for possible mitral valve prolapse.

Focal neurologic symptoms in hypercalcemia.

An unusual clinical presentation of moderate hypercalcemia as a result of primary hyperparathyroidism is described. The patient complained of fatigue, depression, thirst, polyuria, and focal neurologic symptoms including amaurosis fugax, anomia, right upper-extremity dysesthesias, and a left cerebral transient ischemic attack. No structural central nervous system abnormality could be documented. Signs and symptoms disappeared when serum calcium levels were reduced from 13.2 to 9.8 mg/100 ml. They have not recurred in 30 months of follow-up. The association of focal neurologic disease and hyper

Amaurosis fugax associated with SC hemoglobinopathy and lupus erythematosus.

A 23-year-old woman with SC hemoglobinopathy and systemic lupus erythematosus had recurrent amaurosis fugax in her right eye. Extraordinary retinal vascular events were observed and photographed during the amaurotic attacks. Prednisone therapy halted the attacks. We believe a focal lupus vasculopathy involving the central retinal artery caused the amaurosis fugax.

Choroidal occlusive disease in sickle cell hemoglobinopathies.

Two distinct episodes of posterior ciliary artery occlusion were studied in a 32-year-old man with hemoglobin SS disease and multiple episodes of amaurosis fugax. Although posterior ciliary artery occlusions have been observed following photocoagulation of sickle cell retinopathy, their spontaneous evolution in patients with sickling hemoglobinopathies has received little attention. The manifestations of posterior ciliar artery occlusion seen in this case and in other clinical and experimental situations are reviewed. Histopathologic examination of three additional eyes of patients with sickle

Transient ischemic attacks due to increased platelet aggregation and adhesiveness. Ultrastructural and functional correlation.

The authors report 22 cases of transient ischemic attacks (TIA's) manifested by amaurosis fugax or hemiparesis or paresthesia of less than 24 hours' duration. None of the patients demonstrated 1) evidence of atherosclerotic cerebral vascular disease on angiography, 2) evidence of intracranial lesion on brain scan, 3) cardiac source of emboli, 4) arteritis or collagen disease, or 5) history of migraine. The only abnormalities found to explain the TIA's were abnormally increased platelet adhesiveness and/or aggregation. All of these patients were followed from 1 to 5 years, and had repeated coag

Osteoma: an unusual cause of amaurosis fugax.

A 16-year-old white boy presented with a history of amaurosis fugax associated with an afferent pupillary defect, proptosis, discomfort of the left eye. Tomograms of the skull and of the orbit revealed a lobulated 4-cm ivory osteoma arising from the left ethmoid sinus and extending into the maxillary sinus and left orbit. After surgical extirpation, visual acuity returned to normal and the papilledema resolved after 4 months. This patient is 1 of 37 patients with osteoma of the paranasal sinuses or of the orbit (or of both) who underwent surgical treatment at the Mayo Clinic between the years

The morbidity of carotid endarterectomy.

The results of 72 carotid endarterectomy operations carried out by three consultant surgeons were analysed. Thirty-eight operations (32.8 per cent) were performed in patients with mild or resolving stroke and 33 operations (45.9 per cent) in patients with transient ischaemic attacks and/or amaurosis fugax. Five patients (7 per cent) died in the early postoperative period, 2 as a result of strokes and the others from cardiac causes. Seventy-six per cent of patients had a satisfactory long term result, remaining free from storke, blindness or recurrent transient ischaemic attacks over the follow

Role of antiplatelet agents in cerebrovascular disease.

It is now generally accepted by neurologists that most transient ischaemic attacks, particularly in the carotid artery territory, have a thromboembolic basis. These emboli are, for the most part, fibrin-platelet aggregates. Others which contain atheromatous debris are more likely to produce longer lasting neurological deficits. If one assumes this hypothesis then it is reasonable to employ drugs which interfere with platelet aggregation in order to prevent cerebrovascular symptoms and signs. Acetylsalicylic acid (aspirin) prevents aggregation by inhibiting the 'release reaction' initiated by t

Central retinal artery spasm.

Central retinal artery spasm, a seldom witnessed ocular sign, has been recognized in one eye of a patient complaining of transient monocular blindness. Removal of an ulcerative atheromatous plaque of the carotid artery on the same side as the eye with central retinal artery spasm resulted in complete disappearance of symptoms. No other signs of ocular pathology were noted in addition to the central retinal artery spasm. Central retinal artery spasm occurring in both eyes simultaneously may be the mechanism explaining the visual loss of amaurosis fugax. Physiologic mechanisms associating brief

Recurrent stenosis after carotid endarterectomy.

Thirteen of 1,250 patients required a second operation for recurrent stenosis following carotid endarterectomy performed at the Cleveland Clinic between 1958 and 1978. Two other patients underwent reoperation because of recurrent stenosis following primary operations at other institutions. Thirteen of the 15 patients experienced neurologic symptoms caused by recurent stenosis, while two patients remained asymptomatic. Atherosclerosis was responsible for recurrent stenosis in 12 patients and appeared to be related to hypercholesterolemia. Three of the patients had myointimal fibroplasia. Eleven
