BACKGROUND AND PURPOSE: Cerebral perfusion imaging may help detect the presence of, and infer vessel occlusion location in, anterior circulation stroke, but its value in posterior circulation large vessel occlusion (PC-LVO) is less well defined. We evaluated the accuracy and inter-reader reliability of CT and MR perfusion imaging for occlusion classification and posterior circulation vessel localization.
METHODS: We performed a retrospective multicenter diagnostic accuracy study from 11 centers that underwent perfusion imaging for stroke evaluation. The cohort was enriched for posterior circu

PHACES syndrome is characterized by segmental infantile hemangiomas (IHs) accompanied by various extra-cutaneous anomalies, including cerebral artery anomalies, cardiac anomalies, ocular anomalies, sternal deformities, and posterior fossa malformations. This report presents three case series of patients with PHACES syndrome, focusing on challenges encountered in managing stroke risk associated with propranolol therapy due to significant cerebrovascular anomalies. In each case, therapeutic strategies were tailored to individual patients, carefully considering their vascular risk, and the locati

BACKGROUND: The cerebellum has traditionally been regarded as a structure primarily involved in motor coordination. However, accumulating evidence indicates that the cerebellum can exert substantial influence on cognitive and emotional processes. These findings suggest that the cerebellum may play a broader role in higher-order cognitive functions than previously recognized.
CASE PRESENTATION: A right-handed Japanese man in his 80s with no significant prior medical history developed cognitive and behavioral symptoms following an infarction involving the right posterior cerebellar lobe and ver

BACKGROUND: Posterior circulation ischemic stroke (PCIS) accounts for approximately one-quarter of ischemic strokes, but long-term survival and territory-specific recurrence patterns compared with anterior circulation ischemic stroke (ACIS) remain incompletely defined.
METHODS: We analyzed Oracle Health Real-World Data from January 2015 through February 2025, including 118,727 patients hospitalized with acute ischemic stroke across 149 United States (U.S.) health systems. PCIS and ACIS were identified using validated International Classification of Diseases, Tenth Revision, Clinical Modificat

Both bone and intracranial arterial walls are composed of type 1 collagen, and bone mineral density (BMD) may be associated with the presence, size, and multiplicity of intracranial aneurysms. This longitudinal study aimed to investigate whether BMD was associated with aneurysm growth in patients with UIAs < 7 mm. This prospective cohort study consecutively enrolled 425 patients with UIAs < 7 mm between 2016 and 2023. Hounsfield unit (HU) values of the frontal skull on brain computed tomography (CT) were measured as a convenient surrogate for assessing skeletal status. According to the mean HU

BACKGROUND: Tenecteplase has been previously evaluated in large- and medium-sized vessel occlusion subgroups, but its effectiveness in more distal occlusions, particularly those involving the distal middle cerebral artery branches or the anterior cerebral artery and posterior cerebral artery territories, and across varying ischemic core and perfusion profiles, remains uncertain.
METHODS: We performed a secondary analysis of TASTE (Tenecteplase Versus Alteplase for Stroke Thrombolysis Evaluation), a randomized clinical trial comparing tenecteplase and alteplase in patients presenting within 4.

BACKGROUND: Distal posterior inferior cerebellar artery (PICA) aneurysms are rare vascular lesions, comprising less than 0.5% of all intracranial aneurysms. When thrombosed, they can mimic neoplastic or infectious processes, leading to delayed diagnosis and potential mismanagement. The complex anatomy of the PICA and its variable relationships with adjacent neurovascular structures further complicate both the diagnostic process and surgical approach. Here, we present a rare case of a thrombosed distal PICA aneurysm, with emphasis on imaging characteristics, differential diagnosis, and surgical

BACKGROUND: Acute posterior cerebral artery (PCA) occlusion causes disabling deficits, but evidence for endovascular therapy (EVT) is lacking. This gap exists between established efficacy of thrombectomy in anterior and posterior circulations.
OBJECTIVE: To evaluate evidence for PCA thrombectomy via a pre-specified synthesis of the ATTENTION, BAOCHE, and PLATO studies.
METHODS: Three evidence layers were integrated: (1) indirect supportive evidence from two RCTs on basilar artery occlusion (ATTENTION/BAOCHE) for posterior circulation EVT; (2) the targeted observational PLATO study for isolat

Intracranial aneurysms are arterial dilatations most commonly located in the circle of Willis, characterized by a focal outpouching of the arterial wall. Posterior cerebral artery (PCA) aneurysms are very uncommon and, in very rare cases, may present as multiple lesions. They are particularly associated with mass effect due to their proximity to critical midbrain and thalamic structures. Despite their low frequency, they are clinically relevant due to their higher risk of rupture and potential for significant mass effect. We report the case of a 51-year-old postmenopausal female who presented

BACKGROUND: The fogging effect is a well-known phenomenon usually seen two to three weeks post stroke, characterized by a pseudonormalization of the infarcted tissue.
CASE PRESENTATION: A 62-year-old patient underwent emergent thoracic endovascular aortic repair (TEVAR) due to a Stanford Type B aortic dissection. Post-intervention dual antiplatelet therapy and systemic anticoagulation were administered. The following day, the patient developed acute neurological deficits. Cranial CT and perfusion imaging demonstrated an infarction in the left posterior inferior cerebellar artery territory, an

Dupilumab is a human IgG4 monoclonal antibody that inhibits IL-4 and IL-13 signaling through IL-4Rα, thereby reducing Th2-mediated inflammation. Although it has demonstrated significant efficacy in the treatment of atopic dermatitis, its long-term vascular safety profile remains incompletely understood. We report a case of a 27-year-old male who developed sudden headache after physical exertion while receiving Dupilumab therapy for two years. Brain computed tomography revealed subarachnoid hemorrhage, and angiography demonstrated a ruptured dissecting aneurysm in the distal posterior cerebral

PURPOSE: Electrical muscle stimulation (EMS) is increasingly explored as a strategy to potentially enhance cerebrovascular and cognitive health, offering an alternative to voluntary exercise. While previous studies indicate that EMS can increase cerebral blood flow velocity (CBFv), its cognitive effects remain inconsistent. Furthermore, lower-limb EMS is typically delivered as simultaneous bilateral or unilateral stimulation, and the cerebrovascular impact of alternating unilateral stimulation, has not been investigated. This proof-of-concept study examined the effects of two quadriceps EMS pa

Diffuse capillary-malformation-like cutaneous lesions are uncommon during pregnancy, and their clinical implications become more complex when they coexist with focal neurological symptoms or cerebrovascular malformation. This case report describes a 25-years-old primigravida admitted at 36 + 6 weeks of gestation with diffuse congenital capillary-malformation-like skin lesions, a documented history of cerebrovascular malformation diagnosed 10 years earlier, obesity, and hypothyroidism. Physical examination revealed extensive dark red to violaceous patches over the face, trunk, back, buttocks, a

BACKGROUND: Traumatic intracranial pseudoaneurysms (TIPAs) are exceedingly rare, representing <1% of all intracranial aneurysms, and are associated with high morbidity and mortality. They often remain occult on initial imaging, particularly when masked by concomitant subdural hematomas (SDHs).
CASE DESCRIPTION: We report two patients with traumatic pseudoaneurysms presenting as evolving SDHs. To the best of our knowledge, this is one of the few reports describing cortical middle cerebral artery (MCA) and posterior cerebral artery (PCA) pseudoaneurysms manifesting as SDHs in elderly patients.

BACKGROUND: Neuro-Behçet syndrome (NBS) is a heterogeneous inflammatory disorder in which venous involvement is well recognized, while intracranial arterial pathology remains insufficiently characterized. Conventional luminal imaging methods are limited in detecting early or isolated arterial wall abnormalities. Vessel wall imaging (VWI) may provide additional insights into arterial involvement in NBS.
METHODS: Thirty-nine NBS patients (27 men, 12 women; mean age 38.52 ± 9.54 years) underwent intracranial VWI; 16 were imaged during the attack period and 23 during the attack-free period. A tot

PURPOSE: To describe a case of an accessory posterior cerebral artery (PCA)-PCA anastomosis located just distal to the P1-P2 junction.
METHODS: A 73-year-old man underwent cranial magnetic resonance (MR) angiography using a 1.5-Tesla scanner. MR angiography was performed using a standard three-dimensional (3D) time-of-flight technique.
RESULTS: Maximum-intensity-projection (MIP) MR angiography showed no pathological arterial lesions. However, the parieto-occipital branch of the right PCA arose from the internal carotid artery (ICA). On MR angiographic source images, a tiny posterior communic

BACKGROUND: Patients with severe asymptomatic carotid stenosis exhibit inferior baseline cognition. Whether cognition improves after carotid endarterectomy (CEA) remains inconclusive. This study investigated the relationship between regional cerebral perfusion and cognitive function following CEA in patients with asymptomatic carotid stenosis.
METHODS: In this prospective observational cohort study, patients with unilateral severe asymptomatic carotid stenosis (70%-99%) undergoing CEA were recruited. Computed tomographic perfusion, a cognitive battery, and quantitative electroencephalogram we

BACKGROUND: Ischemic stroke is a leading global cause of death and disability. Accurate, rapid infarct territory localization is critical for timely treatment, but ultra-early ischemic changes on non-contrast CT (NCCT) are subtle and highly physician-dependent.
METHODS: This retrospective study enrolled acute ischemic stroke patients with single-territory infarction. Clinical data and NCCT images were collected. Quantitative imaging features (HU values, infarct volume, spatial distribution) were extracted from vascular perfusion territories. A strict leak-proof pipeline was adopted: dataset s

BACKGROUND: Posterior circulation (PC) large vessel occlusions (PC-LVO) include occlusions of vertebral arteries, basilar artery and P1 segment of posterior cerebral artery, although the latter is variably classified as a medium vessel occlusion in some studies and clinical trials. PC-LVOs account for a substantial proportion of PC strokes. PC-LVO remains challenging to diagnose due to nonspecific symptoms and limited sensitivity of CT for posterior fossa ischemic changes. However, PC-LVO is associated with substantial disability and mortality, particularly with basilar artery occlusion (BAO).
