Microsurgical anatomy of the cisternal course of the perforating branches of the posterior communicating artery.

Both the perforating branches--especially the extracerebral segments--and the arachnoidal anatomy at the level of the posterior communicating artery were studied in 60 human brains previously fixed in formalin. The close relationships between this artery and the oculomotor nerve are described, and it is noted that each of them is enclosed in its own arachnoidal compartment, which in the case of the posterior communicating artery is to be distinguished from the interpeduncular cistern. The latter cistern was found to contain only the terminal segments of the posterior communicating artery. The

[PET study on cerebral hemodynamics in internal carotid artery occlusion--the pathogenesis of watershed infarction].

We studied positron emission tomography in nine patients with unilateral internal carotid artery occlusion, selected as having good collateral circulation through the anterior portion of the circle of Willis. Analyses of regional cerebral blood flow, cerebral oxygen metabolic rate, oxygen extraction fraction, and cerebral blood volume allowed quantitative evaluation of the regional hemodynamic status, especially in relation to watershed area. The patient group has a significantly (p less than 0.01) decreased regional blood flow in the middle cerebral artery territory and the surrounding waters

Posttraumatic cerebral infarction diagnosed by CT: prevalence, origin, and outcome.

Posttraumatic cerebral infarction is a recognized complication of craniocerebral trauma, but its frequency, cause, and influence on mortality are not well defined. To ascertain this information, all cranial CT studies demonstrating posttraumatic cerebral infarction and performed during a 40-month period at our trauma center were reviewed. Posttraumatic cerebral infarction was diagnosed by CT within 24 hr of admission (10 patients) and up to 14 days after admission (mean, 3 days) in 25 (1.9%) of 1332 patients who required cranial CT for trauma during the period. Infarcts, in well-defined arteri

Functional impairment due to white matter ischemia after middle cerebral artery occlusion in cats.

We recorded regional cerebral blood flow, somatosensory evoked potentials, and auditory evoked potentials in the thalamic relay nuclei (ventral posterior lateral nucleus and medial geniculate body) and in the somatosensory and auditory cortices during and after 1 hour of transient left middle cerebral artery occlusion in nine cats. Regional cerebral blood flow was also measured in the thalamocortical tracts of five of these cats. Additionally, the integrity of thalamocortical connections was tested by retrograde labeling of the thalamic nuclei with horseradish peroxidase in eight cats (three o

Effectiveness of different task paradigms in revealing blindsight.

Four patients with hemianopia from posterior cerebral artery infarction were tested for residual unconscious vision ('blindsight') in their anopic hemifield. One task tested for spatial summation between the normal and the abnormal hemifields. A second task tested for temporal interactions between the two hemifields. A third task required a manual localization of the site of a brief flash in the hemianopic side. The last test was administered either in a condition of free gaze during pointing or in a condition in which the gaze was kept fixed throughout all phases of the task. The first two ta

Anterior choroidal artery: angiographic analysis of variations and anomalies.

In the first part of this investigation, 216 consecutive carotid angiograms from 108 patients were reviewed to analyze variations of the anterior choroidal artery (AChA) for the origin, size, and course of the stem and for the possible identification of the uncal branch, perforating branches, and plexal segment of the AChA. In the second part of our investigation, we analyzed anomalies of the AChA seen on angiography. The anomalies of this artery were divided into hypoplastic and hyperplastic types, with prevalences of 3% and 2.3%, respectively, as determined from 216 control carotid angiogram

[Glossopharyngeal neurinoma causing symptomatic hemorrhage: case report].

A 86-year-old man was admitted to our hospital because of swallowing disturbance and deterioration of consciousness. He had been aware of hearing disturbance on the right side for twelve months. Computed tomography (CT) scan demonstrated an inhomogeneous hyperdense mass lesion, 3.2 X 2.3 cm in size, at the right cerebello-pontine angle, and ventricular enlargement with intraventricular hemorrhage. Skull tomogram revealed destructive enlargement of the right jugular foramen. The angiogram showed avascular mass with elevation of anterior inferior cerebellar artery, and downward shift of posterio

Vasculature of the paraphysis cerebri of the frog.

The paraphysis cerebri is a glandular structure found in the third ventricle of lower vertebrates. It is well-developed in amphibians and reptiles. The function of the gland is not substantiated, but may play a role in calcium metabolism. To further elucidate its possible endocrine role, the paraphyseal vasculature was examined using casting techniques as well as transmission electron microscopy. Perfusion-fixed paraphyses of Rana pipiens and Rana catesbeiana were either: a) cast with Microfil (with subsequent dehydration, clearing, and macroscopic examination) or Batson's compound (followed b

Origins and distribution of cerebrovascular nerve fibers showing calcitonin gene-related peptide-like immunoreactivity in the major cerebral artery of the dog.

The origins and overall distribution of perivascular nerve fibers showing calcitonin gene-related peptide-like immunoreactivity (CGRP-LI) in the major cerebral arteries were investigated immunohistochemically in the dog by using whole-mount preparations of the arterial trees around the circle of Willis. Perivascular nerve fibers with CGRP-LI were seen most abundantly in the basilar artery, vertebral artery, common anterior cerebral artery, proximal part of the anterior cerebral artery, and terminal part of the internal carotid artery. They were far less numerous in the middle cerebral artery,

[A case of posterior cerebral artery territory infarction with micropsia as the chief complaint].

A 63-year-old man was admitted to the hospital complaining that all objects in his vision suddenly appeared small. Examination on admission revealed amnesia of recent events, recognition memory deficits, emotional disturbance, right upper quadrantanopsia and micropsia. The patient was diagnosed as posterior cerebral artery territory infarction based on brain CT scan and MRI findings which showed an ischemic lesion involving the left occipital lobe and hippocampus. We regarded the quadrantanopsia to have originated in the occipital lobe lesion and memory and emotional disturbances to have origi

Perforating branches of the basilar bifurcation.

The perforating branches of the upper basilar artery and of the first (P1) segment of the posterior cerebral artery were studied in 50 fixed brains obtained from human cadavers. No vertical branches arose from the basilar bifurcation. The upper basilar artery gave rise to horizontal branches, which were studied with reference to their angle of origin. Perforating arteries arising from P1 segments were found in all specimens. Rare branches were found to come from the inferior and anterior surfaces of P1 segments. The anatomical patterns of these branches are discussed in the light of previous r

Exo-focal postischemic neuronal death in the rat brain.

We describe delayed neuronal damage in ipsilateral areas remote from the ischemic area of rat brain after transient focal ischemia induced by embolization of the right middle cerebral artery (MCA). After 15, 30, 60 and 90 min of MCA occlusion, recirculation was achieved by removal of the embolus. Chronological changes in the distribution of the neuronal damage were determined by using the 45Ca autoradiographic technique and the histological method, and the mechanism involved was investigated by measuring local cerebral glucose metabolism. Depending on the duration of ischemia, 45Ca accumulatio

Dolichoectatic basilar artery treated by reducing hemodynamic stress--report of two cases.

Two cases of dolichoectatic basilar artery with a mass effect to the brainstem structure were treated by reducing hemodynamic stress. In one case, angiograms showed the dolichoectatic basilar artery creating a turbulent flow in the vertebrobasilar junction, and unilateral vertebral artery clipping in addition to posterior fossa decompression was performed. The other case, a combination with bilateral internal carotid artery occlusion, underwent bilateral superficial temporal artery-middle cerebral artery anastomoses. The possible surgical treatments of dolichoectasia are discussed.

Transcranial Doppler ultrasonography versus arteriography for assessment of the vertebrobasilar circulation.

The investigation of the vertebrobasilar arterial system by transcranial Doppler (TCD) ultrasonography is difficult due to the numerous anatomical variations in the posterior circulation. To test the validity of TCD, we compared topographic measurements from vertebrobasilar angiograms and corresponding TCD findings in 58 patients with cerebrovascular diseases (N = 37, 21 affecting the vertebrobasilar system), brain tumors (N = 8), and other neurological diseases (N = 13). Normal TCD recordings of the vertebrobasilar system were confirmed by the angiograms in 25 patients, although the noninvasi

[The basilar artery terminal branch syndrome. Paramedian meso-thalamic infarct].

The author describes two cases of paramedian mesothalamic infarction as part of multiple infarctions in occlusions of the top of the basilar and peduncular segment of the posterior cerebral artery. Based on published work and the author's own observations, the clinical syndrome of this infarction was defined and comprises five symptoms: impaired consciousness, amnestic syndrome and paralysis of vertical views due damage of formations in the area of paramedian thalamo-subthalamic (thalamoperforate) arteries by neuropsychic abnormalities and contralateral hemidefects due to damage of structures

Posterior inferior to posterior inferior cerebellar artery anastomosis combined with trapping for vertebral artery aneurysm. Case report.

A case is presented in which a giant intracranial vertebral artery aneurysm gave rise to an associated ipsilateral posterior inferior cerebellar artery (PICA) from its waist. Proximal vertebral artery ligation at C-1 was achieved. The aneurysm filled from the opposite vertebrobasilar junction. Direct intracranial trapping of the right vertebral aneurysm was followed by successful anastomosis of the proximally sectioned right PICA to the adjacent left PICA in an end-to-end fashion.

Brain resection for exposure of deep extracerebral and paraventricular lesions.

In exposing deep extracerebral or paraventricular lesions, it sometimes may be preferable to resect a small amount of noneloquent brain tissue rather than retract the brain. This may be the case in carefully selected instances when, in the surgeon's estimate, brain retraction may result in significant parenchymal damage, in aneurysmal rupture, or in damage to important bridging veins. Some surgical approaches involving brain resection have been well described. This technical note discusses the following approaches that, though undoubtedly used by many surgeons sporadically, have received less

Loss of the insular ribbon: another early CT sign of acute middle cerebral artery infarction.

Although computed tomography (CT) remains the most frequently used imaging examination in acute cerebral infarction, its sensitivity for early detection of strokes remains limited. In middle cerebral artery (MCA) strokes less than 6 hours old, loss of definition of the gray-white interface in the lateral margins of the insula ("insular ribbon") was observed. The acute and subacute CT findings in 11 retrospective and 16 prospective patients are presented. Loss of the insular ribbon was detected in 12 of the prospective cases and in all 11 retrospective cases. The insular ribbon is supplied by t
