Ruptured posterior inferior cerebellar artery aneurysm in a neonate treated with coil embolization: A technical report.

BACKGROUND: Intracranial saccular aneurysms in the neonatal population are exceedingly rare, with fewer than 50 cases in the literature, the majority of which are localized to the anterior circulation. Rarer still is endovascular treatment, occurring in only seven of these cases, with almost all interventions involving the sacrifice of the parent artery. CASE DESCRIPTION: A 1-month-old term male presented lethargic and irritable with bulging fontanelles and no neurologic deficit. Transfontanelle ultrasonography showed intraventricular hemorrhage and hydrocephalus. Non-invasive angiography rev

Pedicled lateral nasal wall flap for endoscopic skull base reconstruction: how I do it.

BACKGROUND: The pedicled nasoseptal flap (Hadad-Bassagasteguy flap, HBF) has been considered as the workhorse for reconstruction of high-flow cerebrospinal fluid (CSF) leak following endoscopic endonasal skull base surgery. However, if the septal mucosa is affected by a tumor or the vascular pedicle is injured due to prior surgery, the use of HBF is precluded. METHOD: We illustrate the harvest techniques of the lateral nasal wall flap (LNWF) based on the posterior lateral nasal artery (PLNA). CONCLUSION: The LNWF is a robust intranasal regional flap with a consistent blood supply and compara

Basilar Artery Occlusion Secondary to Cocaine-Associated Cardiomyopathy: A Case of Cardioembolic Stroke With Poor Outcome Despite Successful Reperfusion.

Basilar artery occlusion (BAO) is a neurologic emergency associated with high morbidity and mortality, in which successful reperfusion does not consistently translate into favorable functional outcomes. While futile recanalization has been well described, the underlying mechanisms contributing to poor outcomes remain incompletely understood. We present the case of a 54-year-old male with polysubstance use who presented with altered mental status and focal neurologic deficit, and was found to have BAO. He underwent emergent endovascular thrombectomy with complete reperfusion (thrombolysis in ce

Endovascular thrombectomy in addition to intravenous thrombolysis versus intravenous thrombolysis alone in medium distal vessel occlusions: Results from the SITS International Stroke Treatment Register.

BACKGROUND: Endovascular thrombectomy (EVT) is the standard of care for large-vessel occlusion stroke. Medium distal vessel occlusions (MDVO) account for 25-40% of acute ischemic stroke cases, but recanalization rates with intravenous thrombolysis (IVT) are often less than 50%. Recent randomized trials have failed to show better outcomes after EVT versus best medical management in MDVO stroke. The main research question of the study: Is the addition of EVT to IVT associated with benefits or harm when treating patients with MDVO stroke? METHODS: We performed a retrospective observational study

Assessment of Peripheral Arterial (Normal) Flows in Multiple Vascular Territories in Patients Receiving Mechanical Cardiac Support.

BACKGROUND: In the last few years, mechanical circulatory support in patients with advanced/terminal heart failure (HF) is increasing. New devices have been developed from the known intra-aortic balloon pump to the total artificial heart. However, vascular complications may occur in up to 30% based on the device implanted. Evaluation of the Doppler flow in all vascular territories (cranial, extracranial, abdominal, and peripheral) in patients with these devices might be of great importance but remains still unexplored. This is a proof of concept study in which we provide a practical analysis o

Bilateral persistence of the proatlantal intersegmental arteries in moyamoya syndrome: illustrative case.

BACKGROUND: The persistent proatlantal intersegmental artery (PPIA) arises from the cervical internal carotid artery (ICA) and joins the vertebral artery near the foramen magnum. In moyamoya syndrome (MMS), such embryonic carotid-vertebrobasilar channels may influence collateralization and procedural strategy. The authors report bilateral PPIA identified concurrently with bilateral MMS. OBSERVATIONS: A 28-year-old woman with Down syndrome presented with sudden severe headache and unresponsiveness. CT showed bilateral intraventricular hemorrhage, a small left periatrial hemorrhage, and obstruc

Infarct Laterality Patterns in Relation to A1 Segment Hypoplasia/Aplasia According to Etiological Subtype.

Background: The Circle of Willis (CoW) is a key collateral pathway that enables communication between the anterior and posterior cerebral circulations. However, anatomical variations in the A1 segment of the anterior cerebral artery, such as hypoplasia or aplasia, can alter hemodynamics and may compromise this collateral function. While incomplete CoW configurations have been linked to aneurysm formation and altered patterns of hemorrhage, their role in the distribution of cerebral infarctions remains controversial. We aimed to explore the association between A1 segment hypoplasia/aplasia and

Transcranial doppler-derived cerebrovascular resistance changes during vagus nerve stimulation in drug-resistant epilepsy.

BACKGROUND: Vagus nerve stimulation (VNS) is an established neuromodulation therapy for drug-resistant epilepsy. Although its clinical efficacy is well documented, its acute effects on cerebral hemodynamics, particularly vascular resistance, are not fully understood. This study aimed to compare cerebral hemodynamic parameters between VNS on and off. METHOD: In this single-center, prospective, within-subject pilot study, 12 patients with left-sided VNS implantation were evaluated. Cerebral blood flow velocities, pulsatility index (PI), and resistive index (RI) were measured from the middle cer

Temporary frame coil technique for ruptured vertebral artery dissection involving major branch presentation: illustrative cases.

BACKGROUND: Endovascular internal trapping is the first-line treatment for ruptured vertebral artery dissection (VAD). However, when critical branches such as the posterior inferior cerebellar artery (PICA) or the anterior spinal artery (ASA) are involved with the dissected segment, the risk of surgery-related ischemia is high. OBSERVATIONS: Two patients with ruptured VAD involving critical branches were treated using the temporary frame coil technique. In the first case, a distal temporary frame coil protected the PICA origin during proximal coil embolization and was subsequently retrieved.

Cadaveric Feasibility of Robot-Assisted V3-Radial Artery-P2 Bypass.

BACKGROUND AND OBJECTIVES: Giant fusiform and dolichoectatic aneurysms of the basilar trunk and vertebrobasilar junction remain among the most challenging cerebrovascular lesions. In selected cases, flow-reversal strategies combined with posterior circulation revascularization can be considered; however, V3-radial artery (RA)-P2 bypass requires deep microvascular suturing within a narrow corridor, limiting its dissemination. The aim of this study is to evaluate the technical feasibility of robot-assisted posterior circulation bypass using the da Vinci Xi system in a fresh cadaveric model. MET

High-resolution cone-beam CT (DynaCT Micro) enhances preoperative and intraoperative visualization of intracranial perforator arteries.

BACKGROUND AND PURPOSE: Preservation of perforator arteries during cerebrovascular interventions remains challenging due to their submillimeter caliber and variable anatomy. High-resolution cone-beam computed tomography (HR-CBCT) offers improved resolution compared with conventional CT angiography (CTA), yet its utility for perforator assessment has not been systematically evaluated. This study assessed HR-CBCT for visualization of intracranial perforators in both preoperative and intraoperative contexts. METHODS: Imaging from 20 patients who underwent aneurysm clipping, arteriovenous malform

Investigation of Contrast Retention in PCOM Aneurysms as a Possible Marker for Instability and Rupture.

Identification of cerebral aneurysms at risk of destabilization and rupture remains challenging. Although flow stagnation has been recognized as a risk factor, the underlying mechanisms and its predictive power are poorly understood. The purpose was to investigate possible associations between flow stagnation determined from computational fluid dynamics (CFD)-based virtual angiograms and aneurysm instability and rupture. A total of 312 aneurysms at the posterior communicating (PCOM) artery were analyzed with image-based CFD. Virtual angiograms were constructed by simulating contrast injections

The enigmatic occipital emissary vein, foramina, and canals: anatomical study with application to skull base surgery.

Compared with mastoid emissary veins, occipital emissary veins have been less studied, and published descriptions of their foramina are inconsistent. This anatomical investigation aimed to clarify their prevalence, morphology, and relevance to skull base surgery. Two hundred fifty adult skulls were examined for internal and external occipital foramina. Selected samples were sagittally sectioned through the foramina. Ten latex-injected adult cadaveric heads were dissected, then sagittally cut to visualize intradiploic and intracranial venous courses. Portions of vein and surrounding bone were s

Minimally Invasive Endovascular Treatment With Integrated Perioperative Management in a 97-Year-Old Patient With Subarachnoid Hemorrhage: A Distal Transradial Access Case Report.

Treatment decisions for subarachnoid hemorrhage (SAH) in nonagenarians are challenging due to the need to balance poor prognosis against treatment invasiveness. A 97-year-old woman with a premorbid modified Rankin Scale (mRS) score of 3 presented with acute-onset headache, dizziness, and vomiting. Computed tomography revealed SAH with intraventricular hemorrhage. Further imaging demonstrated a ruptured dissecting aneurysm of the left posterior inferior cerebellar artery (PICA) (World Federation of Neurosurgical Societies Grade 2, Hunt and Kosnik Grade 3, modified Fisher Group 4). Although cons

Flow Redistribution in Circle of Willis Branches After Flow Diversion: Impetus for Vascular Remodeling?

BACKGROUND AND OBJECTIVES: Flow diverters (FDs) can induce vascular remodeling in nontarget branches of the circle of Willis (CW), yet the hemodynamic drivers of these changes remain unclear. Using in vitro CW models, flow rate alterations were quantified across both target and contralateral CW branches after FD deployment. METHODS: Four 3-dimensional-printed CW models were created (standard and hypoplastic variants) and integrated into a physiological pulsatile flow loop. Flow rates and pressures were recorded in all major branches before and after sequential deployment of 3 concentric FDs.

Case Report: Vagus nerve stimulation in super-refractory status epilepticus: Delayed seizure control and focal cortical knife-blade atrophy.

Super-refractory status epilepticus (SRSE), defined as seizure activity persisting > 24 h despite anesthetic treatment, remains a therapeutic challenge with high morbidity and mortality. We report a 42-year-old man with Crohn's disease who developed SRSE following a right posterior cerebral artery infarction. Despite treatment with multiple antiseizure medications, immunotherapy, intravenous anesthetics, and barbiturate coma, seizures persisted for 80 days. A vagus nerve stimulator (VNS) was implanted on day 57 after status epilepticus onset. Clinical improvement began within days, and complet

Mechanical stress from the hyoid bone as a potential etiology of bony-related ischemic stroke secondary to carotid artery dissection: A case report and systematic review.

Ischemic strokes resulting from internal carotid artery (ICA) dissection are typically associated with trauma, connective tissue disorders, or spontaneous causes. However, rare mechanical factors-including well-documented compression by the styloid process and, more exceptionally, by the hyoid bone-have been identified as potential triggers of such injuries. The close proximity or elongation of the hyoid bone relative to the ICA may exert pressure on or cause injury to the arterial wall, leading to hemodynamic or embolic ischemia in the anterior cerebral circulation, or to a transient ischemic
