Identification and treatment of sleep apnea in patients with chronic headache.

This study investigates the relationship between nocturnal or morning headache and obstructive sleep apnea syndrome (sleep apnea). It is not known if headache of any type is more common in patients with sleep apnea than in other patients, but morning headache is a symptom of sleep apnea. A method is needed for identifying patients with chronic headache who might benefit from evaluation and treatment of sleep apnea. We performed a retrospective assessment of frequency of morning headache in patients grouped according to final diagnosis: sleep apnea (n = 72), periodic leg movements of sleep (n =

[Clinical use of polysomnography (PSG)].

The clinical diagnoses in 695 cases using PSG were analysed. The results are as follows: Obstructive sleep apnea syndrome (OSAS) in 482 cases (69.4%), chronic obstructive pulmonary diseases with nocturnal hypoxemia in 25 cases (3.6%), epilepsy in 31 cases (4.5%), other sleep disorders in 5 cases (0.7%), and essentially normal in 152 cases (21.9%). Some parameters of PSG in 65 cases with OSAS and those in 2 cases before and after treatment were analysed. It is emphasized that PSG is using for diagnosis of different types of OSAS with appraisable seriousness and of other related sleep disorders.

Polysomnography methods and interpretations.

As the field of sleep disorders medicine continues to mature, appropriate diagnostic techniques are becoming properly defined and standardized. This article focuses principally upon diagnostic testing for sleep apnea, although other sleep disorders are discussed briefly. When interpreting a polysomnogram, one must consider a number of complex variables. A critical discussion of the methods for adequately measuring these variables is provided together with guidelines for appropriate interpretation.

Narcolepsy-cataplexy in Israeli Jews is associated exclusively with the HLA DR2 haplotype. A study at the serological and genomic level.

Narcolepsy is a very rare disease among Israeli Jews with a frequency of 7/3 X 10(6). An investigation of the association of narcolepsy with the human leukocyte antigen system was conducted in Israeli Jews at the serologic and genomic levels. The human leukocyte antigen class I and class II antigen typing of 7 clinically diagnosed narcoleptics, 3 individuals suffering from sleep disorders other than narcolepsy, and 11 healthy matched controls revealed that all narcoleptic patients (100%) investigated in the present study carried the HLA-DR2 haplotype, whereas patients with other sleep disorder

Current pharmacologic management of narcolepsy.

Narcolepsy is an incurable sleep disorder characterized by attacks of sleepiness and a series of auxiliary symptoms: cataplexy, sleep paralysis and hypnagogic hallucinations. Classic treatment has included stimulants to control sleepiness and tricyclic antidepressants to control the auxiliary symptoms. Polysomnography is necessary to confirm the diagnosis and to detect other sleep disorders. Recent developments in treatment include the use of codeine for sleepiness and gamma-hydroxybutyrate for auxiliary symptoms.

Sleep disorders in the elderly: rationale for clinical awareness.

Complaints about sleep are extremely common in the elderly, leading to an impression that aging-related sleep problems are virtually normal and benign. However, studies have shown that such complaints as habitual snoring, frequent awakening, nocturnal sweating, and awakening with anxiety, may be signs of genuine sleep disorders. The most prevalent and most serious aging-related sleep disorder is sleep apnea. There is recent evidence of an association between sleep apnea and circulatory disorders, including hypertension, stroke, and angina pectoris, and with reduced life expectancy. The older s

Nocturnal REM latency and sleep disturbance in narcolepsy.

A retrospective study of 92 narcoleptics was undertaken to investigate the significance of prolonged nocturnal REM latencies observed in approximately one in every five narcoleptics undergoing single all-night clinical polysomnograms in our laboratory. Clinical and laboratory findings were examined as a function of REM latency. Our findings emphasize a high incidence of other sleep disorders, particularly sleep-related periodic leg movements, in narcoleptics. Furthermore, sleep disturbance secondary to other sleep disorders, especially during the period of NREM sleep preceding the first REM ep

Sleep and violence.

Most violence connected with sleep disorder is assumed to be related to sleep walking. It is less well known that other sleep disorders can also give rise to violence. The role of narcolepsy in car accidents is mentioned. Sleep drunkenness can lead to confusion resulting in violent behaviour especially on forced awakening. This condition is associated to sleep apnea. Primary or central sleep apnea is caused by disorders of the brain stem affecting the respiratory center. Secondary or upper airway sleep apnea can be caused by virtually any condition that results in cessation of the air flow due

Insomnia and other sleep disorders.

In treating the symptomatology of patients who are chronically ill, it is likely that the physician will encounter a significant number of patients who have sleep disorders. This article has described an approach for the assessment, differential diagnosis, and clinical management of the most prevalent sleep disorders, which are often chronic in nature and have significant psychosocial or medical consequences for the patient and his or her family. We have emphasized the importance for the physician to complement his general medical and diagnostic skills by taking a careful sleep history from th

Taking a sleep history.

In taking a sleep history, the physician determines the onset, course and clinical characteristics of the disorder; assesses the 24-hour sleep/wakefulness patterns; obtains additional information from the bed partner, and inquires about a personal or family history of other sleep disorders. The impact of the disorder on the patient's life is evaluated, and the attitudes of the patient and his family toward the sleep problem are assessed.

Kleine-Levin syndrome associated with fire setting.

Kleine-Levin syndrome is characterized by periodic hypersomnia associated with megaphagia and striking behavioral and psychiatric symptoms; it occurs primarily in adolescent boys. We treated a 17-year-old boy who had typical recurring somnolent episodes. His bizarre behavior included fire setting and stealing, both of which may have represented compulsions. His EEG during a sleepy episode was diffusely abnormal with generalized slowing of background activity; as he awoke and improved clinically, his EEG returned to normal. Between episodes, his EEG was normal. The relationship between Kleine-L
