Median nerve injuries and their management.

Injuries of the median nerve cause two major syndromes. Interosseous nerve injury as it branches from the median just below the elbow produces a characteristic deformity. Treatment involves neurolysis, or primary or secondary repair. Various tendon transfers can be used to compensate for lost function. The other syndrome is produced by median nerve injury at the wrist and consists of thenar muscle paralysis and a sensory deficit on the radial aspect of the palm and radial 3 1/2 digits. The carpal tunnel syndrome is the most common cause. Treatment consists of incising the volar carpal ligament

[Osteitis deformans cranio-orbitalis (Paget's) in a young patient. Operation and clinico-pathology study (author's transl)].

Among the cranio-facial anomalies involving the orbit, Paget's disease is of particular interest due to its progressive evolution and clinico-pathological characteristics. In this article a case of Paget's disease (juvenile type) with orbital expansion in a young woman of 27 years is presented. The onset of clinical symptoms were recorded at the age of 15 years progressing to optic atrophy and blindness in one eye and visual deterioration on the other. The peculiarity of this case lies on the early onset of clinical symptoms due to elective in volvement of the skull and sparing of the facial b

Acute peripheral facial palsy. Part of a cranial polyneuropathy?

In 14 of 16 consecutive patients with acute peripheral facial palsy, one or more (up to four) other nerves were involved. The nerves affected in addition to the facial nerve were as follows: trigeminal (ten patients), vestibular (eight), cochlear (six), vagus (one), and upper cervical (five). Virus was not isolated from any of the patients. A fourfold increase or decrease in complement-fixing antibody titers was present in eight patients (in four, varicella-zoster; in one, varicella-zoster and mumps; in two, cytomegalovirus; in one, mumps). Further, two of the patients with varicella-zoster an

Controlled thermocoagulation of trigeminal ganglion and rootlets for differential destruction of pain fibers: facial pain other than trigeminal neuralgia.

I describe the results of retrogasserian differential lidocaine block to aid in the selection of patients for a differential thermal lesion in the trigeminal ganglion and rootlets. This procedure temporarily duplicates the state of analgesia without anesthesia one seeks to make permanent with the radiofrequency heating. The results of this heating procedure are described in the treatment of 71 patients with facial pain of cancer, postherpetic, periodic migrainous neuralgia, acromegaly, trigeminal neuropathy, central pain, post-traumatic facial neuralgia, and atypical facial neuralgia.

Natural history of bell's palsy: the salivary flow test and other prognostic indicators.

Fifty-one patients with Bell's palsy were evaluated within two days of onset and followed for six months without surgical intervention or effective medical treatment in order to observe the natural history of the disease. Sixty-three percent had a complete return while 37 percent had incomplete return. Age, the presence of pain, and taste alterations had no prognostic value. The progression of the palsy, response to maximal stimulation and salivary flow testing were approximately 80 percent accurate in predicting outcome. The salivary flow test was the most useful prognostic indicator since sa

Cerebellopontine angle tumours, other than acoustic neuromas. A report on 34 cases. A presentation of 7 bilateral acoustic neuromas.

The symptoms, preoperative diagnostic results and histological findings in 34 cerebellopontine angle tumour cases, other than acoustic neuromas, are presented. The meningeomas dominate by far this group of tumours. The symptomatology is much less uniform than that of the acoustic neuromas, where the VIII nerve is dominant. The facial nerve is less involved, whereas the other cranial nerves give symptoms more frequently. One remarkable sign is the presence of severe trigeminal neuralgia, which we have not encountered in acoustic neuromas. Only 35% of the 'non-neuroma' patients had elevated spin

[The prognostic evaluation of Bell's palsy by combined functional diagnosis (author's transl)].

In 82 patients with Bell's palsy nerve excitability test (NET) and electrogustometry (EGM) were performed and the stapedius muscle reflex (SMR) was measured on the day of admission. The comparison of test results with the final outcome of the disease allowed the following observations: in cases with normal SMR a soon and complete regeneration may most probably be expected. The EGM seems to be of prognostic value during the first couple of days after onset only; together with a loss of stapedial function a threshold difference of 15 EGU or more indicates an unfortunate course of the disease. [Dominant generalized cortical hyperostosis with multiple involvement of the cranial nerves].

The authors report an observation of dominant generalized cortical hyperostosis with multiple and unilateral involvement of a certain number of cranial nerves. In other members of the family, who present with the same bone disorder, involvement of the cranial nerves was limited to the facial nerve, either uni or bilaterally, with a partially or totally regressive evolution. This cranial nerve involvement, which is classic in the recessive form (first described) of generalized cortical hyperostosis, has until now, never been reported in the dominant form of this disorder.

Cranial polyneuritis and Bell palsy.

In view of the specific nature of the clinical and neurologic findings in Bell palsy and other acute benign cranial neuritides, the neural component of cutaneous herpes simplex, the predilection of the herpesvirus for sensory nerves, and intrinsic behavior and immunologic interreactions of the herpesvirus within ganglion cells, it is suggested that (1) the entity that has been termed "idiopathic facial paralysis" be recognized as an acute benign cranial polyneuritis; and (2) other acute benign cranial neuritides be recognized as formes frustes of Bell palsy.

Neuro-otologic manifestations of sarcoidosis.

Sarcoidosis is an idiopathic disease which presents in anatomic areas of concern to otorhinolaryngologists. It can cause dysfunction of both auditory and vestibular systems. In patients known previously to have sarcoidosis, this disease should be seriously considered. In patients presenting with otologic disorders and associated facial nerve paralysis or other neuropathies, uveitis, granulomatous meningitis or diabetes insipidus, sarcoidosis should be suspected. An examination of the eyes as well as a chest X-ray is imperative. Sudden and fluctuating neurosensory hearing loss has been reported

The clinical features of sclerosteosis. A review of the manifestations in twenty-five affected individuals.

Sclerosteosis is a unique autosomal recessive condition in which skeletal overgrowth is associated with syndactyly and digital malformation. Analysis of the course and clinical features in 25 affected individuals showed that the condition is progressive and potentially lethal. Facial palsy and deafness are common complications and raised intracranial pressure may develop. The clinical and radiographic stigmata of sclerosteosis permit differentiation from the other disorders of the "osteopetrosis" or "Albers-Schönberg disease" group, in which bony thickening and cranial nerve palsy occur.

Studies of malformation syndromes of man XXXXI B: nosologic studies in the Hanhart and the Möbius syndrome.

We reviewed etiologic and phenotypic aspects of those orofacial and limb anomalies usually diagnosed as Hanhart syndrome and Mobius syndrome but also those described, among others, under names such as aglossia-adactylia syndrome, gloss-palatine ankylosis, ankyloglossia superior, peromelia and micrognathia, cleft palate/lateral synechiae syndrome, and the Charlie M. syndrome. By coding the degree of severity of the limb defects it was possible to compare these cases quantitatively and to determine the nosologic significance of associated cranial nerve palsies and chest abnormalities. We analyze

[Nonaneurysmal vascular pressure lesions of the cranial nerves (author's transl)].

Four cases of facial spasm and 1 case of oculomotor paresis are described. The source of the disorder in all 5 cases is mostlikely not an aneurysm of the vessels of the base of the brain. The literature is discussed and thereby it is shown that mechanical disturbances of other cranial nerves (II, V, VI, IX, XII) can be caused by similar vascular (nonaneurysmal) abnormalities.

Function and innervation of the involuntary m. retroauricularis.

Beside the automatic, obligatory and tonic coinnervation of the involuntary m. retroauricularis in conjugate lateral gaze (oculoauricular phenomenon, nystagmus) several other physiological ways of accidental coinnervation are described. In talking, chewing, swallowing and during involuntary inspiration irregular bursts of innervation may be registered. In sleep regular rhythmic inspiratory innervation is demonstrated as well as myoclonic jerks. With reservation, an allusion is made to rem-sleep. In "nervous subjects" irregular involuntary innervation of the m. retroauricularis might serve as a

[Tick-borne meningopolyneuritis (Garin-Bujadoux, Bannwarth)].

The meningopolyneuritis first described in 1922 by Garin and Bujadoux develops after a tick bite. At the site of the bite in some of the cases, erythema chronicum migrans develops first. Severe pains follow in this area which may spread to other parts of the body. Then peripheral paralysis appears with asymmetrical distribution. The motor cerebral nerves, especially the facial nerve, are also frequently affected. Mild sensitivity disorders occur with no systematic arrangement. There is a lymphocyte pleocytosis of 100-2600/3 mm3 and an increase in protein of varying intensity, meningeal symptom

Prognosis of malignant tumors of the parotid gland with facial paralysis.

A parotid malignant tumor with spontaneous facial nerve involvement is a serious prognostic sign. Of 34 patients reported, 29 have been followed for at least five years. In this group, 20 are dead of disease (one at five years), one is dead of other causes (at seven years), one is living with disease, and seven are free of disease. Four of these seven cases have been followed for more than ten years, with two being free of disease for 12 years. Thus, it is not a hopeless situation, as some studies have shown, and not a contraindication to combined radical surgery and radiotherapy treatment for

Combined limb deficiencies and cranial nerve dysfunction: report of six cases.

Six cases have been presented with combined limb deficiencies and cranial nerve dysfunctions. The major features are congenital disarticulation and congenital amputation associated with various orofacial deficits. Anatomic variations in limb and facial anomalies in these cases, as well as in other cases previously reported; seem to indicate that they are part of a single dysmorphic syndrome rather than a group of distinct clinical syndromes as reported in the past.

The use of soft bandage lenses in corneal disease.

Forty-one patients were treated with soft bandage lenses for various types of corneal pathology. These patients have been monitored over a two-year period. Comfort was consistently achieved with cases of bullous and exposure keratopathies. It was found that the Warner-Lambert Softcon lens gave better protection in patients with VII nerve palsies than the Bausch & Lomb SOFLENS contact lens. However, little difference was noted between the two types of lenses when treating other corneal diseases. It was also found that topical medications, with the exception of steroids, caused no complications.

Cellular immune response to peripheral nerve basic protein in idiopathic facial paralysis (Bell's palsy).

Lymphocytes from patients with Bell's palsy were shown to undergo significant stimulation when cultured in vitro in the presence of a purely neuritogenic basic protein (P1L) isolated from human peripheral nerve myelin. No sensitization was observed to other neural antigens, namely, another periperal nerve myelin basic protein (P2) and the central nerve myelin basic encephalitogenic protein (BE). A similar pattern of response was also demonstrated in patients with Guillain-Barré syndrome (GBS).
