The clinical definition of transient ischemic attacks (TIAs), besides the ischemic pathogenesis is based (i) upon 24 h. duration and (ii) complete reversibility. The limit of 24 h. has been chosen arbitrarily and episodes clearing completely after more than 24 h. do exist. In addition TIAs lasting less than 5 min. would have a higher incidence of subsequent completed stroke. Complete reversibility has also been disregarded: ischemic attacks leaving slight neurological deficits or showing an hypodense area at CT scanning have been shown to have the same significance at TIAs. The evaluation of headache in patients with transient ischemic attacks (TIA) has various sources of difficulty, the definition of TIA being the most relevant. The classical definition needs to be supplemented with a normal CT scan if a misleading diagnostic statement is to be avoided. The syndrome of pure motor hemiplegia (PMH) has been traditionally regarded to be due to lacunar infarcts at various places in the brain. Recently, attention has been drawn towards various other aetiological factors which have been responsible for this clinical presentation. The clinical manifestations and computed tomographic (CT) findings of small intracerebral haematomas (ICHs) were studied in 31 consecutive cases which comprised 6% of 520 cases of non-traumatic, non-neoplastic ICH confirmed by CT in a 3-year period. In this study we assessed regional cerebral blood flow in patients with signs and symptoms of acute stroke using single-photon emission computed tomography (SPECT) and N-isopropyl I-123 p-iodoamphetamine (IMP). Evaluation of a series of 12 chordoid sarcomas suggests that there is a wider range of histological features in this entity then previously appreciated. We studied the relation between blood pressure and type of cerebral infarction (large or lacunar) in 134 patients with acute hemispheral infarcts that were detectable by computed tomography and that could not be attributed to causes other than atherothrombotic arterial disease.
