Molecular Pathophysiology of Ischemic Stroke and Mesenchymal Stem Cell Based Therapeutic Strategies for Neural Tissue Regeneration: A Pre-clinical and Clinical Perspective.

Ischemic stroke (IS), also referred to as cerebral ischemia, is a neurological condition accompanied by long term or permanent physical disability. Various molecular mechanisms such as inflammation, oxidative stress, blood brain barrier (BBB) disruption, energy depletion, mitochondrial dysfunction etc., contribute to its pathophysiology and trigger death of neural tissues. Currently, there are limited therapeutic options for its treatment. Although, thrombectomy or thrombolytic drugs are available, but only beneficial for the management of acute phase and do not address the neurodegenerative a

From cells to vesicles: mechanisms and therapeutic promise of cell- and exosome-based interventions in ischemic stroke.

With the growing burden of ischemic stroke as a leading cause of mortality and long-term disability worldwide, the demand for innovative and effective therapeutic strategies has become increasingly urgent. Traditional interventions offer limited neurorestorative potential, prompting a shift toward regenerative approaches. Cell-based and exosome-based therapies have attracted significant interest due to their potential to regulate inflammation, safeguard neural tissue, and support recovery following ischemic injury. Mesenchymal stem cells (MSCs) exert neuroprotective effects through paracrine s

Aspartame exacerbates cerebral ischemia-reperfusion injury via mitochondrial dysfunction and ERK/CREB1 pathway suppression.

BACKGROUND: Aspartame, a widely used non-nutritive sweetener, has been associated with potential neurotoxicity. However, it remains unclear whether aspartame consumption can exacerbate cerebral ischemia-reperfusion injury (CIRI), a major contributor to stroke outcomes, and the underlying mechanisms are poorly understood. METHODS: In vivo, male C57BL/6 J mice were given free access to 0.1% or 0.2% (w/v, equivalent to human acceptable daily intake level) aspartame, for 7 days, followed by bilateral common carotid artery occlusion (BCCAO) to induce CIRI. In vitro, hippocampal neural stem cells (

Potential of intranasal delivery of human mesenchymal stem cells and extracellular vesicles for stroke therapy.

Cerebral ischemic stroke, caused by interrupted cerebral blood flow, remains a leading cause of mortality and long-term disability worldwide. Current FDA-approved therapies-intravenous tissue-type plasminogen activator (tPA) and mechanical thrombectomy-are constrained by narrow time windows (4.5-24 h) and limited accessibility. Mesenchymal stem cells (MSCs) have emerged as promising candidates for neurorestoration, yet their therapeutic efficacy is hindered by poor blood-brain barrier (BBB) penetration and systemic entrapment. Increasing evidence indicates that MSCs exert their therapeutic eff

Human stem cell-derived neurons establish functional inhibitory-excitatory cortical circuits in a chimeric transplantation model.

Excitation/inhibition balance is crucial to homeostatic brain function and disrupted in several neurodevelopmental and neurodegenerative diseases. Cortical inhibitory neurons derived from the medial ganglionic eminence (MGE) provide input to glutamatergic projection neurons, an essential function in neural processing. While neural grafts for stroke improve motor function following engraftment, these grafts typically contain only excitatory neurons (and glia), and little effort has been directed at understanding the role of inhibitory neurons in this context. Here, we generate a channelrhodopsi

Spatiotemporal dynamics of brain pericytes in ischemic stroke.

Pericytes constitute a vital component of the neurovascular unit (NVU) and play a pivotal role in the pathophysiology of ischemic stroke (IS). They perform a variety of critical functions, involving regulating cerebral blood flow (CBF), modulating inflammatory responses, exhibiting stem cell-like properties, and maintaining the integrity of the blood-brain barrier (BBB). During the hyperacute and acute phases of IS, pericytes undergo contraction, contributing to the no-reflow phenomenon (NRP) and compromising BBB integrity. Concurrently, their activation exacerbates neuroinflammation and secon

Rapamycin improves memory and blood pressure stability in asymmetrical cortico-brainstem connectivity following stroke.

Emerging reports suggest that patients with ischemic stroke involving the insular cortex (IC), particularly in the right hemisphere, may lead to paradoxical reduction in blood pressure-a condition associated with worse outcomes, including increased mortality and cognitive decline-whereas elevated blood pressure is more commonly observed during the acute phase of stroke. To investigate this lateralized effect and its underlying mechanisms, we established focal ischemic stroke models targeting either the left or right IC in male rats. In this study, we demonstrated that infarction of the right i

Duloxetine ameliorates cerebral ischemic injury by inhibiting autophagy.

Ischemic stroke is a severe medical condition characterized by diminished blood flow to the brain, resulting in a shortage of oxygen and nutrients. During ischemia, neurons surrounding the cerebral infarct initiate macroautophagy. However, the implications of this activation for neuronal cell survival are still debated. The identification of new autophagy modulators could aid in understanding autophagy's role in brain ischemia and lay the groundwork for innovative therapeutic strategies aimed at minimizing brain damage in this life-threatening neurological emergency. In this study, we develope

The antiaging protein Klotho is a key factor in susceptibility to cerebral ischemia.

Ischemic cerebrovascular events are among the leading causes of death and disability worldwide, disproportionately affecting the elderly due to vascular aging, oxidative stress, and chronic inflammation. During cerebral ischemia, oxygen and glucose deprivation trigger a cascade of pathological events, including excitotoxicity, mitochondrial dysfunction, blood-brain barrier disruption, and neuroinflammation. Emerging evidence identifies Klotho as a multifunctional protein with anti-aging, antioxidant, and anti-inflammatory properties that may confer neuroprotection against ischemic injury. Agin

Adipose-derived stem cell-conditioned medium mitigates ischemia-induced neuronal injury via the JAK1/STAT3 signaling pathway.

INTRODUCTION: Adipose-derived stem cells (ADSCs) demonstrate therapeutic potential for ischemic stroke, primarily through paracrine actions. However, the specific intracellular signaling pathways underlying these benefits remain unclear. This study investigates the critical role of JAK1/STAT3 signaling in neuroprotection mediated by ADSC-conditioned medium (ADSC-CM). METHODS: We employed a dual-model approach. In vivo, rats subjected to transient middle cerebral artery occlusion (tMCAO) received intravenous ADSC-CM or vehicle at 2, 24, and 48 h post-ischemia, with or without the JAK1 inhibito

Astrocytic Mitochondria Transplantation Rescues Neuron Loss and Dendritic Injuries in Acute Cerebral Ischemic Stroke Mouse Model by Flexibly Regulating Mitochondria Dynamics.

OBJECTIVE: Cerebral ischemic stroke causes neuronal oxygen/energy deprivation, disrupting mitochondrial function including reduced membrane potential and bioenergetics, exacerbating neuronal injury. Mitochondrial defects are, therefore, a central neuropathological node and potential therapeutic target. Previous studies have shown that mitochondria transplantation rescued infarction in cerebral ischemic stroke. However, interactions between transplanted and endogenous mitochondria remain unclear. Here, we proposed astrocytic mitochondria as the optional donor for mitochondria transplantation in

The Effects and Mechanisms of Traditional Chinese Medicine and Extracts on Stem Cells and the Application of Stem Cells to Treat Disease: A Review.

This review examines the synergistic effects and mechanisms of Traditional Chinese Medicine (TCM) and its extracts on mesenchymal stem cells (MSCs), with a focus on enhancing MSC proliferation, differentiation, and immunomodulation for therapeutic applications. A comprehensive analysis of studies from the past five years was conducted, evaluating interactions between TCM/extracts and MSC types (bone marrow-, adipose-, and umbilical cord-derived MSCs) through molecular pathways (e.g., PI3K/Akt, Wnt/β- catenin, TGF-β/Smad) and microRNA regulation. In vitro, in vivo, and clinical trial data were

Phosphorylation and DNA damage resolution coordinate SOX2-mediated reprogramming in vivo.

The stem cell factor SOX2 can reprogram resident glial cells into neurons in the adult mammalian central nervous system, but the molecular mechanisms underlying this process remain poorly understood. Here, we show that both SOX2 phosphorylation and the PRKDC-dependent nonhomologous end joining (NHEJ) pathway are essential for SOX2-mediated in vivo glia-to-neuron reprogramming. A phospho-mimetic SOX2 mutant significantly enhances reprogramming output without altering neuronal fate. Conversely, loss of PRKDC or knockdown of core NHEJ components KU80 and LIG4 abolishes reprogramming. Notably, p53

Intracellular mitochondrial transfer in ischemic stroke: Mechanisms and therapeutic application.

BACKGROUND: Current therapeutic models for ischemic stroke (IS) are shifting from a narrow focus on neuroprotection to a broader concept of cytoprotection. This new paradigm emphasizes rescuing damaged brain cells and maintaining their structural and functional integrity through organelle transfer between healthy and damaged cells. Mounting evidence have supported that intracellular mitochondrial transfer is an intrinsic response to IS, playing a critical role in mitigating neural damage. Consequently, mitochondrial transplantation from stem cell is emerging as a therapeutic avenue for IS. AI

Animal Models for Long-Term Functional Recovery and Rehabilitation After Ischemic Stroke: Beyond Acute Neuroprotection.

Ischemic stroke is one of the leading causes of long-term disability worldwide. Although acute neuroprotection strategies have been extensively studied, the mechanisms underlying long-term functional recovery after ischemic stroke remain incompletely understood. Animal models have played an important role in identifying key processes such as neural plasticity, angiogenesis, blood-brain barrier repair, and regulation of neuroinflammation during the recovery phase. These models enable the investigation of complex, time-dependent biological cascades that are difficult to examine directly in clini

GFAP+ FOXF2+ ependymal cells promote blood-brain barrier repair via DLL4-NOTCH signaling after neural injury.

Ependymal cells in the adult ventricular-subventricular zone are increasingly recognized for functions extending beyond cerebrospinal fluid dynamics; however, their identity and functional specialization remain incompletely understood. While ependymal cells (EP) have been implicated in interactions with the stem cell niche and the vasculature, their role in repair processes following neural injury remains elusive. In this study, we employed region-specific single-cell transcriptomics of the subventricular zone (SVZ) and ipsilesional peri-infarct territory in mice to identify a distinct subpopu

Microglial Inhibition Promotes Proliferation and Differentiation of Neural Stem Cells via STAT3/SDF-1/CXCR4 Signaling Pathway in Hypoxic-Ischemic Encephalopathy.

Neonatal hypoxic-ischemic encephalopathy (HIE) may induce substantial neuronal damage. In particular, an overactivation of microglia following HIE represents a pathogenically important process. Previous studies have shown that microglial inhibitors can exert neuroprotective effects in HIE; however, the specific mechanisms underlying these effects have not yet been elucidated. Ligation of the left common carotid artery and exposure to 5% O2 were utilized to produce an HIE model in rats. A number of experimental approaches were then used to determine the effect of a microglial inhibition, achiev

The Therapeutic Potential of TCM Polysaccharides in Neurological Disorders.

This review systematically evaluates the interactions between traditional Chinese medicine (TCM) polysaccharides and the blood-brain barrier (BBB), positioning BBB modulation as a central mechanistic framework to explain their neurotherapeutic potential. Unlike disease-specific prior works, it identifies BBB dysfunction as a unifying pathological bottleneck in central nervous system (CNS) disorders, offering a cohesive lens to interpret these polysaccharides' multitarget actions. We synthesize evidence on their structural characteristics and bioactivities, focusing on the regulation of tight j
