INTRODUCTION: Automated segmentation of intracranial vessels in 3D black-blood T1W-MRI is feasible. This study evaluates its utility for detecting recurrent aneurysms and compares automatically labeled MRI (AL-BB-MRI) with time-of-flight MRA (TOF-MRA) and digital subtraction angiography (DSA). MATERIALS (PATIENTS) AND METHODS: For model development, the basal intracranial arteries were manually labeled in 37 3D black-blood T1W-MRI examinations from 31 patients with previously endovascularly treated aneurysms. An independent, consecutive series of 84 patients with 90 aneurysms was assessed by t

OBJECTIVE: Mechanical thrombectomy (MT) is currently the standard treatment for large vessel occlusion (LVO). However, advancing a microcatheter and microguidewire into non-visualized distal vessels carries a risk of vascular perforation, potentially resulting in serious hemorrhagic complications. This study aimed to evaluate the usefulness of a new imaging protocol using single-phase CTA guided by the time-enhancement curve (TEC) derived from CT perfusion (CTP) in patients with anterior circulation LVO undergoing MT, for improving distal vessel visualization compared with conventional CTA usi

BACKGROUND: Endovascular therapy (EVT), including mechanical thrombectomy and intraarterial thrombolysis, is increasingly applied in patients with acute ischemic stroke caused by distal medium vessel occlusions. However, its efficacy and safety in this specific population remain inconclusive.
METHODS: We conducted a systematic review and meta-analysis of studies comparing EVT and non-EVT for distal medium vessel occlusion patients, following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines (PRISMA). Data were extracted from PubMed, Embase, and Cochrane Library up

Distal anterior cerebral artery (DACA) aneurysms are rare and account for less than 10% of all intracranial aneurysms. "Kissing" bilateral mirror-image DACA aneurysms are even more uncommon. Securing these aneurysms pose considerable challenges in terms of access, periprocedural risks and difficulty in identifying the culprit aneurysm. We share our experience with the surgical clipping of kissing DACA aneurysm in a 48-year-old patient and discuss the concerns involved.

INTRODUCTION: Decompressive craniotomy or craniectomy (DC) is used to treat refractory intracranial hypertension in severe traumatic brain injury and malignant stroke. Heart rate variability (HRV) and blood pressure variability (BPV) may capture autonomic dysfunction and hemodynamic instability in this setting, but their role remains uncertain. This scoping review maps the evidence on HRV, BPV, and related hemodynamic variability in DC patients.
METHODS: We conducted a scoping review in accordance with PRISMA-ScR guidelines. MEDLINE, Embase, Cochrane CENTRAL, CINAHL, and Web of Science were s

Management of the left subclavian artery can become a very challenging part of a frozen elephant trunk procedure. A new patient-individualized custom-made frozen elephant trunk hybrid prosthesis with a stented left subclavian artery branch is now commercially available, potentially decreasing surgical complexity. We present the implantation of this prosthesis in zone 0, with additional left internal mammary artery to left anterior descending bypass.

Cerebral aneurysms are frequently seen, but the concomitant occurrence of multiple aneurysms along the anterior cerebral artery (ACA) leading to an arteriovenous malformation (AVM) is a rarity. Here we present a case of a woman who presented with subarachnoid haemorrhage due to a ruptured aneurysm. Angiographic imaging revealed multiple aneurysms along the ACA. She underwent successful neuroendovascular coiling, and the recovery was satisfactory.

INTRODUCTION: Acute ischaemic strokes (AIS) due to proximal anterior cerebral artery (ACA) occlusions are rare. Their clinical outcomes following medical management alone have been scarcely described.
PATIENTS AND METHODS: We conducted a retrospective, multicentre, international study of consecutive AIS due to isolated proximal ACA occlusion (A1 or A2 segment) admitted within 6 h of symptom onset and treated with best medical management alone (ie, without endovascular therapy), across 23 centres in France and Switzerland. The primary outcome was poor functional outcome, defined as a mRS score

INTRODUCTION AND IMPORTANCE: Transvenous embolization (TVE) for brain arteriovenous malformations (AVMs) typically requires two microcatheters or a dual-lumen mini-balloon catheter in the draining vein. A single Apollo microcatheter can replace these devices, offering a novel TVE alternative for AVMs with the "Caput Medusa" sign. This case report describes the technique, as such reports remain limited.
CASE PRESENTATION: A 34-year-old male patient suffered intracranial hemorrhage and developed right-sided hemiplegia and aphasia. Angiography revealed a left corpus callosum AVM supplied by the

INTRODUCTION: Computed tomography angiography (CTA) is essential for preoperative evaluation of intracranial vessels; however, the visualisation of small perforating arteries remains challenging with conventional systems. This study investigated whether increasing the reconstruction matrix size from 512 to 1024 pixels improves the visualisation of perforating arteries in cerebral CTA using a standard 64-slice computed tomography (CT) scanner.
METHODS: The study comprised phantom and clinical components. Physical image properties were assessed through Task Transfer Function (TTF) and Noise Pow

Cerebellar arteriovenous malformation complicated by an aneurysm is a critical cerebrovascular disease. Rupture of the aneurysm can lead to severe subarachnoid hemorrhage, with a high mortality and disability rate. This case report describes a 66-year-old male patient who presented with an acute onset of symptoms, including slow response, aphasia, and projectile vomiting. Physical examination upon admission indicated neurological deficits. Computed tomography revealed subarachnoid hemorrhage and cerebral edema. Digital subtraction angiography confirmed the diagnosis of a right cerebellar vascu

BACKGROUND AND PURPOSE: Despite successful recanalization, some patients with acute ischemic stroke remain functionally dependent, which is referred to as futile recanalization (FR). While inadequate perfusion is linked to poor outcomes, hemodynamics (especially time to peak) measured by quantitative DSA immediately post-thrombectomy are underexplored. TTP is defined as the time from contrast injection to peak density at the ROI, indicating perfusion status. Therefore, we investigated the association between TTP and FR.
MATERIALS AND METHODS: We retrospectively screened patients with anterior

BACKGROUND/OBJECTIVES: Selection of the optimal recipient artery in superficial temporal artery to middle cerebral artery (STA-MCA) extracranial-intracranial bypass surgery is essential to ensure adequate cerebral perfusion. Various pre- and intraoperative tools for target vessel selection have been proposed. Indocyanine green fluorescence video angiography (ICG-VA) enables real-time visualization of cerebral hemodynamics, facilitating recipient vessel selection and anastomotic evaluation. Here, we review the literature and present the use of qualitative ICG-VA to support intraoperative decisi

This chapter reviews the vascular anatomy of the brainstem and posterior circulation with emphasis on clinical neurology and angiographic interpretation. Using a caudal-to-cranial approach, we describe the vertebral artery at the craniocervical junction and its key branches, including meningeal, medullary, spinal, and posterior inferior cerebellar arteries (PICA), highlighting common variants and clinically important anastamoses. We then outline basilar artery segments, pontine perforator patterns, and vertebrobasilar perforator groups, linking lesion location to characteristic neurologic synd

Coexisting intracranial aneurysms may be encountered during sellar tumor surgery, but no consensus has been reached on standardized treatment strategies. The simultaneous treatment of sellar tumors resection and coexisting intracranial aneurysms in a single-stage surgery has rarely been reported. Here, we report a case series of sellar tumors coexisting with intracranial aneurysms during a single-stage via purely endoscopic surgery in a single center between January 2015 and July 2022. The patients’ clinical information, including age, sex, symptoms, as well as aneurysm location, treatment mod
