Automated Magnetic Resonance Volumetry to Measure Multiple System Atrophy Disease Progression in Clinical Trials.

BACKGROUND: Recent studies have reported longitudinal brain atrophy in multiple system atrophy (MSA) in relatively small patient cohorts. OBJECTIVE: To compare longitudinal regional brain volume and clinical changes as endpoint measures for MSA clinical trials. METHODS: MSA patients with 1-year longitudinal brain magnetic resonance imaging (MRI) were selected from the placebo arm of the M-STAR Study (NCT03952806) and stratified into MSA-cerebellar (MSA-C) and MSA-parkinsonism (MSA-P) subtypes. A multisite healthy control (HC) cohort from international datasets was included for comparison. Ba

The Bloomy Rind Sign: A Key MRI Marker in Leptomeningeal Carcinomatosis From Lung Adenocarcinoma.

A 48 year-old female with a history of metastatic lung adenocarcinoma to the brain presented with acute onset dizziness for 12 hours. The neurological exam was notable for subtle sustained nystagmus on right and upward gaze. A code stroke was called and CT brain revealed no acute intracranial abnormality. MRI brain demonstrated new subtle restriction diffusion and FLAIR hyperintensity along the anterior surface of the brainstem and the middle cerebellar peduncles without evidence of enhancement, concerning for leptomeningeal disease. The remainder of the supratentorial metastases were stable.

Early venous filling is associated with unfavorable outcomes in acute basilar artery occlusion after mechanical thrombectomy.

BACKGROUND: To investigate the association between early venous filling (EVF) on digital subtraction angiography and poor outcomes after mechanical thrombectomy (MT) for acute basilar artery occlusion (ABAO). METHODS: This retrospective, single-center cohort included consecutive patients with ABAO who underwent MT at a comprehensive stroke center, achieved successful recanalization (modified Thrombolysis in Cerebral Infarction grade ≥ 2b), and had 90-day functional follow-up data. Baseline clinical, imaging, and procedural variables were collected. Candidate predictors were assessed using uni

Ischemic Stroke During Rivaroxaban Therapy: The Importance of Food-Dependent Absorption and Careful Assessment Before Antithrombotic Escalation.

Ischemic stroke occurring during direct oral anticoagulant (DOAC) therapy for atrial fibrillation represents a frequent and challenging clinical dilemma due to increasing stroke rates and increasing DOAC prescription rates. Such events are often interpreted as treatment failure, prompting escalation with antiplatelet therapy, particularly in the presence of concomitant intracranial atherosclerotic disease (ICAD), and thereby exposing patients to increased bleeding risk. However, apparent DOAC failure may reflect correctable pharmacokinetic or adherence-related factors. We report a 75-year-old

Cerebrovascular Manifestations in a Family with a Pathogenic COL4A5 Variant.

Background/Objectives: Pathogenic variants in collagen type 4 α5 chain (COL4A5), responsible for X-linked Alport syndrome, are classically associated with progressive kidney disease, sensorineural hearing impairment, and ocular abnormalities. Cerebrovascular involvement is not considered part of the COL4A5-related phenotype, although rare recent reports suggest it may occur. We describe a family with cerebrovascular events across three generations, segregating in a clear X-linked pattern, and evaluate whether these events could be linked to a pathogenic COL4A5 variant identified in the proband

EXPRESS: Perfusion Extent and Hypoperfusion Severity Determine Diaschisis Patterns in Acute Ischemic Stroke: A CT Perfusion Study.

BACKGROUND: Diaschisis reflects remote functional suppression following focal ischemia and large-scale network disruption. We investigate the prevalence, overlap, and determinants of multiple diaschisis subtypes using acute CTP. METHODS: We retrospectively analyzed 659 patients with hyperacute large-vessel occlusion stroke. Diaschisis was primarily assessed on CBV maps and classified into Crossed Cerebellar (CCD), Ipsilateral Thalamic (ITD), Ipsilateral Hippocampal (IHD), and CCD+ITD. Tmax-based assessment was exploratory. Multivariable logistic regression evaluated severe hypoperfusion exten

Hemi-see-saw nystagmus with superimposed pendular oculopalatal tremor: case report and systematic review.

BACKGROUND: Hemi-see-saw nystagmus (HSSN) is an uncommon subtype, characterized by elevation and intorsion of one eye with contralateral depression and extorsion, typically in a jerk pattern. It is associated with unilateral lesions of the interstitial nucleus of Cajal and the medial longitudinal fasciculus, both of which are involved in the ocular tilt reaction (OTR). Although its association with oculopalatal tremor (OPT) has been described, it is rare. METHODS: We performed a systematic review of seven databases on HSSN, extracting data on demographics, etiology, lesion location, oculomoto

Reversible cerebral vasoconstriction syndrome as a rare postoperative complication after resection of a rosette-forming glioneuronal tumor: a case report and literature review.

INTRODUCTION: Reversible cerebral vasoconstriction syndrome (RCVS) is an uncommon postoperative complication, rarely reported after posterior fossa tumor resections. Rosette-forming glioneuronal tumor (RGNT) is an infrequent, low-grade neoplasm of the central nervous system. To our knowledge, no prior cases of RCVS following RGNT resection have been documented. CASE SUMMARY: We report a male patient in his 20s who underwent suboccipital craniotomy for resection of a cerebellar RGNT. Three weeks postoperatively, the patient presented with acute right hemiparesis and aphasia. Brain MRI demonstr

Three-dimensional Motion Capture Discriminates Extremely Mild Cerebellar Ataxia from Parkinsonism in the Upper Limbs.

Upper-limb motor symptoms in spinocerebellar degeneration (SCD) and Parkinson's disease (PD) frequently overlap, making clinical differentiation challenging in mild cases using the finger-to-nose test (FNT) or the diadochokinesis test (DDKT). This study aimed to identify quantitative kinematic markers that distinguish mild SCD from mild PD using three-dimensional motion capture. Twenty-six individuals per group-mild SCD, mild PD, and normal controls (NCs)--underwent motion analysis during FNT forward, FNT backward, and DDKT. Key parameters including segmental velocity ratios during FNT and rot

Neuromodulation for post-stroke aphasia rehabilitation: established cortical stimulation and emerging network-based targets.

Post-stroke aphasia is a disabling disorder of language and communication for which speech-language therapy remains the standard of care. Because behavioural treatment yields variable gains and many people retain chronic communication limitations, non-invasive neuromodulation has been investigated to increase the responsiveness of residual language networks to therapy. This narrative review critically integrates evidence available through July 2026 for repetitive transcranial magnetic stimulation (rTMS), theta-burst stimulation, transcranial direct current stimulation (tDCS), and emerging appr

Structural and metabolic imaging after mechanical thrombectomy for acute ischemic stroke: A hybrid 18F-FDG PET/MRI pilot study.

Hybrid 2-[18F]-fluoro-2-deoxy-D-glucose positron emission tomography/magnetic resonance imaging (18F-FDG PET/MRI) enables simultaneous, single-session assessment of structural and metabolic stroke damage. We prospectively studied 10 consecutive patients (6 women; mean age 71.6 ± 11.2 years) with anterior-circulation stroke due to middle cerebral artery (MCA) occlusion who underwent successful mechanical thrombectomy (MT). Structural damage was graded with the Alberta Stroke Program Early CT Score (ASPECTS) on admission and 24-h non-contrast CT and on diffusion-weighted imaging (DWI) and fluid-

A reproducible low-dimensional architecture of human white-matter connectivity.

The human brain is organized by large-scale functional gradients, but whether white-matter connectivity defines similarly reproducible and biologically interpretable axes of cortical, subcortical, and cerebellar organization remains unclear. Here, we introduce Structural Connectivity Gradient Mapping (SCGM), a framework for deriving low-dimensional structural connectivity gradients from diffusion MRI tractography using a whole-brain parcellation spanning 438 cortical, subcortical, and cerebellar regions. Across repeated scans, an independent onsite cohort, and the Human Connectome Project Youn

Neural Repair Therapies After Stroke: I. Therapeutic Advances.

Stroke is a leading cause of disability worldwide. Neural repair therapies, distinct from acute stroke therapies, target surviving neural elements to promote plasticity after the acute injury has occurred. Repair therapies encompass a broad and rapidly evolving therapeutic landscape, ranging from behavioral training, drugs, and biological agents to neural stimulation. In this review, we highlight major advances across these therapeutic categories, focusing on the evidence base and readiness for clinical adoption. Intensive behavioral interventions, including constraint-induced movement therapy

Resting-state BOLD fMRI-derived cerebrovascular reactivity for detecting crossed cerebellar diaschisis.

PURPOSE: To study cerebellar cerebrovascular reactivity (CVR) derived from resting-state (RS) blood oxygen level-dependent (BOLD) as a potential imaging biomarker for crossed cerebellar diaschisis (CCD). METHODS: Thirty-nine patients with symptomatic unilateral cerebrovascular occlusion and eighteen age-matched healthy subjects underwent a simultaneous PET/MR scan. CCD was diagnosed as a contralateral cerebellar metabolic reduction present on 18F-FDG PET semi-quantitatively. RS BOLD-CVR maps were derived by extracting signals within specific frequency bands of RS BOLD. The cerebellar asymmetr

Acute Imbalance Syndrome vs Acute Vestibular Syndrome: Clinical and Topographical Differentiation.

BACKGROUND AND OBJECTIVES: Stroke is a major differential diagnosis in patients with acute vertigo/dizziness or imbalance. Current diagnostic algorithms such as head impulse test, gaze-evoked nystagmus, and test of skew are optimized for presentations with spontaneous nystagmus (SPN), traditionally called acute vestibular syndrome (AVS). However, in recent studies, a relevant proportion of patients with stroke-related vertigo/dizziness had no SPN, but rather postural or gait imbalance, a phenotype lately termed acute imbalance syndrome (AIS). We aimed to outline the pathoanatomical differences

Mechanical Thrombectomy for Acute Vertebrobasilar Occlusion Associated with Mycoplasma pneumoniae Pneumonia in a Child: A Case Report.

OBJECTIVE: Due to its rarity, high-quality evidence regarding mechanical thrombectomy for pediatric acute ischemic stroke remains limited. Mycoplasma pneumoniae is a common cause of community-acquired pneumonia in children, and a recognized but uncommon cause of pediatric ischemic stroke. Reports describing large-vessel occlusion associated with M. pneumoniae infection are rare, and the role of mechanical thrombectomy in this setting remains unclear. CASE PRESENTATION: A 5-year-old boy hospitalized for pneumonia developed a sudden onset of impaired consciousness and quadriplegia. Brain MRI de

Diabetes Insipidus as a Rare Complication of Metastatic Breast Cancer With Pituitary Metastases.

Diabetes insipidus (DI) is a rare but significant and life-threatening complication of metastatic breast cancer, typically arising when cancer spreads to the pituitary gland. This condition results from impaired secretion or action of antidiuretic hormone (ADH) or arginine vasopressin (AVP), leading to symptoms including excessive urination and thirst. The posterior lobe of the pituitary and the infundibulum are most susceptible to metastatic involvement due to their direct arterial blood supply, making them more accessible to circulating tumor cells. This phenomenon explains why DI can be a m
