Soluble TIM-3 and galectin-9 in cerebrospinal fluid are associated with disability at 6 months in aquaporin-4 IgG-positive neuromyelitis optica spectrum disorder.

BACKGROUND: Neuromyelitis optica spectrum disorders (NMOSD) are severe autoimmune diseases of the central nervous system characterized by autoantibodies against aquaporin 4 (AQP4-IgG). Despite the availability of several molecular targeted therapies, biomarkers capable of predicting subsequent disability early during the acute relapse phase remain insufficient. Given that abnormalities in immune checkpoint molecules are known to be involved in autoimmunity, this study investigated whether immune checkpoint-related molecules in the cerebrospinal fluid (CSF) of acute-phase AQP4-IgG-positive NMOS

Surgical repair of facial nerve defects by an autologous great auricular nerve transplantation: a retrospective, 12-month, follow-up study.

BACKGROUND: To observe facial nerve function recovery from an autologous great auricular nerve (GAN) transplantation in repairing facial nerve defects. METHODS: Thirty-six patients were included. Facial nerve functions were evaluated by using the House-Brackmann grading system. Abnormal sensations after the GAN sacrifice were recorded. RESULTS: The follow-up lasted for an average of 18.5 (12-60) months. Facial nerve defects were caused by parotid gland malignancies (n = 30) or trauma (n = 6), with an average length of 2.6 ± 1.1 (1.0-6.5) cm. One case showed tumor recurrence during the follow

A Data-Driven Operational Definition of Square-Wave Jerks and Its Diagnostic Utility in Progressive Supranuclear Palsy.

BACKGROUND: The Movement Disorder Society diagnostic criteria for progressive supranuclear palsy (PSP) list frequent square-wave jerks (SWJ) as a suggestive oculomotor feature. A standardised operational definition of SWJ is however presently lacking. OBJECTIVE: To create a data-driven optimal SWJ definition and to assess its diagnostic utility in PSP. METHODS: We prospectively recruited PSP n = 25, Parkinson's disease (PD) n = 32, Alzheimer's disease (AD) n = 35, and controls n = 88. SWJ were recorded with a visual fixation task using video-oculography. Multiple candidate definitions of SWJ

Giant Cell Arteritis in Ophthalmology - Current Treatment Options. A Review.

Giant cell arteritis (GCA) is a  systemic granulomatous vasculitis of medium and large-sized arteries, which represents an ophthalmic emergency because of the risk of sudden, severe and often irreversible loss of vision. Its most important ocular manifestation is arteritic anterior ischemic optic neuropathy; less frequent manifestations include central retinal artery occlusion, cilioretinal artery occlusion, posterior ischemic optic neuropathy, choroidal ischemia and ocular motor nerve palsies. Giant cell arteritis should be actively considered in every patient over 50 years of age presenting

Gelsemine attenuates neuropathic pain in a rat model of sciatic nerve chronic constriction injury.

ObjectiveThe effect of Gelsemine on neuropathic pain was investigated using a sciatic nerve (SN) chronic constriction injury (CCI) rat model.MethodsNetwork pharmacology analysis was performed to identify potential molecular targets of Gelsemine associated with neuropathic pain, followed by molecular docking analysis of the identified targets. Neuropathic pain was induced in male rats by SN-CCI. The rats were divided into four groups: sham, CCI, Gelsemine 2 mg/kg, and Gelsemine 4 mg/kg. Gelsemine (2 or 4 mg/kg, i.p.) was administered to the treatment groups for 30 consecutive days. The effect o

Differential Trajectories of Brain Atrophy in Multiple Sclerosis, MOGAD, and AQP4 + NMOSD: The Role of Disease Activity in MOGAD.

BACKGROUND AND OBJECTIVES: Multiple sclerosis (MS), myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), and aquaporin-4 antibody-positive neuromyelitis optica spectrum disorder (AQP4 + NMOSD) are inflammatory demyelinating disorders with distinct mechanisms of tissue injury. MS is characterized by diffuse neurodegeneration, whereas damage in antibody-mediated diseases is considered attack related. In MOGAD, the relationship between disease activity and brain atrophy remains unclear. We aimed to compare longitudinal trajectories of global and regional brain atrophy across M

Heerfordt-Waldenström Syndrome With Cardiac Sarcoidosis.

Heerfordt-Waldenström syndrome is a rare presentation of sarcoidosis with only a handful of cases reported worldwide. It consists of a tetrad of uveitis, parotitis, and VII nerve palsy and chronic fever. Symptomatic cardiac involvement in sarcoidosis is an uncommon but potentially lethal manifestation. A timely diagnosis and treatment can result in the prevention of morbidity and mortality. We report a case of a young woman with multisystem sarcoidosis who presented with constitutional features, uveitis, multiple cranial nerve palsies, leptomeningitis, and ankle oligoarthritis. Supportive inve

Musculoskeletal Manifestations of Light Chain Amyloidosis: A Systematic Review.

INTRODUCTION: Diagnosing amyloid arthropathy remains challenging because its manifestations often mimic those of other inflammatory musculoskeletal diseases. This review aimed to describe the clinical, biological, and imaging characteristics of musculoskeletal involvement in light-chain (AL) Amyloidosis. METHODS: We conducted a review in accordance with the PRISMA guidelines. We searched SCOPUS and MEDLINE for case reports and series published between January 2000 and December 2023 that detailed musculoskeletal manifestations of AL amyloidosis. To minimize the risk of omitting relevant articl

Recognizing and Differentiating Area Postrema Syndrome Across NMOSD, MOGAD, and GFAP Astrocytopathy.

Area postrema syndrome (APS)-intractable nausea, vomiting, and/or hiccups persisting ≤48 h-is a recognized presenting feature of neuromyelitis optica spectrum disorder (NMOSD) and, less commonly, of myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) and glial fibrillary acidic protein (GFAP)-astrocytopathy. APS is still frequently misattributed to primary gastrointestinal disease, delaying immunotherapy by weeks to months. For years its pathophysiology was inferred largely from lesion topography rather than demonstrated at the level of neuronal signaling. A study presented

The nose-brain axis as a neuroimmune conduit: how allergic rhinitis rewires the central nervous system.

Allergic rhinitis (AR) has traditionally been regarded as a type I hypersensitivity disorder confined to the nasal mucosa. However, persistent symptoms, nasal hyperresponsiveness, and comorbid neuropsychiatric disturbances-including headache, fatigue, cognitive impairment ("brain fog"), anxiety, and depression-cannot be fully explained by peripheral inflammation alone, thereby implicating the involvement of central sensitization. The nose-brain axis serves as a critical bidirectional conduit that relays peripheral inflammatory signals to the central nervous system (CNS) and initiates neuroimmu

TAC-Tic syndromes: a case series on the complex overlap between trigeminal autonomic cephalalgias and trigeminal neuralgia.

INTRODUCTION: Trigeminal autonomic cephalalgias (TACs) and trigeminal neuralgia (TN) are classified as distinct disorders, although overlap syndromes are increasingly recognized. CASE REPORTS: We retrospectively describe five women with TAC-tic overlap: two with SUNCT-tic, two with cluster-tic, and one with paroxysmal hemicrania-tic syndrome. Each had distinguishable neuralgic and autonomic pain phenotypes, appearing either simultaneously or sequentially. In both SUNCT-tic cases, repeat imaging disclosed neurovascular compression after an initially negative study; microvascular decompression

Robot-Assisted Gasless Trans-Hairline Excision of Submandibular Gland Tumors.

Benign submandibular gland tumors are usually resected through a transcervical incision, which provides direct exposure but leaves visible cervical scarring. Remote-access endoscopic techniques improve scar concealment but can be limited by two-dimensional visualization, assistant-dependent camera control, and rigid instrument collision in the narrow submandibular gland space. This article presents a standardized video protocol for robot-assisted submandibular gland excision via a gasless trans-hairline approach for benign tumors. The procedure begins with postauricular trans-hairline incision

Mixed Connective Tissue Disease (MCTD): A Rare Presentation of Probable Central Nervous System (CNS) Vasculitis.

Mixed connective tissue disease (MCTD) may rarely involve the central nervous system (CNS), and CNS vasculitis can present with acute seizures, altered consciousness, and focal neurological deficits. We report a 27-year-old woman with a seven-year history of arthralgia, morning stiffness, intermittent fever, and malaise who developed acute seizures, altered sensorium, and hemiparesis approximately two weeks before hospital admission. Laboratory evaluation demonstrated severe anemia, thrombocytopenia, elevated inflammatory markers, and positive anti-U1 ribonucleoprotein (U1-RNP) antibodies. Cer

Management of High Jugular Bulb in Retrosigmoid Surgery for Vestibular Schwannoma: Analysis of 590 Consecutive Cases.

BACKGROUND AND OBJECTIVES: High jugular bulb (HJB) is an obstacle when performing retrosigmoid intradural temporal drilling for resection of the meatal part of vestibular schwannomas. Some studies reported the technical nuance of the HJB management; however, the indications, impact on surgical outcomes, and long-term outcomes of this challenging management have not been fully analyzed. This study aimed to investigate the clinical and imaging characteristics of patients requiring the intraoperative HJB management, its impact on surgical outcomes, and long-term postoperative follow-up results.

Identification of blood-based inflammatory biomarkers for vestibular neuritis using a proximity extension assay.

ObjectiveThe present study aimed to identify blood-based inflammatory biomarkers that would help investigate the underlying pathogenesis in vestibular neuritis (VN) using a proximity extension assay (PEA).MethodsIn this single-center prospective study, we enrolled both a cross-sectional (acute VN vs normal controls) and a longitudinal (acute phase vs recovery phase) cohorts. We quantified 92 plasma inflammatory proteins using the Olink PEA and identified differentially expressed proteins (DEPs) in each cohort using t-tests. The clinical utility and predictive value of each DEP were evaluated u

Asymmetric Hearing Loss as a Population Screening Signal for Vestibular Schwannoma: A Number-Needed-To-Scan Analysis Using NHANES and SEER.

OBJECTIVE: Asymmetric sensorineural hearing loss (SNHL) is the principal audiometric trigger for magnetic resonance imaging (MRI) to exclude vestibular schwannoma (VS). Although multiple audiometric referral criteria exist, the population-level burden of applying them has not been quantified. To estimate how many adults in the general United States population would be flagged for MRI by each of the 5 recognized audiometric referral criteria per incident VS, and to place that burden in the context of established screening programs and of current clinically indicated imaging. METHODS: We pooled
