The Infundibulum-Aneurysm Conundrum: A Longitudinal Single-Center Analysis.

This study aims to characterize the natural history and diagnostic variability of unequivocal intracranial infundibula and diagnostically uncertain outpouchings labeled "aneurysm versus infundibulum". We retrospectively analyzed 665 intracranial outpouchings detected on CTA, MRA, or DSA at a single-center (2013-2018). Radiology reports classified lesions as unequivocal infundibula (n=321) or as conundrums with an explicit label of "aneurysm versus infundibulum" (n=344). Follow-up imaging through December 2023 was reviewed for growth, rupture, or aneurysmal transformation. Ten conundrums with l

Foraminal Decompression Technique During ACDF for Cervical Radiculopathy that Provides a Better Outcome: Total Uncinatectomy vs Partial Uncoforaminotomy.

Study DesignRetrospective cohort study.ObjectivesAnterior cervical discectomy and fusion (ACDF) provides clinical improvement for cervical radiculopathy. Recently, it was determined that foraminal decompression via uncinate process resection could lead to faster and greater improvement of arm pain. Total uncinatectomy (TU) and partial uncoforaminotomy (PU) are commonly used for direct foraminal decompression. But the advantages and pitfalls of the two techniques remain unknown.MethodsConsecutive patients(n = 306) who underwent single-level ACDF for degenerative cervical radiculopathy and who w

Rescue strategy for management of an undeflating dual lumen balloon micro-catheter.

BACKGROUND: Intracranial balloon microcatheters are vital in neurointerventions, yet balloon non-deflation has not been previously reported in the cerebral vasculature. METHOD: We describe a 32-year-old woman undergoing embolization for a parieto-occipital arteriovenous malformation who developed failure of deflation of a dual-lumen balloon microcatheter placed in the posterior cerebral artery, despite standard manoeuvres. RESULT: Controlled catheter transection distal to the hub, followed by gradual capillary action assisted drainage of diluted contrast, enabled safe balloon collapse and re

Development of the SIMPLE scoring system for predicting malignant brain edema in patients with large hemispheric infarction following acute ischemic stroke.

Malignant brain edema (MBE) is a life-threatening complication of large hemispheric infarction (LHI). While early decompressive hemicraniectomy has been shown to improve outcomes in selected patients, identifying those at risk for MBE remains a clinical challenge. This study aimed to develop a simple and practical scoring system based on non-contrast CT imaging obtained approximately 24 h after stroke onset to support early prediction and timely intervention. We conducted a retrospective cohort study of LHI patients who underwent non-contrast CT imaging within 24 ± 8 h of symptom onset. Radiog

Endovascular Therapy Versus Best Medical Treatment in Posterior Cerebral Artery Stroke: A Systematic Review and Meta-Analysis.

INTRODUCTION: Posterior cerebral artery (PCA) stroke, while comprising a smaller proportion of ischemic strokes, often results in significant neurological deficits and poses distinctive therapeutic challenges. This study aims to systematically review and meta-analyze the efficacy and safety of EVT compared with best medical management (BMM) in patients with PCA stroke. METHODS: Electronic searches of PubMed, Embase, and Scopus up to July 2025 identified studies comparing EVT and BMM for PCA stroke. Eligible studies included patients with PCA stroke, compared EVT with BMM, and reported at leas

Outcome Assessment in Stroke Using Multiparametric MRI: Integrating Infarct Location, Radiomics, and Global Brain Frailty.

BACKGROUND: Accurate assessment of 90-day functional outcomes after anterior circulation large vessel occlusion (LVO) stroke remains challenging. Conventional models relying on a single data dimension have limited assessment power, suggesting that a multidimensional integration strategy could enhance evaluations. PURPOSE: To develop and validate an interpretable machine learning model that integrates radiomics, infarct location, brain frailty, and clinical variables for assessing 90-day functional outcomes in LVO stroke. STUDY TYPE: Retrospective. POPULATION: 1051 patients with anterior cir

Endovascular angioplasty and stenting of a persistent trigeminal artery in acute ischaemic stroke.

A persistent trigeminal artery (PTA) is a rare embryonic vascular remnant that can be implicated in stroke. This case involves a woman in her late 70s who presented following a ground-level fall with diplopia. Her National Institutes of Health Stroke Scale (NIHSS) score was initially 0, but she later developed acute neurological symptoms with an NIHSS of 25. She was found to have a large posterior circulation ischaemic penumbra on CT angiography. The terminal basilar artery was supplied primarily by a stenosed left Saltzman type I PTA. Angioplasty and stenting of the stenosed left PTA were per

Minimally invasive single-stage hybrid strategy for ruptured dissecting fusiform proximal posterior inferior cerebellar artery aneurysm: how I do it.

BACKGROUND: Fusiform posterior inferior cerebellar artery (PICA) aneurysms are technically challenging for both microsurgical and endovascular approaches, particularly when PICA is at high risk for occlusion. Optimal outcomes may be achieved through a collaborative hybrid strategy. METHODS: We present a single-stage hybrid minimally invasive technique for fusiform PICA aneurysm management, involving microsurgical PICA-PICA revascularization and endovascular coil embolization for definitive aneurysm occlusion in the hybrid operating suite. CONCLUSION: Combining microsurgical and endovascular

Acute Impact of FES-Assisted Cycling on Cerebrovascular and Cognitive Function in Healthy Young Adults.

It is well-established that functional electrical stimulation (FES) can enhance physical function, but little is known regarding its acute impact on cerebral blood perfusion and its possible acute repercussions on cognition in comparison to a conventional cycling exercise. Thirteen young healthy participants (1 female, 12 males) underwent two testing sessions (25 min of FES-assisted cycling and cycling control) applied in a randomized order and separated by at least 1 week of recovery. Cerebral blood velocity (CBFv) in the middle (right MCAv) and posterior cerebral arteries (left PCAv), cerebr

CT-derived density of intracranial arteriosclerosis: a population-based cohort study.

BACKGROUND: The CT-derived density of coronary artery calcification is increasingly associated with the risk of ischemic heart disease. Whether this principle also applies to intracranial artery calcifications (IAC) and cerebrovascular disease risk is unknown, primarily due to the lack of population-based estimates of IAC density and its determinants. We investigated these facets in this cohort study. MATERIALS AND METHODS: In 2464 community-living individuals who underwent non-contrast CT, we measured IAC density and assessed its correlation with IAC volume using Spearman's ρ. We described i

Alien hand syndrome as the initial presentation of posterior cerebral artery infarction: a case report.

BACKGROUND: Alien hand syndrome (AHS) is a rare neurological disorder characterized by involuntary, purposeful limb movements without voluntary control. While traditionally associated with frontal or callosal brain lesions, posterior cerebral artery (PCA) involvement remains sparsely reported, making such cases valuable for expanding understanding of the posterior variant of AHS. CASE PRESENTATION: We report a 79-year-old male who presented with a witnessed generalized tonic–clonic seizure followed by acute right-sided weakness. Neurological examination revealed right upper limb movements, des

Posterior fossa penetrating traumatic brain injury from military blast: illustrative case.

BACKGROUND: Posterior fossa penetrating traumatic brain injury (pTBI) in military combat is rare but carries high neurovascular risk. OBSERVATIONS: A 51-year-old male sustained a mine explosion injury in Ukraine, in which a 4 × 1.5-cm metal fragment penetrated through the skull base into the posterior cranial fossa. The fragment was adjacent to the vertebral artery, sigmoid sinus, lower cranial nerves, and occipital condyle. Digital subtraction angiography was completed on arrival, which showed a patent right vertebral artery, occluded transverse-sigmoid sinus, and partial internal jugular ve

Time-Dependent Efficacy of Drag-Reducing Polymers in the Treatment of Permanent Middle Cerebral Artery Occlusion Injury in Rats.

Current treatments for ischemic stroke are not focused on microvascular cerebral blood flow (mvCBF), which actually delivers oxygen to tissue and is significantly impaired during a stroke. We previously showed that drag-reducing polymers (DRPs, 2 μg/ml), injected just after permanent middle cerebral artery occlusion (pMCAO) in rats, effectively restored mvCBF and tissue oxygenation. The aim of this work was to assess the efficiency of DRPs administered at various time points post-onset. DRPs (2 μg/ml) or saline was i.v. injected 0.5, 3, or 6 h after pMCAO induction in rats (n = 10/group). Lase

Improving catheter navigation in neurointerventional procedures: single-center insights on next-generation steerable guidewires.

PURPOSE: Steerable guidewires with deflectable tips enable real-time navigation through complex cerebrovascular anatomy without requiring wire removal and tip reshaping. Despite promising early reports, systematic clinical evaluation remains limited. METHODS: We conducted a retrospective analysis of 58 consecutive neurointerventional procedures utilizing steerable guidewires at two centers from September 2024 to August 2025. The cohort comprised 48 cases using the Drivewire 24 (DW24, 0.024-inch diameter) and 10 cases using the Artiria SmartGUIDE (0.014-inch diameter). Primary outcomes include

Cerebral venous air embolism, a hidden precursor of arterial ischemia: a case report.

BACKGROUND: Cerebral venous air embolism is a rare but potentially fatal complication of central venous catheter insertion and removal. Although most venous air emboli remain clinically silent or cause only transient symptoms, they may exceptionally lead to extensive arterial cerebral ischemia, particularly in patients with impaired pulmonary filtration or pre-existing lung disease. We report a distinctive sequence of computed-tomography-demonstrated cortical venous air followed by extensive right middle and posterior cerebral artery infarction on magnetic resonance imaging despite a negative
