Hypocretin-1/orexin-A fragment1-16 as a potential surrogate marker for diagnosing narcolepsy type 1.

STUDY OBJECTIVES: Loss of cerebrospinal fluid (CSF) hypocretin-1/orexin-A (Hcrt-1) immunoreactivity is a biomarker for narcolepsy type 1 (NT1). The current radioimmunoassay (RIA) does not allow precise quantification, limiting the assessment of disease severity and evolution. This study aimed to reproduce previously proposed workflows to quantify Hcrt-1 and Hcrt-2 by liquid chromatography coupled to tandem mass spectrometry (LC-MS/MS), identify RIA antibody targets, and to compare two purification methods in their ability to identify patients with NT1. METHODS: Solid phase extraction (SPE) an

Postmortem Evidence of CRH Neuron Reduction in Narcolepsy Without Cataplexy With Borderline Hypocretin-1 Levels.

Narcolepsy is classified as type 1 (NT1) or type 2 (NT2) mainly according to hypocretin deficiency rather than cataplexy. While CRH neuron loss in the PVN has been described in NT1, it remains unclear whether similar changes occur in narcolepsy without cataplexy. We report a 42-year-old woman with excessive daytime sleepiness since age 15, without cataplexy. Two multiple sleep latency tests and lumbar punctures performed 8 years apart showed mean latencies of 8.3 min with three SOREMPs and 10.8 min with two SOREMPs, along with borderline low-intermediate hypocretin-1 levels of 100 pg/mL initia

Nocturnal features and daytime characteristics in narcolepsy: reliability and diagnostic relevance for NT1 Versus NT2.

STUDY OBJECTIVES: Assess test-retest reliability of nighttime sleep and daytime maintenance of wakefulness tests (MWTs) over a short time interval in drug-free individuals with narcolepsy; evaluate the diagnostic utility of features with good reliability. METHODS: We analyzed the placebo arms from clinical trials in narcolepsy type 1 (NT1; n = 17; NCT05687903) and narcolepsy type 2 (NT2; n = 19; NCT05687916). Participants visited the clinic three times, four weeks apart, which included nocturnal polysomnography (nPSG) and MWT the following day. From the nPSG and daytime MWT, we derived 440 fe

Slow Wave Activity During Nocturnal Sleep in Hypersomnolence Disorder.

Slow waves during non-rapid eye movement (NREM) sleep are associated with the restorative aspects of sleep. Previous research has suggested reduced normalized slow wave activity (SWA), particularly in the first cycle of NREM sleep, as a feature of hypersomnolence disorder (HD); however, the scarcity of existing research highlights the need for replication studies. Thus, the aim of this investigation was to evaluate normalized SWA in patients with HD, relative to healthy sleeper controls (HSC). Nocturnal polysomnography from 37 unmedicated HD clinical patients were compared against 29 HSC. Six-

Differences in Sleep Spindle Characteristics Between Children With Narcolepsy and Those With Other Causes of Subjective Sleepiness.

Central disorders of hypersomnolence are characterised by excessive daytime sleepiness. There has been growing interest in analysing sleep micro-architecture, to further understand how sleep is affected by these conditions. We investigated if sleep spindle activity differed between children with narcolepsy, idiopathic hypersomnolence (IH) and subjectively sleepy children with a non-diagnostic multiple sleep latency test, and individually age- and sex-matched controls. Twenty eight children with narcolepsy, 11 with IH and 26 with subjective sleepiness (aged 6-19 years) were included. Spindle ac

The Relationship Between Resting State Quantitative EEG and Daytime Sleepiness Complaints in Parkinson's Disease.

Although many studies have reported the presence of daytime sleepiness in Parkinson's disease (PD), either a weak or no association has been found between self-reported and current objective measures of daytime sleepiness in this population. Changes in resting state quantitative electroencephalography (qEEG) have been associated with increased subjective sleepiness induced by sleep deprivation in young and older adults. The aim of this study was to investigate the relationship between resting state qEEG and self-reported daytime sleepiness in adults with PD. Sixty-two participants with PD unde

Preceding night sleep time and multiple sleep latency test outcomes.

PURPOSE: Evidence supporting how much sleep the night before a multiple sleep latency test (MSLT) is required to generate valid results is lacking. We evaluated the association between preceding night total sleep time (TST) and next-day MSLT outcomes. METHODS: We retrospectively analyzed MSLT and preceding overnight diagnostic polysomnogram (PSG) data from January 1, 2008 until December 31, 2018 from St. Michael's Hospital (Toronto, Ontario, Canada). TST on the preceding overnight PSG was categorized as follows: ≥7.0 h vs. 6.00-6.99 h vs. 5.00-5.99 h vs. ≤5.00 h. Descriptive statistics at the

Differential 11-year mortality risk among subgroups of excessive daytime sleepiness in community-dwelling older adults: The Yilan study, Taiwan.

OBJECTIVES: Excessive daytime sleepiness (EDS) is prevalent among older adults, yet its varied manifestations and etiologies often lead to equivocal clinical significance. This study aimed to identify phenotypic subtypes of EDS and examine whether these subtypes carry differential risks for mortality. METHODS: This longitudinal, community-based, fixed-cohort study followed participants aged 65 and older. EDS was defined as a score of ≥ 11 on the Epworth Sleepiness Scale (ESS). K-means clustering analysis with 8 items of the ESS as indicator variables was performed to determine various classes

COVID-19-Induced Narcolepsy Type I With Cataplexy: A Short Report.

Narcolepsy type I is a rare neurological sleep disorder characterised by chronic excessive daytime sleepiness, cataplexy, and low orexin (hypocretin) levels. Infection with SARS-CoV-2 has been linked to neurological and neuropsychiatric sequelae, but cases of post-COVID narcolepsy remain extremely uncommon. We report a previously healthy 42-year-old Caucasian female who developed excessive daytime sleepiness, cataplexy, and hypnagogic/hypnopompic hallucinations within 3 months of confirmed SARS-CoV-2 infection. Polysomnography (PSG) showed disturbed nocturnal sleep with a high arousal index an

Candidate biomarkers in narcolepsy in H1N1-vaccinated cohorts: A proteomic and transcriptomic study.

Narcolepsy is a rare neurological disorder caused by the loss of function in neurons producing orexin or hypocretins, disrupting the sleep-wake cycle. In Sweden and Finland approximately 800 patients developed narcolepsy after swine flu (H1N1) vaccination between 2009 and 2010, probably as an autoimmune side effect. The vaccinated cohorts are of great scientific interest owing to their specific exposomes. We aimed to identify new biomarkers by analyzing serum from 39 narcolepsy type 1 (NT1) patients and 48 controls in Sweden, with subsequent validation in 38 NT1 patients and 44 controls from F

Cytokines in narcolepsy type 1: pathogenic relevance, biomarker potential, and future directions.

Narcolepsy type 1 (NT1) is a central disorder of hypersomnolence characterized by the selective loss of hypocretin/orexin-producing neurons. Its pathogenesis is closely associated with HLA genetic susceptibility, environmental triggers, and T-cell-mediated autoimmune responses. In recent years, increasing attention has been paid to the role of cytokines in the immunopathology of NT1. This review summarizes current evidence regarding cytokines in NT1, including IL-6, TNF-α, IFN-γ, IL-8, MCP-1/CCL2, CCL3, IL-4, and IL-10, and critically evaluates their roles as potential pathogenic mediators, di

Orexin pathway modulation in narcolepsy: from Autoimmune mechanisms to nanotechnology enabled therapeutic delivery.

Narcolepsy is a long-term brain disorder caused by disrupted orexin signaling. Orexin neurons sit in the hypothalamus and control the sleep-wake cycle; damage to these neurons leads to narcolepsy. Early evidence pointed to an autoimmune attack on orexin neurons as the main cause. Current treatments - stimulants, antidepressants, sodium oxybate, and scheduled naps - ease symptoms but do not stop the disease, and they often cause side effects such as anxiety, insomnia, and heart problems. Most of these drugs also cannot cross the blood-brain barrier (BBB) to act directly on the hypothalamus. Thi

The German version of the Sleep Inertia Questionnaire in central disorders of hypersomnolence: a validation study.

BACKGROUND: Sleep inertia, or sleep drunkenness, is a transitional state between sleep and wakefulness characterized by cognitive and physical impairment. It is particularly relevant in central disorders of hypersomnolence (CDH), but validated instruments to assess sleep inertia in German-speaking populations are lacking. This study translated and validated the German version of the Sleep Inertia Questionnaire (SIQ) in patients with CDH and healthy controls. METHODS: We included 109 participants from the international Swiss Primary Hypersomnolence and Narcolepsy Cohort Study (iSPHYNCS): idiop

A multidisciplinary biopsychosocial approach reveals behavioural phenotypes in idiopathic hypersomnia: a prospective single-arm study.

Idiopathic hypersomnia (IH) is clinically heterogeneous yet defined almost entirely in electrophysiological terms, and pharmacotherapy frequently fails to restore daytime functioning. We asked whether IH heterogeneity extends to stable behavioural phenotypes and whether an individualised, multidisciplinary, non-pharmacological programme is feasible and yields a clinically meaningful signal on symptom burden. In this prospective single-arm study, 20 adults with ICSD-3-TR-diagnosed IH at a tertiary sleep centre completed a programme combining psychoeducation, cognitive-behavioural therapy for hy
