A 14-year-old spayed female Siamese cat was presented with signs consistent with a rapidly progressive cranial polyneuropathy. Magnetic resonance imaging of the brain revealed an extra-axial mass at the calvarial floor, extending caudally to involve bilateral trigeminal nerves and rostrally to involve the pituitary gland and optic chiasm. Additional findings included contrast-enhancing content within the bilateral tympanic bullae, mild mandibular and medial retropharyngeal lymphadenopathy, and a left tonsillar mass. Cytologic and histopathologic findings confirmed tonsillar lymphoma. The concu

Endodontic (pulpal and periapical) pain and temporomandibular disorders (TMDs) are often considered to be clinical entities in different dental subspecialties. Both are present in the same sensory system (the trigeminal nerve and its central projections); however, there is recent evidence to suggest that some common neurobiological substrates exist between the two. Mechanistic connections between the inflamed/instrumented tooth and the masticatory muscle and joint pain associated with TMD consist of peripheral sensitization at the inflamed/instrumented tooth, activity-dependent changes in trig

BACKGROUND: Despite the recent therapeutic advances, approximately one-third of patients with chronic migraine fail to achieve clinically meaningful benefit, and existing oral preventive therapies remain limited by tolerability and off-target effects. There is, therefore, a pressing need for new, well-tolerated, mechanistically distinct preventive treatments for chronic migraine. This hypothesis article evaluates the pharmacological and biological rationale supporting guanfacine as a potential preventive therapy for chronic migraine.
HYPOTHESIS: We hypothesize that guanfacine, a selective α2A

Migraine is a highly disabling neurological disorder in which activation of the trigeminovascular system and the ensuing peripheral and central sensitization are major pathological foundations for pain initiation, recurrent attacks, and disease chronification. In recent years, innate immune-mediated sterile neuroinflammation has emerged as a key mechanism linking trigeminal neurovascular system activation to maladaptive remodeling of nociceptive pathways. During the early phase of a migraine attack, trigeminal nerve endings release calcitonin gene-related peptide, pituitary adenylate cyclase-a

Mixed connective tissue disease (MCTD) combines features of systemic lupus erythematosus, scleroderma, and polymyositis. Neurologic involvement, including trigeminal neuropathy, is rare. The present report describes isolated trigeminal sensory neuropathy as the initial manifestation of MCTD. Quantitative sensory testing revealed progressive bilateral trigeminal sensory deficits. Laboratory evaluation showed elevated anti-U1 ribonucleoprotein (anti-U1-RNP) antibodies, and clinical examination revealed lymphadenopathy and hand swelling, confirming the diagnosis. No other systemic manifestations

BACKGROUND: Gamma Knife radiosurgery (GKRS) is a well-established treatment for trigeminal neuralgia (TN) and essential tremor (ET). In patients with coexisting intracranial tumors, radiosurgery can potentially address functional and oncologic targets in a single or staged session. However, data on integrating these treatments and the predictive role of the biologically effective dose (BED), remain limited. This study aims to evaluate clinical outcomes and identify the influence of dosimetric predictors, including BED, in patients undergoing GKRS for TN and/or ET in the context of coexisting i

OBJECTIVE: Trigeminal neuralgia (TN) is a debilitating disorder characterized by severe facial pain. While percutaneous balloon compression (PBC) is an effective surgical treatment for TN, recurrence remains a significant concern, with varying reported rates. The identification of factors that contribute to recurrence after PBC is critical for improving treatment outcomes. However, existing predictive models for recurrence have limitations in accuracy and generalizability. This study aims to explore the influencing factors of TN recurrence after PBC and to construct a TN recurrence risk predic

BACKGROUND: Trigeminal neuralgia, classified as a painful disorder of the trigeminal nerve (PDTN), is characterized by brief, electric shock-like or stabbing pain localized mainly to the maxillary and mandibular regions. The severity of these paroxysmal attacks severely impairs quality of life and, in extreme cases, may lead to suicidal ideation. We hypothesized that PDTN shares a common neuroanatomical signature with other chronic pain disorders, while also exhibiting disorder-specific alterations.
METHODS: High-resolution T1-weighted structural MRI scans from 85 individuals diagnosed with p

OBJECTIVE: Biologically effective dose (BED)-oriented planning is emerging as a potential key consideration in future planning strategies, aiming to achieve the best, most personalized radiosurgery treatment plans possible. In this study, planning parameters and BED variations in previously treated patients were investigated to determine the factors that affect the treatment results in order to achieve better patient outcomes.
METHODS: A highly refined cohort of 191 idiopathic, type 1 trigeminal neuralgia (TN) patients who underwent stereotactic radiosurgery (SRS) with 80 Gy as a first-line i

PURPOSE: Intraoperative bradycardia is a recognized complication during radiofrequency (RF) rhizotomy for trigeminal neuralgia, primarily related to the trigeminal-cardiac reflex. Dexmedetomidine is commonly used for its sedative and analgesic properties during these procedures; however, its hemodynamic impact has not been well characterized. This study aimed to evaluate the association between dexmedetomidine use and intraoperative bradycardia and to identify relevant clinical risk factors.
PATIENTS AND METHODS: This retrospective observational study included 517 patients who underwent RF rh

Nose-to-brain (N2B) drug delivery offers a promising alternative to circumvent the blood-brain barrier and deliver therapeutic agents directly to the central nervous system. Among the intranasal pathways, targeting the olfactory mucosa is particularly attractive due to its direct anatomical and functional connection to the brain. However, effective deposition and retention of drug-loaded formulations in the olfactory region remain significant challenges, owing to complex nasal anatomy, mucociliary clearance, and limited surface area. This review critically examines the physiological and anatom

Alphaherpesvirinae subfamily members are widespread pathogens that cause a range of significant human and animal diseases, from mild mucocutaneous lesions to severe neurological and neonatal infections. Following primary infection of the craniofacial or genital mucosa, lytic replication occurs in epithelial cells, resulting in the production of infectious virus. Virus shed from infected epithelial cells inevitably encounters and enters sensory nerve termini innervating these tissues, after which it is transported to sensory neurons in trigeminal ganglia. Here, lytic viral replication subsides,

INTRODUCTION: Refractory trigeminal neuropathic pain, including classical trigeminal neuralgia (TN) and secondary etiologies, is a chronic neuropathic disorder characterized by recurrent, electric shock-like facial pain that can be disabling and resistant to conventional treatments. Electrical stimulation of the Gasserian ganglion through the foramen ovale has emerged as a minimally invasive technique capable of modulating nociceptive transmission.
OBJECTIVE: This systematic review aimed to assess the efficacy and safety of electrical stimulation of the Gasserian ganglion in patients with ref

Any person that has broken a bone can attest to the existence of sensory innervation of the skeleton. Beyond afferent functions, sensory neurons have been implicated in the orchestration of bone repair via the release of neuroregulatory signals. Yet, these neurosecretory effects have principally been deciphered through loss-of-function studies. Indeed, the potential therapeutic benefit of boosting nerve-to-bone interactions remains cursorily studied. Here, using a mouse calvarial bone defect model, pharmacologic activation of TrkA with a small molecule partial agonist induced bone-associated n

The brain is one of the most delicate & protected organs of the human body. The circulation of blood to the brain is secured by the blood-brain barrier (BBB), blood-cerebrospinal fluid barrier (BCSFB), and cerebrospinal fluid-brain barrier (CBB). These barriers also restrict the distribution of therapeutics to the central nervous system (CNS) for the treatment of any psychotic disorder. Oral & parenteral routes are the main routes for the delivery of anti-psychotics to the brain. Still, associated drawbacks include the stomach's acidic pH, first-pass metabolism, enzymatic degradation, plasma p
