Intranasal administration of enalapril confers acute neuroprotection by mitigating mitochondrial dysfunction and apoptosis in a rat model of ischemic stroke.

BACKGROUND: Stroke remains a leading cause of mortality and long-term disability worldwide, with effective treatments hindered by its complex pathophysiology. This study is the first to explore the neuroprotective potential of intranasal enalapril, in mitigating ischemic stroke outcomes through its influence on mitochondrial function. METHODS: Male Sprague-Dawley rats were subjected to middle cerebral artery occlusion (MCAO), followed by the intranasal administration of enalapril (0.03 mg/kg) post-ischemia. Neurological function was evaluated after 24 hours of reperfusion through the modified

[Emerging Therapeutic Strategies for Central Nervous System Regeneration].

Reperfusion therapy has markedly improved outcomes after acute ischemic stroke; however, many patients remain ineligible for treatment or sustain irreversible brain injury despite successful recanalization. Regenerative medicine therefore represents an important strategy for restoring damaged neural tissue and promoting functional recovery. Stem cell therapies may exert therapeutic effects through multiple mechanisms, including cell replacement, secretion of neurotrophic factors, modulation of inflammation and immune responses, angiogenesis, and intercellular transfer of mitochondria and other

Development of the LARyNGTH score to predict central neurogenic respiratory failure in lateral medullary infarction.

BACKGROUND: Central neurogenic respiratory failure (CNRF) is a fatal complication associated with lateral medullary infarcts (LMI). Its detection may be critical for timely intervention and to reduce the risk of mortality. This study aimed to identify the clinical predictors of CNRF in patients with LMI. METHODS: This study included patients with LMI who were admitted to a university hospital between 2010 and 2022. CNRF was defined as unexpected intubations due to hypercapnia, cardiopulmonary resuscitation owing to respiratory causes, or clinical detection of CNRF. Clinical, neurological, and

Stage-Associated Efficacy of Systemically Administered Neural Stem Cell-Derived Exosomes in Ischemic Stroke through the Prevention of Cx43 Degradation.

Ischemic stroke pathology is highly dynamic, making the optimal timing for biological interventions highly elusive. This study investigated the stage-associated therapeutic efficacy of neural stem cell-derived exosomes (NSC-Exos) for cerebral ischemia-reperfusion. The in vivo evaluations identified a restricted therapeutic window closely associated with the temporal evolution of the ischemic microenvironment. Early exosome intervention (4 or 24 h post-ischemia) significantly reduced infarct volume, restored blood-brain barrier integrity, and improved neurological functional recovery. In contra

A Review of the Molecular Evidence for Near-Infrared Light in Astrocyte-Mediated Neurorehabilitation.

Near-infrared (NIR) light represents a promising, noninvasive therapeutic for neurological disorders, predicated on its capacity to modulate astrocyte function. As the primary glial population in the central nervous system, astrocytes are indispensable for maintaining neuronal homeostasis, regulating glutamate metabolism, and supporting metabolic energetics. This review synthesizes recent advancements in NIR-driven neurorehabilitation, elucidating the molecular signaling cascades and transduction pathways activated within the astrocytic network. We specifically examine translational applicatio

Mechanisms of ubiquitination in central nervous system injury and potential therapeutic targets.

Ubiquitination is a highly conserved and reversible post-translational modification that regulates protein stability, localization, activity, and intracellular signaling through the coordinated actions of the ubiquitin-proteasome system. In recent years, increasing evidence has highlighted the critical role of ubiquitination in maintaining central nervous system proteostasis and regulating pathological responses following central nervous system injury. This review comprehensively summarizes recent advances in research on ubiquitination in traumatic brain injury, spinal cord injury, stroke, and

Harnessing BMSCs for Ischemic Stroke: Decoding Multimodal Repair and Navigating the Path to Clinical Translation.

Ischemic stroke (IS) is characterized by a high incidence rate, high disability rate and high recurrence rate. The existing treatment methods have limitations such as a narrow time window and limited recovery of neurological function. Bone marrow mesenchymal stem cells (BMSCs) have become a research hotspot in the regenerative treatment of IS due to their multi-directional differentiation potential, paracrine regulation and immune regulation characteristics. This review explores the mechanism of action, strategies for enhancing therapeutic efficacy and the progress of clinical transformation o

MARCKS as a mediator and target for pathological tunneling nanotubes between patient-derived glioblastoma cells and astrocytes.

During glioblastoma (GBM) progression, therapeutic resistance is influenced by a heterogeneous network of tumor- and tumor-promoting subpopulations within the tumor microenvironment. Therapeutic interventions against tumor cells, such as chemoradiotherapy, can further induce adaptive defense mechanisms mediated by sophisticated intercellular communication and aberrant signaling pathways. Tunneling nanotubes (TNTs) have emerged as key mediators of this process, promoting tumor cell survival through metabolic rescue while enabling the recruitment and reprogramming of surrounding normal cells int

Phosphorylation-Facilitated CKB Lactylation At K11 By GCN5 Enhances Creatine Kinase Activity and Mitigates Neuronal Damage After Cerebral Ischemia-Reperfusion.

Ischemia-reperfusion (I/R) injury in stroke causes severe neuronal damage through oxidative stress and metabolic dysfunction. Beyond its metabolic role, lactate can induce lysine lactylation, but its impact on neuroprotection remains unclear. Here, we employed integrative proteomic, lactylomic, and phosphoproteomic profiling in a murine I/R model and identified creatine kinase B-type (CKB) as a dual target of K11 lactylation and S199 phosphorylation. These modifications were found to enhance CKB enzymatic activity, promote phosphocreatine metabolism, suppress reactive oxygen species, and suppo

Acid-sensing ion channels in ischemic stroke: amplifying and constraining neuronal injury in a phase-specific framework.

For decades, ischemic neuronal death was understood largely as a story about glutamate. Acid-sensing ion channels (ASICs) have revised that account. When cerebral blood flow stops, the penumbra can develop prolonged extracellular acidosis, and experimental studies implicate ASIC1a in mediating sufficient Ca²⁺ entry to injure neurons even when glutamate receptors are blocked, a contribution shaped by channel expression, subunit composition, and the intracellular signaling context. However, sustained acidosis does not necessarily imply sustained channel opening, given ASIC1a desensitization. Bes

Recent advances in research on novel therapeutic mechanisms and strategies for exosome-based treatment of ischemic stroke.

Ischemic stroke (IS) accounts for over 80% of all stroke cases, featuring high disability, mortality, and recurrence rates. Conventional intravenous thrombolysis and mechanical thrombectomy are limited by narrow therapeutic windows, reperfusion injury, and the blood-brain barrier (BBB), which impedes the entry of neuroprotective agents into the brain parenchyma. Exosomes (Exos) are cell-secreted nanoscale extracellular vesicles (EVs) that carry miRNAs, proteins, and lipids, with low immunogenicity, BBB permeability, and ease of genetic modification, making them promising cell-free therapeutic

Neural Repair Therapies After Stroke: I. Therapeutic Advances.

Stroke is a leading cause of disability worldwide. Neural repair therapies, distinct from acute stroke therapies, target surviving neural elements to promote plasticity after the acute injury has occurred. Repair therapies encompass a broad and rapidly evolving therapeutic landscape, ranging from behavioral training, drugs, and biological agents to neural stimulation. In this review, we highlight major advances across these therapeutic categories, focusing on the evidence base and readiness for clinical adoption. Intensive behavioral interventions, including constraint-induced movement therapy

Alzheimer's disease target and drug discovery by leveraging multiomics and electronic health data.

Despite prolific genomic data from individuals with Alzheimer's disease (AD), translation into treatments has lagged. Here we applied Mendelian randomization to multiple AD genome-wide association studies and functional multiomics datasets to nominate druggable targets, identifying 19 targets for individuals of European ancestry and 7 for African ancestry. This included epoxide hydrolase 2 (EPHX2), for which we identified a protective missense variant (p.Arg287Gln). Experiments in induced pluripotent stem cell-derived neurons and cerebral organoids from patients with AD validated the protectiv

Who owns what? Comprehension of gender information in German possessive pronouns and determiners in aphasia.

Cross-linguistic studies have demonstrated that people with (agrammatic) aphasia often exhibit difficulties in pronoun comprehension, but the role of gender marking has been largely overlooked and prior research has concentrated on only certain pronoun types. This study focused on gender cues in an understudied pronoun type, possessives, in German. German possessive determiners (e.g., seinmasc-efemGabelfem 'his fork') and possessive pronouns (e.g., ihrfem-efem 'hers') comprise two distinct gender cues, with a stem and a suffix providing information about who (possessor) is possessing what (pos

Comparative efficacy and safety of seven stem cell therapies for ischemic stroke: an updated network meta-analysis.

Cell-based therapies represent a promising treatment for ischemic stroke, but the comparative efficacy and safety profiles among stem cell types have not been systematically compared. This network meta-analysis evaluates different stem cell therapies for ischemic stroke. We conducted a literature search using PubMed, the Cochrane Library, and Web of Science up to May 2025. The primary efficacy outcomes included the National Institutes of Health Stroke Scale, the modified Rankin Scale, the Barthel Index, the Fugl-Meyer Assessment, and infarct volume. Safety outcomes included serious adverse eve

Loss of DOT1L disrupts neuronal transcription and leads to a neurodevelopmental disorder.

Individuals with monoallelic gain-of-function variants in the histone lysine methyltransferase DOT1L display global developmental delay and varying congenital anomalies. However, the impact of monoallelic loss of DOT1L remains unclear. Here, we sought to define the effects of partial DOT1L loss by applying bulk and single-nucleus RNA-sequencing, ChIP-sequencing, imaging, multielectrode array recordings and behavioural analysis of zebrafish and multiple mouse models. We present a cohort of 16 individuals (12 females, 4 males) with neurodevelopmental disorders and monoallelic DOT1L variants, inc

hUMSC-derived exosomes alleviate hypoxic cerebrovascular injury via AMPK/NLRP3-mediated pyroptosis suppression and mitochondrial protection.

As the most prevalent cause of death worldwide, ischemic stroke urgently requires innovative therapeutic strategies. The present study demonstrated the therapeutic potential of human umbilical cord-derived mesenchymal stem cell-derived exosomes (hUMSC-Exos) in ameliorating hypoxia-induced cerebrovascular endothelial dysfunction through modulation of the AMPK/NLRP3 signaling pathway. Bioinformatics analysis of DisGeNET and exosomal cargo databases revealed 283 overlapping cerebral ischemia-related genes, implicating hUMSC-Exos in inflammatory regulation. In vitro experiments showed that hUMSC-E

Minimal invasive surgery for primary brainstem intracerebral hemorrhage: a critical mini review.

CONTEXT: Intracerebral hemorrhage (ICH) accounts for half of stroke attributable deaths and two thirds of DALYs loss. Past well designed trials (2005 STICH, 2013 STICH 2, 2019 MISTIE 3, 2024 ENRICH, 2024 SWITCH) did not demonstrate benefits of surgery, aside from ENRICH which suggested improved functional outcome thanks to minimal invasive surgery for lobar ICH, but not for deep seated thalamic ICH. Since 2020, retrospective studies suggest great functional benefit of minimal invasive surgery for brainstem ICH. MATERIALS AND METHODS: Review of weel designed trials and guidelines regarding sur
