Anatomical and clinical factors associated with multiple-stage embolization of brain arteriovenous malformation as a stand alone treatment.

INTRODUCTION: Embolization is one of the modalities used to treat brain arteriovenous malformations (bAVMs), which often require a multiple-stage approach. Factors influencing the need for a multiple-stage embolization are poorly defined and not very well understood. The aim of this study was to establish those factors. PATIENTS AND METHODS: We analysed 117 consecutive patients undergoing embolization due to bAVM with intention to treat. Patients undergoing embolization prior to surgery and with history of previous interventions or stereotactic radiosurgery were excluded. We used univariate a

APOE4 negates the effects of ovarian hormones on cerebrovascular endothelial and mitochondrial function.

The APOEε4 allele and oestrogen deficiency independently predispose females to an increased risk of vascular and metabolic impairments, but their cerebrovascular effects are less understood. The purpose of this study was to determine the interaction between APOE genotype and oestrogen on cerebrovascular endothelial and mitochondrial function. We studied young female homozygous APOEε3 and APOEε4 mice (n = 19-20/group; ∼6 months old) that were fed a high-fat diet and were ovariectomized (OVX), OVX and supplemented with 17β-oestradiol, or left intact. In APOEε3 mice, OVX was associated with impai

Asymmetrical Basilar Tip Fusion and Selected Posterior Circulation Variants: A Comparative 3D Angiography Study in Aneurysm and Clinical Reference Cohorts.

BACKGROUND AND PURPOSE: The configuration of the basilar tip varies from symmetric to markedly asymmetric fusion of the P1 segment of the posterior cerebral artery-superior cerebellar artery complexes. Prior studies have focused mainly on aneurysm cohorts, limiting assessment of whether these anatomical patterns are enriched relative to background anatomy. We compared selected basilar-tip and posterior circulation variants between patients with basilar apex or basilar artery-superior cerebellar artery aneurysms and a clinical reference angiographic cohort without these aneurysms. METHODS: We

Retrospective study on hemodynamic and morphological characteristics of posterior communicating artery aneurysms associated with fetal-type posterior cerebral artery and their correlation with rupture risk.

The fetal-type posterior cerebral artery (fPCA) is a prevalent anatomical variant that may influence the hemodynamics and shape of posterior communicating artery aneurysms (PCoAAs). Nonetheless, its involvement in aneurysm rupture remains contentious. This study investigated hemodynamic and morphologic differences between PCoAA with and without fPCA and assessed the predictive value of inflow angle on the risk of rupture. We conducted a retrospective analysis of 80 PCoAA patients diagnosed via digital subtraction angiography, sourced from the database of the Second Hospital of Hebei Medical Un

Case Report: Structured MRI assessment of posterior thalamic infarction in a distribution compatible with posterior choroidal artery territory presenting as Déjerine-Roussy syndrome in an adolescent: differentiating arterial ischemia from venous thrombosis and thalamic neoplasm.

BACKGROUND: Isolated thalamic lesions with post-contrast T1 hyperintensity in children may mimic arterial infarction, cerebral venous thrombosis, inflammatory disease, or neoplasm. Structured interpretation of standard magnetic resonance imaging (MRI) sequences, particularly diffusion-weighted imaging/apparent diffusion coefficient (DWI/ADC) correlation and interval follow-up, can help distinguish these entities. Posterior thalamic infarction in a distribution compatible with posterior choroidal artery territory is rare in pediatric patients. CASE PRESENTATION: A previously healthy 15-year-ol

Prognostic value of modified magnetic resonance arterial and venous collateral scores in unilateral chronic middle cerebral artery occlusion.

OBJECTIVE: To investigate the value of modified regional leptomeningeal collateral (rLMC) scores based on high-resolution 3D time-of-flight magnetic resonance angiography with magnetization transfer contrast (MRA-MTC-rLMC) and susceptibility-weighted angiography (SWAN-rLMC) in assessing collateral circulation and predicting clinical outcomes in patients with unilateral chronic middle cerebral artery occlusion (CMCAO). METHODS: In this retrospective analysis of 39 CMCAO patients, we developed enhanced rLMC scores by incorporating the posterior cerebral artery territory and deep medullary veins

Flow Diversion in Fetal-Type Posterior Communicating Artery Aneurysms: Anatomical Challenges and Treatment Strategy.

Fetal-type posterior communicating artery (PcomA) aneurysms remain a difficult subgroup in the flow-diverter era. In these lesions, the PcomA supplies a substantial portion of the posterior cerebral artery territory because the ipsilateral P1 segment is absent, hypoplastic, or functionally insufficient. The branch therefore functions as a major outflow channel, and persistent distal demand may maintain circulation through the aneurysm-neck complex after flow diversion. This review summarizes the anatomical basis of fetal-type PcomA aneurysms and the current evidence for clipping, coiling, intr

Repeated ethanol exposure in female rats impairs cerebral endothelial function and increases cerebrovascular stiffness.

There are several lifestyle factors known to increase the risk for age-related cognitive decline and dementia, including chronic alcohol consumption and alcohol use disorder. Chronic alcohol use in adulthood causes decreased gray matter and atrophy in brain regions central to learning and memory, including the hippocampus. Further, people with alcohol use disorder have a high incidence of cerebral white matter hyperintensities, the hallmark of cerebral small vessel disease, suggesting a link between alcohol consumption and cerebrovascular disease. However, how ethanol exposure impacts the cere

Anatomic variations in basilar artery termination: a systematic review and meta-analysis with presentation of an illustrative case.

PURPOSE: Despite extensive literature on anatomical variations of the basilar artery, evidence on its termination patterns is limited. We sought out to systematically pool the prevalence of basilar artery termination variations and describe their neurointerventional implications with an illustrative case. METHODS: We conducted a systematic review and meta-analysis of human basilar artery termination patterns in accordance with PRISMA guidelines. PubMed, Ovid MEDLINE, Web of Science, and Scopus were systematically searched. A risk of bias assessment was performed using the JBI critical apprais

Clinical Efficacy of Onyx Embolization in the Treatment of Distal Intracranial Microaneurysms.

OBJECTIVE: To investigate the feasibility of Onyx embolization in the treatment of intracranial distal microaneurysms. METHODS: Data of 14 patients (5 men and 9 women, aged 51 ± 9 years) who were diagnosed by Digital Subtraction Angiography (DSA) and underwent Onyx embolization were retrospectively analyzed. Anterior circulation was observed in 6 cases, posterior circulation in 8 cases, and rupture in 8 cases. All cases were evaluated by a multidisciplinary team (MDT), and Onyx embolization was performed after excluding the feasibility of conventional coil embolization and surgical clipping.

Microsurgical Anatomy and Surgical Relevance of the Internal Frontal Arteries: A Cadaveric Anatomic Study.

PURPOSE: To investigate the microsurgical anatomy, morphologic variability, and morphometric features of the anterior (AIFA), middle (MIFA), and posterior (PIFA) internal frontal arteries. METHODS: Seventeen formalin-fixed, latex-injected cadaveric heads (34 hemispheres) were examined using microsurgical techniques. Five hemispheres were excluded due to inadequate vascular filling or preservation, resulting in a final sample of 29 hemispheres. RESULTS: The AIFA, MIFA, and PIFA were identified in 96.5%, 93.1%, and 96.5% of hemispheres, respectively. The most common origin of the AIFA and MIFA

The Complementary Role of Size and Morphology in Predicting Intracranial Aneurysm Rupture: An Intrapatient Matched Study.

BACKGROUND: Managing patients with multiple intracranial aneurysms remains challenging, both for identifying the ruptured aneurysm in the acute phase and for deciding whether to treat unruptured bystanders later. As traditional risk scores rely largely on aneurysm size, this study used an intrapatient matched design to better assess the anatomical determinants of rupture beyond patient-specific factors. METHODS: A retrospective analysis was conducted on 55 patients harboring a total of 136 intracranial aneurysms. To inherently control systemic risk factors, the ruptured index aneurysm was dir

Frontobasal bridging vein rerouting for microsurgical clipping of a posterior cerebral artery aneurysm through anterior temporal approach: A technical note.

BACKGROUND: Posterior cerebral artery (PCA) aneurysms are extremely rare and challenging to treat because of their deep location and complex morphology. We describe a case of an enlarging thrombosed PCA aneurysm successfully treated with direct neck clipping through an anterior temporal approach (ATA) incorporating frontobasal bridging vein (FBBV) sacrifice and venous rerouting. METHODS: A 48-year-old man was incidentally found to have a right PCA aneurysm during routine magnetic resonance imaging (MRI). Over an 8-year period, the aneurysm progressively enlarged and developed perifocal edema

Hemorrhagic oculomotor nerve schwannoma mimicking a completely thrombosed posterior circulation aneurysm in an adolescent: illustrative case.

BACKGROUND: Oculomotor nerve schwannomas are rare intracranial tumors, particularly in adolescents. When associated with intralesional hemorrhage, they may radiologically mimic completely thrombosed aneurysms of the posterior circulation, creating diagnostic and operative challenges. OBSERVATIONS: A 15-year-old girl presented with progressive headache and acute partial left third nerve palsy. MRI demonstrated a heterogeneous hemorrhagic mass in the left interpeduncular cistern closely abutting the superior cerebellar artery-posterior cerebral artery complex. MRA revealed no aneurysm. Two mont

Endovascular recanalisation for acute basilar artery occlusion with extended time window of 24-72 hours (ANGEL-BAO): study protocol for a randomised controlled trial.

BACKGROUND: Acute basilar artery occlusion (BAO) carries a disability rate as high as 80% and its optimal treatment window remains a clinical controversy. While landmark studies such as ATTENTION (Endovascular Treatment for Acute Basilar-Artery Occlusion) and BAOCHE (Basilar Artery Occlusion Chinese Endovascular Trial) have confirmed the efficacy of endovascular treatment (EVT) within 24 hours, there is still a lack of high-level evidence supporting intervention strategies for patients beyond this time window. Role of Perfusion Parameters on Outcomes and Safety of Endovascular Therapy in Posterior Cerebral Artery Stroke.

BACKGROUND: The optimal management of isolated posterior cerebral artery occlusion (iPCAO) remains unclear. We investigated whether baseline perfusion imaging parameters are associated with clinical outcomes and whether they modify the association between endovascular therapy (EVT) and outcomes in iPCAO. METHODS: This prespecified secondary analysis of the international, multicenter, observational PLATO (Posterior Cerebral Artery Occlusion) registry (35 centers, 10 countries, 2015-2025) included consecutive adults with unilateral iPCAO and baseline perfusion imaging (computed tomography or ma

Flow diversion in proximal superior cerebellar artery aneurysms: Systematic review of clinical outcomes and exploratory hemodynamic analysis.

Flow diverters (FDs) are emerging as a treatment strategy for superior cerebellar artery aneurysms (SCAs) despite the limited data on their efficacy and safety. This study evaluated flow diversion (FD) treatment for proximal SCA aneurysms through a single center analysis, systematic review of the literature, and exploratory computational fluid dynamics (CFDs) simulations. Retrospective review of patients with proximal SCA aneurysms treated with FDs at our neurovascular service between 2013 and 2024. We then conducted a systematic review of the literature using the EMBASE, PubMed, and Web of Sc

Establishment of a machine learning prediction model for Wallerian degeneration after ischemic stroke.

BACKGROUND: Wallerian degeneration (WD) is a common and clinically significant complication of ischemic stroke (IS). Due to the multifactorial and nonlinear characteristics of its underlying mechanisms, accurately identifying high-risk patients early remains challenging. This study aimed to develop and validate an interpretable machine learning (ML) model to predict WD after IS. METHODS: We retrospectively analyzed clinical data from 269 patients with IS, all admitted to the Xinhua Hospital of Dalian University. The patients were randomly divided into a training set (70%) and an internal vali

Aneurysm at an Extremely Proximal Posterior Communicating Artery Origin Treated With Flow Diverter-Assisted Coiling: A Case Report.

The posterior communicating artery (Pcom) arises from the supraclinoid internal carotid artery (ICA), just proximal to the anterior choroidal artery. Although caliber variations of the Pcom are common, marked proximal displacement of its origin is extremely rare. We report a case of an aneurysm initially diagnosed as a paraclinoid ICA aneurysm that was subsequently identified as a branch-incorporated internal carotid-posterior communicating artery (IC-PC) aneurysm associated with an aberrant Pcom arising from the ICA siphon proximal to the ophthalmic artery. A man in his 60s underwent magnetic
