BACKGROUND: The pretemporal anteromedial interdural approach provides direct extradural access to Meckel's cave by exploiting the natural plane between the temporal dura propria and the meningeal layer of the lateral wall of the cavernous sinus. After a pretemporal craniotomy, the foramen rotundum guides interdural dissection along V2 toward the corridor between V1 and V2, enabling controlled exposure of the Gasserian ganglion and proximal trigeminal rootlets. Tumor resection proceeds through internal debulking and circumferential dissection, with extension into the posterior fossa when necess

OBJECTIVE: The optimal surgical approach for trigeminal schwannomas (TSs), particularly between the endoscopic endonasal approach (EEA) and endoscopic transorbital approach (ETOA), remains unclear. This study aimed to analyze radiological factors influencing surgical outcomes and establish criteria to guide approach selection.
METHODS: A retrospective review of the records of TS patients treated with the EEA or ETOA at three tertiary centers from 2015 to 2023 was conducted. The authors introduced the orbito-ovale line (OOL) to classify tumors as low-lying or high-lying. A scoring system (rang

OBJECTIVE: Microvascular decompression (MVD) is an effective treatment for trigeminal neuralgia (TGN), but anatomical/clinical predictors of long-term outcomes are incompletely defined, and many studies emphasize only immediate complications. This study investigates long-term outcomes after MVD for TGN and predictors of postoperative pain relief and recurrence.
METHODS: One hundred thirty-two MVDs performed for TGN in 124 patients (February 2012-November 2021) were retrospectively analyzed. Demographics, prior therapies, preoperative magnetic resonance imaging (MRI), intraoperative findings,

BACKGROUND: Trigeminal neuralgia (TN) is a severe neuropathic pain disorder in which peripheral neurovascular compression alone does not fully explain the associated central pain manifestations. Although diffusion tensor imaging studies have reported white matter (WM) alterations in TN, findings have been heterogeneous, partly due to limited sensitivity to complex fibre architecture. Fixel‑based analysis (FBA) enables fibre‑specific assessment of WM microstructural and macrostructural properties and may provide improved anatomical and biological specificity. METHODS: This cross‑sectional study

BACKGROUND: Extraction of lower third molars (3 Ms) carries a risk of inferior alveolar nerve (IAN) injury. Anatomical proximity is a well-known risk factor, although the effect of patient age remains insufficiently explored.
PURPOSE: The study purpose was to measure the association between age and IAN injury among patients whose 3 Ms are close to the mandibular canal.
STUDY DESIGN, SETTING, SAMPLE: This case-control study included patients at increased risk for IAN injury due to close proximity of the lower 3 Ms to the mandibular canal, as assessed by preoperative cone-beam computed tomogra

BACKGROUND: Immediate implant placement in the posterior mandible is associated with a risk of inferior alveolar nerve (IAN) injury due to anatomical variations such as lingual concavities and variability in mandibular canal positioning. Accurate three-dimensional assessment of these structures is therefore essential.
OBJECTIVES: This study aimed to evaluate the root apex-mandibular canal distance (RAC), prevalence and morphology of lingual concavities, and cross-sectional ridge patterns in the posterior mandible to identify high-risk sites for IAN injury during immediate implant placement.

Idiopathic trigeminal-mediated headshaking is a painful neuropathic disorder characterized by sudden, violent, predominantly vertical head movements of unknown etiology. Clinical signs may be seasonal or persistent and triggered by light, sound, exercise, or tactile nasal stimulation. The pathophysiology is multifactorial, involving abnormal trigeminal nerve excitability influenced by hormonal, environmental, and dietary factors. Treatment is challenging and typically multimodal, aiming to reduce trigeminal hypersensitivity and improve quality of life.

BACKGROUND: Trigeminal neuralgia (TN) is a debilitating neuropathic pain disorder characterized by severe paroxysmal facial pain. Increasing evidence indicates that Schwann cell dysfunction and altered nerve repair mechanisms contribute to TN pathology. MicroRNAs (miRNAs), as key regulators of Schwann cell plasticity, myelination, inflammation, and nerve regeneration, represent promising biomarker candidates. This study aimed to investigate differential expression patterns of Schwann cell-associated miRNAs (miR-221-3p, miR-132-3p, miR-210-5p, miR-340-3p, miR-98-5p, miR-182-5p) in patients with

Trigeminal nerve injury can lead to chronic and difficult-to-treat orofacial neuropathic pain. Here, we uncover a key role for the cation channel TRPM3 in the chronic constriction injury of the infraorbital nerve (IoN-CCI) mouse model of trigeminal neuropathic pain. Wild-type (WT) mice develop spontaneous pain and mechanical allodynia for up to 6 weeks following IoN-CCI, whereas Trpm3-/- mice do not develop such symptoms. Using longitudinal RNA sequencing (RNA-seq) analysis, we obtain a detailed time course of transcriptome alterations in trigeminal ganglia during progression of the IoN-CCI mo

BACKGROUND: Mandibular defects resulting from trauma, tumors, and infections present a considerable clinical challenge, profoundly affecting patients' psychological and physical well-being. Native bone tissue relies on tightly coordinated neurovascular crosstalk to regulate bone development, remodeling, and regeneration. Vascularization is well-studied in bone tissue engineering, whereas neural integration remains relatively unexplored. Consequently, the development of neurovascularized bone tissue engineering scaffolds represents a promising strategy for enhancing the outcomes of bone tissue

PURPOSE: Neurotrophic keratopathy (NK) results from trigeminal nerve impairment, leading to loss of corneal sensation (CS), epithelial breakdown, and in advanced stages, stromal ulceration or perforation. Cenegermin, a recombinant human nerve growth factor, is the first and only FDA-approved therapy for NK, although existing studies are limited by small sample sizes. This systematic literature review evaluated cenegermin's effects on CS and corneal innervation (CI) in patients with NK.
METHODS: English-language studies published between August 2018 and June 2024 measuring CS or CI after ceneg

BACKGROUND: Idiopathic trigeminal neuralgia (ITN) affects approximately 0.16% to 0.3% of the population, with severe complications occurring in 15% of cases. Existing treatments often yield suboptimal outcomes.
CASE REPORT: This study evaluates the clinical efficacy of Gasserian ganglion stimulation (GGS) for ITN management in 2 patients. GGS led to significant symptom improvement in both patients, with one patient achieving complete remission. Compared to conventional therapies, GGS demonstrated a 30% higher efficacy rate, suggesting its potential as a superior therapeutic option. However, l

BACKGROUND: Dolichoectatic vertebrobasilar arteries (DBA)-induced trigeminal neuralgia (TGN) is rare, and both medication and surgical decompression often have a substantial treatment failure rate. Surgical techniques that provide sufficient decompression and long-term pain relief are challenging in cases of large DBA with limited posterior fossa space, and have been scarcely described due to their rarity and technical complexity. We present our novel technique involving mobilization of the trigeminal nerve (CN5) combined with macrovascular transposition of the gigantic DBA for curative treatm

BackgroundClassical trigeminal neuralgia (CTN) is a severe chronic pain disorder. While microvascular decompression (MVD) is an effective treatment, outcomes are variable, and preoperative predictive biomarkers are needed. Furthermore, the relationship between brain microstructural alterations and neurotransmitter systems in CTN is not well understood. This study aimed to characterize these alterations and assess their prognostic value for MVD.MethodsWe evaluated microstructural alterations in both white matter (WM) and gray matter (GM) in 101 CTN patients and 84 healthy controls (HCs) using N
