Transient and lasting oculomotor disturbances during visual-vestibular interaction are described in 9 patients with Wallenberg's lateral medullary syndrome. In all patients magnetic resonance imaging (MRI) demonstrated a single focal area of pathological signal intensity in the (dorso)-lateral medulla suggesting infarction. In 2 of these 9 patients and in 3 further patients with no medullary signs, the infarction involved the cerebellar territory of the posterior inferior cerebellar artery (PICA). Acutely, all patients with Wallenberg's syndrome (except 1) had saccadic lateropulsion and sponta

We report 10 cases of cerebellar infarction in the territory of the medial branch of the posterior inferior cerebellar artery (mPICA). Axial sections on MRI through the middle of the medulla and the cerebellum showed the infarction as a triangular area with a dorsal base and a ventral apex directed towards the fourth ventricle. The infarct also involved the lateral and dorsal medulla when the mPICA supplied all or part of these regions. Three clinical patterns were observed: 1) pseudolabyrinthine signs with or without dysmetria and ataxia when the medulla was spared; marked axial lateropulsion

Brain perfusion single photon emission CT (SPECT) has been used effectively in the evaluation of neurological diseases including stroke. Recently the introduction of annular special purpose systems has resulted in substantially improved spatial resolution. High resolution SPECT allows us to look accurately at the functional abnormalities resulting from small structural defects. We present a case of infarction in the territory of the posterior inferior cerebellar artery which illustrates the utility of this method.

We sought to determine whether high spatial resolution magnetic resonance imaging is useful for noninvasive quantitation of the ischemic infarct produced by occlusion of the middle cerebral artery and for detection of reduced infarct volume elicited by electric stimulation of the cerebellar fastigial nucleus. Male rats of the spontaneously hypertensive strain were anesthetized, the middle cerebral artery was occluded, and the fastigial nucleus was stimulated for 1 hour. Twenty-four hours later, rats were reanesthetized and T1- and T2-weighted images were obtained. Rats were killed and the volu

Two pediatric cases of unilateral Moyamoya disease are reported. The clinical symptoms and the findings of electroencephalography, angiography, and positron emission tomography in these cases were quite the same as those in the cases with bilateral involvement, which is typical of this disease. In both cases angiography demonstrated unilateral occlusion not only of the internal carotid artery but also of the posterior cerebral artery together with so-called Moyamoya vessels on the same side. Although infectious diseases were mentioned in the patients' histories, no obvious basic diseases relat

We reported a 49-year-old male with brain stem infarction who had bilateral hearing impairment and tinnitus at the onset and subsequently developed various neurological symptoms, including bilateral lateral inferior pontine syndrome, one and a half syndrome and upward gaze palsy. Although CT scan failed to reveal any abnormalities initially, MRI revealed symmetrical foci bilaterally from the lateral inferior pons to the middle cerebellar peduncle, as well as in the paramedian portion of the mid-pons. Cerebral angiography: The left vertebral artery (VA) occluded at the 4th segment. The right VA

A rare case of ruptured aneurysm originating from the vertebral artery, and presenting an isolated clot in the fourth ventricle is reported. The patient underwent surgery via a right lateral suboccipital craniectomy on the third day. As the aneurysm was huge and partially thrombosed, trapping was performed and completed successfully. If isolated intraventricular hematoma was found without obvious parenchymal hemorrhage or subarachnoid clot, the diagnosis of vertebral artery aneurysm should be considered as a possibility in addition to posterior inferior cerebellar artery aneurysm. Treatment of intracranial arterial aneurysms by interventional neurovascular techniques is now being performed in selected cases. From a transfemoral approach, under local anesthesia, a detachable silicone microballoon can be guided through the intracranial circulation, directed into the aneurysm, inflated with a polymerizing agent for solidification, and detached. The goal is to exclude the aneurysm from the circulation and preserve flow through the parent artery. Since 1984, 84 patients have been treated by this technique. The patients ranged in age from 15 to 83 years (mean age, 48) and inc

BACKGROUND AND METHODS: The use of cocaine, especially one of its alkaloidal forms ("crack"), has been increasingly associated with cerebrovascular disease. To clarify the clinical, radiologic, and pathological features of the events associated with cocaine use, we identified 28 patients at four medical centers who had stroke temporally related to the use of alkaloidal cocaine (during or within 72 hours of use).
RESULTS: The 28 patients had the following types of cerebrovascular event: cerebral infarction (n = 18 [2 hemorrhagic; 1 fatal]) in the areas supplied by the middle cerebral artery (n

Two cases treating aneurysms of the distal PICA were reported, and 36 cases with 39 aneurysms in the literatures in Japan were reviewed concerning the distribution of aneurysms and their findings on CT. Case 1; a 68-year-old female suffered from sudden onset of severe headache and nausea. On admission, it was found she was lethargic. However, her consciousness deteriorated down to semicoma with tetraparetic condition soon after. CT revealed subarachnoid hemorrhage in the basal, quadrigeminal and supravermian cisterns and blood clots in the entire ventricle. Cerebral angiography demonstrated an

A 45-year-old woman was admitted to our department because of intracerebral hemorrhage. Neurological examination showed upward gaze palsy, right hemihypesthesia including face and nuchal rigidity. CT scan demonstrated a high density spot in the left quadrigeminal plate. Postcontrast CT scan demonstrated an abnormal vessel in the left midbrain. We suspected the presence of an arterio-venous malformation (AVM) and angiography was performed. Left vertebral angiography demonstrated early filling as far as the basal vein of Rosenthal with the contrast medium. However, no nidus suggesting AVM was ob

A case of intrasellar and suprasellar meningioma with hypopituitarism is reported. A-64-year-old woman was admitted to our hospital with chief complaints of reduced consciousness and inactivity. She had a history of subarachnoid hemorrhage 20 years previously, and developed right third nerve palsy. Physical examination demonstrated that, in consciousness, she was stuporous, and she had impaired visual acuity and palsy in the right third nerve. An X-ray film of the sella turcica showed enlargement and intrasellar calcification. A CT scan with contrast enhancement revealed a homogenously enhance

We studied 305 patients with a first stroke and a potential cardiac source of embolism (PCSE), on the basis of the Lausanne Stroke Registry. We have compared these patients with 1006 patients with stroke but without PCSE, admitted into the Registry during the same period, in order to assess the potential role of PCSE and the coexistence of other causes of stroke. Analysis of the various types of PCSE suggests that some neurologic characteristics are more frequent in patients with than without PCSE (infarcts of the posterior division of middle cerebral artery with Wernicke's aphasia and lateral

Eighty-three patients underwent 85 intracranial to extracranial pedicle bypass anastomosis procedures to the posterior circulation. There were 15 patients with occipital artery (OA) to posterior inferior cerebellar artery (PICA) anastomosis, 20 patients with OA to anterior inferior cerebellar artery (AICA) anastomosis, and 50 patients with superficial temporal artery (STA) to superior cerebellar artery (SCA) anastomosis. All patients had transient ischemic attacks (TIA's) suggestive of vertebrobasilar ischemia. Twenty-seven patients had crescendo TIA's or stroke in evolution and were considere

Dementia occurs infrequently in patients with giant cell (temporal) arteritis (GCA). Three elderly women with biopsy-proven GCA showed abrupt cognitive decline during periods of clinically active GCA, 1 to 6 months after diagnostic temporal artery biopsy, during periods of corticosteroid taper. One patient had additional clinical signs of cerebral infarction and other ischemic phenomena. Reinstitution of higher oral doses of corticosteroids successfully prevented further cognitive losses and permitted gradual but incomplete improvement of cognitive function in 1 patient. Neuropsychologic data

Our pathogenetically oriented classification system of hemispheric brain infarctions is reviewed. New data are presented to validate this classification from various points of view. A retrospective analysis of 73 patients with large striato-capsular infarcts demonstrated that two-thirds of them had a source of embolism either in the carotid bifurcation or in the heart. SPECT-imaging in patients with hemispheric brain infarctions of various origin revealed that the area of exhausted cerebral perfusion reserve largely exceeds the area of the infarct visible on CT if a hemodynamically caused low-

Less than 10% of the 250 reported cases of traumatic intracranial aneurysms have involved the posterior circulation. Traumatic aneurysms of the superior cerebellar artery are extremely rare, with only three cases previously reported. This is the first report of a traumatic superior cerebellar artery aneurysm in which the diagnosis was suggested by computed tomographic scan. The potential for a good outcome suggests the value of early angiography when the history and diagnostic imaging studies suggest the possibility of a traumatic aneurysm.

Three patients underwent transarterial platinum coil embolization of unusual posterior inferior cerebellar artery aneurysms. In one case, a giant, bilobed, partially thrombosed aneurysm exhibited marked mass effect on the adjacent medulla. In the second case, diffuse severe cerebral vasospasm, 3 days after subarachnoid hemorrhage, rendered transvascular treatment of the aneurysm difficult. Increasing vertigo and nausea caused by mass effect from an aneurysm previously coated with methyl methacrylate warranted treatment in the third case. Indications for transvascular coil treatment included re

A case with infarction in the territory of the anterior choroidal artery (AChA) due to embolic occlusion of the internal carotid artery (ICA) is rare. We described two cases and investigated the mechanism of the territory of the AChA. Case 1 was a 69-year-old man. Case 2 was a 71-year-old woman. The neurological examination in both cases showed left homonymous hemianopsia, left facial palsy, left hemiparesis and left hemisensory disturbance. CT scan in these cases showed infarctions of the right uncus, amygdaloid nucleus, genu and posterior limb of the internal capsule, globus pallidus, latera
