Cerebellar infarction in the territory of the superior cerebellar artery: a clinicopathologic study of 33 cases.

We reviewed the clinical and pathologic findings in 33 patients with infarcts in the territory of the superior cerebellar artery (SCA). The clinical manifestations included the rostral basilar artery syndrome (8); coma at onset, often with tetraplegia (11); cerebellar and vestibular signs (9, with delayed coma due to cerebellar swelling in 6); and, in only 1 patient, the "classic" syndrome of the SCA. Clinical features were overshadowed by an infarct in the territory of the middle cerebral artery in 3 other patients, and the diagnosis was made only at autopsy in a fourth. Pathologically, SCA i

[The foramen of Monro--blockage caused by a giant aneurysm of the basilar artery. A case report and review of the literature].

A case of a giant aneurysma of the basilar artery bifurcation with occlusive hydrocephalus due to bilateral foramen of Monro occlusion is reported. The patient presented with progressive neurological deterioration which led to computerized tomography examination and implantation of a ventricular shunt as an emergency procedure. After the patient had recovered to a certain extent, four-vessel angiography was performed that demonstrated a giant basilar tip aneurysm. As the posterior communicating, the posterior cerebral as well as the superior cerebellar arteries originated from the aneurysm sac

Ruptured cerebral aneurysms missed by initial angiographic study.

The authors reviewed the computed tomographic (CT) scans of patients with subarachnoid hemorrhage whose initial angiograms were negative, to investigate the validity of CT scans in predicting the presence of an angiographically missed aneurysm in such patients. During the past 14 years, additional angiograms have been obtained for 38 of the 45 patients with subarachnoid hemorrhage whose initial angiograms disclosed no aneurysm. Aneurysms were found in 8 patients; 7 on the anterior communicating artery and 1 at the junction of the internal carotid and posterior communicating arteries. CT scans

Non-invasive radiological investigation for oculomotor palsy.

An application of computed tomography (CT) is described in which multiplanar high resolution images of the terminal carotid and basilar arteries are obtained. This has been applied in a series of 32 patients with IIIrd nerve palsy in whom the underlying pathology was thought to be a posterior communicating artery aneurysm. The results of the CT were compared with conventional angiography. Seventeen aneurysms were detected in 13 patients by CT and all were confirmed by angiography. Vessels considered to be normal on CT were confirmed to be normal by angiography. This CT technique is a simple no

An operative complication in a patient with a true posterior communicating artery aneurysm: case report and review of the literature.

Intraoperative oculomotor nerve injury in a patient with a true posterior communicating artery aneurysm is reported in detail. A comparison of internal carotid artery aneurysms at the posterior communicating artery junction with true posterior communicating artery aneurysms deserves special attention, because the vascular relationships of the aneurysm are more complex. A clip along the internal carotid artery does not occlude blood flow to the aneurysm, and the aneurysmal neck and the distal posterior communicating artery are closer to the oculomotor nerve. This is the 27th reported case of a

Impairment of vertical motion detection and downgaze palsy due to rostral midbrain infarction.

We present two cases with acute onset of vertical gaze palsy, mainly consisting of impaired downgaze and apraxia of downward head movements, together with neuropsychological deficits (hypersomnia, impaired attention and disorders of memory and affective control). CT and MRI revealed bilateral post-ischaemic lesions in the dorsomedial thalamus and the mesodiencephalic junction, dorsomedial to the red nucleus, thus being restricted to the territory of the posterior thalamosubthalamic paramedian artery, which includes the region of the rostral interstitial nucleus of the medial longitudinal fasci

[Four cases of intracranial AVM successfully treated by radiation therapy].

We have treated three patients with conventional radiotherapy and one with stereotactic radiosurgery and report the follow-up results. Case 1 was of a 63-year-old woman who had a dural AVM in the right cavernous sinus. She was irradiated using 10MV X-rays with a total of 30Gy. Post irradiation angiography showed a complete absence of dural AVM. Case 2 was of a 62-year-old woman who had a dural AVM fed by the left external carotid artery in the right cavernous sinus. Embolization by gelfoam was performed before irradiation. The irradiation was given with a total dose of 30Gy. Post irradiation a

Distal posterior inferior cerebellar aneurysms--report of 12 cases.

The authors review 12 cases of distal posterior inferior cerebellar artery (PICA) aneurysm. Nine of the aneurysms were discovered after their rupture and three were incidentally detected. Seven (78%) of the nine ruptured aneurysms rebled, leading in four cases to coma and respiratory arrest. Computed tomography revealed intraventricular hematomas, mainly in the fourth ventricle, and a subarachnoid clot at the cisterna magna. In five of the nine cases involving aneurysm rupture, radical surgery was performed. The remaining four, whose clinical status was poor, were treated conservatively and su

Surgical treatment of unruptured aneurysms of the posterior circulation.

With the ever-increasing number of intact aneurysms revealed by modern imaging, the options for their management are assuming great importance. While some knowledge has emerged as to their natural history and the results of surgical treatment of those in the anterior circulation, little information has been published concerning unruptured aneurysms arising from the posterior circulation. The authors report their experience since 1971 with 167 patients operated on for 179 unruptured vertebrobasilar aneurysms up to 25 mm in diameter. Overall, 160 aneurysms were treated by direct clip obliteratio

[Fenestrated basilar artery with ruptured cerebral aneurysms: case report].

Two cases of aneurysms with basilar artery fenestration are presented. A 45 year-old lady had a saccular aneurysm associated with a fenestrated basilar artery. The fenestration was located in the proximal third of the basilar artery. The aneurysm arose from the proximal end of the fenestration, projecting antero-inferiorly. Via a left retromastoid craniectomy, the aneurysm was successfully obliterated through the space between the eighth cranial nerve and the lower cranial nerve complexes. Postoperative deficit in the lower cranial nerves was minimal and transient. The other patient, a 63 year

Surgical approach to arteriovenous malformation of the medial temporal lobe--report of three cases.

We report three cases of arteriovenous malformation (AVM) of the medial temporal lobe and the surgical approaches used. The AVM was fed by the anterior choroidal artery (AChA) in two cases (Cases 1 and 2) and by the posterior cerebral artery in one (Case 3). The trans-Sylvian approach was first used for cerebrospinal fluid aspiration to retract the brain in all cases, and for confirming the feeding arteries to prevent premature bleeding from the AVM in Cases 1 and 2. In Case 1, a corticotomy was then made in the fusiform gyrus via the subtemporal approach to avoid the development of speech dis

[Two cases of subcortical hemorrhage with asymptomatic occlusion of the main trunk of cerebral artery].

We report two cases of asymptomatic occlusion of the main trunk of the cerebral artery associated with subcortical hemorrhage in the area fed by collateral circulation. The first patient, a 51-year-old female who had suffered from untreated hypertension for 20 years, was hospitalized in a state of coma. Computed tomography (CT) revealed a subcortical hemorrhage in the right parietal lobe as well as subdural and subarachnoid hemorrhages. Cerebral angiography disclosed occlusion of the right internal carotid artery at its origin. The region normally supplied by the right anterior (ACA) and middl

Basilar arterio-venous pseudoparallelism due to persistence of embryonal venous pattern.

A vascular malformation, consisting of a venous vessel bridgeing the right inferior petrosal sinus and the anterior spinal veins, was found in the posterior fossa. The vessel presented a ring-like course around the right trigeminal root, and it was parallel and dorsal to the basilar artery. The malformation was associated with cutaneous and hepatic angiomas and peri-osteal lipomas. It had been clinically silent for 52 years, when it thrombosed causing death. The authors think that, within a general mesenchymopatic state, this is a result of the persistence of an embryonal cerebral venous patte

Cerebellar infarction in the territory of the anterior and inferior cerebellar artery. A clinicopathological study of 20 cases.

Clinicopathological reports on infarction in the territory of the anterior inferior cerebellar artery (AICA) are rare. In the present series cerebellar infarcts involved only the AICA territory in 13 cases. They were bilateral in 3 cases. In these pontocerebellar infarcts the middle cerebellar peduncle was the core of the affected territory. It was seldom selectively affected. In a few cases, the infarction extended to the flocculus. Next most frequently involved was a thin band of tissue in the adjacent cerebellar white matter, comprising laterally the neighbouring cerebellar lobules. The lar

[Transcranial Doppler measurement of hemodynamics in the major cerebral arteries].

Using a pulsed Doppler velocity meter, the authors assessed by a transcranial approach, in order to obtain an idea on the normal haemodynamics of the basal cerebral arteries, the maximal systolic blood flow (Vmax., m.s-1) in these arteries in a group of 25 healthy young men and in 20 healthy children. In different arteries of the two groups the following mean values of Vmax. were recorded: middle cerebral artery 0.73 +/- 0.11 parallel 0.80 +/- 0.17 m.s-1; anterior cerebral artery 0.55 +/- 0.06 parallel 0.51 +/- 0.09 m.s-1 and posterior cerebral artery 0.44 +/- 0.04 parallel 0.40 +/- 0.02 m.s-1

Borderzone hemodynamics in cerebrovascular disease.

To investigate the possible existence of chronic selective hemodynamic impairment in the arterial borderzone regions of the brain, we used positron emission tomography (PET) to measure regional mean vascular transit time (rt, equal to the ratio of regional cerebral blood volume to cerebral blood flow) and regional oxygen extraction fraction (rOEF) in 32 patients with either severe internal carotid artery stenosis or occlusion and 11 normal controls. Twenty-four of the patients had had TIAs or amaurosis fugax from 1 to 60 days before PET; all had normal brain CT. We used a stereotactic localiza

Remote effect of deep-seated vascular brain lesions on cerebral blood flow.

We measured regional cerebral blood flow using the xenon-133 inhalation method, at approximately 1 month after onset, in 60 stroke patients who had no evidence of major carotid artery stenosis or occlusion. Their single lesions (43 infarcts and 17 hematomas) were located in the capsulothalamolenticular region, sparing the cortex. Hemispheric mean cerebral blood flow was reduced on the side of the lesion in 25 patients and on both sides in 20. Regional hypoperfusion was observed in 46 patients (ipsilaterally in 34, bilaterally in 10, and contralaterally in two). Regional hypoperfusion was obser

[Dissecting aneurysm of the vertebral artery as a cause of Wallenberg's syndrome].

Although it is well known that Wallenberg's syndrome is caused by occlusion of the vertebral artery (VA) or the posterior inferior cerebellar artery (PICA), the etiology of the occlusion is rarely documented. During the course of Wallenberg's syndrome, patients often complain of headache. We thought that these headaches might be caused by dissecting aneurysm (DA) of the vertebral artery, and so we studied the incidence of DA in our cases with Wallenberg's syndrome. Although many variants exist, Wallenberg's syndrome encompasses several neurological symptoms due to a disorder of the nucleus and

A 49-year old right-handed taxi-driver experienced right upper limb weakness and global aphasia following internal carotid artery occlusion. Five months later, aphasia and hemiparesis had resolved but he complained of difficulties in his daily activities, termed "inner conflict". Specific testings disclosed typical features of a callosal syndrome: left unilateral ideomotor apraxia, left hand agraphia, left tactile dysnomia, right hand constructional difficulties, and left ear extinction on dichotic listening. Naming and recognition of tachistoscopically presented images or words was similar to
