Normal and pathologic radiographic anatomy of the motor innervation of the face.

Facial motor disorders, including facial paralysis, myokymia, dyskinesia, and hemifacial spasm, are common clinical problems in which radiographic evaluation plays a crucial role. Since every segment of the motor innervation of the face from the brain to the parotid gland can now be seen radiographically, radiologists must understand the normal anatomy, the common pathologic lesions at each level, and the clinical findings that help localize the abnormality so that the most sensitive and accurate radiographic approach can be planned. Though computed tomography alone allows for visualization of

Current medical treatment for facial palsy.

Medical treatment for facial palsy includes an accurate diagnosis and reliable estimate of prognosis as well as appropriate medication. Cranial polyneuritis (Bell's palsy and Ramsay Hunt syndrome), the most common cause of facial palsy, is an inflammatory, autoimmune, demyelinating disease best treated by parenteral steroids without surgical intervention. The antiviral agent acyclovir is now being tested as an adjunct to or replacement for steroid therapy. Trauma, the second most common cause of facial palsy, is often treated with steroids, but no controlled study has ever been performed. Howe

Peripheral nerve involvement in Bell's palsy.

A group of patients with Bell's palsy were studied in order to disclose the presence of subclinical peripheral nerve involvement. 20 patients, 8 male and 12 female, with recent Bell's palsy as their unique disease were examined. In all cases other causes of polyneuropathy were ruled out. Patients were investigated with CSF examination, facial nerve latencies in the affected and in the sound sides, and maximal motor nerve conduction velocities, as well as motor terminal latencies from the right median and peroneal nerves. CSF laboratory examination was normal in all cases. Facial nerve latencie

Retrolabyrinthine section of the vestibular nerve.

When symptoms of dizziness and episodic vertigo cannot be controlled through medical management or drainage procedures such as endolymphatic subarachnoid shunt operations, selective vestibular nerve section may be necessary. In the 1920s eighth cranial nerve sections were performed by neurosurgeons through the suboccipital approach but were frequently associated with hearing loss and facial paralysis. The middle fossa approach has been popularized by Dr. William House and others as a method of selectively sectioning the vestibular nerve and preserving facial and cochlear function. More recentl

Histopathology of neurosensory deafness in sarcoidosis.

Sarcoidosis is an idiopathic systemic granulomatous disease which occasionally causes fluctuating auditory and vestibular dysfunction. The temporal bones from a 32-year-old man deaf for 5 years from CNS sarcoidosis were examined histologically and compared with other nervous system tissues. It was found that the acoustic, vestibular, and facial nerves were involved in a striking perivascular lymphocytic infiltration resulting in myelin and axonal degeneration. The cochlear and labyrinthine neuroepithelium and stria vascularis had degenerated. It is hypothesized that neurosensory deafness and v

Traumatic third nerve palsy.

Twenty patients with a traumatic third nerve palsy had sustained a closed head injury with prolonged loss of consciousness in a high-speed deceleration accident. Sixteen were male, and the average age was 25 years. Seven had skull or facial fractures, 15 damage to the anterior visual pathways, and 16 other permanent neurological damage. Nineteen developed the misdirection/regeneration syndrome. Thirteen had strabismus surgery, and an area of binocular single vision was enlarged or achieved in three.

[Kawasaki's syndrome: description of 4 cases].

In this study we present four cases of Kawasaki syndrome; in two of them were present some particular aspects, besides the characteristic signs of the lymphonodal-mucocutaneous syndrome. One little girl showed, during the acute phase of the disease, paralysis of facial nerve, without other signs of neurological involvement; in the second patient, the outcome of the syndrome was quite atypical, because of an osteoarticular involvement; we also observed the presence of immature white blood cells in the peripheral blood. Three of the four cases were treated with acetyl-salicylic acid for almost o

Pharmacotherapy for chronic facial pain.

The clinical usefulness of a drug therapy program for the management of chronic head and neck pain requires an understanding of the conditions in which the program will be used. The practitioner should remember that drug therapy is not the keystone of chronic pain management, but only a useful adjunct that should be eliminated or reduced as soon as feasible. The other various modalities for chronic pain management, which include physical therapy, relaxation therapy, transcutaneous electrical nerve stimulation, injection therapy, and occlusal reconstruction should be utilized to their fullest e

Treatment of essential blepharospasm. I. Comparison of facial nerve avulsion and eyebrow-eyelid muscle stripping procedure.

Benign essential blepharospasm is an incurable disease for which many treatment modalities have been suggested. The two surgical procedures that have been used most commonly are avulsion of the facial nerve and stripping of the protractor muscles of the eyelid and brow. We compare two matched series of 22 patients; one group underwent facial nerve avulsion ("Reynold's" procedure), and the other underwent "muscle stripping" ("Anderson's" procedure). We noted the number of procedures required for the patient to obtain a functional visual result and the side effects produced. Additional surgical

Relapsing polychondritis with multifocal neurological abnormalities.

A sixty-five year old woman developed relapsing polychondritis with three of the diagnostic criteria established by McAdam et al. (1976), namely bilateral auricular chondritis, ocular inflammation and both cochlear and vestibular dysfunction. Many authors have mentioned other neurological symptoms including unilateral facial weakness. This patient is the first case described with bilateral facial weakness and cerebral manifestations. Relapsing polychondritis, an uncommon, recurrent, inflammatory disorder affecting the cartilaginous tissues of the body (Jaksch-Wartenhorst, 1923; Herman, 1981),

Multiple sclerosis with onset after age 60.

The onset of multiple sclerosis (MS) after age 60 is uncommon. The clinical features of three such cases are described. Considerable diagnostic delay was seen in each case as symptoms were initially attributed to other conditions that more commonly afflict the elderly, including atypical facial pain, neuropathy, peripheral and cerebral vascular disease, arthritis, benign prostatic hypertrophy, and ischemic optic neuropathy. Although MS should be considered a diagnosis of exclusion in this age group, cerebral evoked potentials and CSF immunoglobulin studies can increase the diagnostic yield and

Styloid-stylohyoid syndrome in the differential diagnosis of atypical facial pain.

Differential diagnosis of atypical facial pain can be a perplexing problem. Similarities in symptomatology and nonspecific subjective findings make diagnosis difficult. The astute diagnostician, regardless of his speciality, must be acutely aware of key findings that will differentiate the myriad of chronic pain syndromes in his patients. Accurate correlation of clinical and radiographic findings in patients with "Eagle's syndrome" may assist the clinician in distinguishing this entity from other pain syndromes, such as glossopharyngeal or vagoglossopharyneal neuralgia.

Granular cell tumor arising in myelinated peripheral nerves. Light and electron microscopy and immunoperoxidase study.

The histologic, immunohistochemical and ultrastructural characteristics of two granular cell tumors arising from the right recurrent laryngeal and left facial nerves are described. S-100 protein was detected both in the nuclei and cytoplasm of the granular cells using the peroxidase-anti-peroxidase method. The ultrastructural findings in both cases support a Schwann cell derivation of the granular cells. It is suggested that the granularity of cells of granular cell tumor may represent a lysosomal disorder affecting most frequently neoplastic and nonneoplastic Schwann cells and occasionally ot

Rhabdomyosarcoma of the ear in childhood. A report from the Intergroup Rhabdomyosarcoma Study-I.

Twenty-four children with rhabdomyosarcoma of the middle (22 patients) or external ear (two patients) were entered on the Intergroup Rhabdomyosarcoma Study-I protocol from 1972 to 1978. After surgery, treatment consisted of radiotherapy plus vincristine, dactinomycin, and cyclophosphamide, with or without Adriamycin. Nine of 19 patients (47%) who presented with localized sarcoma are free of disease at 2.2 to 6.5 years after diagnosis (median, 3.6 years). One is alive with regional recurrence at 6.7 years; another developed a contralateral cerebellar astrocytoma 4.4 years from diagnosis and die

Stereotactic radiosurgery in cases of acoustic neurinoma: further experiences.

Fourteen patients with acoustic neurinomas varying in size from 7 to 30 mm were treated by stereotactic radiosurgery and then were followed for 4 years. The tumors of eight patients decreased 1 to 10 mm in diameter, no change was found in two, and the tumor increased slightly in three patients. Loss of the ability to enhance with contrast administration on computed tomographic scan was a common effect of the treatment. One patient in poor general condition died from intercurrent disease 6 months after radiosurgery. At postmortem examination, a large central necrosis was found. Among five patie

Hemifacial spasm: treatment by microsurgical facial nerve decompression.

The literature on hemifacial spasm and its surgical therapy is reviewed, and the authors' experiences with 20 patients are described. Vascular cross compression of the facial nerve adjacent to the brain stem is seen in 90% of the surgical patients. Mobilizing the offending vessel will cure or significantly improve approximately 80% of the patients. Complications occur in 25% of the patients and usually involve decreased hearing or facial weakness. Mortality is virtually zero, and this operation is vastly superior to any other medical or surgical therapy for hemifacial spasm. The pathophysiolog

Blepharospasm associated with brainstem lesions.

We studied six patients with clinical and radiographic evidence of rostral brainstem lesion and bilateral blepharospasm. Two patients also had other facial dystonic movements. Four patients suffered rostral brainstem strokes, and two had multiple sclerosis. None had been treated with antipsychotic drugs prior to the onset of blepharospasm. Medical treatment was helpful in two patients, and bilateral selective facial nerve section was used in another patient. Possible pathogenic mechanisms are discussed.

[Circulating immune complexes in uveitis].

The serum of 63 patients suffering from various clinical types of endogenous uveitis with unknown etiology was investigated for the presence of circulating immune complexes (CIC). None of the patients was suffering from a systemic disease usually associated with CIC. Significantly elevated CIC levels were detected in 2 women with chronic cyclitis. One of them was also suffering from paresis of the facial nerve. The investigation reported here indicates that C1q-binding CIC are of no importance in the pathogenesis of endogenous uveitis. On the one hand one should consider the possibility of loc

[Mixed connective tissue disease (Sharp syndrome) with involvement of the trigeminal nerve and facial paralysis].

The authors report a case of mixed connective tissue disease with involvement of the fifth and seventh cranial nerves. While involvement of the trigeminal nerve is well-known, that of the seventh nerve is much less so. However, seventh nerve involvement is beginning to be recognized in other connective tissue diseases such as scleroderma. Neurological manifestations in Sharp syndrome are reviewed.
