Transient Ischemic Attack Due to Delamination of a Steerable Guiding Sheath: A Potential Complication During Atrial Fibrillation Ablation.

We report a case of transient ischemic attack due to a thromboembolic event secondary to delamination of the steerable guiding sheath, CARTO VIZIGO™ (J&J MedTech, New Brunswick, NJ, USA), indicating a device deficiency in structural integrity. This case illustrates an unforeseen device-related complication, emphasizing the importance of heightened vigilance for this potential complication during catheter ablations incorporating this sheath.

Ultra-high-resolution photon-counting CT argues against a suspected bilateral carotid web: a case report.

PURPOSE: Carotid web is diagnosed almost entirely from a shelf-like filling defect at CT angiography (CTA), yet the posterolateral carotid bulb at which webs arise is also a site of flow separation in which blood of differing contrast density mixes. We describe a bilateral web-like appearance at this site and determine whether it arose from reconstruction or acquisition. METHODS: A 75-year-old woman with a right-hemispheric transient ischemic attack had three carotid CTA examinations: a dual-energy energy-integrating-detector CT (EID-CT) angiogram five months before presentation, a stroke-pro

Comparison of angioplasty, stents and conventional medical therapy for symptomatic intracranial atherosclerosis: a network meta-analysis.

BACKGROUND: Medical treatment and endovascular therapy are widely employed in the management of symptomatic intracranial atherosclerotic stenosis, however, the comparative effectiveness of these treatment modalities remains uncertain. METHODS: We searched major Chinese and English databases to identify randomized controlled trials (RCTs) comparing four different endovascular modalities against other comparable non-endovascular therapies for patients with symptomatic intracranial atherosclerotic stenosis (sICAS). Eligible participants were individuals who had experienced a transient ischemic a

Joint association of growth hormone and cerebral small vessel disease with antihypertensive treatment burden after stroke: an exploratory prospective study.

Growth hormone (GH) and cerebral small vessel disease (SVD) may influence blood pressure regulation. This study investigated the individual and joint associations of GH and SVD severity with antihypertensive management following stroke. We prospectively enrolled patients admitted for acute stroke between October 2020 and December 2022. Patients with pre-existing or newly diagnosed hypertension during the initial hospitalization were included. We examined whether baseline GH levels and severe SVD burden (total SVD score ≥2) during the index hospitalization were associated with high antihyperten

High Yield of Pharmacogenetic Testing for Clopidogrel Resistance in a Canadian Outpatient Stroke Population.

BACKGROUND: Clopidogrel is commonly prescribed for secondary stroke prevention, but routine pharmacogenetic testing is not typically performed. As the CYP2C19 enzyme activates clopidogrel into its effective form, clopidogrel-treated patients who carry CYP2C19 loss-of-function alleles may face an increased risk of recurrent stroke. We examined the prevalence of CYP2C19 loss-of-function variants and their potential influence on patient management within a Canadian stroke clinic population. METHODS: We randomly selected and invited outpatients attending a regional stroke prevention clinic in Tor

Bleeding With Asundexian Versus Placebo in Patients With Noncardioembolic Ischemic Stroke or High-Risk TIA: OCEANIC-STROKE Randomized Trial.

BACKGROUND: OCEANIC-STROKE (XXX) showed that asundexian added to antiplatelet therapy reduced ischemic stroke without significantly increasing bleeding in patients with noncardioembolic ischemic stroke or high-risk transient ischemic attack. We characterized bleeding categories, sites, and key subgroups. METHODS: In the safety population, 12 254 participants received at least 1 dose of asundexian (n=6124) or placebo (n=6130). Bleeding was categorized by International Society on Thrombosis and Haemostasis criteria as major, clinically relevant nonmajor (CRNM), or minor. While-on-treatment anal

Pharmacological interventions for reducing the risk of cognitive decline and dementia after stroke/transient ischemic attack: A systematic review.

BACKGROUND: Stroke is a leading cause of death and disability worldwide. Advances in stroke care have improved survival and recovery, but it remains unclear whether pharmacological treatments can prevent long-term consequences including cognitive decline and dementia. This systematic review examined whether pharmacological interventions reduce the risk of cognitive decline, mild cognitive impairment (MCI), or dementia in adults with a history of stroke or symptomatic transient ischemic attack (TIA). METHODS: We searched Medline, EMBASE, PsychINFO, and Cochrane Central from inception to Januar

A prospective study evaluating clopidogrel resistance among patients with ischemic cerebrovascular events across different ethnic groups in Israel.

INTRODUCTION: Clopidogrel is commonly used for secondary prevention after ischemic cerebrovascular events, but substantial interindividual variability in pharmacodynamics response has been reported. Individuals demonstrating resistance to clopidogrel face a higher likelihood of recurrent ischemic events. This study evaluated clopidogrel responsiveness among Jewish and Arab patients treated within the same regional healthcare setting. METHODS: This prospective cohort included 102 patients with ischemic cerebrovascular events (acute ischemic stroke or transient ischemic attack) receiving clopid

One-Year Systolic Blood Pressure and Subsequent Cardiovascular Outcomes After Acute Coronary Syndrome: Insights from a Swiss Prospective Cohort.

AIMS: Current guidelines recommend targeting systolic blood pressure (SBP) below 130 mmHg in patients at high cardiovascular risk, including after acute coronary syndrome (ACS), although evidence specific to the post-ACS setting remains limited. We evaluated the prognostic value of SBP measured 1 year after ACS on subsequent cardiovascular outcomes. METHODS: The ELIPS cohort prospectively enrolled ACS patients across four Swiss tertiary-care centers (2009-2017). Patients alive at 1 year with available SBP measurements were included. The association between SBP at 1 year and 3-point major adve

Oral Anticoagulants in Patients With Atrial Fibrillation and Advanced or End-Stage Renal Disease: A Systematic Review and Meta-Analysis.

BACKGROUND: Patients with atrial fibrillation (AF) and advanced chronic kidney disease (CKD) face a markedly elevated risk of both thromboembolic and bleeding events. While oral anticoagulation is the cornerstone of stroke prevention in AF, the optimal use of this treatment in patients with stage 4-5 CKD and end-stage renal disease (ESRD) undergoing dialysis remains uncertain, as these populations were largely excluded from the landmark trials that established the efficacy of direct oral anticoagulants (DOACs) in the general AF population. This study aimed to systematically evaluate and compar

Pathway-Guided and Patient-Centered Therapeutic Strategies in Atherosclerotic Cardiovascular Disease.

Atherosclerotic cardiovascular disease (ASCVD), encompassing coronary artery disease (CAD), cerebrovascular disease (ischemic stroke, IS; and transient ischemic attack, TIA), and peripheral artery disease (PAD) of atherosclerotic origin, remains a leading cause of morbidity and mortality worldwide despite major advances in prevention and treatment. Although atherosclerosis arises from the convergence of endothelial injury, lipid accumulation, inflammation, and thrombosis, the relative contribution of these pathogenic processes varies markedly among individuals, due to the effects of genetics,
