Opposing blood-contacting surfaces of the human cavernous internal carotid artery: a segmental and topographic immunofluorescence study.

PURPOSE: The cavernous segment of the internal carotid artery (ICA) courses within the cavernous sinus (CS). In this segment, the luminal surface of the intima is in contact with arterial blood, whereas the endothelial covering over its CS-facing outer surface is in contact with cranial venous blood within the CS. Thus, the opposite surfaces of the same arterial wall are associated with different circulatory microenvironments. This study aimed to compare the immunoreactivity distributions of pH sensitive G protein coupled receptor 4 (GPR4) and mechanosensitive PIEZO1 across ICA segments and su

Maternal Plasma Afamin Levels in Pregnancies Complicated by Fetal Growth Restriction and Their Association With Doppler Markers of Placental Dysfunction.

AIM: To compare maternal plasma afamin between pregnancies with fetal growth restriction (FGR) defined by fetal biometry and healthy controls, to test whether the cross-sectional association between afamin and gestational age differs in FGR, and to examine associations with Doppler indices. METHODS: In this prospective study, 84 women assessed at 28-40 weeks (40 FGR, 44 controls) underwent same-day maternal blood sampling and umbilical-, middle-cerebral-, and uterine-artery Doppler. FGR was defined by an estimated fetal weight or abdominal circumference below the 5th percentile. Plasma afamin

Functional connectivity recovers canine-human network homology along the evolutionary cortical hierarchy.

Mapping functional brain organization between species is a prerequisite for translating connectivity biomarkers from animal models to patients, but connectivity-based correspondence has been demonstrated almost exclusively in primates. We tested whether canine-human network homology can be recovered from functional connectivity alone, using only five network-level anatomical priors, and benchmarked a spectral manifold alignment framework (Laplacian-eigenmap embedding with Procrustes alignment on network centroids) against a simple connectivity-signature matching baseline. Data comprised restin

The clinical application of a low-profile stent for symptomatic intracranial atherosclerotic stenosis in the middle cerebral artery: a retrospective study of the Atlas stent.

BACKGROUND: Intracranial atherosclerotic stenosis (ICAS) is a major cause of ischemic stroke, conferring high recurrence risk especially among East Asian populations. Landmark trials revealed limitations of Wingspan stenting, including restenosis concerns. The low-profile, flexible Neuroform Atlas stent, initially designed for intracranial aneurysms, may be suitable for tortuous intracranial vessels, yet its real-world evidence for middle cerebral artery (MCA)-segment symptomatic ICAS (sICAS) remains insufficient. This study aimed to evaluate the safety and efficacy of the Neuroform Atlas sten

Understanding Anti-Diuretic Hormone Dynamics in Off pump Coronary Artery Bypass Grafting (OPCABG) Patients: A Case Series.

OBJECTIVES: The conundrum of Polyuria witnessed post-cardiac surgery has been attributed to altered antidiuretic hormone (ADH) release secondary to both osmotic and non-osmotic stimulus. This transient DI-like state is more often seen in on-bypass cardiac surgeries but a few off-bypass occurrences have also been reported. A lot of conflicting evidence has accumulated trying to explain this altered water balance but a broader conceptual framework is lacking, which we have attempted to provide, with an objective of designing a future prospective study. DESIGN SETTING PARTICIPANTS INTERVENTIONS:

Microsurgical Revascularization for Thrombosed Middle Cerebral Artery Dissecting Aneurysms: Classification Scheme and Surgical Strategies.

OBJECTIVE: Thrombosed dissecting aneurysms represent highly challenging lesions in neurosurgery. Among these, thrombosed middle cerebral artery (MCA) dissecting aneurysms are clinically rare and exceptionally difficult to manage. Their defining feature is the extensive organized thrombus within the aneurysmal lumen and the subsequent progressive mass effect, which presents a significant dilemma for traditional endovascular treatment and microsurgical clipping to balance safety and efficacy. To address this, the authors summarize the clinical experience of a single senior surgeon at our center

Acute partial sleep deprivation attenuates cardiac output during moderate-intensity cycling in young adults.

Acute partial sleep deprivation (PSD) can attenuate systolic blood pressure (BP) during cycling exercise; however, the mechanisms responsible are unknown. This study aimed to test if acute PSD attenuates cardiac output (Q̇) during cycling. Twenty-five adults (22±3 yrs; 13 females; V̇O2peak: 40.1±7.7 ml.kg-1.min-1) completed a randomized, crossover trial where they slept normally or underwent acute PSD (early awakening; 40% normal sleep duration). BP, Q̇ (impedance cardiography), and middle and posterior cerebral artery blood velocity (transcranial Doppler ultrasound) were assessed at rest, thr

Multiterritory Bihemispheric Ischemic Stroke in the Setting of Unilateral Internal Carotid Artery Disease.

Accurate determination of stroke mechanism is essential for secondary stroke prevention. Bilateral, multiterritory ischemic infarcts are commonly attributed to a central embolic source, prompting evaluation for cardioembolism, paradoxical embolism, or hypercoagulable states. However, this diagnostic approach assumes conventional intracranial vascular anatomy despite the high prevalence of Circle of Willis variants. We report the case of a 66-year-old man who presented with acute bilateral cortical and subcortical ischemic infarcts involving the right middle cerebral artery (MCA), right anterio

Retinal Microvascular Abnormalities, White Matter Injury, and Cognitive Impairment in Carotid Stenosis or Occlusion.

BACKGROUND: Severe carotid artery stenosis or occlusion (CASO) is associated with white matter injury and cognitive impairment, but markers of accompanying microvascular dysfunction remain incompletely characterized. We investigated the relationships among retinal microvascular abnormalities on optical coherence tomography angiography, white matter hyperintensities (WMH), and cognitive impairment in severe unilateral CASO. METHODS: In this multicenter cross-sectional study, 860 participants were included: 566 patients with severe unilateral CASO without acute cerebral infarction and 294 contr

Highly potent, brain-penetrant EP2 receptor antagonists for the treatment of permanent focal cerebral ischaemia in male and female mice.

BACKGROUND AND PURPOSE: Cerebral ischaemia is a leading cause of death in older adults and imposes a substantial global economic burden. Recent studies indicate that inhibiting the Gαs-coupled EP2 receptor reduces ischaemic brain injury by modulating neuroinflammation. However, these findings derive from the transient middle cerebral artery occlusion (tMCAO) model, which represents only a small subset of large-vessel occlusion (LVO) cases with successful recanalisation. EXPERIMENTAL APPROACH: We characterised the pharmacokinetic parameters of selective EP2 antagonists CJ-18, CJ-19 and CJ-21 t

Constraint-induced movement therapy after ischemic stroke: From cortical reorganization to molecular plasticity.

Stroke recovery is a dynamic, multilevel process involving structural, functional, and molecular reorganization of surviving neural circuits. Constraint-induced movement therapy (CIMT) has emerged as one of the most effective rehabilitation strategies for improving upper-limb motor function after stroke by overcoming learned nonuse and promoting activity-dependent plasticity. This review integrates clinical and experimental evidence to provide a comprehensive mechanistic framework of CIMT-induced recovery. At the systems level, motor recovery is governed by peri-infarct plasticity, diaschisis

Clinical Characteristics Associated with Fetomaternal Hemorrhage: A Retrospective Multicenter Descriptive Study (2009-2026).

Fetomaternal hemorrhage refers to the transfer of fetal erythrocytes into the maternal circulation. Although clinically significant fetomaternal hemorrhage is rare, it can result in severe fetal anemia with substantial morbidity and mortality. Early recognition remains challenging due to a nonspecific clinical presentation. This study aimed to identify maternal and obstetric characteristics associated with fetomaternal hemorrhage that may serve as early clinical warning signs. In this retrospective multicenter descriptive study, singleton pregnancies with a positive Kleihauer-Betke test from f

β-Adducin Restrains PLA2G4A-Dependent Lysosomal Membrane Permeabilization and Neuronal Death in Ischemic Stroke.

Failure to preserve vulnerable neurons in the ischemic penumbra remains a barrier in ischemic stroke. Lysosomal membrane permeabilization (LMP) is increasingly recognized as a critical event in ischemic neuronal death, yet the endogenous mechanisms that preserve lysosomal integrity remain poorly defined. Here, we identify β-adducin (Add2) as an endogenous protector of lysosomal homeostasis in ischemic neurons. We found that β-adducin was markedly reduced in neurons after cerebral ischemia in both middle cerebral artery occlusion and oxygen-glucose deprivation/reoxygenation models. Neuron-speci

Gut microbiota-derived indole-3-propionic acid preserves dorsal hippocampal catecholamines to prevent post-stroke cognitive impairment.

UNLABELLED: Gut dysbiosis has been increasingly implicated in post-stroke cognitive impairment (PSCI); however, the causal contribution and therapeutic potential of gut microbiota-derived metabolites remain unclear. This study aimed to identify key microbiota-derived metabolites involved in PSCI and elucidate their underlying mechanisms. We found that both PSCI patients and middle cerebral artery occlusion (MCAO) mice exhibited distinct gut microbial alterations, characterized by a marked reduction in tryptophan-metabolizing bacteria and indole-3-propionic acid (IPA), a gut microbiota-derived

Cardiac Metastasis From Lung Cancer: A Hidden Cardioembolic Source of Acute Ischemic Stroke.

Acute ischemic stroke requires prompt identification of the underlying mechanism to guide treatment and secondary prevention. Although common causes account for most cases, occasionally the diagnostic work-up reveals an unexpected cardioembolic source. We report the case of a woman in her 50s with stage IVB non-small cell lung carcinoma who was admitted with presyncope, acute disorientation, and dysarthria, followed by anomia and right-sided neurological deficits. Brain magnetic resonance imaging revealed multiple acute ischemic infarcts in the left middle cerebral artery territory. Cardiac im

The clinical value of combined assessment of substantia nigra morphology and intracranial hemodynamics via transcranial ultrasound in the diagnosis and differentiation of Parkinson's disease.

BACKGROUND: Parkinson's disease (PD) is a common neurodegenerative disease, and its clinical differentiation from Parkinson-plus syndromes (PPS) remains challenging due to the lack of highly specific biomarkers. Although dopamine transporter single-photon emission computed tomography (DAT-SPECT) provides objective evidence of dopaminergic denervation, its limited value in distinguishing PD from PPS and restricted availability underscore the need for accessible and reliable diagnostic tools. This study aimed to evaluate a novel, non-invasive, and convenient auxiliary diagnostic approach for the
