Function of submaxillary gland following iatrogenic damage to chorda tympani nerve.

Flow rate, protein and amylase content of submaxillary saliva were determined before and after iatrogenic damage to the chorda tympani nerve during otosurgery. Parallel to these investigations, gustatory function was checked. Chronic otopathy itself did not lead to a disturbance of either the sense of taste or the function of the submaxillary gland in any of the 14 patients. On the other hand, the flow rate of the affected gland dropped after chorda division (8 patients) and after chorda stretching (6 patients), and the protein concentration of the saliva was increased while the concentration

[Disorders of the coordination of masticatory muscles in lesions of the brain stem].

Observations of 10 patients were reported. In addition to other neurologic deficits, coordination of the trigeminus, a motor nerve, may also be disturbed with brain lesions. Disturbance of bilateral synergism can be demonstrated with the EMG. The muscles closing the jaw on the affected side are paradoxically innervated when the jaw is opened, i.e., they function like antagonists to the muscles responsible for jaw closure on the opposite side. This condition may produce trismus. Similar central disturbances in bilateral coordination of the ocular muscles have also been established. Bilateral sy

Epidemiological, clinical, and pathomorphological characteristics of epidemic poliomyelitis-like disease caused by enterovirus 71.

In May-September, 1975, an outbreak of epidemic disease clinically and pathomorphologically simulating nearly all known forms of poliomyelitis occurred predominantly among young infants in Bulgaria. Most cases presented benign aseptic meningitis, sometimes with a short period of general cerebra- symptoms. Paralytic forms, such as bulbar polioencephalitis, anterior poliomyelitis, isolated pareses of the facial nerve, occasional cases of encephalomyocarditis, etc., were observed in about 21% of all cases. Over one-fourth of the paralytic cases with bulbar symptoms ended fatally. In March, 1976 a

[Clinical aspect of herpes zoster and the pathogenesis of symptomatic cranial nerve zosters].

The prognosis of shingles is generally good, however if cranial nerves are affected, the course of disease may be very serious. It is still doubtful if latent virus-in the sense of a trigger mechanism-may be activated by dental surgical interventions and cause a symptomatic herpes zoster of cranial nerves. Serious loss of teeth as an oral manifestation of shingles, with panostitis and retarded healing show the seriousness of the tissue changes. The virus can invade the plexus between the facial and trigeminal nerves and can thus travel within the same epineural sheath from one nerve to the oth

[Bell's palsy (etiology, pathogenesis, clinical picture, course, outcome)].

The authors studied 908 patients with Bell's paralysis. In 373 cases the etiology was cooling, in 52--vascular diseases, in 207 cases the etiology was not established. An analysis of the clinical picture development permits to assume that different modern theories of the pathogenesis of Bell's paralysis (the theory of primary ischemia, secondary ischemia a combination of primary and secondary ischemia) do not exclude each other, finding a different application depending upon the etiology of the paralysis. The report contains a detailed description of the clinical picture and development of Bel

Moebius syndrome, peripheral neuropathy and hypogonadotrophic hypogonadism.

The association of congenital ophthalmoplegia and facial paresis (Moebius syndrome) with a variety of other developmental somatic defects has been widely recognised. Its co-existence with hypogonadism of hypothalamic/pituitary origin and subclinical peripheral neuropathy has been reported and in this paper we describe the second case of the Moebius syndrome in association with hypogonadotrophic hypogonadism and a progressive peripheral neuropathy of mixed axonal and demyelinating type.

[Facial paralysis during progressive chronic otitis. Reflections apropos of 32 cases].

The authors report their experience of 32 cases of facial paralysis occurring during progressive chronic otitis. Twenty were pre-operative. The conclusions which may be drawn are as follows: 1) It occurs in general in cases of severe chronic otitis with large destructive cholesteatomatous and osteitic lesions. 2) Surgery is often difficult, long full of unexpected findings and risks. 3) The development of facial paralysis in a case of progressive chronic otitis is an indication for immediate surgery, on the same basis as a fistula of the H.S.C.C., which is, in fact, oftan present in associatio

Diagnosis and surgery of acoustic tumours.

In the diagnosis of acoustic neuromata the stapedial reflex test, the forward vs reverse Békésy test, the loudness balance test, and the adaptation test have all been found superior among the special tests used in predicting a tumour diagnosis. Conventional X-ray studies of the internal acoustic meati are always performed, followed by tomograms and in suspected cases by meatocisternography. In the present series translabyrinthine total removal was performed in 21 cases, a planned decompression in 5, while in 2 the removal was subtotal. Only two permanent facial paralyses arose, one corrected b

[Unilateral deafness (author's transl)].

Unilateral conductive deafness has a readily determined aetiology in most cases. In contrast, unilateral sensorineural hearing loss requires more refined and extensive investigation. The most frequent causes of unilateral sensorineural hearing loss in our patients were sudden deafness, Menière's disease, cranio-vertebral dysplasia and cerebellopontine angle tumors. Early diagnosis of acoustic neuroma or other lesions of the internal auditory meatus or cerebellopontine angle requires special attention. The definitive diagnosis of these tumors often demands intensive clinical investigation but a

Speech, language and hearing in Moebius syndrome: a study of 22 patients.

A comprehensive profile of communicative disorders in patients with Moebius syndrome was attempted. Seven patients were evaluated by the authors and the findings were added to 15 contributed case-histories. Most patients demonstrated some degree of dysarthria due to congenital paralysis of the facial nerves. Other problems such as cleft palate, hearing loss, mental retardation and delayed development of language were found in a few of the patients. As most of the older children in the sample and in reports in the literature eventually developed intelligible speech, a program of oral language s

Congenital atresia of the ear.

Congenital atresia of the ear requires prompt diagnosis and an early assessment of hearing. In bilateral atresia, aural rehabilitation should be started early in life to avoid late sequelae of speech problems. In unilateral atresia, the need is not urgent if the child hears normally in the other ear. Polytomography and audiometric testing are the two most important parts of the patient's evaluation. Precise audiometric testing may be difficult, or even impossible, in these children. The indications for surgery are presented. The risk of injuring an abnormally placed facial nerve remains a dete

Neurological manifestations of Paget's disease.

The neurological features of 96 patients whose skull and spinal X-rays showed the features of Paget's disease of bone were reviewed. The clinical phenomena were compared with those in another group of patients, identified by having a normal skull X-ray. Cranial nerve lesions and spinal syndromes could be attributed to the Paget's disease in many cases but the evidence did not suggest that other clinical phenomena such as dementia and epilepsy were related to the bony changes. Amongst cranial nerve lesions deafness is certainly attributable to Paget's disease, hemifacial spasm when combined wit

Adult-onset of Tangier disease: 1. Morphometric and pathologic studies suggesting delayed degradation of neutral lipids after fiber degeneration.

A 67-year-old woman, with the typical biochemical features of Tangier disease, had a syringomyelia-like syndrome which has now been observed in several patients with symptomatic onset in adult life. She developed progressive facial diplegia, bilateral wasting of hand muscles and loss of sensation over cranial, cervical and brachial dermatomes over 17 years. Nociception alone was first affected, then nociception and thermal discrimination and finally all modalities of sensation. Quantified tests of cutaneous sensation confirmed that sensation was normal in lower limbs but markedly abnormal in u

Adenoidcystic carcinoma (cylindroma) of the parotid gland.

Adenoidcystic carcinoma or cylindroma of the parotid gland is a slow-growing, relentless, and often lethal disease which kills usually by local occurrence and direct extension of the disease. The reported incidence of adenoidcystic carcinoma with reference to other parotid malignancies varies from 5% to 37%. The incidence in this series is 36%. The presence of facial nerve involvement adversely affects the long-term survival. Some authors regard facial nerve involvement as an indication of noncurability. The incidence of local pain is reported to be as high as 50% and is thought to be caused b

[Facial palsy in benign tumours of the parotid gland (author's transl)].

Facial paralysis caused by benign parotid gland disease in 2 patients is reported. In one patient there was an acute parotid inflammation and a parotid cyst, to the medial wall of which the facial nerve was adherent. In the other patient the paralysis was assumed due to the toxic effects of a necrotizing lymph node, which was adherent to the nerve.

Lymphomatoid granulomatosis clinically confined to the CNS. A case report.

A 38-year-old man developed pain and peripheral-type weakness on the right side of his face and was discovered to have decreased hearing bilaterally, as well as optic nerve swelling on the right. The pain and optic nerve swelling subsided over a period of six weeks, but hearing loss and facial weakness persisted. Thirty months later, he developed dysphagia, ataxia, dysarthria, nystagmus, and progressive spastic quadriparesis. He died approximately four years after the onset of the illness. Although no evidence of disease was found other than in the central nervous system during life, two nodul

Facial palsy: unusual etiology.

This is a report of three patients who presented at the Mayo Clinic over a two-year period. All were initially diagnosed as having Bell's palsy but were later found to have a malignant neoplasm causing the paralysis. Two of the patients had breast carcinoma metastases involving the mastoid portion of the facial nerve. The third patient had an adenocarcinoma of the deep lobe of the parotid that involved the facial nerve distal to the stylomastoid foramen. The course of the facial paralysis in the two patients with the metastitic breast disease was almost identical. It consisted of episodes of p

Immunological concept for Bell's palsy: further experimental study.

This report concerns itself with additional experimental evidence to support the immunologic concept for the pathogenesis of Bell's palsy, using the mast cell as an index of immunological activity. In a previous experimental study, we postulated that degranulation of mast cells activated by complement or specific allergens with release of histamine and other substances may be the mechanism leading to nerve edema, ischemia, and paralysis. In this study we observed a loss of granulated mast cells in the more severely damaged facial nerves of immunized dogs after the intrafallopian canal injectio

Management and rehabilitation of the facial nerve in parotid surgery.

The facial nerve is the single most important consideration in the surgical management of most parotid disorders. Its surgical anatomy should be well known by the head and neck surgeon. Very few conditions of the parotid gland necessitate the sacrifice of the facial nerve. When this decision is made, the benefits to be derived should be very thoughtfully measured against the tremendous cosmetic and functional sequelae that follow, and the patient should understand preoperatively to the fullest the magnitude of this decision and all its consequences. Most facial nerve deficits resulting from in

[The trigeminus nerve in neuro-otological diagnosis (author's transl)].

The Trigeminus nerve is represented by a large area in brainstem and therefore we often find symptoms of this nerve, if there are dysfunctions in this region. Especially when we suspect disorders of the central part of the vestibular or auditory systems the Trigeminus can give distinct signs. Besides other qualities (tactile, nociceptive) the sensitivity for warmth can be measured exactly with the thermic-impulse-test (TITT). The importance of this test is shown in some cases (tumor of the cerebello-pontine angle, tumors in the brainstem and multiple sclerosis). On the other hand in lesons of
