Pediatric Post-COVID-19 Neuromyelitis Optica Spectrum Disorder: A Case Report.

Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune astrocytopathy affecting the central nervous system, particularly the optic nerves and spinal cord. Post-infectious triggers, including SARS-CoV-2 (COVID-19), have been increasingly recognized as potential catalysts for this immune-mediated condition. We report the case of a nine-year-old boy who developed AQP4-IgG-positive NMOSD following confirmed SARS-CoV-2 infection. The patient's clinical onset began 1.5 months prior to admission with a seven-day episode of intractable vomiting, indicative of area postrema syndrome (APS).

Lesion Expansion in Craniofacial Fibrous Dysplasia: Natural History and Indicators of Progression in the Frontal, Zygomatic, and Sphenoid Bones.

Fibrous dysplasia (FD) is characterized by expansile fibro-osseous lesions that may occur in association with endocrinopathies as part of McCune-Albright syndrome (MAS). Craniofacial FD may result in substantial morbidity, including facial asymmetry and compression of vital neurovascular structures. There is a critical need to understand the natural history and risk factors for craniofacial lesion expansion to develop preventative trials and identify candidates for intervention. The purpose of this study was to evaluate expansion rates in periorbital bones (frontal, zygomatic, and sphenoid). P

Miescher's Granulomatous Cheilitis: A Rare Cause of Recurrent Lip Inflammation.

Miescher's granulomatous cheilitis (GC) is a rare, noninfectious, chronic, idiopathic granulomatous disorder characterized by recurrent swelling of the lips. It is considered part of the triad of Melkersson-Rosenthal syndrome, a neurological and cutaneous disorder characterized by orofacial edema, peripheral facial nerve palsy, and a fissured tongue. However, the complete triad is uncommon, and Miescher's GC may present as a monosymptomatic form. Due to its rarity, the condition has a broad differential diagnosis and poses a diagnostic challenge. We report the case of a 33-year-old woman with

Hotspots and trends in peripheral facial paralysis treatment: A bibliometric analysis.

BACKGROUND: Peripheral facial paralysis (PFP) is a nonspecific inflammatory disease of the facial nerve, and the resulting sequelae seriously affect social function and mental health. Bibliometric analysis determines the focus and status of current research. However, no comprehensive bibliometric study has yet focused on PFP treatment. METHODS: Relevant publications from 2004 to 2024 were retrieved from the Web of Science. Visualization and bibliometric analyses were conducted using VOSviewer, CiteSpace, and Scimago Graphica to explore collaborative networks among countries, institutions, and

What happens next: unexpected malignancies after extracapsular dissection of parotid tumors.

BACKGROUND: Extracapsular dissection (ECD) is commonly used for benign parotid tumors because of its low morbidity. Unexpected malignant diagnoses after ECD raise concerns regarding oncologic adequacy and postoperative management. AIMS/OBJECTIVES: To evaluate the long-term oncologic and functional outcomes of patients with unexpected malignant parotid tumors treated with ECD. MATERIAL AND METHODS: A retrospective review was performed of patients who underwent ECD for presumed benign parotid tumors between 2012 and 2020. Patients with a final malignant histopathological diagnosis and a minimu

Implantable Gasserian ganglion stimulation for refractory trigeminal neuropathic pain: systematic review of observational clinical evidence.

INTRODUCTION: Refractory trigeminal neuropathic pain, including classical trigeminal neuralgia (TN) and secondary etiologies, is a chronic neuropathic disorder characterized by recurrent, electric shock-like facial pain that can be disabling and resistant to conventional treatments. Electrical stimulation of the Gasserian ganglion through the foramen ovale has emerged as a minimally invasive technique capable of modulating nociceptive transmission. OBJECTIVE: This systematic review aimed to assess the efficacy and safety of electrical stimulation of the Gasserian ganglion in patients with ref

Clinical features and diagnostic pathways of trigeminal neuralgia: a retrospective study in a tertiary orofacial pain clinic.

BACKGROUND: Trigeminal neuralgia (TN) is a neuropathic facial pain disorder that frequently mimics odontogenic and other orofacial pain conditions, leading many patients to initially seek dental care. This study aimed to characterize the clinical features and diagnostic pathways of patients with TN presenting to a tertiary orofacial pain clinic. METHODS: This single-center retrospective study included newly diagnosed patients with TN who attended a tertiary orofacial pain clinic between 2024 and 2025. Data regarding their demographic characteristics, pain features, pain intensity, trigger fac

Conversion surgery following trastuzumab and docetaxel combination therapy for unresectable HER2-positive parotid gland cancer.

Salivary gland cancers (SGCs) are rare and histologically diverse tumors. Among them, salivary duct carcinoma and carcinoma ex pleomorphic adenoma frequently exhibit HER2 overexpression, for which trastuzumab (Tmab) combined with docetaxel (DTX) has shown clinical efficacy. We report two cases of initially unresectable, HER2-positive parotid gland carcinoma successfully treated with Tmab and DTX, enabling curative surgery. In Case 1, a 68-year-old woman with T4bN2bM0 parotid carcinoma experienced marked tumor shrinkage, allowing surgical resection while preserving the skin and facial nerve. In

Identification of a Hidden Lesion in Meckel's Cave: A Diagnostic Odyssey of Unilateral Facial Pain in a Nonagenarian.

Fibrous dysplasia of the temporal bone and petrous apex is an uncommon benign fibro-osseous disorder, particularly in elderly individuals. Involvement of the skull base may present with atypical cranial neuropathies and may mimic inflammatory, vascular, or neoplastic diseases. This report describes a 90-year-old woman with recurrent unilateral facial pain and episodic facial muscle contractions. Initial assessment considered neoplastic pathology and inflammatory vasculopathy. Serial clinical evaluations, laboratory investigations, and neuroimaging established the diagnosis of left petrous apex

Decremental responses following repetitive nerve stimulation in spinal and bulbar muscular atrophy.

OBJECTIVE: The presence of decremental responses following repetitive nerve stimulation (RNS) in amyotrophic lateral sclerosis (ALS) is well established. However, in spinal and bulbar muscular atrophy (SBMA), a rare X-linked recessive lower motor neuron disease, the incidence and distribution of decremental responses across different muscles have not been thoroughly investigated. METHODS: Patients with SBMA were retrospectively identified in our database. RNS at a frequency of 3 Hz was performed on five muscles: the abductor pollicis brevis (APB), abductor digiti minimi (ADM), upper trapezius

Doxazosin Alleviates Chronic Orofacial Pain.

Central to the linkage of pain circuitry with the limbic system is its initial NAα2-mediated antinociceptive effect in acute pain models, followed by contradictory pronociceptive activation by the locus coeruleus seen in chronic pain models. Rats with a stable, long-term (>10 weeks) inflammatory compression of the trigeminal infraorbital nerve (FRICT-ION) preclinical model were given daily doxazosin, a slow-release NAα1 receptor antagonist, in weeks 8-10. Facial hypersensitivity was reversed back to baseline in male and female rats, but anxiety was only reduced in male animals. Doxazosin-decre

Clinical experience with the diagnosis and treatment of middle ear adenomatous neuroendocrine tumors.

Middle ear adenoma, also known as middle ear adenomatous neuroendocrine tumors (MEANTs), is a rare neoplasm, accounting for < 2% of all middle ear tumors. It is characterized by slow growth, non-invasive behavior, and nonspecific symptoms, which often complicate preoperative diagnosis. This review summarizes our clinical experience in the diagnosis and treatment of 11 cases of MEANT. In this cohort, four cases were preoperatively diagnosed as cholesterol granuloma, five as cholesteatoma, and two as middle ear tumors. The median age at diagnosis was 36.7 years, with all patients (100%) experien

The Takotsubo Syndrome: Where the Sympathetic Stress Reflex and the Dive Reflex Conflict.

Understanding of the pathophysiology of Takotsubo syndrome (TTS) remains incomplete. This is a hypothesis-driven editorial, proposing a new autonomic mechanism that may explain several important and puzzling features of TTS. Rather than involving a single reflex pathway, we suggest that TTS may result from the interaction and conflict of two evolutionarily conserved autonomic reflex responses. The first is the sympathetic stress response (SSR), a cardiovascular predominantly sympathomimetic reaction characterised by heightened adrenergic activity, tachycardia and cardiovascular stress. The sec

Otologic Manifestations and Peripheral Facial Nerve Palsy As the Initial Presentation of Granulomatosis With Polyangiitis: A Case Report.

Granulomatosis with polyangiitis (GPA) is a rare antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis that predominantly affects the upper and lower respiratory tracts and the kidneys. Although otolaryngological manifestations are common, isolated otologic symptoms as the initial presentation are rare and may delay diagnosis. We report the case of a 65-year-old man who initially presented with refractory bilateral otitis media associated with progressive mixed hearing loss, vestibular dysfunction, and subsequent peripheral facial nerve palsy. Initial temporal bone imaging was unrem

[Complications of acute otitis media].

Complications of acute otitis media (AOM) are rare but can occur intratemporally or intracranially. Clinically relevant mastoiditis usually presents with subperiosteal abscesses, whereas subclinical opacification of the mastoid cells is common but does not require specific treatment. Other potential complications include labyrinthitis, facial nerve palsy, intratemporal abscess, cerebral venous sinus thrombosis, or meningitis. Diagnosis relies on clinical examination, laboratory tests, and targeted imaging. Intratemporal complications are primarily treated with antibiotics. Surgical interventio
