The cyclic alternating pattern is a hallmark of the dynamic organisation of non-rapid eye movement sleep, reflecting the brain's oscillatory regulation of arousal and sleep stability. Since the original publication of the rules for scoring the cyclic alternating pattern in 2001, major advances in sleep neurophysiology, signal analysis and international sleep staging standards have necessitated a comprehensive revision. This consensus report by a Taskforce of the European Sleep Research Society presents the updated 2025 Atlas and Rules for the scoring of the cyclic alternating pattern in human

BACKGROUND: Obstructive sleep apnoea (OSA) is a prevalent disorder. No consistent mortality benefit from treating OSA has been reported in randomised controlled trials. This analysis of an age-and-sex-representative anonymised German public health insurance database from 2015-2020 investigated mortality and hospitalisation rates in positive airway pressure (PAP)-treated versus non-PAP-treated individuals with OSA, accounting for key confounders, including demographic factors, socioeconomic status, comorbidities and medication use.
METHODS: Treatment-naïve individuals with OSA were selected an

PURPOSE: To assess whether a Network Physiology approach using Sample Entropy and Transfer Entropy can be applied to simple physiological signals obtained during sleep studies, to accurately distinguish between different types of sleep disordered breathing (SDB) and streamline the diagnostic process.
METHODS: Retrospective study on patients who underwent a sleep study between January 2017 and December 2021. The training dataset, used for algorithm development, included four clinically important groups: normal, obstructive sleep apnoea alone, sustained nocturnal hypoxemia with a high AHI (≥ 30

BACKGROUND: Dietary melatonin, naturally occurring in plant- and animal-based foods, has been linked to beneficial effects on sleep, mood and metabolic health. Although evidence suggests that food-derived melatonin may elevate circulating levels, few studies have assessed its intake through habitual diets or explored associations with chronic disease outcomes in adults.
METHODS: We conducted a cross-sectional analysis within an open cohort of university graduates (baseline data). Dietary intake was evaluated using a validated 144-item Food Frequency Questionnaire (FFQ), and melatonin intake w

INTRODUCTION: Positive airway pressure (PAP) therapy is the recognised treatment for sleep disordered breathing (SDB), delivered via a tight-fitting face mask (interface). Conventional interfaces do not consider facial geometries, often resulting in poor fit and ineffective therapy. Three-dimensional (3D) printing of customised interfaces may improve comfort and outcomes.
OBJECTIVES: To evaluate the clinical impact of customised versus conventional oronasal interfaces in adults with obstrcutive sleep apnoea (OSA). The primary outcome was residual Apnoea Hypopnea Index (AHI) at 6 months; secon

The consequences of untreated obstructive sleep apnea (OSA) on health are well established. OSA alterations to sleep microarchitecture include a reduction in sleep spindles during NREM sleep, a neurophysiological biomarker that reflects sleep stability, cognitive function and brain health. However, the effects of OSA treatment on reversing sleep spindle abnormalities remains unclear. This systematic review evaluated the effect of continuous positive airway pressure (CPAP) treatment on spindles during polysomnography in adults diagnosed with OSA. We identified five papers meeting inclusion crit

BACKGROUND: The Non-Specific Pattern (NSP) is characterized by a reduced Forced Expiratory Volume in 1 s (FEV₁) and/or Forced Vital Capacity (FVC), a preserved FEV₁/FVC ratio, and a normal Total Lung Capacity (TLC). Although recognized in recent American Thoracic Society/European Respiratory Society (ATS/ERS) recommendations, this pattern remains poorly understood.
OBJECTIVE: To systematically review the literature addressing the NSP in adult patients, with a focus on its physiological definition, clinical correlates, underlying mechanisms, and longitudinal outcomes.
METHODS: A systematic se

OBJECTIVES: Hypoxic burden (HB) quantifies obstructive sleep apnoea (OSA)-associated nocturnal intermittent hypoxemia as the area under the peripheral desaturation curve near an apnoeic or hypopnoeic event. We assessed whether HB is a better marker of the cardiovascular effects of OSA than usual OSA severity markers.
METHODS: In a continuous positive airway pressure (CPAP) withdrawal study in OSA patients (apnoea-hypopnoea-index (AHI) > 10/h), randomized in a 1:1 ratio to two weeks of CPAP continuation or sham CPAP, the relationship between the treatment effect on HB (ΔHB) and the treatment e

UNLABELLED: Utility-based quality-of-life measures are generally recommended for economic evaluation. However, disease-specific, non-utility outcome measures are used more frequently than the former, particularly in sleep health studies. This paper aimed to establish a mapping algorithm between two of the most used instruments in sleep health and the general population, respectively: the Functional Outcomes of Sleep Questionnaire 10 items (FOSQ-10) and the EuroQoL 5 Dimensions 5 Level (EQ-5D-5 L). In-sample cross-validation approach was performed using a k-fold technique to randomly divide the

We examined the urinary metabolome of those with obstructive sleep apnoea hypopnoea syndrome (OSAHS), and how continuous positive airway pressure (CPAP) affects it. We compared it with their non-OSAHS counterparts to identify any distinguishing biomarkers that could be used to diagnose and monitor, as well as highlight those at increased cardiometabolic risk. Observational, prospective and longitudinal study including 70 consecutive attenders referred to our sleep service. OSAHS was defined as apnoea-hypopnoea index ≥ 15 events/h and Epworth sleepiness score (ESS) > 10. Patients treated with C

Dentists in general practice currently play a very limited role in the care of sleep apnoea patients. Patients are referred by a primary care physician, usually a general practitioner, to a secondary medical care specialist if sleep apnoea is suspected. A medical specialist provides further analysis, diagnosis, and initiation of treatment. Oral healthcare providers who are trained for this can play an important role in early detection and referral of sleep apnea to the general practitioner and in treatment with a mandibular repositioning appliance if indicated by a medical specialist.

Bimaxillary osteotomy is skeletal surgery for the treatment of obstructive sleep apnoea. Several studies have shown bimaxillary osteotomy to significantly improve respiratory parameters - including the apnoea-hypopnoea index, zero oxygen saturation, and the respiratory disturbance index - in the short and the long term. Additionally, research has shown that bimaxillary osteotomy contributes to a reduction in systemic blood pressure and to improvements in sleep architecture and neurocognitive functions. Subjectively, a significant proportion of patients report a decrease in daytime sleepiness,

Obstructive sleep apnoea is a sleep-related breathing disorder characterized by repeated episodes of partial or complete obstruction of the upper airway during the sleep. The severity of obstructive sleep apnoea is determined by the apnoea-hypopnea index, which represents the number of apnoea s and hypopnea s per hour of sleep. A systematic literature review on the association between obstructive sleep apnoea and the periodontal condition was conducted in the PubMed and Dentistry and Oral Sciences databases. Independent selection and assessment of methodological quality were performed by 2 res

Obstructive sleep apnoea is a prevalent disorder, with a significant portion of patients being edentulous. Continuous positive airway pressure therapy is the gold standard for treating obstructive sleep apnoea. Due to low compliance to this treatment method, however, effective alternative solutions are often looked for. The mandibular advancement device presents a potential option but is often dismissed for edentulous patients because of perceived issues with retention and stability. On the basis of a review of the literature regarding mandibular advancement device therapy in edentulous patien

A recent NTVT reader survey (n = 75) shows that oral healthcare professionals increasingly acknowledge obstructive sleep apnoea as part of their field. 73% consider screening for OSA a dentist s role, 80% believe this competency should be included in the curriculum, and 87% are positive about collaboration with medical specialists. At the same time, only 57% feel sufficiently equipped to recognize obstructive sleep apnoea, and just 27% have insight into its prevalence in their own practice. Willingness to pursue further training is high (72%). These findings, summarized in an infographic, demo

PURPOSE: Continuous Positive Airway Pressure (CPAP) is the main treatment for patients with obstructive sleep apnoea (OSA). Hypoxemic patients with sleep-disordered breathing may require CPAP and additional low-flow oxygen. However, specific recommendations on where to attach the oxygen bleed-in connector are lacking. METHODS: This randomised cross-over trial assessed the fraction of inspired oxygen (FIO₂) in a simulated OSA patient’s lung resulting from different rates of low-flow oxygen (1, 2 and 3 l/min) added via three different bleed-in connector locations under different levels of either
