Facial Neuropathy in Patients with Recurrent Painful Ophthalmoplegic Neuropathy.

INTRODUCTION: Involvement of non-ocular motor cranial nerves, such as the trigeminal, facial or hypoglossal nerves, has rarely been reported in recurrent painful ophthalmoplegic neuropathy (RPON). This study aimed to determine the incidence and clinical significance of multi-cranial nerve involvement in RPON. METHODS: Eight patients (females = 5; mean age = 53.3 ± 15.6 years) who met International Classification of Headache Disorders-3 diagnostic criteria for RPON were enrolled at a single tertiary hospital. Repeated evaluations were performed for differential diagnosis, including cranial ner

A Case Report of Guillain-Barré Syndrome Presenting as Facial Diplegia: A Deviation From the Classical Ascending Paralysis.

Guillain-Barré syndrome (GBS) is an immune-mediated inflammatory disorder affecting the peripheral nerves, typically presenting with ascending paralysis and areflexia in the lower limbs, but can also have other variable presentations. Such variable presentations can rarely include an isolated involvement of the facial nerve as the initial presenting feature. We explore a case of a 53-year-old male patient who initially came with bilateral lower motor neuron facial nerve palsy, preliminarily diagnosed as inflammatory Bell's palsy. However, the patient later re-presented with further neurologica

Paediatric Peripheral Facial Nerve Palsy-The Role of Ear Pathology.

BACKGROUND: Nearly half of the acquired cases of peripheral facial nerve palsy (PFNP) are of unknown aetiology, a condition termed Bell's palsy (BP). PFNP in children can often be associated with acute otitis media (OM). Differences between the clinical course and prognosis in OM-associated PFNP (OM-PFNP) and BP in children are not well characterised. METHODS: Retrospective review of hospitalised children in a tertiary care children's hospital due to non-traumatic and non-surgery associated with PFNP. RESULTS: Out of 177 children hospitalised with facial nerve palsy, 46 were excluded. A tota

Management of Bell's palsy in a 2-month-old following COVID-19 infection: a case report and review of the literature.

BACKGROUND: Bell's palsy (idiopathic peripheral facial nerve paralysis) is uncommon in children and is particularly rare in infants younger than one year of age. Its occurrence following viral infections, including coronavirus disease 2019 (COVID-19), is exceedingly rare. We report a case of Bell's palsy in an infant following COVID-19 infection and provide a review of the relevant pediatric literature. CASE PRESENTATION: We report the case of a 2-month-old female infant who developed left-sided facial weakness shortly after a COVID-19 infection. She had previously been healthy, except for a

Electrophysiological evidence of severe marginal mandibular branch injury in Bell palsy: Implications for optimizing clinical assessment and treatment - a retrospective study.

Bell palsy (BP) is an acute idiopathic peripheral facial neuropathy typically presenting with unilateral weakness. However, the differential electrophysiological involvement of its 6 main branches remains poorly defined, limiting branch-level diagnosis and targeted management. This retrospective study aimed to characterize nerve conduction parameters across these branches, compare injury severity, and refine electrophysiological protocols to enhance clinical decision-making. This retrospective study enrolled 56 patients with BP. Compound muscle action potential (CMAP) amplitude and distal moto

Structural Mimics of Recurrent Bell's Palsy: A Case Report Highlighting Diagnostic Red Flags in Facial Nerve Schwannoma and Metastasis.

Background and Clinical Significance: Recurrent peripheral facial palsy is most often attributed to idiopathic Bell's palsy (BP), yet a minority of patients harbor underlying structural lesions. Early recognition of such lesions is essential for timely and appropriate management. Case Presentation: We retrospectively reviewed recurrent unilateral facial palsy at a single tertiary center and selected two illustrative patients in whom serial clinical, electrodiagnostic, and magnetic resonance imaging (MRI) findings ultimately indicated structural pathology. The first patient experienced three st

Supranuclear infarction mimicking Bell's palsy: a case report of splenial infarction.

BACKGROUND: The classic distinguishing feature between peripheral and central facial palsy is the preservation of forehead wrinkles; however, in rare instances, supranuclear lesions can present as peripheral-type facial palsy involving both the upper and lower face, and cases caused by splenial infarction are particularly uncommon among them. CASE PRESENTATION: We report a 61-year-old woman who presented with isolated left-sided peripheral-type facial palsy. No predominance of lower facial involvement or dissociation between emotional and volitional facial movements was observed. Magnetic res
