BACKGROUND AND OBJECTIVE: Trigeminal schwannomas (TSs) are rare, mostly benign tumors that exhibit an indolent clinical course. The lateral orbitotomy approach (LOA), with or without endoscopic assistance, has emerged as a novel skull base route, particularly for accessing lesions confined to the middle cranial fossa. Herein, we elucidate the advantages and limitations of the LOA for the treatment of TSs.
METHODS: All TS cases that underwent LOA through a lateral canthus incision during the past decade in our department were retrospectively reviewed. The operative technique was detailed, and

PURPOSE OF REVIEW: Despite the scarce literature regarding the matter, this current article identifies the most important factors that lead to the best outcomes in the therapeutic approach of trigeminal neuralgia and hemifacial spasm. However, notwithstanding the central focus lies on the neurosurgical perspective, the authors also integrate insights from adjacent medical specialties. Thus, the main objective of this comprehensive narrative review is to enhance understanding of the factors that can dramatically influence the course of these conditions.
RECENT FINDINGS: This article provides a

BACKGROUND/OBJECTIVE: Trigeminal neuralgia (TN) is a severe neuropathic pain disorder that may necessitate surgical intervention when medical therapy fails. Among minimally invasive procedures, radiofrequency (RF) lesioning and percutaneous balloon compression (PBC) are most widely used; however, their relative efficacy and safety remain controversial. This systematic review and meta-analysis included only double-arm comparative studies that directly evaluated RF and PBC outcomes in TN.
METHODS: Following PRISMA guidelines, PubMed, Embase, Scopus, and Web of Science were searched to August 20

BACKGROUND: Cranial nerve involvement is a well-recognized feature in Guillain-Barré syndrome (GBS) but remains less well understood in chronic forms of autoimmune neuropathies. Earlier studies of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) were conducted before updated diagnostic criteria and the recognition of autoimmune nodopathy (AN), which may limit the interpretation of their findings.
METHODS: We retrospectively analyzed 582 patients with chronic autoimmune neuropathies-CIDP (n = 431), multifocal motor neuropathy (MMN) (n = 64), anti-myelin-associated glycoprotein

BACKGROUND: Trigeminal neuralgia (TN), or Fothergill’s disease, is a neurological condition, known for episodic paroxysmal electrical shock-like pains across the sensory divisions of the trigeminal nerve. Being a serious cause of orofacial pain, referral to a dentist is sometimes necessitated. However, diagnosis can easily be confused, resulting in undertreated patients. OBJECTIVE: This study aimed to evaluate the patterns of dentists’ interventions prior to accurate TN diagnosis and to assess the effectiveness of such interventions. METHODS: Approval for conducting this retrospective analysis

BACKGROUND/AIMS: Facial nerve palsy (FNP) impacts the ocular and periocular regions. Preganglionic trigeminal nerve palsy (PrGTP) may complicate preganglionic FNP (PrGFP), resulting in neurotrophic keratopathy (NK), risking corneal sequelae. We present a case series where comprehensive eyelid surgery (CES) to manage the eyelid sequelae of PrGFP, complicated by PrGTP, resulted in the recovery of corneal sensation, avoiding corneal neurotisation surgery (CNS).
METHODS: A retrospective case series of three patients with PrGFP and PrGTP in whom eyelid abnormalities were addressed through CES. Out

BACKGROUND: Trigeminal neuralgia (TN) resistant to pharmacologic treatment can be treated with microvascular decompression, percutaneous procedures, or radiosurgery. Regarding radiosurgery, there is variability in target definition, dose selection, radiation delivery and positioning, and immobilization.
METHODS: This observational prospective study included patients with TN treated with 90 Gy prescribed to an anterior cisternal isocenter, delivered with a TrueBeam LINAC, under mask immobilization, Novalis couch positioning, and ExacTrac X-ray and infrared positioning and motion monitoring. Tr

BACKGROUND: Inferior alveolar nerve (IAN) and lingual nerve (LN) injury are recognized complications of oral and maxillofacial trauma or surgical procedures, often requiring subsequent microsurgical repair. This study explores the potential of photochemical tissue bonding (PTB), a light-activated technique that forms molecular crosslinks between collagen molecules, to seal a commercial nerve connector in place for IAN and LN nerve repair.
PURPOSE: The purpose of this study was to measure and compare the repair strength of conventional suture and PTB repair in a nerve transection model.

BACKGROUND: Cognitive impairment affects prognosis and quality of life in patients with trigeminal neuralgia (TN). Modulating hippocampal microglial activation is a promising approach for addressing TN-related cognitive deficits. Nuclear receptor subfamily 4 group A member 1 (Nr4a1) is involved in hippocampal-dependent cognition and microglial homeostasis. However, its role in trigeminal neuralgia-associated cognitive dysfunction remains unclear. We aimed to investigate the role of Nr4a1 in regulating neuroinflammation mediated by microglial hyperactivation, and to establish the relationship b

BACKGROUND: The pretemporal anteromedial interdural approach provides direct extradural access to Meckel's cave by exploiting the natural plane between the temporal dura propria and the meningeal layer of the lateral wall of the cavernous sinus. After a pretemporal craniotomy, the foramen rotundum guides interdural dissection along V2 toward the corridor between V1 and V2, enabling controlled exposure of the Gasserian ganglion and proximal trigeminal rootlets. Tumor resection proceeds through internal debulking and circumferential dissection, with extension into the posterior fossa when necess

OBJECTIVE: The optimal surgical approach for trigeminal schwannomas (TSs), particularly between the endoscopic endonasal approach (EEA) and endoscopic transorbital approach (ETOA), remains unclear. This study aimed to analyze radiological factors influencing surgical outcomes and establish criteria to guide approach selection.
METHODS: A retrospective review of the records of TS patients treated with the EEA or ETOA at three tertiary centers from 2015 to 2023 was conducted. The authors introduced the orbito-ovale line (OOL) to classify tumors as low-lying or high-lying. A scoring system (rang

OBJECTIVE: Microvascular decompression (MVD) is an effective treatment for trigeminal neuralgia (TGN), but anatomical/clinical predictors of long-term outcomes are incompletely defined, and many studies emphasize only immediate complications. This study investigates long-term outcomes after MVD for TGN and predictors of postoperative pain relief and recurrence.
METHODS: One hundred thirty-two MVDs performed for TGN in 124 patients (February 2012-November 2021) were retrospectively analyzed. Demographics, prior therapies, preoperative magnetic resonance imaging (MRI), intraoperative findings,

BACKGROUND: Trigeminal neuralgia (TN) is a severe neuropathic pain disorder in which peripheral neurovascular compression alone does not fully explain the associated central pain manifestations. Although diffusion tensor imaging studies have reported white matter (WM) alterations in TN, findings have been heterogeneous, partly due to limited sensitivity to complex fibre architecture. Fixel‑based analysis (FBA) enables fibre‑specific assessment of WM microstructural and macrostructural properties and may provide improved anatomical and biological specificity. METHODS: This cross‑sectional study

BACKGROUND: Extraction of lower third molars (3 Ms) carries a risk of inferior alveolar nerve (IAN) injury. Anatomical proximity is a well-known risk factor, although the effect of patient age remains insufficiently explored.
PURPOSE: The study purpose was to measure the association between age and IAN injury among patients whose 3 Ms are close to the mandibular canal.
STUDY DESIGN, SETTING, SAMPLE: This case-control study included patients at increased risk for IAN injury due to close proximity of the lower 3 Ms to the mandibular canal, as assessed by preoperative cone-beam computed tomogra

BACKGROUND: Immediate implant placement in the posterior mandible is associated with a risk of inferior alveolar nerve (IAN) injury due to anatomical variations such as lingual concavities and variability in mandibular canal positioning. Accurate three-dimensional assessment of these structures is therefore essential.
OBJECTIVES: This study aimed to evaluate the root apex-mandibular canal distance (RAC), prevalence and morphology of lingual concavities, and cross-sectional ridge patterns in the posterior mandible to identify high-risk sites for IAN injury during immediate implant placement.

Idiopathic trigeminal-mediated headshaking is a painful neuropathic disorder characterized by sudden, violent, predominantly vertical head movements of unknown etiology. Clinical signs may be seasonal or persistent and triggered by light, sound, exercise, or tactile nasal stimulation. The pathophysiology is multifactorial, involving abnormal trigeminal nerve excitability influenced by hormonal, environmental, and dietary factors. Treatment is challenging and typically multimodal, aiming to reduce trigeminal hypersensitivity and improve quality of life.
