Investigation of miRNAs affecting schwann cells in patients with trigeminal neuralgia.

BACKGROUND: Trigeminal neuralgia (TN) is a debilitating neuropathic pain disorder characterized by severe paroxysmal facial pain. Increasing evidence indicates that Schwann cell dysfunction and altered nerve repair mechanisms contribute to TN pathology. MicroRNAs (miRNAs), as key regulators of Schwann cell plasticity, myelination, inflammation, and nerve regeneration, represent promising biomarker candidates. This study aimed to investigate differential expression patterns of Schwann cell-associated miRNAs (miR-221-3p, miR-132-3p, miR-210-5p, miR-340-3p, miR-98-5p, miR-182-5p) in patients with

TRPM3 mediates spontaneous pain and mechanical allodynia in a mouse model of chronic orofacial neuropathy.

Trigeminal nerve injury can lead to chronic and difficult-to-treat orofacial neuropathic pain. Here, we uncover a key role for the cation channel TRPM3 in the chronic constriction injury of the infraorbital nerve (IoN-CCI) mouse model of trigeminal neuropathic pain. Wild-type (WT) mice develop spontaneous pain and mechanical allodynia for up to 6 weeks following IoN-CCI, whereas Trpm3-/- mice do not develop such symptoms. Using longitudinal RNA sequencing (RNA-seq) analysis, we obtain a detailed time course of transcriptome alterations in trigeminal ganglia during progression of the IoN-CCI mo

KDM2B-PP1 Promotes Remyelination and Functional Recovery After Facial Nerve Injury.

BACKGROUND: Peripheral nerve injury (PNI) often causes persistent sensory and motor deficits because of incomplete axonal regeneration and insufficient remyelination. Schwann cells (SCs) are essential for peripheral nerve repair, but effective recovery depends on their timely transition from a repair state to a myelinating phenotype. OBJECTIVE(S): To investigate whether KDM2B-PP1, a bioactive peptide designed to disrupt the interaction between lysine-specific demethylase 2B (KDM2B) and enhancer of zeste homologue 2 (EZH2), promotes SC redifferentiation, remyelination, and functional recovery

Digitally Guided Intraoral Segmental Mandibulectomy and Fibula Flap Reconstruction for Locally Invasive Odontogenic Tumors.

Radical treatment of ameloblastoma and other benign locally invasive odontogenic tumors frequently requires mandibular segmentectomy followed by functional reconstruction. Conventional reconstruction is typically performed through an extraoral approach to obtain sufficient exposure; however, this technique often results in visible facial scarring, extensive soft-tissue dissection, and potential injury to the marginal mandibular branch of the facial nerve. This protocol describes a reproducible digitally guided workflow for mandibular segmental resection and reconstruction using a free vascular

AI-Based Angle Map Analysis of Facial Asymmetry in Peripheral Facial Palsy.

Peripheral facial palsy (PFP) causes pronounced facial asymmetry and functional impairment, highlighting the need for reliable, objective assessment. This study presents a novel, fully automated, reference-free method for quantifying facial symmetry using artificial intelligence (AI)-based facial landmark detection. A total of 405 datasets from 198 PFP patients were analyzed, each including nine standardized facial expressions covering both resting and dynamic movements. AI detected 478 landmarks per image, from which 225 paired landmarks were used to compute local asymmetry angles. Systematic

Iba1 deficiency impairs microglial synaptic remodeling and neuronal survival after axonal injury.

BACKGROUND: Microglia remodel neuronal circuits in pathological conditions; however, the molecular requirements for these responses and their consequences for motoneuron survival remain unclear. METHODS: Aif1 (Iba1) knockout mice were generated using CRISPR/Cas9-mediated deletion, and baseline phenotypes and responses to unilateral facial nerve axotomy were assessed using immunohistochemistry, transmission electron microscopy, and single-nucleus RNA sequencing of the facial motor nucleus. Motoneuron survival and nuclear γH2AX foci were evaluated 28 days post-axotomy. FINDINGS: Under baseline

Novel clinical classification system for peripheral facial nerve defects after oncological resection.

Peripheral facial nerve defects resulting from oncological resection necessitate early repair, and underscore the need for accurate intraoperative assessment of their extent. This study proposes a span-based classification system for peripheral facial nerve defects, based on the length and location of the defect. The system delineates specific defect characteristics and provides practical guidance for intraoperative decision-making and personalised repair plans.

Modified Radical Neck Dissection for Cervical Metastasis.

Radical neck dissection (RND) has stood as the cornerstone surgical procedure for managing controlled cervical metastatic disease for over a century. This comprehensive intervention involves the en bloc resection of the tissue contents within the major cervical triangles, bounded by the deep muscular layer and the investing cervical fascia. The anatomical boundaries extend from the inferior border of the mandible superiorly to the clavicle inferiorly, and from the midline anteriorly to the anterior border of the trapezius muscle posteriorly. A standard RND specimen systematically incorporates

Activating the vagus nerve-adrenal anti-inflammatory axis to delay aging: A randomized controlled trial protocol.

BACKGROUND: Aging is a primary risk factor for chronic diseases, yet safe and effective interventions remain limited. Traditional Chinese Medicine (TCM) highlights the role of acupuncture in health preservation, with ST36 (Zusanli) being a key acupoint for longevity. Preclinical studies suggest ST36 acupuncture modulates inflammation and oxidative stress, but robust clinical evidence is lacking. This trial aims to evaluate the efficacy of ST36 acupuncture in delaying aging and preventing age-related diseases through biomarkers and clinical outcomes. METHODS: A single-center, randomized, singl

Trigeminal nerve transposition by sling-pulling technique in combination with macrovascular transposition for giant basilar artery-induced trigeminal neuralgia via a combined transpetrosal approach.

BACKGROUND: Dolichoectatic vertebrobasilar arteries (DBA)-induced trigeminal neuralgia (TGN) is rare, and both medication and surgical decompression often have a substantial treatment failure rate. Surgical techniques that provide sufficient decompression and long-term pain relief are challenging in cases of large DBA with limited posterior fossa space, and have been scarcely described due to their rarity and technical complexity. We present our novel technique involving mobilization of the trigeminal nerve (CN5) combined with macrovascular transposition of the gigantic DBA for curative treatm

Predictive value of preoperative white and gray matter microstructural alterations for microvascular decompression outcome in classical trigeminal neuralgia.

BackgroundClassical trigeminal neuralgia (CTN) is a severe chronic pain disorder. While microvascular decompression (MVD) is an effective treatment, outcomes are variable, and preoperative predictive biomarkers are needed. Furthermore, the relationship between brain microstructural alterations and neurotransmitter systems in CTN is not well understood. This study aimed to characterize these alterations and assess their prognostic value for MVD.MethodsWe evaluated microstructural alterations in both white matter (WM) and gray matter (GM) in 101 CTN patients and 84 healthy controls (HCs) using N
