Sustained Effectiveness of Doxycycline Post-Exposure-Prophylaxis in a Large Sexual Health Clinic Over 96 Weeks: An Interrupted Time Series Analysis.

Doxycycline post-exposure prophylaxis (doxy-PEP) reduced bacterial sexually transmitted infection (STI) incidence in a sexual health clinic over 96 weeks (N = 4592; 2524 on doxy-PEP), particularly for chlamydia and syphilis, with smaller effects for gonorrhea. Continued surveillance for gonorrhea and antibiotic resistance is essential to evaluate doxy-PEP's long-term effectiveness.

Regression-based risk scores using sociodemographic and sexual behaviour data do not predict asymptomatic sexually transmitted infections among HIV PrEP users.

OBJECTIVES: Among users of oral HIV pre-exposure prophylaxis (PrEP), condom use is low and incidence of sexually transmitted infections (STIs) is high, hence guidelines recommend STI screening every 3-6 months. Identifying individuals with higher asymptomatic STI risk for targeted screening may offer an opportunity to reduce the burden of STI screening. METHODS: In the Netherlands, PrEP has been offered through the National PrEP Pilot Program since 2019, which includes screening every 3 months. We included data of all individuals who received care through the PrEP programme between July 2019

Prevalence of chlamydial, gonococcal and syphilis positivity by anatomical site and sex via self-collected biospecimens using at-home testing kits among a large and diverse cohort of men and women enrolled in the MACS/WIHS Combined Cohort Study (MWCCS) in the USA.

INTRODUCTION: Cisgender men who have sex with men (MSM), transgender women, cisgender women and cisgender men who have sex with women only (MSW) have differential risk of acquiring Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (GC), the two most commonly reported bacterial sexually transmitted infections (STIs) in the USA. Similarly, MSM and transgender women have a higher number of syphilis infections in the USA than other groups. The presence of any of these three STIs is a significant risk factor for HIV acquisition and transmission, unless taking antiretroviral medication for HIV pr

Newer and emerging sexually transmitted infections: A narrative review.

Sexually transmitted infections (STIs), including traditionally curable diseases like syphilis and gonorrhoea, have seen a recent resurgence. Concurrently, newer atypical infections presenting as STIs have emerged, driven by changes in pathogen virulence, host behaviours, and environmental factors. The spectrum of STIs is expanding due to evolving behaviours, globalisation, and pathogen adaptability. This review aims to highlight key viral, bacterial, fungal, and enteric infections that are newly identified or increasingly transmitted through sexual contact, introducing the concept of sexually

Aortic paleopathology: osteological evidence of an aortic aneurysm in historic human remains from Lithuania (17th18th centuries AD).

This report investigates a probable aortic aneurysm in a skeleton from Kedainiai, Lithuania, assessing cardiovascular signs within a sample of 22 adults. Notably, sternal bone changes indicative of chronic aneurysmal pressure were observed. The remains, identified as those of a middle-aged adult male (ca. 50 years old), exhibited an oval-shaped perforation and surrounding erosion and remodeling on the sternal body, consistent with chronic pressure exerted by a dilated ascending aorta. These findings were systematically compared with documented cases of aortic aneurysms from osteological specim

Prevalence of antenatal syphilis remains positively associated with HIV in South African districts.

BACKGROUND: Previous analyses from South Africa have found that the ecological association between the prevalence of HIV and syphilis switched from positive to negative in the wake of the HIV pandemic. This was attributed to the impact of HIV mortality on sexual network connectivity. We conducted an ecological analysis of HIV prevalence and syphilis seropositivity among all districts in South Africa in 2010 and 2022. METHODS: We used Spearman's correlation to compare the weighted prevalence of HIV and syphilis per district using South Africa's 2022 Antenatal HIV Sentinel Survey. We calculated

Should we still treat asymptomatic Neisseria gonorrhoeae and Chlamydia trachomatis contacts in the era of NAATs? Reviewing data from the GetaKit study to inform practice.

OBJECTIVES: We sought to determine positivity rates for participants in the GetaKit study who identified as asymptomatic contacts of Neisseria gonorrhoeae (NG) and/or Chlamydia trachomatis (CT). Our goal was to shed light on current discrepancies between international sexually transmitted infection (STI) guidelines, some of which recommend blanket empiric treatment for all NG and CT contacts (eg, Canadian and United States guidelines) versus others which recommend waiting for positive results (BASHH, Australian, and European). METHODS: We extracted data for the period of 1 June 2023 to 30 Jun

Syphilis testing and treatment outcomes among people experiencing homelessness: a street medicine intervention in South Los Angeles, California.

OBJECTIVE: To evaluate the outcomes of a street medicine intervention offering syphilis testing and treatment to people experiencing homelessness in South Los Angeles, California-a population disproportionately affected by syphilis and underserved by traditional healthcare systems. METHODS: From September 2024 to January 2025, a multidisciplinary street medicine team provided testing and treatment for syphilis during outreach visits to patients experiencing homelessness. A total of 99 patients were tested using the reverse syphilis testing algorithm. Demographic data included gender, race/eth

The Relationship Between HIV Preexposure Prophylaxis Use and Laboratory-Confirmed Bacterial Sexually Transmitted Infection Positivity Among a Diverse, Multi-City Cohort of Sexually Active Sexual and Gender Minorities in the United States.

The presence of chlamydia, gonorrhea, or syphilis infection is a significant risk factor for HIV acquisition and transmission and disproportionately impacts men who have sex with men (MSM) and transgender women. While HIV preexposure prophylaxis (PrEP) reduces HIV risk, its use may influence sexual behaviors, potentially increasing sexually transmitted infection (STI) exposure. Conversely, PrEP users are often more engaged in care, regularly screened and treated for STIs, and may access other prevention tools such as doxycycline postexposure prophylaxis. Studies on the relationship between PrE

Pitfalls in the Serologic Diagnosis of Maternal Syphilis: A Case Series.

Pregnancy is associated with false-positive treponemal and nontreponemal screening test results. We present 5 pregnant women who had reactive treponemal screening and nontreponemal test results but nonreactive Treponema pallidum- particle agglutination test result. Reliance on treponemal screening tests may result in misdiagnosis and unnecessary treatment, unless a specific treponemal test is performed.

Are we there yet? A roadmap from a statewide population-based analysis of STI patterns among people with HIV, with implications for morbidity, transmission and ending the HIV epidemic.

BACKGROUND: Ongoing sexually transmitted infection (STI) transmission reflects inequitable access to sexual healthcare services. STI reinfection pre-HIV and post-HIV infection is a proxy measure of sexual behaviour for people with HIV (PWH). Surveillance data show that recommended screenings are not occurring, leading to missed opportunities. Gaps exist about the true picture of STI infection and reinfection among PWH from pre-HIV to post-HIV diagnosis. To end the HIV epidemic and achieve national HIV goals of preventing new infections, synergistic STI/HIV screening is important to limit coinf

Syphilis Transmission: Contagion of Bodies and Words.

Syphilis is a sexually transmitted infection caused by the bacterium Treponema pallidum (subsp. pallidum), associated with severe symptoms culminating in cardiovascular complications or neurological damage. In the late 15th century, a syphilis epidemic occurred in Europe with an outbreak in Naples that caused numerous deaths and impacted people's lives, shaping their thoughts, behaviors, and language. Here, we traced the origin of syphilis, exploring its social implications and reviewing therapeutic approaches. Using examples of Neapolitan folk sayings, we illustrated how dialects preserve fra

Identifying Pregnant Women at High Risk of HIV Acquisition: An External Validation Assessment of an HIV Risk Assessment Tool for Pregnant Women.

BACKGROUND: An HIV risk assessment tool for pregnant and lactating women was developed in Kenya. We evaluated the performance of this tool in Malawi. METHODS: We conducted a case-control study among pregnant women in Lilongwe, Malawi (2017-2020) to externally validate the Kenyan HIV risk assessment tool. Pregnant women with a recent HIV infection at their first antenatal care (ANC) visit served as cases (n = 44), whereas those who tested HIV-negative at their first ANC visit served as controls (n = 349). Recent HIV infections were identified using a validated, laboratory-based, recent infecti

Characterizing STI testing and diagnosis disparities between Asian people with HIV and other racial/ethnic subpopulations in Washington, District of Columbia: a longitudinal analysis of the DC Cohort, 2018-2023.

BACKGROUND: People of Asian ethnicity represent one of the fastest-growing populations in the United States and may experience unique social and cultural challenges to accessing sexual healthcare. This analysis explores disparities in annual sexually transmitted infection screening and diagnosis rates per person-year between Asian and non-Asian people with human immunodeficiency virus in Washington, District of Columbia (DC). METHODS: Using District Columbia cohort data, a longitudinal study of people living with human immunodeficiency virus receiving care in Washington, DC, we analyzed peopl

Prevalence and co-occurrence of sexually transmitted infections among heterosexual couples in southern Uganda: a population-based study.

OBJECTIVE: Partner-level sexually transmitted infection (STI) data from eastern Africa are rare, despite high STI burden. To address this gap, we examined STI prevalence and clustering among cohabiting couples in two high HIV-burden Ugandan communities, representing the largest population-based, couple-level STI study in the region. METHODS: We analysed data from the Sexually Transmitted Infection Prevalence Study (STIPS), a cross-sectional, population-based study in southern Uganda which tested participants for chlamydia, gonorrhoea, trichomonas, syphilis, high-titre syphilis and herpes simp

Emergency department diagnosis and management of patients with syphilis.

Syphilis is a sexually transmitted infection known as the "great imitator" due to its large variety of presentations, depending on the stage of the disease. Because of its prevalence and the possibility of severe outcomes, emergency clinicians must remain vigilant when evaluating patients, especially those in high-risk groups. Clinicians should be familiar with the treatment recommendations for the various stages of syphilis, including alternative regimens, when national drug shortages limit the availability of first-line treatments. This review discusses the various presentations, diagnostic

Evaluating the Impact of Implementing and Scaling-Up the Use of Syphilis Rapid/Point-of-Care Tests: An Interrupted Time Series Analysis of New Syphilis Positivity Rates in Alberta, Canada.

BACKGROUND: In July 2019, a syphilis outbreak was declared in Alberta, Canada, affecting key populations. Syphilis rapid/point-of-care testing (RPOCT) provides opportunities to test individuals in nontraditional settings and provide same-day treatment. This study aimed to evaluate whether RPOCT resulted in a decline in new syphilis positivity rates. METHODS: Starting August 2020, syphilis RPOCTs were implemented in a single (Edmonton) health zone (EDM phase) and in March 2022 were scaled up across the province of Alberta (ProvScaleUp phase). To evaluate the impact of RPOCTs on new syphilis po
