First-in-human prospective pilot study of a bipolar dual-center total wrist arthroplasty: early mechanical stability and clinical outcomes.

PURPOSE: Total wrist arthroplasty (TWA) is limited by implant loosening, instability, and non-physiological load transfer, partly because most contemporary implants rely on a single center of rotation and do not preserve native wrist kinematics. This first-in-human pilot study evaluated the feasibility, early mechanical stability, and clinical outcomes of a novel bipolar dual-center TWA system designed to preserve radiocarpal and midcarpal motion. METHODS: This prospective single-center pilot case series included five consecutive patients with end-stage wrist arthritis treated with a bipolar

Simultaneous Carpal Tunnel Release With Forearm Fasciotomy: Are We Doing Enough?

BACKGROUND: Simultaneous carpal tunnel release (sCTR) with emergent forearm fasciotomy in the setting of acute compartment syndrome (ACS) has been reported, but the national incidence and cost of delayed carpal tunnel release (CTR) remain unknown. This large database study aims to evaluate how often sCTR is performed, the frequency of delayed CTR when not performed simultaneously, and the associated costs. METHODS: The PearlDiver Mariner database was queried for patients who underwent forearm fasciotomies between January 1, 2015, and October 31, 2022. Patients were grouped based on CTR timing

Recurrent trigeminal neuralgia caused by an anomalous cerebellar artery arising from the persistent primitive trigeminal artery: illustrative case.

BACKGROUND: Persistent primitive trigeminal artery (PTA) and its related variants-the PTA family-are rare vascular anomalies that can cause trigeminal neuralgia through atypical neurovascular compression patterns, requiring careful anatomical evaluation. The authors describe a rare case of recurrent trigeminal neuralgia caused by an anomalous cerebellar artery arising from a PTA and further complicated by a long-standing Teflon granuloma. OBSERVATIONS: A 79-year-old man developed recurrent trigeminal neuralgia 19 years after microvascular decompression. Imaging revealed an anomalous anterior

Effect of Esketamine Pretreatment on Necroptosis in Mice with Trigeminal Neuralgia and Anxiety-Like Behavior via the RIP1/RIP3/MLKL Pathway.

To explore the effect of esketamine (ES) pretreatment on necroptosis in mice with trigeminal neuralgia accompanied by anxiety-like behavior and its correlation with the RIP1/RIP3/MLKL signaling pathway. Thirty male C57BL/6J mice were randomized into five groups (n=6): Control, Sham, model group (PIONT), PIONT+NS, and PIONT+ES. Trigeminal neuralgia model was established by left infraorbital nerve transection. ES (10 mg/kg, i.p.) was given daily for 5 days preoperatively (preoperative pretreatment) in the PIONT+ES group before molding, and equal-volume saline was administered identically in the

[Research progress on the autonomic-immune mechanism of acupuncture in regulating ovarian function].

Ovarian dysfunction is an important cause of ovulatory disorders, infertility and endocrine disorders in women, which is commonly observed in conditions including polycystic ovary syndrome, primary ovarian insufficiency, diminished ovarian reserve and ovarian aging-related states. Recent studies have demonstrated that besides abnormalities in the classic hypothalamic-pituitary-ovarian axis, autonomic nervous imbalance and immune-inflammatory disturbance also serve as critical mechanisms underlying impaired ovarian function. Central inflammatory responses, chronic stress and metabolic abnormali

False-Negative Ligamentum Teres Tendon Sampling for the Detection of Systemic Amyloidosis.

This case report involves a 60-year-old man with bilateral carpal tunnel syndrome, hip osteoarthritis, and lumbar spinal stenosis who underwent biopsies of the ligamentum teres, ligamentum flavum, and carpal flexor tenosynovium. Amyloid deposits were identified in the ligamentum flavum and flexor tenosynovium, but not the ligamentum teres. Amyloid was typed by mass spectrometry, cardiac scintigraphy was negative, and genetic testing remains pending. This supports that ligamentum flavum and carpal flexor tenosynovium biopsies may have better diagnostic value for the early diagnosis of amyloidos

Electrophysiological Monitoring of Multifurcated Recurrent Laryngeal Nerve With Motor Multibranches.

Extralaryngeal bifurcation of the recurrent laryngeal nerve (RLN) is common anatomical variation, but the multifurcation of the RLN with more than three branches is quite rare. Intraoperative neuromonitoring evaluating functional status of the nerve branches shows that the branched RLN may have functional variation. Identification and electrophysiological monitoring of the RLN can localize motor fibers within nerve branches and help preserve motor function.

Neuromuscular Blockade Depth and Oculocardiac Reflex in Pediatric Strabismus Surgery: A Randomized Clinical Trial.

BACKGROUND: Although neuromuscular blockade reduces oculocardiac reflex (OCR) incidence compared with no neuromuscular blockade, the effect of blockade depth on OCR has not been prospectively evaluated. This study aimed to evaluate whether the depth of neuromuscular blockade influences the incidence of the OCR during pediatric strabismus surgery. METHODS: In this prospective, double-blinded randomized controlled trial, 204 pediatric patients aged 3 to 18 yr undergoing strabismus surgery under general anesthesia were randomly assigned to moderate to deep neuromuscular blockade (MD-NMB; train-o

Gut microbial and metabolomic alterations associated with herpetic trigeminal neuralgia: an exploratory multi-omics study.

BACKGROUND: Herpetic trigeminal neuralgia (HTN) is a neuropathic pain disorder caused by varicella-zoster virus invasion of the trigeminal nerve, but its mechanisms remain poorly elucidated. Accumulating evidence implicates gut microbiota dysbiosis and metabolic perturbations in modulating chronic pain via the gut-brain axis, yet their roles in HTN have not been systematically explored. METHODS: Integrated multi-omics analysis was performed on 19 patients with HTN and 18 healthy controls. Fecal metagenomic sequencing characterized gut microbial composition and function. Untargeted metabolomic

[The role of neuroprotection in treating oculomotor neuropathy in a child following endoscopic cystocisternostomy].

This report presents a clinical case involving a 13-year-old patient diagnosed with an instrumentally confirmed arachnoid cyst located in the right Sylvian fissure (Galassi type III). Following the surgical procedure of endoscopic cystocisternostomy, the patient exhibited signs consistent with oculomotor nerve neuropathy on the right side. Consequently, Cytoflavin (inosine+nicotinamide+riboflavin+succinic acid) was included in the treatment regimen, administered intravenously at a dose of 10.0 g in 200 mL of 0.9% sodium chloride solution daily for 10 days. During the treatment, the patient's c

Research Progress on Intraocular Pressure-lowering Drugs, Commonly Used Clinical Adjuvants, and Emerging Neuroprotective Agents for Glaucomatous Optic Neuropathy.

Glaucomatous optic neuropathy (GON) is the leading cause of irreversible blindness worldwide, characterized by progressive retinal ganglion cell (RGC) loss. Current clinical treatment relies primarily on IOP-lowering drugs, but a subset of patients continues to experience progressive optic nerve damage. Adjuvant therapies such as antioxidants and calcium channel blockers have limited efficacy. Emerging neuroprotective strategies including NMDA receptor antagonists, neurotrophic factors, and gene therapy remain in preclinical or early-stage trials. This review systematically summarizes the mech

Tau A152T risk variant drives distinct cell type transcriptional signatures in AD and PSP.

The A152T point mutation is a rare genetic variant in the tau gene (MAPT) that is linked to risk of Alzheimer's disease (AD) and frontotemporal dementia spectrum disorders, including progressive supranuclear palsy (PSP) and corticobasal degeneration (CBD). To uncover mechanisms through which the A152T variant contributes to risk of tauopathy, we performed single-nucleus RNA sequencing (snRNA-seq) on postmortem brain tissue collected from A152T carriers and pathology-matched non-carriers with either AD or PSP, along with age-matched controls. These efforts revealed that despite similar cell typ
