The diagnostic criteria for narcolepsy continue to evolve as more is learned about the features of this and other sleep disorders. A variety of symptoms have been said to distinguish narcolepsy from other sleep disorders, including cataplexy, character of daytime sleepiness, sleep paralysis, hypnagogic hallucinations, and automatic behavior. Other diagnostic assessments, such as determination of human leukocyte antigen (HLA) haplotype and findings of sleep laboratory assessments, also contribute to the differential diagnosis. As diagnostic and analytic techniques have become more sophisticated

The concept of idiopathic hypersomnia (IHS) was reviewed. The minimal criteria for IHS in the International Classification of Sleep Disorders (ICSD) are not clear enough to designate a clinical entity. We propose our criteria for IHS, which consist of 6 items. 1. Recurrent daytime sleep episodes with duration of longer than one hour that occur almost everyday for at least 6 months. 2. Absence of refreshed feeling after the daytime sleep episodes. 3. Length of nocturnal sleep is either normal or prolonged. 4. Total sleep time per day exceeds 10 hours. 5. Absence of cataplexy. 6. Does not meet c

Excessive daytime sleepiness (EDS) is a common and debilitating symptom of narcolepsy and other sleep disorders. Modafinil is a novel stimulant which effectively treats EDS, yet lacks many of the undesirable side-effects commonly encountered with currently available compounds. The specific mode of action of modafinil is not well understood, but it may promote sleep by indirectly influencing adrenergic or GABAergic neurotransmission. Modafinil-induced wakefulness is not associated with rebound hypersomnolence or the potential for abuse as is often encountered with other stimulants such as amphe

Background: Sleep paralysis is one of the many conditions of which visual hallucinations can be a part but has received relatively little attention. It can be associated with other dramatic symptoms of a psychotic nature likely to cause diagnostic uncertainty. Methods and results: These points are illustrated by the case of a young man with a severe bipolar affective disorder who independently developed terrifying visual, auditory and somatic hallucinatory episodes at sleep onset, associated with a sense of evil influence and presence. The episodes were not obviously related to his psychiatric

BACKGROUND: Non-gestational ovarian choriocarcinomas (NGOC) are rare, distinct, highly aggressive tumors, primarily affecting young women. It accounts for less than 0.6% of malignant ovarian germ cell tumors. It is associated with a poorer prognosis compared to gestational choriocarcinoma.
CASE PRESENTATION: A 36-year-old woman (P2L2) presented with intermittent heavy menstrual bleeding for the past three months. The urinary pregnancy test was positive. On abdominal examination, a solid mass consistent with 20-weeks gravid uterus was palpated in right iliac fossa. Bimanual pelvic examination

OBJECTIVE: Disruptive nocturnal behaviors (DNBs) of older people often threaten the caregiving arrangements on which their community tenure depends. Dementing disorders are especially prone to result in disrupted sleep and agitated behaviors ("sundowning"). The objective here was to develop an objective correlate of DNBs, by which their severity and effects on caregivers can be measured.
DESIGN: Quantitative comparison of subjective sleep and motor activity patterns in older people and their caregivers. It was hypothesized that older people with reported DNBs would be more motorically active

To explore further the relationship between ease of falling asleep, ability to maintain wakefulness, attention and information processing in sleep apnea and other sleep disorders, we conducted a thorough analysis of the similarities, differences and correlations between auditory and visual P300 amplitudes and latencies, and tests of sleepiness. The 283 consecutive patients presenting with hypersomnia were administered nocturnal polysomnography. Next day they underwent auditory and visual P300 recordings, Multiple Sleep Latency Test (MSLT) and Maintenance of Wakefulness Test (MWT). Correlation

Children with sleep disorders are often inattentive or hyperactive, and some carry a diagnosis of attention deficit/hyperactivity disorder (ADHD) until their sleep disorder is detected. However, the potential behavioral impact of undiagnosed sleep disorders is not known. We sought to determine whether children with higher levels of inattention and hyperactivity more frequently have symptoms of sleep-related breathing disorders (SRBDs) or periodic limb movement disorder (PLMD). We surveyed parents of 2-18-year-old patients at a child psychiatry clinic (n = 70) and a general pediatrics clinic (n

Restless legs syndrome (RLS) is a common sensorimotor disorder with an estimated prevalence of between 1% and 5%. The symptomatology is characterized by unpleasant sensations experienced predominantly in the legs and rarely in the arms. The symptoms occur only at rest and become more pronounced in the evening or at night. In addition, the patients suffer from a strong urge to move the limbs, typically manifest as walking around, which leads to complete but only temporary relief of the symptoms. Most of the patients with RLS have periodic leg movements (PLMS) during sleep and relaxed wakefulnes

OBJECTIVE: To determine whether upper airway resistance syndrome (UARS) can be recognized and distinguished from obstructive sleep apnea syndrome (OSAS) in prepubertal children based on clinical evaluations, and, in a subgroup of the population, to compare the efficacy of esophageal pressure (Pes) monitoring to that of transcutaneous carbon dioxide pressure (tcPCO2) and expired carbon dioxide (CO2) measurements in identifying UARS in children.
STUDY DESIGN: A retrospective study was performed on children, 12 years and younger, seen at our clinic since 1985. Children with diagnoses of sleep-di

Narcolepsy is a commonly under diagnosed or misdiagnosed problem that results in severe daytime sleepiness. There is a strong genetic component with some immune system involvement. It may occur in combination with other sleep disorders such as sleep apnea complicating its treatment. An objective diagnosis requires a polysomnogram and Multiple Sleep Latency Test. Management depends on the careful use of stimulant medication to control the sleepiness and other medications to control the auxiiliary symptom of cataplexy.

Over the past 30 years, significant progress has been made in understanding the physiologic mechanisms of sleep. Insomnia, a common complaint in general medical practice, and other sleep disorders have become increasingly recognized. In 1986, a heritable total insomnia was described and termed fatal familial insomnia; since then, the pathology of this disease has been shown to involve an accumulation of prion particles in the brains of affected patients. Prions have been more commonly associated with the transmission of spongiform encephalopathies such as scrapie (in sheep), Creutzfeldt-Jakob

Ambulatory EEG (A-EEG) allows longterm polygraphic recording over 24 hours or more and proves to be particularly useful in the diagnosis of narcolepsy-cataplexy (N/C). We performed A-EEG using the Medilog 9000-8 channel system over a total of 36 24-hour periods in 20 consecutive N/C patients and recorded an average of 3.5 daytime sleep episodes, of which 2.2 were with SOREMP, AND 21 evening SOREMP (58%). A MSLT was performed using A-EEG in 16 cases. Continuous A-EEG proved to be more informative than the MSLT, and may represent a valid alternative to the classic continuous polygraphic recordin

The National Comission on Sleep Disorder Research evaluated the status of insomnia and other sleep disorders in the US. The Commission found that insomnia is prevalent affecting 20-30 million Americans, has significant morbidity especially related to attention and memory processes and cost the US an estimated $ 15 billion a year. It also estimated that all of these figures should rise significantly by the year 2000. Other sleep disorders (eg, narcolepsy and sleep apnea) although less prevalent have significant costs to patients and society associated with them. The Commission recommended that

Complaints of sleep disturbance increase with age. Objective sleep assessments using polysomnography reveal sleep impairments (increased wakefulness and arousal from sleep; decreased slow wave sleep) even in healthy seniors. Both polysomnographic sleep and subjective sleep worsen in the presence of health impairments related to drug use, pain, cardiovascular disease, diabetes, depression, or other emotional disorders. In addition to normal aging and chronic disease, sleep complaints can also result from poor sleep habits, specific occult disorders during sleep, or some combination of these fac

Questionnaire data from patients presenting at three sleep disorders centers were used to develop and assess a screening tool for sleep apnea based on the reporting of the frequency of various symptoms of sleep apnea and other sleep disorders plus age, body mass index (BMI) and gender. Patients were not specifically referred for suspicion of sleep apnea. Separate factor analyses of survey responses from 658, 193 and 77 respondents from the first, second and third sites, respectively, each yielded four orthogonal factors, one of which accounted for all the questions concerned with the frequency

OBJECTIVE: Delayed sleep phase syndrome is a common but little reported cause of severe insomnia. Since it was first described, few detailed reports of delayed sleep phase syndrome have appeared, and treatment methods have not been reviewed. From the literature, the authors provide diagnostic descriptions and review treatment methods, and from their sleep disorder clinic, they describe the management and outcome of the largest series of patients with delayed sleep phase syndrome thus far reported.
METHOD: The authors reviewed all articles with primary data on delayed sleep phase syndrome publ

The objectives of this study were to test the sensitivity of the short form 36 health survey questionnaire (SF 36) to sleep disruption in patients with obstructive sleep apnoea (OSA) and assess its use as an outcome measure for treatment with nasal continuous positive airway pressure (CPAP). Two hundred and twenty-three subjects under investigation for snoring and/or daytime somnolence completed the questionnaire at presentation and again after a six month period. Subjects with OSA requiring treatment scored lower on all dimensions of the SF 36 (P < 0.05) than normative scores for the general

It is well established that, as a group, insomnia patients overestimate sleep onset latency (SOL) and underestimate total sleep time (TST) when compared to objective polysomnographic (PSG) findings. Whether a similar phenomenon occurs with other sleep disorders is not fully established. We compared the PSG sleep of 84 patients with suspected sleep apnea (SA) to their subjective experience of sleep reported on a sleep diary the morning after PSG testing. Both patients with SA (SA+) and those without (SA-) tended to overestimate SOL, but the SA+ group (n = 50) made larger overestimations (p < 0.
