No Glymphatic Dysfunction in Trigeminal Neuralgia: A Preliminary Study.

BACKGROUND: Trigeminal neuralgia (TN) patients are at risk for glymphatic dysfunction due to their prolonged headaches and sleep disturbances. OBJECTIVE: The aim of this study is to compare the glymphatic function changes in TN patients with healthy controls, to investigate whether there are differences in glymphatic function after decompression surgery, and to examine their relationship with clinical parameters. METHODS: The data for this hybrid design study were obtained from the OpenNeuro dataset titled "A large-scale dataset of pre- and post-surgical MRI data in patients with chronic tri

Predictors of Postoperative Complications After Microvascular Decompression for Trigeminal Neuralgia and Hemifacial Spasm: A Retrospective Cohort Study.

BACKGROUND: Microvascular decompression (MVD) is a definitive treatment for trigeminal neuralgia and hemifacial spasm, but a subset of patients experiences postoperative complications. Identifying risk factors for these complications could improve perioperative management and patient counseling. METHODS: We retrospectively analyzed 453 patients who underwent MVD for classic trigeminal neuralgia or hemifacial spasm from January 2020 to March 2024 at the First Affiliated Hospital of Anhui Medical University. Univariate analyses compared patients with versus without complications. Multivariate l

Shredded vs. pledget teflon in microvascular decompression for trigeminal neuralgia: a retrospective comparative study.

OBJECTIVE: Trigeminal neuralgia (TN) is a debilitating craniofacial pain disorder that often necessitates surgical intervention when medical therapy fails. This study evaluates the impact of two Teflon implantation techniques - pledget interposition and shredded Teflon - on clinical outcomes following microvascular decompression (MVD). METHODS: A retrospective analysis was conducted on 121 patients with classical trigeminal neuralgia (TN) who underwent microvascular decompression (MVD) between 2001 and 2020. Patients were grouped based on the Teflon technique used: pledget interposition (n =

Safety and Efficacy of Microvascular Decompression for Trigeminal Neuralgia in the Octogenarian Population: Insights From a Propensity Score-Adjusted and Matched Analysis.

BACKGROUND AND OBJECTIVES: Although microvascular decompression (MVD) is the most effective long-term treatment for trigeminal neuralgia (TN), its risks make some neurosurgeons favor ablative procedures in elderly patients, despite their limited durability and higher risk of sensory deficits, sparking debate over the optimal approach. This retrospective analysis aims to evaluate the safety and efficacy of MVD in octogenarian patients with TN compared with younger patients. METHODS: In this matched cohort analysis, a total of 280 patients were initially included, with 33 in the octogenarian gr

Selecting Neuromodulation Devices For Chronic Pain Conditions: A Narrative Review.

BACKGROUND: Neuromodulation is a rapidly advancing field in pain medicine, providing targeted, reversible interventions for patients with chronic pain unresponsive to conventional therapies. Advances in waveform technology, device design, and stimulation strategies have shifted neuromodulation from a last-resort approach to a core element of multidisciplinary pain management. Despite its growing adoption, variability in training, terminology, and clinical implementation underscores the need for consensus-driven frameworks to ensure safety, efficacy, and uniformity across practice settings. Microvascular decompression of aberrantly looped anterior inferior cerebellar artery: illustrative case.

BACKGROUND: The anterior inferior cerebellar artery (AICA) is the second most common neurovascular etiology of trigeminal neuralgia. OBSERVATIONS: Here the authors build on a prior reported technique for noncompressive decompression of a curious anatomical AICA variant, where a loop of the main artery nearly completely encircles the trigeminal nerve to circumferentially impact it. LESSONS: Circuitous anatomy of AICA may present particular challenges for microvascular decompression, but it is still possible to achieve a noncompressive configuration to best limit future irritation of the trige

CyberKnife Stereotactic Radiosurgery for Vestibular Schwannomas: Systematic Review and Meta-Analysis of Long-Term Tumor Control and Hearing Preservation Outcomes.

BACKGROUND AND OBJECTIVES: Stereotactic radiosurgery is a popular intervention for small vestibular schwannomas in patients with preserved hearing. Gamma Knife radiosurgery (GKRS) and CyberKnife radiosurgery (CKRS) represent 2 alternative modalities, with CKRS potentially allowing for the delivery of higher doses over multiple sessions. However, the optimal dosing and hearing outcomes of CKRS remain unclear. This study systematically reviewed all literature describing outcomes following treatment of vestibular schwannomas with CKRS, with a focus on tumor control, hearing preservation, and dosi

Lateral Orbitotomy for Trigeminal Schwannomas: Surgical Technique, Outcomes, and Limitations.

BACKGROUND AND OBJECTIVE: Trigeminal schwannomas (TSs) are rare, mostly benign tumors that exhibit an indolent clinical course. The lateral orbitotomy approach (LOA), with or without endoscopic assistance, has emerged as a novel skull base route, particularly for accessing lesions confined to the middle cranial fossa. Herein, we elucidate the advantages and limitations of the LOA for the treatment of TSs. METHODS: All TS cases that underwent LOA through a lateral canthus incision during the past decade in our department were retrospectively reviewed. The operative technique was detailed, and

Pivotal Factors for Minimizing the Risks of Therapeutic Failure in Trigeminal Neuralgia and Hemifacial Spasm.

PURPOSE OF REVIEW: Despite the scarce literature regarding the matter, this current article identifies the most important factors that lead to the best outcomes in the therapeutic approach of trigeminal neuralgia and hemifacial spasm. However, notwithstanding the central focus lies on the neurosurgical perspective, the authors also integrate insights from adjacent medical specialties. Thus, the main objective of this comprehensive narrative review is to enhance understanding of the factors that can dramatically influence the course of these conditions. RECENT FINDINGS: This article provides a

On the topical issue of assisted dying: the insurmountable challenges of human existence, and the right to exit.

Although the issue of medical-assisted dying or medical aid in dying (MAID, euthanasia, and medical-assisted suicide) has a long history, it has become an increasingly important topic in recent years. Around the world, many governments remain opposed to MAID. In places where MAID is legally permitted, it is highly regulated, and typically, unbearable suffering limited to a few serious and incurable medical conditions determines eligibility. MAID eligibility based on unbearable suffering but limited to a few medical conditions is unfair because suffering is inescapable in life, is not limited t
