[Amaurosis fugax in obliterations of the carotid artery].

Lesions at the carotid bifurcation frequently cause transient cerebral ischemia and especially visual disturbances by microembolization. An early diagnosis is vital in order to prevent definitive blindness. Transient monocular blindness is an early diagnostic sign of ulcerated atheroma of the extracranial carotid artery. Symptomatology, diagnosis and therapeutic aspects are discussed.

Clinical and arteriographic comparison of amaurosis fugax with hemispheric transient ischemic attacks.

Eighty-seven patients with either amaurosis fugax (40 patients) or hemispheric transient ischemic attacks (47 patients) were studied to determine whether the two symptom groups could be differentiated clinically and arteriographically. Clinical data assessed were age of patient, incidence of cardiac disease, and presence of claudication, hypertension, diabetes, and carotid bruits. Contrary to a prior report, our results indicate no significant difference between the 2 patient groups based on clinical and arteriographic findings.

[Amaurosis fugax and obliteration of the carotid artery (author's transl)].

Obliterative processes of the carotid bifurcation can be a starting point for cerebral and especially retinal microemboli. Early diagnosis is essential in protecting such patients from a definitive insult, or a reduction of sight which can go as far as permanent blindness, whereas the amaurosis fugax being the most common ocular symptom of the internal carotid insufficiency. Due to the fact ca. 65%--75% of the patients with ocular symptoms of a carotid insufficiency show forms of amaurosis fugax or photopical sensations, this symptom complex, especially in combination with temporary contralate

Amaurosis fugax.

We reviewed 112 patients with amaurosis fugax to refine our indications for carotid angiography. Eighty patients had a carotid endarterectomy. Thiry-four patients with ipsilateral hemispheric transient ischemic attacks plus amaurosis fugax all had an operable carotid lesion. The presence or absence of a carotid bruit, a normal or abnormal ophthalmologic examination, or a normal or abnormal cerebrovascular Doppler examination did not predict which patients would or would not have an operable carotid lesion. All patients with amaurosis fugax being evaluated for an operable carotid stenosis requi

Relationship of transient ischemic attacks and angiographically demonstrable lesions of carotid artery.

Eighty-eight percent of arteries in patients with amaurosis fugax or hemispheric transient ischemic attacks had angiographically demonstrable lesions at the carotid bifurcation. Eighty-one percent had stenoses or occlusions at the carotid bifurcation; 7 percent had ulcerative lesions without stenoses at this site. Forty-nine percent of arteries in these patients demonstrated ulcerative lesions with or without stenosis at the carotid bifurcation. There was no significant difference in the incidence or types of ulcerations between those patients with amaurosis fugax and those with hemispheric tr

Evidence of silent cerebral embolism in patients with amaurosis fugax.

In 34 patients who presented with attacks of amaurosis fugax with no evidence of cerebral involvement, EEG, angiographic, and regional cerebral blood flow studies were carried out. Six of 23 EEGs, one of 21 angiograms, and five of six regional blood flow studies were thought to provide evidence of silent cerebral embolism. The implications for the management of patients who present with isolated amaurosis fugax are discussed.

Amaurosis fugax and carotid artery disease: indications for angiography.

A prospective study of 80 patients presenting with amaurosis fugax was performed in an attempt to relate clinical features to angiographic findings in the internal carotid artery. Carotid bruit, transient cerebral ischaemic attacks, hypertension, and claudication were associated with a high prevalence of angiographic abnormality. Every patient who showed all these features had an operable lesion, as did 88% of those who had three features. In patients over 50 years of age carotid stenosis and atheromatous ulceration were occasionally found in the absence of bruit and transient cerebral ischaem

Amaurosis fugax from disease of the external carotid artery.

Three patients with proved internal carotid artery occlusion and diseased external carotid artery or common carotid artery had amaurosis fugax. Evidence for retinal embolization through external carotid collaterals is retinal emboli and infarction after carotid artery manipulation, amaurosis during external carotid angiography, and relief of amaurosis by endarterectomy of this vessel. The abundance of external carotid collaterals, their size, and natural incidence are discussed. Reports clinically support embolization to the central retinal artery and ophthalmic artery through these collateral

Surgical management of the totally occluded carotid artery.

Surgical revascularization of the completely occluded carotid artery was evaluated in 19 symptomatic patients who had recurrent episodes of amaurosis fugax or transient hemiparesis. Nine had complete internal carotid occlusion on arteriography and 10 had severe stenosis which had progressed to total occlusion by the time of surgical exploration. At operation irreparable fibrotic changes were observed in four patients, but in 15 a pattern of proximal focal obstruction with relatively healthy distal internal carotid vessels was observed. Thromboendarterectomy produced good back-bleeding in most

Amaurosis fugax caused by a prolapsed mitral valve leaflet in the midsystolic click, late systolic murmur syndrome.

A 28-year-old man had recurrent episodes of amaurosis fugax of eight years' duration and had a congenital, late systolic mitral insufficiency murmur with the characteristics of the midsystolic click, late systolic murmur syndrome. The ophthalmologist must consider the clinical entity of the midsystolic click, late systolic murmur syndrome in the differential diagnosis of amaurosis fugax, especially when such attacks occur in young patients.

The reliability of clinical predictors of extracranial artery disease.

The records of 628 patients admitted to the Joint Study of Extracranial Arterial Occlusion with transient symptoms of carotid system ischemic disease were examined to determine the accuracy of predicting disease of the extracranial internal carotid artery on the basis of clinical information alone. A patient with a history of episodes of amaurosis fugax is more likely to have a lesion of the internal carotid artery on the same side than if he were having only transient cerebral ischemic attacks. In patients with transient symptoms and a carotid bruit on the appropriate side, the incidence of a

Bypass surgery for vascular disease of the carotid system.

A series of 58 operations on 56 patients, in whom a branch of the superficial temporal artery was anastomosed to a branch of the middle cerebral artery (STA-MCA bypass or Yasargil procedure), is reviewed. These operations were performed chiefly for occlussions or for inaccessible stenotic lesions of the internal carotid or middle cerebral arteries. Patency in eight patients operated on from April 1971 through November 1973 was low (25%). Patency in patients operated on since July 1974 has been high (95%). There have been no deaths and no major ischemic strokes attributable to the surgery. The

Indications for angiography and surgery in carotid artery disease.

The results of angiographic investigation of 211 patients suffering from transient cerebral ischaemic attacks in the carotid territory have been reviewed. The greatest proportion of patients with carotid stenosis and who were referred for endarterectomy were those with a neck bruit ipsilateral to the affected hemisphere. Ocular involvement (amaurosis fugax) was associated with a still higher prevalence of angiographic abnormality, and carotid occlusion was commonest in this group. The implications for the management of patients with transient ischaemic attacks are discussed.

Amaurosis fugax: a clinical comparison.

Amaurosis fugax (transient monocular blindness) is a symptom of retinal ischemia just as contralateral hemiparesis and sensory loss are symptoms of cerebral ischemia. These symptoms are produced by atherosclerotic stenosis of the carotid vessels at the ipsilateral carotid bifurcation and emboli from these areas causing focal, repetitive, retinal ischemia. A study of 31 endarterectomy patients was undertaken to see if eight patients with amaurosis fugax (25%) could be differentiated from 22 patients with transient cerebral ischemia. The patients with amaurosis fugax were found to be younger. Th

Granulomatous arteritis.

Granulomatous, or temporal, arteritis is a disease of older people and should be considered when any patient over 55 years old has complaints consistent with a multi-system disease and/or visual loss, especially if combined with a history of diplopia, amaurosis fugax, loss of vision, or with the findings of ischemic optic neuritis or central retinal artery occlusion. Treatment should be instituted immediately with high doses of corticosteroids. The sine qua non for the diagnosis is a positive temporal artery biopsy when ocular involvement is a feature of the disease. Treatment includes adminis
