A numerical flow experiment for assessing the risk of rupture in anterior communicating artery aneurysms in relation to aneurysm projection.

Anterior communicating artery (AcoA) aneurysms may project in different directions - anterior, posterior, superior, or inferior - each of which can substantially alter intra-aneurysmal flow patterns and potentially influence rupture risk. The present study investigates the impact of aneurysm projection direction on hemodynamic characteristics associated with rupture and thrombosis. Transient computational fluid dynamics (CFD) simulations were performed using idealized, parametric models of AcoA aneurysms with four defined projection types: anterior (toward the frontal lobe), posterior (toward

A rare combination of anterior cerebral arterial ring with accessory middle and duplicated posterior cerebral arteries.

PURPOSES: To report a unique configuration of the cerebral arterial circle identified incidentally during magnetic resonance angiography (MRA). METHODS: An 87-year-old female patient was evaluated using MRA on a 3-Tesla scanner with the time-of-flight (TOF) technique. RESULTS: On the anterior circulation, an arterial ring was identified at the junction of the left anterior cerebral and anterior communicating arteries, suggesting a double origin of the left A2 segment. An accessory middle cerebral artery originated from the proximal left A1 segment. Additionally, the right A1 segment was aplast

Dolichoectasia of Anterior and Posterior Circulation With Thrombosed Saccular Aneurysm of Vertebral Artery Presenting as Obstructive Hydrocephalus: A Case Report.

Vertebrobasilar dolichoectasia (VBD) is characterized by vascular elongation, widening, and tortuosity of cerebral vasculature. There are only a few reported cases of VBD as a cause of hydrocephalus. Here, we report a case in a 55-year-old male who presented with sudden vomiting for a day and worsening of headache for two weeks. He was diagnosed with CT imaging with angiographic findings: thrombosed saccular aneurysm of the vertebral artery along with dolichoectasia of the anterior and posterior circulation.

Skull base revascularization corridors to the high cervical, petrous, and cavernous internal carotid artery: microsurgical anatomy for optimizing surgical exposure.

OBJECTIVE: Revascularization enables the total removal of skull base tumors encasing the internal carotid artery (ICA) and can be used to treat complex aneurysms requiring parent artery occlusion when clipping is unfeasible. Authors of this study aimed to analyze the microsurgical anatomy involved in revascularization procedures involving the high cervical, petrous, and cavernous segments of the ICA in relation to the anterior circulation, with special emphasis on various skull base approaches to optimize the surgical exposure. They also aimed to assess the various surgical corridors used in r

Characteristics of brain structural damage related to cerebral small vessel disease in general population with intracranial artery stenosis.

BACKGROUND AND AIMS: Covert MRI markers of cerebral small vessel disease (CSVD) can coexist with large artery atherosclerosis. We aimed to explore whether the spatial distributions of these markers were diverse in people with or without intracranial artery stenosis (ICAS). METHODS: This cross-sectional analysis included 1206 stroke-free participants (aged 55.69±9.27, 62.94% female) with brain MRI and MR angiography from community-based Shunyi cohort. We analysed the relationships between ICAS and CSVD markers. We also compared the probability maps of lacunes, cerebral microbleeds (CMB), white

High-Flow Bypass with Vertebral Artery (V3) to Middle Cerebral Artery (M2) Bypass Using Radial Artery Graft for Common Carotid Artery Occlusion.

Common carotid artery (CCA) occlusion is a rare condition that can result in diverse neurological symptoms due to insufficient cerebral blood flow. We present a case of CCA occlusion treated with high-flow vertebral artery (VA) to middle cerebral artery (MCA) bypass using a radial artery (RA) graft. A 68-year-old man experienced recurrent ischemic events in the months preceding surgery. Preoperative diffusion-weighted imaging showed left anterior border zone infarction, and single-photon emission computed tomography revealed severe hemispheric hypoperfusion. Giant serpentine aneurysm: Imaging features including potential diagnostic utility of arterial spin labeling.

We present a case of an otherwise healthy mid 30s male with right sided vision loss. CT and MR imaging demonstrated a giant serpentine aneurysm of the right anterior cerebral artery A1-A2 segments. The patient was successfully treated with craniotomy for intracranial A3-A3 bypass followed by endovascular coil embolization of the inflow vessel. We highlight the imaging findings, reviewing both the pertinent findings and potential diagnostic utility of arterial spin labeling, an emerging noninvasive, MRI technique to assess brain perfusion.

Serial Intracranial Flow Rate Measurements Using Quantitative Magnetic Resonance Angiography Following Large-Vessel Occlusion Stroke.

Background/Objectives: Haemodynamic changes following ischaemic large-vessel occlusion (LVO) stroke might affect clinical outcome, including after endovascular recanalization. Using non-invasive quantitative MRA (qMRA), we report for the first time serial intracranial flow rate measurements following LVO and investigate flow rate changes, collateral pathway development, and their possible clinical significance. Methods: We report data from the prospective IMPreST study (Interplay of Microcirculation and Plasticity after Ischemic Stroke, registered at clinicaltrials.gov, no. NCT04035746). Patie

Association of A1 Segment Morphology with the Rupture Risk and Morphology of Anterior Communicating Artery Aneurysms: A Retrospective, Single-Center Study.

Background/Objectives: A1 segment asymmetry, including hypoplasia and aplasia, is a well-recognized anatomical variation associated with altered hemodynamic stress and anterior communicating artery (ACoA) aneurysm formation. However, its influence on subsequent aneurysm rupture risk remains controversial. This study aimed to evaluate the relationship between A1 segment morphology and aneurysm rupture risk, as well as its association with aneurysm size and morphological complexity. Methods: A retrospective single-institution analysis was conducted on 211 patients treated for ACoA aneurysms betw

Clinical Significance of Microembolic Signal Detection in Acute Ischemic Stroke: Insights Into Ongoing Cerebral Embolization and Occult Etiologies.

BACKGROUND AND PURPOSE: Transcranial Doppler (TCD) detects microembolic signals (MES), reflecting ongoing cerebral embolization. MES have been studied across stroke subtypes and may clarify etiology, monitor treatment, and predict recurrence. We aimed to determine the prevalence of MES in acute ischemic stroke (AIS), explore clinical and laboratory associations, and assess their role in etiological workup. METHODS: We conducted a prospective single-center study over 3 years including anterior circulation AIS patients within 24 h of onset. Stroke subtype was classified by TOAST criteria. Bilat

Clinical evidence of the absence of brain perfusion during thoraco-abdominal normothermic regional perfusion in donation after circulatory determination of death.

The use of thoraco-abdominal normothermic regional perfusion (TA-NRP) has been associated with improved graft utilization and superior recipient outcomes compared with rapid recovery in donation after circulatory determination of death (DCDD). A major barrier to its broader adoption is the concern for inadvertent brain reperfusion, which would invalidate the determination of death. We aimed to confirm that, during TA-NRP in DCDD, clamping and sectioning of the supra-aortic vessels, with venting the cephalad ends to atmospheric pressure, effectively prevents brain perfusion. A prospective study

A rare coexistence of hypoplasia of the middle cerebral artery with aortic arch origin of the vertebral artery: a case report and analysis of hemodynamic vulnerability.

BACKGROUND: Congenital cerebrovascular anomalies, such as middle cerebral artery hypoplasia and vertebral artery with an aortic arch origin, are well-documented in isolation but exceedingly rare in combination. Their coexistence may critically disrupt cerebral hemodynamics, yet clinical implications remain poorly characterized, highlighting the novelty of this case. CASE PRESENTATION: A 68-year-old male presented with chronic refractory dizziness that worsened with positional changes and showed no improvement after standard management for benign paroxysmal positional vertigo. The detour technique: transcirculation stent coiling for complex cerebellar artery aneurysms. Illustrative cases.

BACKGROUND: Cerebellar artery aneurysms are exceedingly rare, accounting for only 0.1%-0.3% of all intracranial aneurysms. They often present with wide necks and are usually treated with endovascular means through the vertebral arteries. Transcirculation approaches can offer ergonomic advantages when conventional anterograde endovascular approaches are technically challenging, particularly with incorporation of cerebellar artery origin from the aneurysm and less optimal stent placement angles. OBSERVATIONS: The authors present the transcirculation cases of 2 patients, a 41-year-old female wit

Mechanical thrombectomy for tandem internal carotid artery-middle cerebral artery occlusion in a patient with a persistent hypoglossal artery: illustrative case.

BACKGROUND: Mechanical thrombectomy (MT) is the established treatment for acute ischemic stroke due to large-vessel occlusion. However, rare cerebrovascular anomalies, such as a persistent hypoglossal artery (PHA), present unique challenges that require procedural modifications. OBSERVATIONS: The authors present the case of a woman in her 80s who developed sudden-onset impaired consciousness and left hemiparesis, with a National Institutes of Health Stroke Scale (NIHSS) score of 20. Imaging revealed a tandem occlusion of the right internal carotid artery (ICA) and middle cerebral artery (MCA)

Anatomic Variants of the Anterior Cerebral Artery and Anterior Communicating Artery: A Digital Subtraction Angiographic Study.

Background Several anatomic variants can result from developmental anomalies of the cerebral arteries during the embryonic period. Some of them predispose to the development of aneurysms or cerebrovascular ischemic disease. Failure to identify them may lead to misdiagnosis. This study aimed to document the frequency and type of anatomic variants of the anterior cerebral artery (ACA) and the anterior communicating artery (ACoA). Methodology A retrospective, record-based, cross-sectional study was conducted among 500 subjects (188 females, 312 males) whose four-vessel digital subtraction angiogr

A Study on Branching Pattern of Middle Cerebral Artery and its Variations: A Systematic Review and Meta-analysis.

Middle cerebral artery (MCA) anatomy is essential to prevent catastrophe during intravascular procedures such as thrombolysis and cerebral angiography and neurosurgical practices, example - stent insertion, bypass grafting, etc. Eighty percent of blood supply to the brain is through MCA. The motor and sensory homunculus of brain representing arm and face receive blood supply from MCA. MCA consists of four segments. M1 is between the origin of MCA and the genu of insula (sphenoidal); M2 runs from limen insula to insula's circular sulcu (insular) s; M3 from circular sulcus to Sylvian fissure sur

Early T2-weighted magnetic resonance imaging changes after microsurgical clipping of unruptured aneurysms: Biomarkers of corridor-related tissue impact.

BACKGROUND: Diffusion-weighted imaging (DWI) detects ischemic injury after microsurgical clipping of unruptured intracranial aneurysms (UIAs), yet non-ischemic parenchymal effects remain poorly characterized. This study evaluated early postoperative T2-weighted imaging (T2WI) abnormalities to characterize corridor-related tissue impact and identify anatomical determinants of non-ischemic parenchymal changes. METHODS: This retrospective study analyzed 797 anterior circulation UIA clipping procedures performed by a single surgeon under uniform protocol. Early postoperative magnetic resonance im
