Reversible cerebral vasoconstriction syndrome as a rare postoperative complication after resection of a rosette-forming glioneuronal tumor: a case report and literature review.

INTRODUCTION: Reversible cerebral vasoconstriction syndrome (RCVS) is an uncommon postoperative complication, rarely reported after posterior fossa tumor resections. Rosette-forming glioneuronal tumor (RGNT) is an infrequent, low-grade neoplasm of the central nervous system. To our knowledge, no prior cases of RCVS following RGNT resection have been documented. CASE SUMMARY: We report a male patient in his 20s who underwent suboccipital craniotomy for resection of a cerebellar RGNT. Three weeks postoperatively, the patient presented with acute right hemiparesis and aphasia. Brain MRI demonstr

Three-dimensional Motion Capture Discriminates Extremely Mild Cerebellar Ataxia from Parkinsonism in the Upper Limbs.

Upper-limb motor symptoms in spinocerebellar degeneration (SCD) and Parkinson's disease (PD) frequently overlap, making clinical differentiation challenging in mild cases using the finger-to-nose test (FNT) or the diadochokinesis test (DDKT). This study aimed to identify quantitative kinematic markers that distinguish mild SCD from mild PD using three-dimensional motion capture. Twenty-six individuals per group-mild SCD, mild PD, and normal controls (NCs)--underwent motion analysis during FNT forward, FNT backward, and DDKT. Key parameters including segmental velocity ratios during FNT and rot

Neuromodulation for post-stroke aphasia rehabilitation: established cortical stimulation and emerging network-based targets.

Post-stroke aphasia is a disabling disorder of language and communication for which speech-language therapy remains the standard of care. Because behavioural treatment yields variable gains and many people retain chronic communication limitations, non-invasive neuromodulation has been investigated to increase the responsiveness of residual language networks to therapy. This narrative review critically integrates evidence available through July 2026 for repetitive transcranial magnetic stimulation (rTMS), theta-burst stimulation, transcranial direct current stimulation (tDCS), and emerging appr

Delirium after Aortic Arch Surgery with Hypothermic Circulatory Arrest: Clinical and Neuroimaging Insights from the GOT ICE Randomized Trial.

BACKGROUND: Postoperative delirium (POD) is a complication of arch surgery with hypothermic circulatory arrest (HCA) and may contribute to long-term cognitive decline, but prospective data are limited. We examined its incidence, predictors, and neurocognitive and neuroimaging correlates. METHODS: The Cognitive Effects of Body Temperature During Hypothermic Circulatory Arrest trial compared deep versus moderate hypothermia during elective arch repair. Delirium was assessed preoperatively and on postoperative days 1-3 using the Confusion Assessment Method. Thirty-day outcomes, cognition, qualit

Structural and metabolic imaging after mechanical thrombectomy for acute ischemic stroke: A hybrid 18F-FDG PET/MRI pilot study.

Hybrid 2-[18F]-fluoro-2-deoxy-D-glucose positron emission tomography/magnetic resonance imaging (18F-FDG PET/MRI) enables simultaneous, single-session assessment of structural and metabolic stroke damage. We prospectively studied 10 consecutive patients (6 women; mean age 71.6 ± 11.2 years) with anterior-circulation stroke due to middle cerebral artery (MCA) occlusion who underwent successful mechanical thrombectomy (MT). Structural damage was graded with the Alberta Stroke Program Early CT Score (ASPECTS) on admission and 24-h non-contrast CT and on diffusion-weighted imaging (DWI) and fluid-

Neural Repair Therapies After Stroke: I. Therapeutic Advances.

Stroke is a leading cause of disability worldwide. Neural repair therapies, distinct from acute stroke therapies, target surviving neural elements to promote plasticity after the acute injury has occurred. Repair therapies encompass a broad and rapidly evolving therapeutic landscape, ranging from behavioral training, drugs, and biological agents to neural stimulation. In this review, we highlight major advances across these therapeutic categories, focusing on the evidence base and readiness for clinical adoption. Intensive behavioral interventions, including constraint-induced movement therapy

Resting-state BOLD fMRI-derived cerebrovascular reactivity for detecting crossed cerebellar diaschisis.

PURPOSE: To study cerebellar cerebrovascular reactivity (CVR) derived from resting-state (RS) blood oxygen level-dependent (BOLD) as a potential imaging biomarker for crossed cerebellar diaschisis (CCD). METHODS: Thirty-nine patients with symptomatic unilateral cerebrovascular occlusion and eighteen age-matched healthy subjects underwent a simultaneous PET/MR scan. CCD was diagnosed as a contralateral cerebellar metabolic reduction present on 18F-FDG PET semi-quantitatively. RS BOLD-CVR maps were derived by extracting signals within specific frequency bands of RS BOLD. The cerebellar asymmetr

Acute Imbalance Syndrome vs Acute Vestibular Syndrome: Clinical and Topographical Differentiation.

BACKGROUND AND OBJECTIVES: Stroke is a major differential diagnosis in patients with acute vertigo/dizziness or imbalance. Current diagnostic algorithms such as head impulse test, gaze-evoked nystagmus, and test of skew are optimized for presentations with spontaneous nystagmus (SPN), traditionally called acute vestibular syndrome (AVS). However, in recent studies, a relevant proportion of patients with stroke-related vertigo/dizziness had no SPN, but rather postural or gait imbalance, a phenotype lately termed acute imbalance syndrome (AIS). We aimed to outline the pathoanatomical differences

Mechanical Thrombectomy for Acute Vertebrobasilar Occlusion Associated with Mycoplasma pneumoniae Pneumonia in a Child: A Case Report.

OBJECTIVE: Due to its rarity, high-quality evidence regarding mechanical thrombectomy for pediatric acute ischemic stroke remains limited. Mycoplasma pneumoniae is a common cause of community-acquired pneumonia in children, and a recognized but uncommon cause of pediatric ischemic stroke. Reports describing large-vessel occlusion associated with M. pneumoniae infection are rare, and the role of mechanical thrombectomy in this setting remains unclear. CASE PRESENTATION: A 5-year-old boy hospitalized for pneumonia developed a sudden onset of impaired consciousness and quadriplegia. Brain MRI de

Diabetes Insipidus as a Rare Complication of Metastatic Breast Cancer With Pituitary Metastases.

Diabetes insipidus (DI) is a rare but significant and life-threatening complication of metastatic breast cancer, typically arising when cancer spreads to the pituitary gland. This condition results from impaired secretion or action of antidiuretic hormone (ADH) or arginine vasopressin (AVP), leading to symptoms including excessive urination and thirst. The posterior lobe of the pituitary and the infundibulum are most susceptible to metastatic involvement due to their direct arterial blood supply, making them more accessible to circulating tumor cells. This phenomenon explains why DI can be a m

Clinical Characteristics of Japanese Cases of Anti-Metabotropic Glutamate Receptor Type 1 Antibody-Associated Cerebellar Ataxia.

Anti-metabotropic glutamate receptor type 1 (mGluR1) antibody-associated cerebellar ataxia is a rare autoimmune cerebellar ataxia. Although its clinical spectrum has been recognized mainly from Western cohorts, its features in other populations remain uncharacterized. We investigated the clinical characteristics of this disease in a Japanese cohort with reference to overseas cases. We retrospectively reviewed patients testing positive for anti-mGluR1 antibodies in our laboratory between 2018 and 2025, comparing their clinical, laboratory, and radiological profiles with reference to 40 previous

Clinical and MRI morphometric characterization of symptomatic cerebellar tonsillar ectopia.

BACKGROUND: Cerebellar tonsils normally extend up to 5 mm below the foramen magnum (FM). Some individuals with tonsillar ectopia (TE) measuring under 5 mm may present with symptoms of Chiari type 1 malformation (CM1). We analyzed MRI data of the posterior cranial fossa (PCF) in affected patients to assess TE's impact on clinical manifestations. METHODS: The retrospective study examined 83 adults with 2-4 mm symptomatic cerebellar TE without syringomyelia (SM), 16 with syndromic CM1 without SM, and 19 controls. Participants with TE were divided into three groups: syndromic (TEs, n = 15), incom

Cerebellar Theta-Burst Stimulation Induces Distinct Functional Connectivity Patterns in Post-Stroke Aphasia: A Randomized Controlled Trial.

BACKGROUND: Recent studies have identified the right cerebellar Crus I (rCrus I) as a promising target for rTMS in improving language rehabilitation. However, it remains unclear whether continuous theta burst stimulation (cTBS) over the rCrus I improves language outcomes in patients with subacute post-stroke aphasia (PSA). OBJECTIVE: To determine whether inhibitory cTBS over rCrus I improves language recovery in PSA and to explore underlying functional connectivity (FC) mechanisms. METHODS: Fifty-one PSA patients were randomized to receive either real cTBS (n = 25) or sham stimulation (n = 2

Multiple Posterior Circulation Infarctions in a Patient With Complex Aortic Atheroma and Left Vertebral Artery Stenosis: A Case Report.

Complex aortic atheroma is a recognized potential source of cerebral embolism, particularly when plaques are thick, protruding, irregular, ulcerated, or mobile. However, establishing a direct causal relationship between aortic atheroma and cerebral infarction remains challenging when competing arterial mechanisms coexist. We describe a 65-year-old man with hypertension, dyslipidemia, and active tobacco use who presented with headache and an acute confusional state. Neurological examination revealed left-sided dysmetria and gait instability. Brain imaging demonstrated multiple ischemic lesions

Morphometric analysis of the inferior petrosal sinus and cerebellopontine angle cistern to assess feasibility of endovascular shunting in pediatric patients.

BACKGROUND: Hydrocephalus is commonly treated using invasive surgical shunt placement with associated morbidity and frequent revision in children. The eShunt System, an endovascular miniature transdural shunt implanted via the inferior petrosal sinus (IPS), has shown promise in adults, but its pediatric application remains undefined because of limited data on IPS and cerebellopontine angle cistern (CPAC) morphometry. MATERIALS AND METHODS: Consecutive patients from two pediatric hospitals with high-resolution brain MRI were retrospectively analyzed. IPS and CPAC measurements were evaluated ag

Study of Cerebellar Network Dynamics in Patients with Poststroke Aphasia Based on Resting-State Functional MRI.

BACKGROUND AND PURPOSE: The cerebellum is increasingly recognized as a key contributor to language and cognitive processing, but its dynamic network alterations in poststroke aphasia remain poorly understood. This study investigated dynamic cerebellar networks in patients with poststroke aphasia using resting-state functional MRI. We examined intracerebellar and cerebellar-cortical dynamic functional connectivity and quantified their temporal properties and graph-theoretical topology. MATERIALS AND METHODS: Seventy-seven right-handed patients with poststroke aphasia and 79 healthy controls un

Beyond the Database: Integration of Network Pharmacology and UHPLC-QTOF-MS/MS to Reveal the Neuroprotective Potential of Bai Mi Decoction in Stroke Therapy.

Network pharmacology is instrumental in understanding how TCM works by targeting specific pathways. This study examines different mechanisms using three network pharmacology approaches, focusing on the TCM "Bai Mi Decoction" (BMD). The neuroprotective effects of BMD were evaluated in a middle cerebral artery occlusion-reperfusion (MCAO/R) rat model. The composition of BMD extract (BMDE) and its components in brain (BMDB) were analyzed using UHPLC-QTOF-MS/MS, alongside documented constituents from the database (BMDD). A network pharmacological analysis was conducted to explore the similarities

Mitochondrial DNA Variation in the Aging Human Cerebral Cortex and Cerebellum.

Somatic differences in mitochondrial DNA (mtDNA) have been observed with aging and between brain regions for mutations, structural variation, and abundance, which are represented by single nucleotide variants (SNVs), large deletions, and copy number, respectively. We used bioinformatic methods to interrogate mtDNA changes and their relation to cortical and cerebellar aging using whole genome sequencing data from the North American Brain Expression Consortium. This dataset contained 292 unpaired postmortem samples from frontal cortex (n = 143) and cerebellum (n = 149), ranging in age from 0.4 t
