Temporomandibular joint ankylosis--experience with a case of twenty years' duration.

The clinical and radiographic findings and operative management of a patient having temporomandibular joint ankylosis for 20 years is presented. The patient had fibrous ankylosis on one side and complete bony ankylosis on the other. There was substantial growth of the mandible on the side with fibrous ankylosis, and marked growth retardation on the side with bony ankylosis. Both a preauricular and an intraoral approach were required to release structures restricting mandibular opening. Complications included temporary upper facial nerve palsy and a persistent anterior open bite requiring furth

Facial nerve paralysis: it is always Bell's palsy?

Most patients with facial nerve paralysis have idiopathic, or Bell's palsy. Nevertheless, other more serious causes should be carefully excluded before Bell's palsy is diagnosed. Several tests can help the physician determine the site of the lesion and the physiologic extent of involvement of the facial nerve. The type of treatment depends on the cause of the paralysis and whether it is complete or partial. Familiarity with facial nerve anatomy and the various causes of facial paralysis should facilitate early diagnosis and appropriate treatment.

Non-neoplastic disorders of the parotid gland.

The parotid glands are subject to many pathological disorders other than neoplastic diseases. This large group of disorders is classified for the parotid glands into infectious, metabolic and endocrine, traumatic, congenital, and acquired cystic diseases. The other major salivary glands also can be affected by many of these conditions. In recent years more and more patients have been examined and treated who have had diseases classified according to our concepts. Surgical procedures are now commonly recommended for these groups of patients not only for diagnosis but for treatment as well. The

Temporal bone histopathology of necrotizing external otitis. A case report.

Histopathological examination of the temporal bone of an individual with necrotizing external otitis revealed severe inflammation with necrosis in the subcutaneous granulation tissue in the external auditory canal. Erosion of the bony walls of the canal had created a defect in the anterior wall, through which infection spread to the preauricular region. The lateral part of the middle ear cavity was also filled with granulation tissue and purulent exudate. A dehiscence of the horizontal portion of the facial canal had apparently allowed spread of an inflammatory round cell infiltrate along the

[Neurologic syndromes in patients with influenza and other viral infections].

The authors show that in the Ukrainian SSR, the viral diseases of the nervous system are associated primarily with influenza, epidemic parotitis, enteroviral infections, and less frequently with parainfluenza, adenoviral infections and their combinations. These nervous diseases manifested themselves as meningeal and meningoencephalic syndromes, serous meningitis, encephalitis, meningoencephalitis, encephalomyelitis, mono- and polyradiculoneuritis. The character of the regression of the morbid manifestations depended on the gravity and the type of the nervous system affections, time of the inst

Acoustic neuroma surgery. Translabyrinthine-transtentorial approach via the middle cranial fossa.

In our approach for acoustic tumors, the method of Morrison and King and that of Bochenek and Kukwa have been modified into one method. This modified method is basically a neuro-otological-neurosurgical team approach, extending the operative field by drilling the temporal bone and cutting the superior petrosal sinus, tentorium, and posterior fossa dura according to the size of the tumor. Therefore, for tumors slightly protruding into the posterior fossa from the prous of the internal auditory canal, only the bone adjacent to it is removed (Bochenek et al's method). For larger tumors, labyrinth

The effects of extreme cold on sensory nerves.

The effects of extreme cold on sensory nerves are discussed and a clinical application of these effects is proposed. The structural changes observed following the freezing of sensory nerves in the rat are described and correlated with the clinical results in patients with chronic facial pain treated by cryogenic peripheral nerve blockade. It is suggested that this technique offers features which are not shown by any other method for interrupting peripheral pain pathways and provides a useful alternative to existing methods of treatment for chronic pain.

Early diagnosis in Bell's palsy. A nerve conduction study.

In 100 consecutive patients affected by idiopathic facial palsy the final degree of recovery was found to be inversely related to the distal latency time (DLT) measured within 72 hours of the onset of the disease. Incomplete recovery occurred in 77% of patients in whom the DLT was prolonged more than 10.0 msec, while complete recovery was found in 92% of cases in whom the DLT was less than 6.0 msec. This inexpensive test is easily and quickly performed and does not depend on the subjective responses or the cooperation of the patients. It may fill the gap between the onset of the palsy and the

[Principal features of the pathogenesis and pathogenetic treatment of certain neurologic syndromes of the facial region].

Results of many-year studies and literary data on the clinical course, pathogenesis and pathogenetic therapy of motor, sensor, and vegetotrophic disturbances in the facial region are reviewed. A concept is substantiated, that neuralgia of the trigeminal, glossopharyngeal and vagal nerves, secondary contracture of the mimic muscles, and facial hemiatrophy are diseases with a specific pathogenesis having no analogy with pain, motor, and trophic disturbances in other parts of the body. This is reflected in the special methods of the pathogenetic therapy of the diseases.

Hearing impairment and facial palsy as initial signs of Wegener's granulomatosis.

In two patients with Wegener's granulomatosis, treatment-resistant serous otitis media was followed by unilateral facial nerve palsy. Later both patients developed uraemia due to extracapillary glomerulonephritis. In one of them, who was treated with prednisone and cyclophosphamide, renal function improved, while in the other patient it remained impaired. Both patients suffered lasting hearing impairment in spite of general improvement during the course of the disease. The facial nerve palsy improved slightly in one, while no improvement was seen in the other. The possibility of Wegener's gran

Histopathology of cryosurgery for Meniere's disease.

The histopathologic findings in the temporal bones of two patients who underwent a cryo-otic-perotic shunt procedure are reported. Morphologic changes in the inner ear were limited to the area of cryoprobe application on the promontory. The hook region of the cochlear duct showed severe atrophy of the spiral ligament, stria vascularis, and sensorineural structures. The ampulla of the posterior semicircular canal showed fibrosis in the perilymphatic space and ruptured membranous walls. No changes were found in the rest of the vestibular labyrinth or in the facial nerve. The long-term improvemen

[Moebius syndrome : one case (author's transl)].

The Moebius syndrome is a congenital anomaly characterized by paralysis of the sixth and seventh nerves. In some instances, there may be weakness of the adductors. The other muscles are normal. Visual acuity is not affected. Other congenital defects may be associated: polydactyly, defects of the ear and of the tongue. Etiology is probably an embryonal egression, cause of aplasia of the facial and abducens nucleï. The impossibility to move the eyes laterally in either direction, points to supranucleus involvement by defective development of the medial longitudinal fasciculus. Some people sugges

Surgical treatment of vertigo.

Endolymphatic saccotomy, vestibular neurectomy via middle fossa, labrinthectomy and translabyrinthine vestibular neurectomy have been used by the authors in 54 patients with episodic vertigo, chiefly from Ménière's disease. Over a mean follow-up period of 15 months hearing retention and vertigo control were of the same general order as in the reports of other surgeons. Our current ideas on indications for the different surgical procedures are discussed.

Non-Hodgkin lymphoma of the parotid gland: diagnosis, evaluation, and treatment.

Malignant lymphoma of the parotid gland not infrequently presents as a solitary mass indistinguishable from other commonly occurring lesions. Two new cases are presented here with a comprehensive literature review. When encountered, the lesion should be excised with the superficial lobe of the gland. If it is within the deep lobe, the tumor should also be excised with preservation of the facial nerve. Recommendations for further evaluation and treatment are given.

[Tolosa-Hunt syndrome (ophthalmoplegia dolorosa)].

In the reported case unilateral external ophthalmoplegia developed in a young woman, with concomitant pain and anaesthesia in the area innervated by the 1st branch of the trigeminal nerve. After ruling out an expanding lesion of the central nervous system, aneurysm, diabetes, myasthenia, multiple sclerosis and other diseases dexamethasone was administered in a total dose of 30 mg. Complete clinical remission was achieved. In the light of observations and a survey of the peritinent literature it has been accumed that the disease was caused by a non-specific process in the small vessels in the v

Bell's palsy; prognostic accuracy of case history, sialometry and taste impairment.

A prognostic test in cases of Bell's palsy must be accurate and applicable early in the course of the disease. The case history is examined in 239 patients with Bell's palsy. The prognostic value of the case history, taste impairment and sialometry is investigated in a series of untreated patients who were followed up for 12 months. Sialometry is advocated as a reliable progostic indicator in the individual patient if a strong stimulant is used. None of the other examined parameters discrimates between good and poor outcome with reasonable accuracy. A follow-up time of 12 months is advocated i

Progressive extrinsic ophthalmoplegia with peripheral neuropathy and storage of muscle glycogen.

A case of progressive extrinsic ophthalmoplegia associated with peripheral neuropathy and hypertriglyceridemia type IV is described. Motor and sensory conduction velocities of the spinal nerves were severely decreased, while the EMG of the facial muscles was more suggested of a myopathic disorder. Electron microscopic study of biopsies of biceps and peroneus brevis muscles disclosed many ragged red fibers, mainly type I, which contained typical abnormal mitochondria. Other fibers, all type II, contained increased amounts of glycogen between myofibrils or beneath the sarcolemmal membrane, but t

Muscular and other abnormalities in a case of Edwards' syndrome (18-trisomy).

This paper describes the anatomical variations observed in a specimen exhibiting Edwards' Syndrome (18-trisomy). The clinical and autopsy data are compared with those reported in earlier literature. Aside from having a tracheoesophageal fistula, the viscera were characterized by abnormalities of the heart, lungs, liver and kidneys. The facial musculature was relatively undifferentiated. Only a few abnormalities were recorded in the otomandibular and suprahyoid structures. The infrahyoid region had three pairs of supernumerary muscles, including the "sternohyoideus azygos." The bluk of abnormal

Hemifacial spasm: clinical and electrophysiologic observations.

Twenty-three patients with hemifacial spasm were studied clinically and electrodiagnostically. Seven patients had mild facial weakness. All patients had clinical evidence of synkinesis, which often varied considerably. Facial nerve conduction and blink reflex latencies were normal. Facial synkinesis could be measured objectively on the involved side in all patients by simultaneously recording from the orbicularis oculi and orbicularis oris muscles at the time of supraorbital nerve stimulation. Using this procedure, synkinesis was also observed in association with aberrant regeneration after Be

Congenital unilateral lower lip palsy.

Thirty-nine cases of congenital facial asymmetry, in which one corner of the mouth does not dip downward symmetrically with the other, are presented. The author has termed this condition 'congenital unilateral lower lip palsy' (CULLP). CULLP was the most frequent condition in congenital facial anomalies seen in our clinic. Complaints were purely cosmetic. No functional disorders in pronouncing labial sounds or in suckling were noticed. As for pathogenesis, an insufficiency of the unilateral depressor labii inferior muscle is the most likely cause. An important point is that the lower lip, when
