Therapeutic Prospects of Ketamine for Cataplexy in Narcolepsy Type 1: Evidence from a Human Case and a Murine Model.

Narcolepsy type 1 is characterized by cataplexy, for which current treatments targeting monoaminergic, histaminergic, and GABAergic pathways may be ineffective or poorly tolerated. We report the case of a 34-year-old man with narcolepsy type 1, intolerant to standard therapies, who experienced marked and sustained cataplexy reduction with morning intranasal ketamine, with symptom recurrence upon discontinuation. To confirm this observation, ketamine was compared with vehicle in a crossover study in orexin knockout mice implanted for polysomnography. Ketamine dramatically reduced time spent in

Sleep underestimation and subjective sleep appraisal: Limited value of routine polysomnographic and electroencephalographic features.

OBJECTIVES: Whether routine polysomnographic (PSG) and electroencephalographic (EEG) features distinguish patients who underestimate sleep remains unclear. We examined associations of clinical, subjective, PSG, and EEG features with sleep underestimation and tested whether EEG features added discrimination beyond a parsimonious three-predictor baseline model. METHODS: We analysed 225 adults undergoing clinically indicated in-laboratory PSG in this single-centre retrospective cross-sectional study. The misperception index ([objective - subjective total sleep time]/objective total sleep time) w

Altitude-induced plasticity of respiratory central-peripheral chemoreflex interaction.

Stable breathing depends on the integrated action of central and peripheral chemoreceptors, but the nature of this interaction in humans and its physiological importance remain controversial. Previous studies reported additive, hypo-additive and hyper-additive responses across species, preparations and protocols. Here, we used a novel method to quantify peripheral chemoreflex sensitivity across graded levels of central chemoreceptor activation to test whether central-peripheral chemoreceptor interaction in awake humans is fixed or state dependent. At low altitude, the form of interaction varie

The role of histamine in narcolepsy type 1: a narrative review.

Narcolepsy type 1 (NT1) is a chronic neurological disorder characterized mainly by impaired wakefulness maintenance and dysregulated rapid eye movement (REM) sleep, which markedly affects patients' daytime functioning and quality of life. The core pathological basis of NT1 is the selective loss of orexinergic neurons. The orexinergic system regulates histaminergic neurons in the tuberomammillary nucleus (TMN) through anatomical projections and functional excitation, while the histaminergic system contributes to cortical activation, arousal maintenance, and attention regulation. Therefore, the

Why Sleep Matters in COPD: Cluster-Derived Sleep Phenotypes Reveal Distinct Physiological, Comorbidity and Survival Profiles.

BACKGROUND: Sleep disturbances are highly prevalent in chronic obstructive pulmonary disease (COPD) but are not incorporated into current phenotypic classifications. We examined whether multidimensional sleep data identify clinically relevant patterns in COPD and whether these patterns are associated with all-cause mortality. METHODS: In a clinical cohort of 205 patients with COPD, home-based sleep assessment was performed using the WatchPAT™ device. Fifteen complementary variables reflecting sleep duration and architecture, nocturnal oxygenation, sleep-disordered breathing, and daytime sleep

Orofacial Myofunctional Therapy to Treat Residual Obstructive Sleep Apnoea in Children Treated with Adenotonsillectomy and Maxillary Expansion: An Interventional Study.

Background: In children with residual obstructive sleep apnoea (OSA) despite combined adenotonsillectomy (TA) and semi-rapid maxillary expansion (SRME), the optimal further management remains unclear. This study evaluated the effect of a 12-month programme of orofacial myofunctional therapy (OMFT) on residual OSA in this population. Materials and Methods: In this retrospective cohort study, 80 children (aged 7-9 years) with polysomnography-confirmed residual OSA (respiratory disturbance index [RDI] ≥ 1 event/h) after combined TA and SRME were followed for 12 months. Forty-one children complete

Lung Restriction From Childhood Is Associated With Obstructive Sleep Apnoea in Middle-Age.

BACKGROUND AND OBJECTIVE: Whether lifetime lung function trajectories and spirometry patterns are associated with Obstructive Sleep Apnoea (OSA) is unknown. We aimed to investigate this association to widen the understanding of OSA pathophysiology. METHODS: We previously identified six FEV1/FVC (forced expiratory volume in the first second/forced vital capacity) and five FVC trajectories within a population-based prospective cohort (n = 2422) using spirometry assessed at ages 7, 13, 18, 43, 50, and 53 years. These trajectories were combined into four lifetime spirometry patterns: restrictive-

Childhood-onset parkinsonism, dystonia and severe muscle wasting in a Sudanese girl caused by a novel homozygous DnaJ heat shock protein family (heat shock protein 40) member C6 frameshift variant, with an incidental pathogenic breast cancer 1 gene secondary finding: a case report.

BACKGROUND: DNAJC6-related juvenile parkinsonism is a rare autosomal recessive disorder of auxilin, a co-chaperone essential for clathrin-mediated synaptic vesicle recycling. Fewer than 50 kindreds have been reported worldwide, and the condition is easily missed when first-line investigations come back normal. We describe a Sudanese adolescent in whom a previously unreported homozygous DNAJC6 frameshift variant was identified only after a prolonged diagnostic journey that included a period during which her illness was mistakenly attributed to a functional neurological disorder. Clinical whole-

Relapsing Polychondritis With Severe Airway Involvement Mimicking Refractory Asthma: A Diagnostic Challenge.

Relapsing polychondritis (RP) is a rare immune-mediated disorder that can cause severe tracheobronchial involvement and life-threatening airway compromise. We present a diagnostically challenging case of a 30-year-old non-binary patient whose progressive respiratory symptoms were initially attributed to severe refractory asthma. Despite repeated courses of systemic corticosteroids and escalation of asthma-directed therapy, they developed recurrent breathlessness, wheeze, and inspiratory stridor. Pulmonary function testing demonstrated severe fixed airflow obstruction with minimal bronchodilato

De-escalating Therapy in Obstructive Sleep Apnoea After Glucagon-Like Peptide-1 Receptor Agonist-Induced Weight Loss: A Narrative Review.

Incretin-based therapies now achieve weight reduction of a magnitude previously attainable only through bariatric surgery and have demonstrated substantial reductions in the apnoea-hypopnoea index (AHI) in adults with obesity-related obstructive sleep apnoea (OSA). Sleep services are consequently managing growing numbers of patients who undergo rapid, substantial weight loss while established on positive airway pressure (PAP) therapy, and who ask whether that therapy can be stopped. No guideline addresses this question. This narrative review synthesises three literatures that have developed la

Computed tomography-derived cardiac parameters versus biochemical inflammatory indices for discriminating disease severity and identifying cardiovascular comorbidity in obstructive sleep apnoea: A cross-sectional study.

PURPOSE: It is unclear whether haematological inflammatory indices or CT-derived cardiac parameters add discriminative information beyond standard clinical variables in obstructive sleep apnoea syndrome (OSAS). We compared both modalities, alone and combined, against age, sex and body mass index (BMI) for identifying severe OSAS and associated cardiovascular comorbidity. METHODS: This was a retrospective cross-sectional study of 401 participants (240 OSAS, 161 controls). Four nested logistic regression models were fitted in the OSAS cohort: M0 (age, sex, BMI); M1 (M0 plus biochemical indices

Case Report: Narcolepsy associated with anti-Ma2 antibody positivity.

BACKGROUND: Several intraneuronal antibodies have been identified in association with paraneoplastic encephalitis, including anti-Ma and anti-Yo antibodies. There is a pure case of narcolepsy associated with anti-Ma2 antibody positivity with concomitant anti-Yo antibody. CASE PRESENTATION: We report a 42-year-old male patient who presented with recurrent cataplexy as the primary and dominant symptom without typical hyperphagia in the early stages of narcolepsy associated with anti-Ma2 antibody positivity. Despite extensive screening, no underlying malignancy was detected over a 3-year disease

Sleep stage-specific heart rate variability feature-driven clustering analysis: identifying cardiac autonomic endotypes with long-term cardiovascular predictive value.

AIMS: Cardiovascular risk assessment in obstructive sleep apnoea (OSA) has traditionally relied on the Apnea-Hypopnea Index (AHI). However, AHI primarily reflects respiratory event burden and may not adequately capture inter-individual heterogeneity in sleep-related cardiorespiratory regulation. This study aimed to identify sleep stage-specific heart rate variability (HRV) phenotypes and evaluate their associations with long-term cardiovascular outcomes. METHODS AND RESULTS: Data were obtained from the Sleep Heart Health Study (SHHS). After exclusion of participants with missing clinical cova
