Association between intraoperative balloon morphology and postoperative sensory dysfunction after percutaneous balloon compression for trigeminal neuralgia: a propensity score-matched cohort study.

BACKGROUND: Percutaneous balloon compression (PBC) is widely used for trigeminal neuralgia, but the association between intraoperative balloon morphology and postoperative sensory outcomes remains unclear. Defining this relationship may improve technique and risk stratification. Prior studies suggest selective injury to large myelinated fibers rather than generalized sensory fiber or ganglion injury, and balloon morphology should be interpreted as a procedural marker rather than a direct measure of neural injury. METHODS: We retrospectively identified patients who underwent PBC for trigeminal

Trigeminal sensory neuropathy developing as a late complication after the treatment of surgically induced necrotizing scleritis following pterygium surgery: a case report and literature review.

BACKGROUND: Surgically induced necrotizing scleritis (SINS) is an immune-mediated disease that occurs infrequently after ocular procedures, with latency ranging from days to years. There are various early and late complications that can negatively affect quality of life after SINS. This case report aims to present late trigeminal sensory neuropathy (TSN), corresponding in this patient to painful post-traumatic trigeminal neuropathy (ICHD-3 13.1.2.3), as a complication in a patient who developed SINS after pterygium surgery (PS). CASE PRESENTATION: A 31-year-old woman was admitted to our clini

Bilateral cranial polyneuropathy associated with an intracranial mass in a cat with concurrent tonsillar lymphoma.

A 14-year-old spayed female Siamese cat was presented with signs consistent with a rapidly progressive cranial polyneuropathy. Magnetic resonance imaging of the brain revealed an extra-axial mass at the calvarial floor, extending caudally to involve bilateral trigeminal nerves and rostrally to involve the pituitary gland and optic chiasm. Additional findings included contrast-enhancing content within the bilateral tympanic bullae, mild mandibular and medial retropharyngeal lymphadenopathy, and a left tonsillar mass. Cytologic and histopathologic findings confirmed tonsillar lymphoma. The concu

Interpreting the S1BF-ACC-LHb pathway in trigeminal neuralgia-associated affective behavior.

The study by Wang and colleagues proposes that three connected brain areas link facial nerve injury to pain and depression-like behavior in mice. The experiments are ambitious and potentially important. However, the nerve-injury model differs from the defining pattern of human trigeminal neuralgia, several behavioral and drug controls leave alternative explanations open, and measurements from many cells were not clearly separated from the animals that supplied them. Clarifying these issues, and distinguishing results from whole tissue from results specific to neurons, would make the causal and

Efficacy and Safety of Selective Extracranial Radiofrequency Thermocoagulation and Percutaneous Balloon Compression for V3 Trigeminal Neuralgia.

BACKGROUND: Trigeminal neuralgia (TN) is a neuropathic disorder causing paroxysmal severe facial pain. Radiofrequency thermocoagulation (RFTA) and percutaneous balloon compression (PBC) are standard surgical treatments. While RFTA is constrained by its lack of branch selectivity and the risks of repeated intracranial punctures, selective extracranial RFTA (SRFTA) has attracted attention due to its high targeting specificity and fewer neural complications. This study aims to compare the efficacy and safety of SRFTA and PBC in patients with idiopathic TN involving the V3 branch (TN V3). METHODS

The efficacy of surgical interventions for trigeminal neuralgia in multiple sclerosis: A systematic review.

Trigeminal neuralgia affects approximately 3% of patients with multiple sclerosis - a far higher prevalence than in the general population. The associated pain often proves refractory to medication, prompting consideration of neurosurgical intervention. The variable efficacy, tolerability, and durability of available procedures, compounded by factors such as multiple sclerosis subtype and neuroimaging findings, require evidence to guide personalised surgical decision-making. This systematic review evaluates surgical interventions for trigeminal neuralgia in multiple sclerosis, analysing how pa

Resting-state local brain functional synchronization after a single trigger-zone stimulation: with versus without evoked pain in classical trigeminal neuralgia patients.

BACKGROUND: Classical trigeminal neuralgia (CTN) is characterized by transient, stimulus-evoked pain attacks, but post-stimulation changes in local brain synchronization remain unclear. We characterized static and dynamic local functional synchronization at baseline and during resting-state scans initiated after a single trigger-zone stimulation. METHODS: Baseline analyses included 44 healthy controls (HC), 80 CTN patients with stimulation-evoked pain (CTN_pain), and 68 CTN patients without evoked pain (CTN_no_pain). Longitudinal analyses included 39 HC, 75 CTN_pain patients, and 64 CTN_no_pa

[Advances in innate immune-mediated neuroinflammatory mechanisms of migraine].

Migraine is a highly disabling neurological disorder in which activation of the trigeminovascular system and the ensuing peripheral and central sensitization are major pathological foundations for pain initiation, recurrent attacks, and disease chronification. In recent years, innate immune-mediated sterile neuroinflammation has emerged as a key mechanism linking trigeminal neurovascular system activation to maladaptive remodeling of nociceptive pathways. During the early phase of a migraine attack, trigeminal nerve endings release calcitonin gene-related peptide, pituitary adenylate cyclase-a

Intraoperative computed tomography-guided neuronavigation for radiofrequency rhizotomy in trigeminal neuralgia: optimizing cannulation trajectories for individual anatomy.

OBJECTIVE: Fluoroscopy-guided radiofrequency rhizotomy for trigeminal neuralgia relies on biplanar fluoroscopic imaging and surface landmarks. However, anatomical variations and imaging limitations often necessitate multiple attempts, leading to patient discomfort and increased procedural risks. This study evaluated the procedural outcomes of radiofrequency rhizotomy using preplanned trajectories and intraoperative computed tomography with neuronavigation. DESIGN: Retrospective study. SETTING: Single-center study conducted in a neurosurgical department. PARTICIPANTS: Forty-six patients with

Facial airflow enhances the benefits of exercise training in people with chronic lung disease: a randomised controlled trial.

INTRODUCTION: Breathlessness limits exercise training intensity in people with chronic lung disease (CLD). Stimulation of the trigeminal nerve via fan-to-face (F2F) therapy (facial airflow) can reduce exertional breathlessness and improve exercise endurance in CLD. This randomised controlled trial tested the hypothesis that adding F2F therapy to an exercise training programme could enhance the benefits of exercise training on exercise endurance time (EET) and exertional breathlessness in adults with CLD by allowing them to train at higher intensities. METHODS: 23 participants with COPD (n=19)

External trigeminal nerve stimulation in youth with ADHD: a randomized, sham-controlled, phase 2b trial.

External trigeminal nerve stimulation (TNS) received US Food and Drug Administration clearance in 2019 as the first device-based, non-pharmacological treatment for attention-deficit/hyperactivity disorder (ADHD), based on a small pilot sham-controlled randomized controlled trial (RCT) that reported symptom improvement in 62 children with ADHD. Here we conducted a confirmatory multicenter, double-blind, randomized, sham-controlled, parallel-group, phase 2b RCT to investigate short-term and long-term efficacy (6 months) of real versus sham TNS in 150 children and adolescents with ADHD. HCN2 channels: a potential therapeutic target for orofacial neuropathic pain after trigeminal nerve injury.

Hyperpolarization-activated cyclic nucleotide-gated (HCN) channels have recently emerged as promising targets for the treatment of neuropathic pain. This study investigated the potential involvement of HCN2 channels in the development of trigeminal neuropathic pain following peripheral nerve injury. Infraorbital nerve chronic constriction injury (ION-CCI) model was adopted to rats, and head withdrawal thresholds (HWT) to mechanical stimulation were assessed pre- and postoperatively, as well as after pharmacological intervention. In the trigeminal ganglion (TG), intracellular cyclic adenosine m

Inhibition of H3K14 lactylation promotes diabetic corneal nerve regeneration via Wnt1/β-catenin signaling.

AIMS: This study bridges diabetes-induced metabolic alterations and corneal neuropathy, proposing lactylation as a key mechanistic and therapeutic target in diabetic keratopathy (DK). METHODS: We used a streptozotocin-induced Type 1 diabetes mellitus (T1DM) mouse model to examine metabolic reprogramming in trigeminal ganglion (TG) neurons and its effects on corneal nerve regeneration. Glycolysis, lactate levels, and histone lactylation (H3K14la) were analyzed via Western blot, qPCR, immunofluorescence, and enzymatic assays. Genome-wide H3K14la profiling was conducted using CUT&Tag, followed b

Neurotrophic keratopathy in childhood: advances in understanding of pathogenesis and management.

Neurotrophic keratopathy (NK) is an uncommon corneal disorder caused by trigeminal nerve dysfunction, leading to loss of ocular surface sensation, impaired corneal epithelial maintenance, and possible progressive stromal lysis. NK is of added potential visual significance in children on account of the risk of amblyopia resulting from stromal opacification. Unlike acquired NK in adult-onset disease, NK in childhood is frequently congenital or inherited, linked to genetic pain insensitivity syndromes, cranial dysinnervation disorders or broader developmental anomalies. Visual function can be wel
