Common and distinct circulating microRNAs in four neurovascular disorders.

BACKGROUND: Familial cerebral cavernous malformations (FCCM), Sturge-Weber Syndrome (SWS), and hereditary hemorrhagic telangiectasia (HHT) are driven by genetic mutations causing varying vascular dysmorphism and risk of brain bleeding. Cerebral microbleeds (CMBs) are associated with the aging process with less characterized genetic drivers. This study hypothesizes that common and distinct circulating microRNAs (miRNAs) can reflect mechanisms of vascular dysmorphism and bleeding, which can serve as potential biomarkers in clinical contexts. METHODS: Differentially expressed (DE) plasma miRNAs

On Neurocardiology Updates: an Interdisciplinary Field at the Intersection of Neurology and Cardiology.

PURPOSE OF REVIEW: This review examines the evolving field of neurocardiology, tracing its development from early homeostatic theories to modern understandings of the bidirectional neural networks that link the heart and brain. We propose an integrative framework to explain neuro-cardiac interactions in health and disease, with a focus on clinical applications and emerging therapies. RECENT FINDINGS: The neuro-cardiac axis comprises hierarchical neural networks, from the intrinsic cardiac nervous system to subcortical and cortical brain regions. This structure underlies a range of clinical sy

Depressive Symptoms Profiles and Cognitive Outcomes After Stroke.

INTRODUCTION: Post-stroke depressive symptoms are heterogeneous and variably associated with other psycho-cognitive features. We employed cluster analysis to identify distinct profiles of post-stroke depressive symptomatology and their association with cognitive performance. METHODS: We included consecutive patients undergoing neuropsychiatric evaluation 6 months after stroke. Cluster analysis incorporated the Center for Epidemiologic Studies Depression Scale, along with the apathy and anxiety items from the Neuropsychiatric Inventory questionnaire. Baseline clinical/neuroimaging variables an

Hemoglobin SC Sickle Cell Disease Presenting as Acute Ischemic Stroke With Moyamoya Syndrome in an Adult: A Case Report.

Moyamoya disease is a rare condition where blood vessels at the base of the brain become narrow or blocked. This leads to the growth of small, fragile vessels that look like a "puff of smoke" on imaging tests like CT or MRI scans. These vascular changes predominantly involve the terminal portion of the internal carotid arteries and the proximal segments of the anterior and middle cerebral arteries, leading to progressive stenosis and the development of collateral networks. Although the exact cause of moyamoya disease is unknown, it increases the risk of having a stroke. In this disease, there

Alzheimer's disease diagnostic progression is associated with cerebrovascular disease and neuroinflammation in adults with Down syndrome.

INTRODUCTION: Despite having few vascular risk factors, people with Down syndrome (DS) have MRI evidence of cerebrovascular disease (CVD) and neuroinflammation that worsens with Alzheimer's disease (AD) severity. We investigated whether markers of CVD and inflammation are associated with AD-related diagnostic progression in people with DS. METHODS: We included 149 participants (mean age [SD] = 44.6 [9]) from the Alzheimer's Biomarkers Consortium-Down Syndrome who had two (n = 24) or three follow-up visits (n = 125). We derived white matter hyperintensity (WMH) volume and plasma biomarker (gli

Autonomic phenotyping, brain blood flow control, and cognitive-motor-integration in Long COVID and myalgic encephalomyelitis/chronic fatigue syndrome: A pilot study.

Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) and the prolonged sequelae after COVID-19 (>3 months; Long COVID) have similar symptomology, are both associated with autonomic dysfunction, and a growing proportion of Long COVID patients are developing ME/CFS. We aimed to determine an autonomic phenotype of patients with ME/CFS vs Long COVID. We hypothesized that the groups would differ from controls yet be similar to one another. We recruited sedentary controls (n = 10), mild/moderate ME/CFS patients (n = 12), and Long COVID patients (n = 9) to undergo 1) breathing 5 % CO2, 2) brea

Rare Variants in HTRA1, SGTB, and RBM12 Confer Risk of Atherosclerotic Cardiovascular Disease Independent of Traditional Cardiovascular Risk Factors.

BACKGROUND: Atherosclerosis is a pathophysiological process common to a range of cardiovascular diseases. We reasoned that considering clinical presentations of atherosclerosis across the coronary, peripheral, and cerebrovasculature as a single entity would enhance statistical power to identify rare genetic variation driving pathological processes across multiple vascular beds. METHODS: We performed an exome-wide association study of atherosclerotic cardiovascular disease in 434 438 UK Biobank participants of European ancestry. RESULTS: We identified rare, predicted damaging variants in HTRA

Complex Mechanisms Underlying the Coexistence of Diabetic Complications, Chronic Kidney Disease, and Hemorrhagic Stroke: A Case Report.

Diabetes mellitus type 2 is a chronic metabolic disease that presents a growing global health challenge. Its long-term complications, including nephropathy, neuropathy, and retinopathy, significantly contribute to patient morbidity and the healthcare burden. Diabetic kidney disease, one of the significant complications, often progresses to end-stage renal failure, necessitating renal replacement therapy. This case report describes a 67-year-old man with longstanding type 2 diabetes mellitus and hypertension complicated by progressive chronic kidney disease, diabetic foot syndrome, and subseque

A Fatal Case of a Ruptured Posterior Communicating Artery Aneurysm in a Patient With Suspected Klippel-Feil Syndrome.

Klippel-Feil syndrome (KFS) is a rare congenital disorder characterized by cervical vertebral fusion and often associated with multisystem anomalies. Although primarily skeletal, KFS has been linked to vascular abnormalities, including arterial dissections, agenesis, and aneurysms. We present the case of a 33-year-old previously healthy woman who presented with diffuse subarachnoid hemorrhage. Imaging revealed a ruptured 5.5 mm posterior communicating artery aneurysm arising from the left supraclinoid internal carotid artery, along with congenital cervical vertebral fusion consistent with undi

Vascular parkinsonism: an update.

Vascular parkinsonism (VP), resulting from cerebrovascular disease, is a rare disorder with a characteristic motor and non-motor clinical profile distinct from sporadic/idiopathic Parkinson disease (PD) and other parkinsonian disorders. It accounts for 3-6% of all parkinsonian syndromes and may overlap with other parkinsonisms. Clinical features of VP are heterogenous and include bilateral rigidity with lower body predominance, bradykinesia, postural instability, shuffling gait, falls, corticospinal symptoms and cognitive impairment, tremor being rare or absent. Macular vessel density in the superficial plexus is not a proxy of cerebrovascular damage in non-demented individuals: data from the NORFACE cohort.

INTRODUCTION: Optical coherence tomography angiography (OCT-A) is a novel tool that allows the detection of retinal vascular changes. We investigated the association of macular vessel density (VD) in the superficial plexus assessed by OCT-A with measures of cerebrovascular pathology and atrophy quantified by brain magnetic resonance imaging (MRI) in non-demented individuals. METHODS: Clinical, demographical, OCT-A, and brain MRI data from non-demented research participants were included. We analyzed the association of regional macular VD with brain vascular burden using the Fazekas scale asse

Hyperbaric oxygen therapy for thalamic pain syndrome: case report.

Thalamic pain syndrome is a distressing type of central post-stroke pain (CPSP) that occurs in up to 10% of cases following a cerebrovascular accident, typically with a delayed onset of signs and symptoms, and is often chronic or even life-long. Thalamic pain syndrome, as is the case for other CPSPs, is difficult to treat, and the response is typically moderate at best. Central pain also occurs after vascular insults in parts of the CNS other than the thalamus. Only a few patients present with the classic "Dejerine and Roussy syndrome," so the term CPSP is preferred for describing neuropathic

Sneddon Syndrome: A Case Report From Saudi Arabia.

Sneddon syndrome, also known as livedo reticularis with cerebrovascular accidents, is a rare but chronic condition that affects blood vessels in the skin and brain. This syndrome is characterized by a net-like appearance on the skin, known as livedo reticularis, which occurs due to the constriction of blood vessels. In addition to skin manifestations, Sneddon syndrome is often associated with repeated neurological events, such as strokes or transient ischemic attacks. These neurological symptoms can vary in severity and can lead to various complications. Upon admission to the stroke unit, a 28

Cortical microinfarcts in adults with Down syndrome assessed with 3T-MRI.

BACKGROUND: Cortical microinfarcts (CMI) were attributed to cerebrovascular disease and cerebral amyloid angiopathy (CAA). CAA is frequent in Down syndrome (DS) while hypertension is rare, yet no studies have assessed CMI in DS. METHODS: We included 195 adults with DS, 63 with symptomatic sporadic Alzheimer's disease (AD), and 106 controls with 3T magnetic resonance imaging. We assessed CMI prevalence in each group and CMI association with age, AD clinical continuum, vascular risk factors, vascular neuroimaging findings, amyloid/tau/neurodegeneration biomarkers, and cognition in DS. Cerebrovascular disease emerges with age and Alzheimer's disease in adults with Down syndrome.

Adults with Down syndrome have a genetic form of Alzheimer's disease (AD) and evidence of cerebrovascular disease across the AD continuum, despite few systemic vascular risk factors. The onset and progression of AD in Down syndrome is highly age-dependent, but it is unknown at what age cerebrovascular disease emerges and what factors influence its severity. In the Alzheimer's Biomarker Consortium-Down Syndrome study (ABC-DS; n = 242; age = 25-72), we estimated the age inflection point at which MRI-based white matter hyperintensities (WMH), enlarged perivascular spaces (PVS), microbleeds, and i
