Neuronavigation‑guided rTMS of the facial motor cortex for atypical facial pain: Two case reports.

OBJECTIVE: Evaluate safety and analgesic effect of neuronavigation-guided 10 Hz rTMS precisely targeting facial M1 in atypical facial pain (AFP). METHODS: Two adults with refractory AFP received 10 daily sessions (2,500 pulses/day) of 10 Hz rTMS at 110 % resting motor threshold. Targeting used MRI/CT-integrated neuronavigation to the facial M1. Pain severity (Barrow Neurological Institute [BNI] scale I-V) and adverse events were recorded through treatment and short follow-up. RESULTS: Both completed treatment without serious adverse events. Case 1 improved from BNI V at baseline to II at end

Internal neurolysis for atypical facial pain: Clinical outcomes and validation of an intraoperative trigeminal pathology scoring system.

BACKGROUND: Atypical facial pain (AFP) differs from typical trigeminal neuralgia (TTN) in clinical features and pathophysiology. Internal neurolysis (IN) has been proposed as a treatment for refractory AFP; however, objective intraoperative assessment methods remain limited. METHODS: We retrospectively analyzed 20 consecutive patients with AFP who underwent IN. Intraoperative findings revealed superior cerebellar artery (SCA) compression in five cases, venous compression in four, and no vascular compression in 11. When vascular compression was identified, the vessel was dissected and mobilize

Temporalis Muscle-Originated Supraorbital Neuralgia Misdiagnosed as Trigeminal Neuralgia.

In patients with pharmacotherapy-refractory supraorbital neuralgia, a myofascial etiology should be included in the differential diagnosis. The authors report a case of a 73-year-old man with left supraorbital pain triggered by mastication, initially misdiagnosed as trigeminal neuralgia (V1). Physical examination identified a myofascial trigger point (MTrP) in the ipsilateral temporalis muscle, leading to a revised diagnosis of temporalis muscle-originated supraorbital neuralgia. Management involved ultrasound-guided pulsed radiofrequency of the supraorbital nerve combined with dry needling of

Non-invasive brain stimulation for neuropathic orofacial pain: a mini-review.

Neuropathic orofacial pain (NOP) can seriously affect the quality of life of patients. Due to the concentration of pain in the central area of the craniofacial region, it is not only highly invasive but also easily distracts the patient's attention, resulting in its destructive nature far exceeding that of distal limb pain. As emerging non-invasive therapies, Repetitive Transcranial Magnetic Stimulation (rTMS) and Transcranial Direct Current Stimulation (tDCS) are bringing hope for the treatment of NOP. This brief review summarizes existing evidence on their efficacy, highlighting that pain ph

Efficacy of internal neurolysis for older patients suffering from trigeminal neuralgia without neurovascular compression.

BACKGROUND: The efficacy of internal neurolysis (IN) for older patients with trigeminal neuralgia (TN) without neurovascular compression remains unclear. We evaluated the clinical outcomes of IN in older patients with TN without neurovascular compression, comparing them with those of their younger counterparts. METHODS: We retrospectively reviewed 22 patients with TN who underwent IN without neurovascular compression. They were divided into older (≥70 years, n = 11) and younger (<70 years, n = 11) groups. Clinical variables included pain type (typical vs. atypical), recurrence status, and int

Vaso-occlusive crisis in the cervicofacial region in sickle cell anemia.

BACKGROUND: Sickle cell anemia (SCA) is an autosomal recessive disorder characterized by a point mutation in the β-globin chain of the hemoglobin protein, causing abnormal folding. This can lead to sickling of red blood cells, agglutination, and ultimately vascular obstruction, which rarely occurs in the cervicofacial region. CASE DESCRIPTION: This report reviews a rare case of vaso-occlusive crisis (VOC) in the cervical vertebrae and mandible, manifesting as severe mandibular pain of unknown origin in a patient with SCA. The patient experienced diffuse body pain, 3 days in duration, and most

Restless Eyes at Night: A Novel Variant of Restless Legs Syndrome.

INTRODUCTION: Restless legs syndrome (RLS) is a common sensorimotor disorder that primarily affects the lower extremities. This condition is characterized by unpleasant sensations and an irresistible urge to move the affected body regions, typically during periods of rest or at night. While RLS most commonly involves the legs, atypical variants affecting other body parts, including the arms, abdomen, face, and even the head, have increasingly been reported. This has led to the emergence of a broader diagnostic concept termed "restless X syndrome". However, no prior cases involving the ocular r

Genetically predicted the causal relationship between gut-brain axis and chronic pain: a Mendelian randomization study.

BACKGROUND: Although previous studies have indicated a potential association between gut–brain axis dysfunction and chronic pain, the causal relationship and underlying pathogenic mechanisms remain unclear. Therefore, this study employed Mendelian Randomization (MR) analysis to investigate the causal effect of the gut–brain axis on chronic pain from a genetic perspective. METHODS: This study utilized the largest available genome-wide association study (GWAS) summary statistics to date, incorporating data on gut microbiota (n = 18,340 from MiBioGen consortium), brain structural connectivity (n

Isolated facial pain as the sole presentation of a type I branchial cleft cyst in an adult male.

First branchial cleft cysts (FBCCs) are rare developmental anomalies that typically present in childhood. We report a unique case of a Type I FBCC in a 43-year-old male whose only symptom was isolated facial pain without infection or mass. Imaging and biopsies were inconclusive, prompting surgical excision for definitive diagnosis and symptomatic relief. Histopathology confirmed a branchial cleft anomaly. This case highlights the importance of including congenital anomalies in the differential diagnosis for atypical facial pain even in the adult population and underscores the value of a thorou
