Cerebellar functional reorganization after intermittent theta burst stimulation over vermis on balance recovery in subacute stroke patients.

BACKGROUND: This study aims to investigate whether intermittent theta-burst stimulation (iTBS) over the cerebellar vermis enhances balance recovery in subacute stroke, and examine the underlying neural mechanisms. METHODS: Fifty-two patients with subacute stroke and balance impairment were randomized to receive either three weeks of iTBS (n = 26) or sham stimulation (n = 26). The primary outcome was the Berg Balance Scale (BBS). Secondary outcomes included additional motor function measures and surface electromyography (sEMG). Clinical assessments were conducted at baseline and at weeks 1, 2,

Dizziness Found to Be Candida Parapsilosis Prosthetic Valve Endocarditis.

Candida parapsilosis is an opportunistic pathogen with a propensity for biofilm formation and adhesion to medical devices. Although rare, fungal prosthetic valve endocarditis (PVE) is a feared complication of disseminated candidemia or direct intraoperative contamination from fungal adherence to prosthetic valves. C. parapsilosis is one of the most common species underlying fungal endocarditis. We present a 78-year-old male patient 2 months after transcatheter aortic valve replacement with persistent dizziness and a cerebellar stroke found to be secondary to treatment-resistant disseminated ca

Repeated Oral Methylphenidate Administration Evokes Changes in Brain Plasticity Proteins in Juvenile Wistar Kyoto Rats: Evidence for Sex-Related Differences.

Methylphenidate (MPH) is first-line pharmacotherapy for Attention Deficit Hyperactivity Disorder (ADHD). Misdiagnosis and misuse raise concerns about exposing children and adolescents to MPH. This study aimed to assess how clinically relevant oral doses of MPH influence the expression of brain proteins involved in synaptic plasticity and neuronal growth in both sexes. Thirty-seven Wistar-Kyoto (WKY) rats (18 males and 19 females) were divided into an MPH group (daily oral dose of 5 mg/kg MPH in a 5% sucrose solution) and a control group (equivalent volume of 5% sucrose solution). Daily gavage

Somatosensory Induced Cerebellar Responses to Peripheral Nerve Stimulation: A Time and Time-Frequency EEG Study.

Background/Objectives: The cerebellum plays a central role in sensorimotor integration and temporal processing, yet its direct electrophysiological investigation in humans remains challenging, and cerebellar contributions to somatosensory responses remain poorly defined. This study aimed to determine whether cerebellar responses to peripheral nerve stimulation can be detected using scalp EEG and whether time-frequency analysis provides advantages over time-domain approaches. Methods: Scalp EEG was recorded during electrical stimulation of the median nerve and tibial nerve in 16 healthy partici

Metronidazole-Induced Encephalopathy in a Patient With Metastatic Cancer.

Metronidazole-induced encephalopathy (MIE) is a rare adverse effect from long-term metronidazole use. We describe a patient with metastatic cancer and complicated abscesses on long-term antibiotics who presented to the emergency department with acute encephalopathic features including reported weakness and confusion. Physical examination was remarkable for chronic right-sided weakness from a previous stroke and reported abnormal right-sided finger-to-nose testing, which may have been due to weakness from his prior stroke. Head CT was non-contributory, and brain MRI showed bilateral edema in ce

Clinical and Radiological Features of Aphasia Caused by Brainstem Infarction: Two Case Reports.

Cognitive impairment resulting from cerebellar damage, known as cerebellar cognitive affective syndrome (CCAS), can cause language impairment and aphasia. The disruption of the cerebro-cerebellar pathway is a possible mechanism for CCAS. Consequently, aphasia may also arise from brainstem lesions. However, the precise mechanisms of brainstem-induced aphasia are not fully understood owing to the scarcity of published reports describing this condition. Herein, we investigated characteristics of patients with aphasia attributed to the brainstem infarction. We retrospectively analyzed medical reco

Pediatric Stroke Associated With a Rare Pathogenic PKP2 Variant: A Diagnostic Challenge.

Pediatric ischemic stroke is an uncommon but serious neurological condition with highly variable etiologies. Cerebellar infarctions are particularly rare in children and often present with nonspecific symptoms, contributing to delays in diagnosis. We report a case of a previously healthy 15-year-old female who presented with acute worsening headache, dizziness, vomiting, dysarthria, and left-sided neurological deficits. Neuroimaging revealed an acute non-hemorrhagic infarct in the left cerebellar hemisphere within the superior cerebellar artery territory. Given her low National Institutes of H

The epidemiology and clinical presentation of acute vestibular syndromes: a systematic review of the literature.

BACKGROUND: Acute vertigo and dizziness are among the most frequent complaints presenting to the emergency department (ED). If acute-onset and persistent vertigo/dizziness (lasting > 24 h) are accompanied by motion intolerance, gait imbalance, nausea/vomitus, and (usually also) nystagmus, this refers to the acute vestibular syndrome (AVS). We aimed at collecting epidemiologic data about the frequency of AVS etiologies for improving the diagnostic workup. METHODS: We conducted a Prospero-registered systematic literature search (MEDLINE, Embase - in June 2024) retrieving information on AVS's cl

Genotype and Age at Onset Drive Vermis Atrophy in CACNA1A- and GAA-FGF14-related Ataxias.

CACNA1A- and GAA-FGF14-related channelopathies are among the most frequent genetic etiologies of cerebellar ataxia. They display overlapping features consisting of both chronic and episodic neurological symptoms and usually show only a mild cerebellar atrophy in imaging studies, with prevalent involvement of the vermis. To evaluate cerebellar volumetry as a biomarker for distinguishing between these disorders and for understanding their neuroanatomical correlates. We applied a deep learning method (CerebNet) for the lobular segmentation and volumetry assessment of the cerebellum on 3.0 Tesla M

Prevalence of signs and symptoms in posterior circulation ischemic stroke: a systematic review and meta-analysis.

Posterior circulation (PC) ischemic stroke accounts for approximately a fourth of all acute ischemic strokes and is associated with high disability and mortality. Rapid diagnosis and management can be challenging as it often presents with non-specific signs and symptoms. We performed a systematic review and meta-analysis to describe the prevalence of signs and symptoms in PC ischemic stroke. Two databases (PubMed and EMBASE) were searched for English-written observational studies reporting data on signs and symptoms in patients with a diagnosis of PC ischemic stroke. Data were extracted by two

Investigation of Possible Cardiac Manifestations of Dysautonomia in a Patient With Acquired Ondine's Curse Syndrome: A Rare Clinical Case Report.

Ondine's curse, also known as central alveolar hypoventilation syndrome, is a very rare condition characterized by failure of breathing mechanisms during sleep. The most usual cause of acquired Ondine's curse syndrome is a stroke of the brainstem, causing dysautonomia, which is primarily manifested by sleep apnea, but can also cause cardiac rhythm disturbance. We present the case of an 81-year-old female with a history of a brainstem and cerebellum ischemic stroke one month ago, which resulted in acquired Ondine's curse syndrome. The patient had been sleeping with respiratory support at a reha

Comparison of neurosurgical and medical management options of space-occupying cerebellar infarction.

BACKGROUND/OBJECTIVES: Despite space-occupying cerebellar infarctions (SOCIs) carrying a high morbidity and mortality due to mass effect in the posterior fossa, optimal management remains uncertain: particularly regarding patient selection, timing, and surgical technique. We conducted a systematic review and network meta-analysis to compare outcomes between medical versus surgical management, and to identify prognostic thresholds that may guide treatment. METHODS: A search of PubMed, Embase, and CENTRAL was performed from database inception through October 30, 2024. Studies were included if t

Hyperbaric oxygen-assisted neurological recovery in a patient with classic heat stroke: a case report.

BACKGROUND: Heat stroke (HS) is a critical condition involving severe central nervous system injury. While standard treatments-such as cooling, ventilation, and fluid resuscitation-can stabilize patients, they are often inadequate in addressing long-term neurological complications, including impaired consciousness and paralysis. The persistent deficits in neurological function remain a major clinical challenge. Hyperbaric oxygen therapy (HBOT), which improves brain oxygenation and supports neural repair, offers a potential solution. In the case we present of a patient with HS and brain injury,

Herpes Simplex Virus Encephalitis With Cerebellar Infarction and Cortical Laminar Necrosis.

Herpes simplex virus (HSV) encephalitis typically involves the temporal lobes, and cerebellar involvement with stroke and cortical laminar necrosis (CLN) is rare. A 40-year-old man with no prior comorbidities presented with 10 days of positional vertigo, vomiting and intermittent fever. Initial otorhinolaryngology assessment showed bilateral nystagmus and a positive fistula test, and he was treated as having a peripheral vestibular disorder. He re-presented with persistent vertigo and new confusion; on admission, he was febrile, hypertensive and ataxic, and within 24 hours, he developed worsen

Association of serum albumin levels with multidomain functional impairment (motor, balance, ADL, and cognitive impairment) in Chinese post-stroke patients: a multicenter cross-sectional study.

BACKGROUND: Malnutrition, frequently indicated by hypoalbuminemia, is prevalent post-stroke and associated with adverse functional outcomes. However, the independent role of serum albumin (ALB) in multidomain functional recovery-encompassing motor, balance, cognitive, and daily living domains-remains underexplored in Chinese populations. This multicenter study aimed to quantify the independent association between serum ALB levels and functional impairment in Chinese post-stroke patients. METHODS: In this cross-sectional study, 1,741 patients from rehabilitation centers across China were enrol

White matter predictors of cerebellar tDCS treatment effects in aphasia rehabilitation.

INTRODUCTION: Cerebellar transcranial direct current stimulation (tDCS) combined with language therapy can aid in chronic aphasia recovery, but the neural mechanisms and biomarkers of treatment efficacy remain uncertain. METHODS: In this secondary analysis of data from a previously conducted clinical trial, we used a randomized, double-blind, sham-controlled, within-subject crossover design with a study sample of 19 participants with post-stroke aphasia. We assessed the degree to which baseline properties of cerebro-cerebellar white matter tracts can predict or moderate longitudinal treatment

Long-term intensive golf training induces reconfiguration of brain structural covariance networks.

Long-term motor training is thought to reshape brain organization, yet how golf expertise influences large-scale brain networks remains unclear. Using T1-weighted MRI and an individualized structural covariance network (SCN) approach, we compared 20 expert golfers, 20 novice golfers, and 20 non-golfer controls. Experts showed higher global clustering coefficient and local efficiency than novices, indicating enhanced modular processing. At the nodal level, experts exhibited increased clustering in regions supporting visual-sensorimotor integration (e.g., right supramarginal gyrus, Heschl's gyru

Cerebellar Infarction Following Pulseless Electrical Activity Arrest in Advanced Heart Failure With Reduced Ejection Fraction: A Post-resuscitation Diagnostic Pitfall.

Neurologic deterioration following cardiac arrest is frequently attributed to global hypoxic-ischemic brain injury. However, focal ischemic stroke may occur concurrently and represents an important diagnostic pitfall in post-resuscitation care. We report the case of a 59-year-old woman with end-stage heart failure with reduced ejection fraction who developed acute neurologic impairment following prolonged pulseless electrical activity arrest. Early non-contrast CT of the head demonstrated a focal left cerebellar infarction without diffuse cerebral edema. MRI of the brain performed four days la
