[Treatment of obstructive sleep apnoea with a mandibular advancement device in the dental practice].

Obstructive sleep apnoea is a common sleep-related breathing disorder with significant health and socio-economic consequences. Obstructive sleep apnoea is characterized by recurrent obstructions of the upper airway during sleep, leading to disrupted sleep due to frequent arousal and drops in blood oxygen levels. Research has demonstrated mandibular advancement devices to be effective in reducing upper airway obstructions and in alleviating obstructive sleep apnoea symptoms such as snoring and excessive daytime sleepiness. The application of mandibular advancement device therapy in patients wit

Orexin pathway modulation in narcolepsy: from Autoimmune mechanisms to nanotechnology enabled therapeutic delivery.

Narcolepsy is a long-term brain disorder caused by disrupted orexin signaling. Orexin neurons sit in the hypothalamus and control the sleep-wake cycle; damage to these neurons leads to narcolepsy. Early evidence pointed to an autoimmune attack on orexin neurons as the main cause. Current treatments - stimulants, antidepressants, sodium oxybate, and scheduled naps - ease symptoms but do not stop the disease, and they often cause side effects such as anxiety, insomnia, and heart problems. Most of these drugs also cannot cross the blood-brain barrier (BBB) to act directly on the hypothalamus. Thi

[Obstructive sleep apnoea in children with Down s syndrome: challenges and treatment strategies].

Obstructive sleep apnoea poses a significant health challenge for children with Down s syndrome. Its high prevalence is attributed to unique anatomical, physiological, and comorbid factors in this population. Early diagnosis and effective treatment are crucial, as children with Down s syndrome are particularly vulnerable to the health consequences of obstructive sleep apnoea. While adenotonsillectomy is the first-line treatment, its efficacy in children with Down s syndrome can be lower. This often necessitates further airway evaluation and interventions. Alternative treatment options include

The German version of the Sleep Inertia Questionnaire in central disorders of hypersomnolence: a validation study.

BACKGROUND: Sleep inertia, or sleep drunkenness, is a transitional state between sleep and wakefulness characterized by cognitive and physical impairment. It is particularly relevant in central disorders of hypersomnolence (CDH), but validated instruments to assess sleep inertia in German-speaking populations are lacking. This study translated and validated the German version of the Sleep Inertia Questionnaire (SIQ) in patients with CDH and healthy controls. METHODS: We included 109 participants from the international Swiss Primary Hypersomnolence and Narcolepsy Cohort Study (iSPHYNCS): idiop

Genial swing orthomorphic osteotomy: a novel technique for retrogenia-associated obstructive sleep apnoea.

Retrogenia, despite its anatomical significance, remains under-recognized as a contributing factor in obstructive sleep apnoea (OSA). While retrognathia and micrognathia are well-documented, retrogenia as an isolated morphological variant has received limited attention in the context of OSA. This technical note introduces the genial swing orthomorphic osteotomy (GSOO), a novel orthomorphic surgical technique specifically designed to address dysmorphology associated with retrogenia. The procedure repositions the genial segment to enhance posterior airway space, while also achieving favourable f

A multidisciplinary biopsychosocial approach reveals behavioural phenotypes in idiopathic hypersomnia: a prospective single-arm study.

Idiopathic hypersomnia (IH) is clinically heterogeneous yet defined almost entirely in electrophysiological terms, and pharmacotherapy frequently fails to restore daytime functioning. We asked whether IH heterogeneity extends to stable behavioural phenotypes and whether an individualised, multidisciplinary, non-pharmacological programme is feasible and yields a clinically meaningful signal on symptom burden. In this prospective single-arm study, 20 adults with ICSD-3-TR-diagnosed IH at a tertiary sleep centre completed a programme combining psychoeducation, cognitive-behavioural therapy for hy

Methodological insights and correlations of short-term BP variability indices.

BACKGROUND: Blood Pressure Variability (BPV) is an independent risk factor for cardiovascular events. Short-term indices of BPV, such as standard deviation (SD), coefficient of variation (CV), average real variability (ARV), and Time Rate Index (TRI) are calculated using blood pressure (BP) measurements from 24-hour ambulatory blood pressure monitoring (ABPM). Although these indices have distinct units and amplitudes, their relationships have not been thoroughly investigated. This study aims to explore the correlation between the SD, CV, ARV, and TRI indices of short-term BPV. METHODS: Data w

Treatment of a skeletal Class II malocclusion growing patient using clear aligners with solid occlusal blocks: A case report.

This case report describes a two-phase orthodontic treatment of an 11-year-old adolescent patient with a Class II malocclusion. Before orthodontic treatment, the patient exhibited adenoid hypertrophy, mouth breathing, and snoring. He was referred to an otolaryngologist and diagnosed with paediatric obstructive sleep apnoea (PedOSA), which was treated with tonsillectomy and adenoidectomy. Thereafter, given that the patient was at the pubertal growth peak and preferred aesthetic appliances, clear aligner functional therapy was selected for orthodontic treatment. During the first phase (mandibula

"I know a little, but I can't do more": a COM-B-based qualitative study of CPAP use and sleep behaviour change among hospitalised patients with COMISA in China.

BACKGROUND: Co-morbid insomnia and sleep apnoea (COMISA) is a highly prevalent condition associated with worse health outcomes and greater treatment complexity than either disorder alone. Patients with COMISA demonstrate lower acceptance and use of continuous positive airway pressure (CPAP) therapy compared with those who have obstructive sleep apnoea (OSA) alone, yet the subjective mechanisms underlying this adherence penalty remain poorly understood. Qualitative research on CPAP use and sleep behaviour change in patients with COMISA is scarce, the COM-B (Capability, Opportunity, Motivation-B

A decade of change in age, sex distribution, and comorbidities of obstructive sleep apnoea in Finland.

There is limited information on changes over time in the incidence, age, sex, and comorbidities of obstructive sleep apnoea. We extracted data from the Finnish Secondary Care Register to assess the incidence of obstructive sleep apnoea, the age and sex distribution, and the prevalence of 26 comorbidities of incident obstructive sleep apnoea patients in Finnish specialized care in 2010 and 2020. Analyses were conducted for three age groups (18 - 39, 40 - 64, and ≥65 years), stratified by sex, and for the total population. From 2010 to 2020, the incidence of clinically diagnosed obstructive slee

Co-exposure to multiple fine particulate matter constituents and adults obstructive sleep apnoea (OSA) in Southern China.

Obstructive sleep apnea (OSA) is a common sleep-related breathing disorder. OSA affects approximately 936 million individuals globally and 176 million in China. Nevertheless, the effects of PM2.5 constituents on OSA remains unclear. We aimed to evaluate long-term impacts of PM2.5 components [ammonium (NH4+), sulfate (SO42-), nitrate (NO3-), organic matter (OM), and black carbon (BC)] on OSA indices and identity the dominant component. We carried out a cross-sectional study, incorporating long-term exposures utilizing a database (n = 3925) encompassing 21 cities in Guangdong Province from 2005

Age Does Not Affect Respiratory Characteristics in Children With Prader-Willi Syndrome Before and After Growth Hormone Therapy.

AIM: Children with Prader-Willi syndrome (PWS) are at increased risk of both central (CSA) and obstructive sleep apnoea (OSA). Studies examining the effects of growth hormone have focused on older children; however, therapy is often initiated before the age of 2 years. We determined the effects of age on (1) the number of children diagnosed with OSA and CSA; (2) the sleep and respiratory characteristics and (3) the effects of growth hormone on OSA and CSA. METHODS: Retrospective review of children with PWS who underwent polysomnography pre- and post-growth hormone between January 2011 and Jun

Hypoxia in MASLD: a spatial determinant of the pathogenesis.

The liver has a unique microarchitecture, with hepatic sinusoids receiving blood from the portal vein and hepatic artery and draining into the central vein. This flow establishes an oxygen gradient along the sinusoids critical for defining the liver zonation. In metabolic dysfunction-associated steatotic liver disease (MASLD), fat accumulation and fibrosis disrupt this architecture, contributing to localised hypoxia. Mounting evidence implicates hypoxia in MASLD, including the activation of canonical hypoxia sensors such as hypoxia-inducible factors. Moreover, chronic intermittent hypoxia, cha

Ventilatory control in young patients with narcolepsy type 1: A case-control study.

OBJECTIVES: Narcolepsy type 1 (NT1), characterized by orexin deficiency, may impair ventilatory control due to orexin neurons' role in CO2/pH chemosensitivity. Animal studies suggest attenuated hypercapnic-hyperoxic responses in orexin-deficient models, but human data are limited. METHODS: In this cross-sectional case-control study, we compared ventilatory control in children and adolescents with NT1 to that of controls matched for sex, age, body mass index, and apnea-hypopnea index (AHI). Ventilatory control was assessed using the hypercapnic, hyperoxic ventilatory response (HCVR) rebreathin

Weight-adjusted-waist index as a mediator in the association between atherogenic index of plasma and obstructive sleep apnoea: insights from NHANES 2015-2020.

BACKGROUND: Although extensive evidence has identified epidemiological risk factors for comorbidities related to obstructive sleep apnea (OSA), few studies have explored the linear relationship between the atherogenic index of plasma (AIP) and OSA, particularly regarding the mediating role of weight-adjusted-waist index (WWI). This study aimed to elucidate the linear relationship between AIP and OSA symptoms and to quantify the mediating effect of WWI.
