iSPHYNCS: Unsupervised Clustering in Questionnaires and Metadata Reveals Distinct Subtypes in the Narcolepsy Borderland.

The international Swiss Primary Hypersomnolence and Narcolepsy Cohort Study (iSPHYNCS) is a multicentre study aimed at identifying novel biomarkers for central disorders of hypersomnolence (CDH). We analysed questionnaires and metadata to uncover distinct clusters of participants and explore phenotypic variability within CDH. Data were collected from 227 patients with CDH and 33 healthy controls. Participants completed validated clinical questionnaires and study-specific questions addressing CDH-related symptoms such as excessive daytime sleepiness, fatigue, cataplexy, disrupted sleep, and sle

Mazindol Immediate-Release/Sustained-Release (IR/SR): A 50-Year Legacy of Multifaceted Mechanisms and Emerging Therapeutic Potential.

Mazindol, an imidazo[2,1-a]-isoindole anorectic from the 1970s, has recently re-emerged as a candidate therapy for disorders of arousal and reward dysregulation, due to its unique multimodal receptor profile. Despite its 1999 withdrawal from US/EU markets, off-label narcolepsy efficacy spurred immediate-release/sustained-release (IR/SR) development. Clinical trials demonstrate mazindol IR/SR efficacy, reducing excessive daytime somnolence and cataplexy in narcolepsy and attention-deficit/hyperactivity disorder (ADHD) severity symptomatology, with mild adverse events (e.g., dry mouth). Preclini

Real-world effectiveness and tolerability of wake-promoting agents for residual sleepiness in treated obstructive sleep apnea.

Residual sleepiness despite effective positive airway pressure (PAP) therapy remains a persistent clinical problem in obstructive sleep apnea (OSA). This retrospective study evaluated real-world use, effectiveness, and tolerability of modafinil, armodafinil, and solriamfetol in 50 adults with OSA and residual sleepiness. Most patients received modafinil (80%) as their initial agent. Complete, partial, and poor responses occurred in 26%, 44%, and 30% of patients, respectively. Adverse events were reported in 38%-predominantly neurologic and psychiatric-and led to discontinuation in ten cases. R

Clinical and polysomnographic changes in idiopathic hypersomnia ten years after the diagnosis.

PURPOSE: Idiopathic hypersomnia (IH) is characterized by excessive daytime sleepiness and an excessive need for sleep. It is unknown whether the disorder is stable or improves with time. Our goal was to characterize the evolution of clinical and sleep patterns (including measures of excessive sleep need) in patients with IH over a ten-year period. METHODS: A standardized questionnaire focusing on the progression of clinical symptoms was sent to all patients with IH. At the time of diagnosis, all patients underwent overnight polysomnography and a multiple sleep latency test (MSLT), followed by

Clinical significance of ADHD traits in central disorders of hypersomnolence.

AIM: Attention-deficit/hyperactivity disorder (ADHD) is increasingly recognized as a potential comorbidity in central disorders of hypersomnolence (CDHs), including narcolepsy type 1 (NT1), narcolepsy type 2 (NT2), and idiopathic hypersomnia (IH). This study aimed to characterize ADHD traits within these CDH populations and evaluate their association with daily functioning. METHODS: Adult participants (≥ 18 years old) with a clinical diagnosis of NT1, NT2, or IH were enrolled. Matched controls were recruited through an online platform. All participants completed validated self-report questionn

Cataplexy-Like Episodes Following Suvorexant Use in a Non-Narcoleptic Patient: A Brief Report.

Dual orexin receptor antagonists (DORAs) such as suvorexant are effective treatments for insomnia but are associated with rapid eye movement (REM) sleep-related adverse effects. While sleep paralysis and hypnagogic hallucinations are recognised, cataplexy is not typically reported. We describe a 39-year-old male US Navy Veteran with chronic pain, long-standing insomnia and anxiety who developed recurrent, emotionally triggered episodes of bilateral ptosis, slurred speech, unsteadiness and subjective "euphoric" sensations shortly after initiating suvorexant. Neurologic evaluation, electroenceph

The impact of paediatric periodic limb movement disorder and obstructive sleep apnoea syndrome on sleep quality and daytime symptoms.

PURPOSE: There is little available literature comparing paediatric periodic limb movement disorder (PLMD) and paediatric obstructive sleep apnoea syndrome (OSA). This retrospective study aimed to examine the impact of PLMD, OSA and OSA with an elevated periodic limb movement index (PLMI) on sleep quality and daytime symptoms. METHODS: This retrospective study examined 132 children who had been diagnosed with OSA (with or without elevated PLMI) and PLMD. All participants underwent overnight polysomnography. Patients were divided into three groups, patients with OSA with low PLMI, patients with

Samelisant (SUVN-G3031), a histamine H3 receptor inverse agonist, as a potential treatment for narcolepsy: a phase-2 drug evaluation.

INTRODUCTION: Narcolepsy is a debilitating chronic neurological disorder, and currently, there is no cure. Activation of the histaminergic system through the blockade of histamine 3 (H3) autoreceptors promotes wakefulness. Blockade of H3 heteroreceptors modulates adrenergic, serotonergic, cholinergic, and dopaminergic systems. These properties of H3 receptors may offer therapeutic options for managing the symptoms of narcolepsy. Samelisant is an investigational H3 receptor inverse agonist/antagonist being developed for the treatment of hypersomnia disorders. AREAS COVERED: This review include

How sleeping minds decide: State-specific reconfigurations of lexical decision-making.

Sleep has traditionally been conceptualized as a state of cognitive disconnection, yet emerging evidence indicates that decision-making capacities persist across sleep stages. Here, we elucidate the computational mechanisms underlying real-time lexical decision-making during polysomnographically-verified sleep, using facial electromyography and hierarchical drift diffusion modeling in both healthy individuals and participants with narcolepsy. We found that lexical decision-making was preserved during N1 and lucid REM sleep, but relied on distinct computational strategies: in N1 sleep, both enh

Initial validation of a pediatric cataplexy survey for children and adolescents with narcolepsy type 1.

PURPOSE: Cataplexy is core symptom of pediatric narcolepsy type 1 (NT1) but there is currently no validated cataplexy scale that captures the frequency, severity, and impact of cataplexy for children and adolescents to guide care. In this study, we developed and validated a 7-item Pediatric Cataplexy Scale (PCS). METHODS: We performed psychometric evaluation of the PCS using item response theory, specifically a graded response model (GRM). Sixty-nine children and adolescents with NT1 (age 9–21) from six North American sites completed the study. We examined the 7-item scale for internal consist

Pitolisant-associated mania - a case report.

BACKGROUND: Narcolepsy is a chronic neurological disorder treated with medications that promote wakefulness. Pitolisant, a histamine H3 receptor inverse agonist, is used to reduce excessive daytime sleepiness. Although pitolisant clinical trials describe mainly mild adverse effects, severe psychiatric reactions have been reported, yet detailed clinical descriptions remain limited. This case describes a first-onset manic episode with psychotic features shortly after pitolisant initiation in an adult without any psychiatric history, highlighting a rare but clinically significant potential reacti

Perspectives on idiopathic hypersomnia: diagnostic challenges, unknown pathophysiological, and emerging therapeutic strategies.

Idiopathic hypersomnia is a rare chronic neurological condition that may be characterized by excessive daytime sleepiness, and/or prolonged nocturnal sleep, and also sleep inertia, yet its underlying mechanisms and diagnostic boundaries remain poorly defined. A key challenge is distinguishing excessive sleep duration from excessive daytime sleepiness, as patients may present one or both features, and similar symptoms can arise from diverse medical, psychiatric, and sleep disorders. Current diagnostic criteria rely heavily on the multiple sleep latency test, which frequently underestimates hype

Incidence and prevalence of obstructive sleep apnoea and narcolepsy in the UK: a population-based descriptive study.

BACKGROUND: Obstructive sleep apnoea (OSA) and narcolepsy are estimated to affect approximately 4.8% and 0.047% of the UK population, respectively. We do not know how many people have been diagnosed or how this varies over time and by demographic factors. METHODS: We, therefore, conducted a historical population-based descriptive study estimating prevalence and incidence of diagnosed OSA and narcolepsy in England from 2000 to 2019 stratified by demographic factors, and compared estimates to Scotland, Wales and Northern Ireland. Data were from Clinical Practice Research Datalink (CPRD) primary

Narcolepsy and metabolic disease: A meta-analytic synthesis.

Narcolepsy is a chronic neurological disorder marked by sleep-wake dysregulation and an elevated burden of endocrine and metabolic comorbidities. However, evidence on specific metabolic outcomes across narcolepsy subtypes remains heterogeneous. We conducted a systematic review and meta-analysis (PROSPERO CRD42024576070) to primarily quantify the prevalence and risk of metabolic syndrome (MetS) in narcolepsy, with additional cardiometabolic conditions assessed as secondary outcomes. Five databases were searched through April 8, 2025, included 54 studies after screening. Pooled prevalence estima

Unmasking the hidden role of sleep bruxism in obstructive sleep apnea patients' sleepiness.

BACKGROUND: Obstructive sleep apnea (OSA) and sleep bruxism (SB) are both sleep-related conditions that may affect sleep architecture and potentially contribute to daytime sleepiness (DS). AIM: This study aimed to explore the association between SB and DS as measured by Epworth Sleepiness Scale in patients with OSA. METHODS: A cross-sectional study was conducted involving 345 patients diagnosed with OSA. The presence of SB was assessed through multidisciplinary clinical examination and audio-video recordings during polysomnography. DS was measured using the Epworth Sleepiness Scale (ESS). Demo

Homeostatic dysregulation of slow wave activity during sleep in idiopathic hypersomnia.

STUDY OBJECTIVES: Sleep homeostasis describes the accumulation of sleep pressure during wakefulness and its dissipation during sleep. Electroencephalographic delta power (SWA, [0.5-4 Hz]) is a known marker of sleep pressure. Dysregulation of this process has been proposed in idiopathic hypersomnia (IH), a debilitating disorder characterized by severe daytime sleepiness and excessive sleep duration. We investigated SWA regulation in IH with (IH-LST) and without (IH-nLST) long-sleep time and healthy participants in a controlled ad libitum sleep protocol. METHODS: We recorded SWA in 208 IH parti
