Trigeminal neuralgia or hemifacial spasm due to vertebrobasilar dolichoectasia: a single-center case series and systematic review.

OBJECTIVE: Dolichoectasia of the intracranial arteries is a rare vascular disorder. Patients with vertebrobasilar dolichoectasia (VBD) can present with cranial nerve deficits, such as cranial rhizopathies. The aim of this study was to systematically review existing literature and to share an institutional experience in treating VBD-associated trigeminal neuralgia (TN) and hemifacial spasm (HFS), as well as to provide an overview for optimal therapeutic decision-making in these patients. METHODS: A systematic search was conducted across three major databases to identify publications on VBD in

Nerve matters: Longitudinal microstructural change in the trigeminal nerve is associated with durable pain relief after surgery for trigeminal neuralgia.

BackgroundMany patients with medically-refractory trigeminal neuralgia (TN) fail to achieve lasting pain relief following surgery targeting the trigeminal nerve (cranial nerve five; CNV). While some studies using MRI diffusion tensor imaging (DTI) suggest that preoperative CNV microstructure may predict surgical response, the findings remain inconsistent. Furthermore, the relationship between post-surgical CNV microstructural changes and long-term pain relief is not well understood. Using a novel CNV-nerve specific DTI protocol, the present study aimed to determine whether: (1) preoperative CN

The Role of Neuron-specific Enolase as a Biomarker of Response in Percutaneous Balloon Compression for Drug-Resistant Trigeminal Neuralgia.

BACKGROUND AND OBJECTIVES: Neuron-specific enolase (NSE) has been identified as a marker of neuronal damage in different clinical conditions. Percutaneous balloon compression (PBC) is a well-established palliative destructive procedure for the treatment of trigeminal neuralgia (TN). The aim of our study was to evaluate the prognostic role of NSE in predicting pain recurrence after PBC for drug-resistant TN. METHODS: We retrospectively analyzed the prospectively collected data of 41 drug-resistant TN patients (21 M, 20 F; mean age 65 ± 13 years) who underwent PBC between December 2020 and June

Sex differences of synaptic plasticity and microglial remodeling in the dorsal hippocampus following trigeminal nerve injury in mice.

Peripheral nerve injury patients have chronic pain as well as poor memory and cognitive functioning that is associated with hippocampal abnormalities. Despite sex-dependent changes in the hippocampus of patients with chronic pain, detailed sex differences in hippocampal plasticity after peripheral nerve injury are not well known. In mice with infraorbital nerve chronic constriction injury (ION-CCI), we found complex sex differences in the dorsal hippocampus (dHC). ION-CCI suppressed population spikes amplitude and long-term potentiation (LTP) in the dHC compared to sham group, which was consis

Sensory and Sympathetic Nerve Localization in Mouse Temporomandibular Joint and Knee Joint Neuro-Musculoskeletal Tissues.

OBJECTIVE: Sympathetic-sensory coupling is increasingly implicated in joint homeostasis and disease. Towards the long-term goal of deciphering its role in temporomandibular disorders (TMDs), we characterised the spatial relationships of sympathetic and sensory nerves and their receptors in the mouse temporomandibular joint (TMJ) and trigeminal ganglion (TG), using the knee joint for comparison. MATERIALS AND METHODS: RNAscope was used for localisation of sympathetic nerve markers tyrosine hydroxylase (Th) and adrenergic receptor alpha 2a (Adrα2a); and brain-derived neurotrophic factor (Bdnf)

Epipharyngeal Abrasive Therapy (EAT) Field Theory: A Clinical Framework for EAT in Modulating Psycho-Neuro-Endocrino-Immune (PNEI) Dynamics.

Chronic epipharyngitis/chronic nasopharyngitis has traditionally been understood as a localized inflammatory disease, but clinically, it often presents with systemic symptoms that span the nervous, immune, and endocrine systems, such as headaches, fatigue, low-grade fever, impaired concentration, orthostatic dysregulation, joint pain, anxiety, and sleep disorders. Epipharyngeal abrasive therapy (EAT) is a treatment that applies chemical and physical stimulation to the mucous membranes of the epipharynx/nasopharynx using a cotton swab soaked in a 1% zinc chloride solution. Recently, there have

The protocol for a randomized, sham-controlled trial of transcutaneous auricular neurostimulation for chronic pain and opioid withdrawal symptoms during a 4-day opioid taper for adults in the United States.

BACKGROUND: Reducing opioid use is challenging due to limited evidence-based weaning methods and a lack of interventions to mitigate withdrawal symptoms. An emerging intervention using transcutaneous auricular neurostimulation (tAN) is being developed to reduce opioid withdrawal symptoms, but its mechanisms of action are not yet well understood. This is a clinical trial performed to investigate the mechanisms of tAN in managing pain and opioid withdrawal during opioid taper in adults with chronic pain. METHODS: This is a single-site, randomized, double-blind, and sham-controlled superiority f

Transcutaneous auricular neurostimulation to reduce heavy menstrual bleeding in women with and without von Willebrand disease.

Prior studies have revealed that electrical stimulation of the vagus nerve modifies platelet phenotype and reduces blood loss in preclinical models of soft tissue injury. This pilot trial (NCT06064851) sought to determine whether the use of a wearable transcutaneous auricular neurostimulation (tAN) device targeting both the vagus and trigeminal nerve branches correlated with reduced menstrual blood loss in participants with or without von Willebrand disease (VWD). Participants with qualified heavy menstrual bleeding (HMB) gave informed consent to participate in an IRB-approved, decentralized,

Cranial nerves as pathways for human cerebrospinal fluid efflux: In vivo evidence.

In vivo evidence for cerebrospinal fluid (CSF) efflux along cranial nerves in humans is scarce. This study investigated whether the trigeminal, facial, and vestibulocochlear nerves serve as efflux routes for CSF in humans. A magnetic resonance imaging (MRI) contrast agent, used as a CSF tracer, was administered intrathecally at the lumbar level, and consecutive MRI acquisitions measured tracer enrichment along the trigeminal, facial, and vestibulocochlear nerves. The study included 27 patients undergoing evaluation for potential CSF disturbances, but none of whom exhibited evidence of CSF path

Substitution of human olfaction by the trigeminal system.

Smell loss is a sensory impairment that has major consequences in many areas of daily life and for which current therapies are insufficient. A prosthesis-type technology enabling patients to sample their olfactory environment has not yet been developed. The aim of our study was, therefore, to test whether stimulation of the intranasal trigeminal system by a device combining an artificial nose with an intranasal electrical stimulator will enable patients to detect and discriminate odorant molecules. Four experiments involving normosmic individuals (n = 13) and patients with olfactory loss (n =

Fully Endoscopic Microvascular Decompression for Neurovascular Compression Syndrome: Early Outcomes and Technical Note.

BACKGROUND: Neurovascular compression syndromes (NVCSs) are a series of vascular compression diseases wherein there is usually entrapment or distortion of cranial nerves due to redundant or aberrant culprit vessels. Microvascular decompression (MVD) is an effective treatment for NVCS. However, the global adoption of fully endoscopic MVD (E-MVD) remains limited. METHODS: This retrospective study reviewed 105 patients with NVCS (102 cases for initial operation and 3 cases of disease recurrence) who underwent E-MVD. The cohort comprised of 59 hemifacial spasm (HFS), 34 trigeminal neuralgia (TN),
